A large portion of the world's population is affected by acne vulgaris (AV), with many of these individuals being adolescents. The underlying mechanism of AV is hyperkeratinization and Cutibacterium acnes infection of the pilosebaceous follicle secondary to excessive stimulation of sebaceous glands by androgens. Metformin is a biguanide medication primarily used in efforts to lower patients' sugar levels in the management of type 2 diabetes. It has been proven to reduce levels of circulating androgens in patients with insulin resistance, indicating its potential for treating AV. A search strategy was developed and performed using the databases Ovid Medical Literature Analysis and Retrieval System Online (MEDLINE), Excerpta Medica database (EMBASE), Cumulative Index to Nursing and Allied Health Literature (CINAHL), Cochrane Controlled Register of Trials (CENTRAL), and Web of Science. The keywords "metformin" and "acne" were searched, along with related Medical Subject Headings (MeSH) and other subject headings. Studies that met the inclusion criteria were controlled trials, published after 2010, and in the English language. Participants with and without comorbidities such as polycystic ovary syndrome (PCOS) were considered. Two independent reviewers screened studies based on predefined criteria and extracted data from each study, which were quantitatively combined. A total of 15 studies were included in this systematic review. Across the 15 studies, there were 1,046 participants, with 13 studies looking exclusively at women with PCOS. Of the remaining two studies, one examined males with altered metabolic profiles, while the other included men and women with moderate AV. Notable risks of bias included studies that did not exclusively state the blindness of the study. Of the studies that were examined, 13 showed that metformin reduces AV, with seven studies showing statistical significance. Acne vulgaris is an inflammatory condition that has plagued patients for years due to the limited treatment options available. The hyperglycemic medication metformin, used in the management of type 2 diabetes, is being explored as a novel therapeutic that can possibly be repurposed for the treatment of AV. The use of metformin in AV is hypothesized to disrupt the proposed linkage between insulin resistance and AV proliferation. This proposed research could offer physicians a new option for the treatment of AV as well as render an alternative AV treatment for patients.

RESUMO

The growth of digital health has led to more app-based solutions entering the market which aim to provide therapeutic outcomes for a variety of diseases. This column explores the emerging field of digital therapeutics (DTx) as evidence-based and FDA-cleared digital interventions - setting them apart from the majority of commercially available solutions. In an effort to identify high quality and evidence-based options, medical and health science librarians can benefit from recognizing the qualifications of DTx, types of available DTx, and use cases for their patrons.

RESUMO

As the largest public funder of biomedical research in the world, the National Institutes of Health's (NIH) new Data Management and Sharing (DMS) Policy is a large step toward shifting the culture of medical research toward a broader sharing of scientific data. Librarians in the health sciences support researchers by assisting with data management plans, research dissemination, abiding by publisher/grant requirements on data sharing, and suggesting recommended repositories for data preservation. This article will serve as a primer on open data, data sharing, the NIH's DMS Policy and its implications, and how librarians can support researchers in this landscape.

As virtual and augmented reality technologies continue to advance, a new concept has risen above the rest that aims to advance the virtual experience to create a complex, interactive and interconnected world - known as the metaverse. Offering a near limitless potential, the metaverse would provide new 3-D environments for people to communicate, shop, learn, work, and do anything that would normally be done on the internet. This column will introduce the metaverse, provide current examples of development, and consider a few takeaways for medical librarians.

The Internet of Things (IoT) has transformed the way businesses and consumers collect and use data. By using disparate sets of data, companies and individuals alike can identify meaningful patterns of behavior. This new trend called "Internet of Behavior" (IoB) enables not only the evaluation of people's behavior, but also ways to modify or influence behavior. This column will provide a brief overview of IoB, how it is utilized, and how it may be useful for libraries.

The explosive growth of digital information in recent years has amplified the information overload experienced by today's health-care professionals. In particular, the wide variety of unstructured text makes it difficult for researchers to find meaningful data without spending a considerable amount of time reading. Text mining can be used to facilitate better discoverability and analysis, and aid researchers in identifying critical trends and connections. This column will introduce key text-mining terms, recent use cases of biomedical text mining, and current applications for this technology in medical libraries.

RESUMO

Telehealth services have slowly yet steadily increased over the past few decades as new technologies emerge. However, social distancing mandates, state shutdowns, and an overburdened healthcare system during the COVID-19 pandemic forced a dramatic surge forward in telehealth use and policy. While many of these emergency measures are temporary, the successes, failures and lessons learned during this period will change the way telehealth is administered, moving forward. This column will review recent changes to telehealth and telemedicine services during the pandemic and their impact on healthcare systems.
