Decabromodiphenyl ether (BDE-209) is recognized as an emerging and hazardous pollutant in numerous ecosystems. Despite this, only a few studies have concurrently investigated the biodegradation of BDE-209 by a microbial consortium comprising both bacteria and fungi. Consequently, the interactions between bacterial and fungal populations and their mutual effects on BDE-209 degradation remain unclear. Our main objective was to concurrently assess the changes and activity of bacterial and fungal communities during the biodegradation of BDE-209 in a real soil matrix. In the present study, various organic substrates were employed to promote soil biomass for the biodegradation of BDE-209. Soil respiration and molecular analysis were utilized to monitor biological activity and biomass community structure, respectively. The findings revealed that the use of wheat straw in the soil matrix resulted in the highest soil respiration and microbial activity among the treatments. This approach obviously provided suitable habitats for the soil microflora, which led to a significant increase in the biodegradability of BDE-209 (49%). Biomass survival efforts and the metabolic pathway of lignin degradation through co-metabolism contributed to the biodegradation of BDE-209. Microbial community analysis identified Proteobacteria (Alphaproteobacteria-Betaproteobacteria), Firmicutes, Bacteroides (bacterial phyla), as well as Ascomycota and Basidiomycota (fungal phyla) as the key microorganisms in the biological community involved in the biodegradation of BDE-209. This study demonstrated that applying wheat straw can improve both the biological activity and the biodegradation of BDE-209 in the soil of polluted sites.

The present study aims to determine the relationship between the concentration of arsenic in the groundwater of Hamadan province and the mortality rate due to various types of malignancies, congenital anomalies, diabetes mellitus and Alzheimer's. Mortality data due to various causes of death in Hamadan province were collected for five years (2016-2020). Sampling of drinking water was determined in the reference laboratory using polarography method. Poisson regression was used to investigate the relationship between arsenic level and the death rate due to various types of disease, at a significant level (p value < 0.05). According to the results of Poisson regression, among the various causes of death (N = 8042), Alzheimer's 5.94 (3.67-9.61), diabetes mellitus 4.05 (3.5-5.37), congenital malformations 2.98 (1.88-4.72), breast cancer 2.72 (1.56-4.71), leukemia 1.90 (1.24-2.92), stomach cancer 1.64 (1.28-2.10), Liver cancer 1.58 (1.58-2.30), other digestive organs 5.86 (3.38-10.16), meninges and brain cancer 1.57 (1.02-2.41) showed the highest relationship with arsenic contamination. The results of this study could be evidence for a positive and significant relationship between arsenic concentrations and mortality rates due to cancers, diabetes mellitus, Alzheimer disease, and congenital malformations. Therefore, it's necessary to use appropriate water treatment methods to remove arsenic at the source in contaminated areas.

This study aimed to evaluate the occurrence and likelihood of health risks related to arsenic in drinking water of all counties of the Hamadan province in the northwest of Iran. In this work, 370 samples were collected from all of the water resources of urban and rural regions, during 5 years (2017 to 2021). Oracle Crystal Ball software was used to perform the Monte Carlo simulation and investigate the potential health risks. According to the results, the average values of arsenic in the nine counties were in the order Kabudarahang (40.1 ppb), Malayer (13.1 ppb), Nahavand (6.1 ppb), Bahar (2.05 ppb), Famenin (0.41 ppb), Asadabad (0.36 ppb), Tuyserkan (0.28 ppb), Razan (0.14 ppb), and Hamadan (< 0.1 ppb). The highest concentration of arsenic occurred in Kabudarahang with a maximum value of 185 ppb. In the spring season, the average concentration of the cations, including calcium, magnesium, sodium, lead, cadmium, and chromium, obtained 109.51 mg/l, 44.67 mg/l, 20.50 mg/l, 88.76 ppb, 0.31 ppb, and 0.02 ppb, respectively. Based on the Delphi classification, the P 90% of oral lifetime cancer risk, in Hamadan province, were within level II (low risk) to VII (extremely high risk). The risk analysis revealed there was a possible carcinogenic risk to humans from oral exposure to As-contaminated groundwater, especially in Kabudarahang county. Therefore, there is an urgent need for management and precise measures in contaminated areas to reduce and prevent the adverse health effects of arsenic.

Introduction: The Sinopharm BBIBP-CorV vaccine produces a variety of cutaneous adverse effects. Scleromyxedema is a mucinous connective tissue disorder that causes skin thickness and sclerodermoid changes. According to our findings, this is the first case of scleromyxedema induced by the Sinopharm immunization. Case description: We discuss the case of a 75-year-old woman who acquired progressive thickening of the skin in her limbs and trunk after getting the Sinopharm vaccination. Examination, laboratory testing, and a biopsy were used to verify scleromyxedema diagnosis. Intravenous immunoglobulins, mycophenolate mofetil, and prednisolone were used in the treatment of the patient. The outcomes from the 4-month follow-up were reassuring. Conclusion: This study emphasizes the need of considering scleromyxedema as a connective tissue pathology in patients who have recently received Sinopharm vaccine and have similar cutaneous signs.

Increased levels of nutrients and algae can cause drinking water problems in communities. Harmful algal blooms affect humans, fish, marine mammals, birds, and other animals. In the present study, we investigated the use of a combined system [Hydrodynamic Cavitation, Ozone (O3), and Hydrogen Peroxide (H2O2)] on the removal of Chlorophyll a and Organic substances in the raw water was investigated. The Effect of different operating conditions such as pH, cavitation time, pressure, distance, flow rate, ozone dose, and hydrogen peroxide concentration was studied. Utilizing the Taguchi design method, experiments were planned and optimized. The combined system treatment yielded a maximum reduction in Chlorophyll a and Total Organic Carbon (TOC) at an optimum condition of pH 5, cavitation pressure 5 bar, flow rate of 1 m3/h, a distance of 25 cm from the orifice plate, O3 3 g/h and 2 g/l of H2O2 concentrations. The most efficient factor in the degradation of TOC and Chlorophyll a, was cavitation pressure based on the percentage contributions of each parameter (38.64 percent and 35.05 percent, respectively). H2O2 was found to have the most negligible impact on degradation efficiency (4.24 percent and 4.11 percent, respectively).

An amino-functionalized MIL-68(Al) metal-organic framework (amino-MIL-68(Al) MOF) was synthesized by solvothermal method and then characterized by FESEM, XRD, FTIR, EDX-mapping, and BET-BJH techniques. In order to predict arsenate (As(V)) removal, a robust quadratic model (R2 > 0.99, F-value = 2389.17 and p value < 0.0001) was developed by the central composite design (CCD) method and then the genetic algorithm (GA) was utilized to optimize the system response and four independent variables. The results showed that As(V) adsorption on MOF was affected by solution pH, adsorbent dose, As(V) concentration and reaction time, respectively. Predicted and experimental As(V) removal efficiencies under optimal conditions were 99.45 and 99.87%, respectively. The fitting of experimental data showed that As(V) adsorption on MOF is well described by the nonlinear form of the Langmuir isotherm and pseudo-second-order kinetic. At optimum pH 3, the maximum As(V) adsorption capacity was 74.29 mg/g. Thermodynamic studies in the temperature range of 25 to 50 °C showed that As(V) adsorption is a spontaneous endothermic process. The reusability of MOF in ten adsorption/regeneration cycles was studied and the results showed high reusability of this adsorbent. The highest interventional effect in inhibiting As(V) adsorption was related to phosphate anion. The results of this study showed that amino-MIL-68(Al) can be used as an effective MOF with a high surface area (> 1000 m2/g) and high reusability for As(V)-contaminated water.

Introduction: Nephrotic syndrome (NS) is a common chronic kidney disorder during childhood. The most important characteristic of this disease is proteinuria. The Persian medicine (PM) has important dietary recommendations for strengthening the kidney function and treatment of this disease. The aim of this study was to investigate the effect of a diet including PM recommendations and general principles of Western medicine. Materials and Methods: Twenty children with nephrotic syndrome were randomly divided into intervention and control groups and monitored for one month. The control group received a diet based on the general principles of Western medicine. In the intervention group, in addition to the Western medicine diet, dietary recommendations of PM were also prescribed including the pomegranate (Cydonia oblonga mill.), quince (Cydonia oblonga mill.), and whole grains (wheat and barley). A 24-hour dietary questionnaire was applied and anthropometric and biochemical indices including spot urine protein (proteinuria), albumin (Alb), urea, creatinine (Cr), total cholesterol (TC), and triglyceride (TG) were measured before and after the study. Results: The amount of protein intake reduced significantly in the diet of both groups but the differences between the two groups were not significant. Proteinuria reduced significantly in both the Western and PM groups; however, proteinuria was significantly lower in the Persian medicine group compared to the control group. TC and Cr levels reduced significantly in the intervention group, although the changes were not significant compared to the control group. Conclusion: The results of this study showed that adding dietary recommendations of the Persian medicine to the general rules of the Western medicine diet reduced proteinuria and improved the combat against nephrotic syndrome.

Background and Aims: Diabetes is a leading cause of renal failure. High levels of oxidative stress and inflammation in patients with renal diabetes lead to various disorders and mortality. This study was performed to determine the effect of Nigella sativa (NS) supplementation on superoxide dismutase (SOD), malondialdehyde (MDA), total antioxidant capacity (TAC), high-sensitivity C-reactive protein (hs-CRP), glycosylated hemoglobin (HbA1c), fasting blood sugar (FBS), and insulin (INS) in patients with diabetes mellitus undergoing hemodialysis (HD). Methods: In this randomized, double-blind, placebo-controlled clinical trial, a total of 46 diabetic HD patients were randomly divided into NS (n = 23) and placebo (n = 23) groups. NS group received 2 g/day of NS oil, and the placebo group received paraffin oil for 12 weeks. Serum levels of SOD, MDA, TAC, hs-CRP, HbA1C, FBS, and INS were measured before and after the study. Results: Compared to baseline values, SOD, TAC, and INS levels increased, whereas MDA, hs-CRP, HbA1c, and FBS significantly decreased. After adjusting for covariates using the ANCOVA test, changes in the concentrations of SOD (p = .040), MDA (p = .025), TAC (p=<.001), hs-CRP (p = .017), HbA1c (p = .014), and FBS (p = .027) were statistically significant compared to the placebo group. Intergroup changes in INS were not significant. Additionally, there were no notable side effects during the research. Conclusions: This study found that NS supplementation significantly enhanced the levels of SOD, MDA, TAC, hs-CRP, HbA1c, and FBS in diabetic HD patients.

BACKGROUND AND OBJECTIVES: The kidney is probably the most crucial target of microvascular damage in diabetes, which can ultimately eventuate end-stage renal disease (ESRD). Hemodialysis is the most usual way of renal replacement therapy in ESRD. Patients receiving hemodialysis are susceptible to many complications like hyperglycemia, inflammation, depression, anxiety, and poor quality of life. So, they are constrained to consume many drugs. Medicinal herbs are used in different cultures as a reliable source of natural remedies. This study aims to determine the efficacy of Nigella sativa (NS) oil supplementation on blood glucose, kidney function tests, inflammation, oxidative stress, quality of life, and depression in hemodialysis patients. METHODS AND ANALYSIS: This double-blind, randomized controlled trial will enroll 46 patients with diabetes mellitus who give hemodialysis thrice a week. Patients who have an inflammatory or infectious disease and who are receiving nonsteroidal anti-inflammatory drugs will be excluded. Patients will be randomized to the treatment and control group, which will be recommended using two soft gels of NS and paraffin oil, respectively. Laboratory tests will be assessed at baseline and end of the study, including fasting blood sugar, glycated albumin, insulin, creatinine, blood urea nitrogen, urea, uric acid, superoxide dismutase, malondialdehyde, total antioxidant capacity, high sensitive C reactive protein, and 24-h urine volume. Also, the kidney disease and quality of life and hospital anxiety and depression scale questionnaires will be evaluated. DISCUSSION: Previous studies have reported a positive effect of Nigella sativa supplementation in chronic kidney disease, but there is no evidence that this plant is safe in hemodialysis patients. The results of this study can be helpful in better control of blood sugar and kidney function and reduce complications in diabetic hemodialysis patients. BACKGROUND AND AIMS: The Coronavirus disease 2019 (COVID-19) pandemic globally changed the priorities of medical and surgical procedures. It has caused many healthcare systems to stop performing their routine screenings. Altering medical clinics to COVID-19 referral centers, lockdowns, and the public fear to refer to medical centers caused a significant reduction in the referral rate; especially in the elderly. This short review article highlights the transform in clinical practice during the post-COVID era and the need for future medical revolutions. METHODS: A comprehensive literature search was separately performed by both authors. The articles published between 2019 and August 2021 were included in this study and selected according to a quality appraisal method. RESULTS: We have summarized the possible corresponding changes in the diagnosis and treatment of all fields of medical care including internal medicine, surgical, and minor subcategories after the COVID-19 pandemic. We have also discussed the potential impacts of the pandemic on all these different categories and subcategories of medicine, including the outpatient setting and clinical work. We do believe that the lack of routine check-ups has led to an increase in the stage of disease in patients with a previously diagnosed problem. On the other hand, the dramatic change in the lifestyle of the population including restricted mobility and increased consumption of unhealthy food has caused metabolic syndrome and other new diseases that have not been diagnosed and properly managed. CONCLUSION: Our findings revealed the urgent need for public health awareness. It indicated the need to carry out both psychological and screening approaches in the post-COVID era to not miss patients with a chronic disease and new cases who were undiagnosed during the COVID pandemic.

A three-dimensional electrochemical reactor with Ti/SnO2-Sb/ß-PbO2 anode and granular activated carbon (3DER-GAC) particle electrodes were used for degradation of 2,4-dichlorophenol (2,4-DCP). Process modeling and optimization were performed using an orthogonal central composite design (OCCD) and genetic algorithm (GA), respectively. Ti/SnO2-Sb/ß-PbO2 anode was prepared by electrochemical deposition method and then its properties were studied by FESEM, EDX, XRD, Linear sweep voltammetry and accelerated lifetime test techniques. The results showed that lead oxide was precipitated as highly compact pyramidal clusters in the form of ß-PbO2 on the electrode surface. In addition, the prepared anode had high stability (170 h) and oxygen evolution potential (2.32 V). A robust quadratic model (p-value < 0.0001 and R2 > 0.99) was developed to predict the 2,4-DCP removal efficiency in the 3DER-GAC system. Under optimal conditions (pH = 4.98, Na2SO4 concentration = 0.07 M, current density = 35 mA cm-2, GAC amount = 25 g and reaction time = 50 min), the removal efficiency of 2,4-DCP in the 3DER-GAC system and the separate electrochemical degradation process (without GAC particle electrode) were 99.8 and 71%, respectively. At a reaction time of 80 min, the TOC removal efficiencies in the 3DER-GAC and the separate electrochemical degradation system were 100 and 57.5%, respectively. Accordingly, the energy consumed in these two systems was calculated to be 0.81 and 1.57 kWh g-1 TOC, respectively. Based on the results of LC-MS analysis, possible degradation pathways of 2,4-DCP were proposed. Trimerization and ring opening reactions were the two dominant mechanisms in 2,4-DCP degradation.

Traditional planar PbO2 anodes have been used extensively for the electrocatalytic degradation process. However, by using porous PbO2 anodes that have a three-dimensional architecture, the efficiency of the process can be significantly upgraded. In the current study, carbon felt (CF) with a highly porous structure and a conventional planar graphite sheet (G) were used as electrode substrate for PbO2 anodes. Both CF/ß-PbO2 and G/ß-PbO2 anodes were prepared by the anodic deposition method. The main properties of the electrodes were characterized by XRD, EDX-mapping, FESEM, and BET-BJH techniques. The electrocatalytic degradation of diuron using three-dimensional porous CF/ß-PbO2 anode was modeled and optimized by a rotatable central composite design. After optimizing the process, the ability of porous CF/ß-PbO2 and planar G/ß-PbO2 anodes to degrade and mineralize diuron was compared. The electrocatalytic degradation of the diuron was well described by a quadratic model (R2 > 0.99). Under optimal conditions, the kinetics of diuron removal using CF/ß-PbO2 anode was 3 times faster than the G/ß-PbO2 anode. The energy consumed for the complete mineralization of diuron using CF/ß-PbO2 anode was 2077 kWh kg-1 TOC. However, the G/ß-PbO2 anode removed only 65% of the TOC by consuming 54% more energy. The CF/ß-PbO2 had more stability (115 vs. 91 h), larger surface area (1.6287 vs. 0.8565 m2 g-1), and higher oxygen evolution potential (1.89 vs. 1.84 V) compared to the G/ß-PbO2. In the proposed pathways for diuron degradation, the aromatic ring and groups of carbonyl, dimethyl urea, and amide were the main targets for HO• radical attacks.

The present study investigates the efficiency of a three-dimensional electro-peroxone (3D/E-peroxone) reactor filled with TiO2-GAC in removing diuron from aqueous solution and in the remediation of real pesticide wastewater. The behavior of the system in terms of the effect of independent variables on diuron was investigated and optimized by RSM-CCD and ANN-GA methods. Both approaches proved to have a very good performance in the modeling of the process and determined the optimum condition of the independent variables as follows: initial pH = 10, applied current = 500 mA, supporting electrolyte = 0.07 M, ozone concentration = 10 mg L-1, and reaction time = 10 min. The 3D/E-peroxone process achieved a synergistic effect in diuron abatement and reduced significantly energy consumption, as compared to its individual components. H2O2 concentration generated in the electrolysis system was notably increased in the presence of TiO2-GAC microparticles. The BOD5/COD ratio of the real pesticide wastewater increased from 0.049 to 0.571 within 90 min treatment. Giving to the considerable enhancement of the biodegradability of the wastewater, this study strongly suggests that the 3D/E-peroxone process can be considered as a promising pretreatment step before a biological treatment process to produce intermediates which are more easily degradable by microorganisms.

The present study examined the removal of bisphenol A (BPA) and total organic carbon (TOC) from aqueous solutions by the Fe/CMK-3 as peroxymonosulfate activator used in the sono-photo-catalytic process. The synthesis of Fe/CMK-3 was carried out using the co-precipitation method, and it was characterized by FTIR, XRD, BET, EDX, and TEM. The results showed that the iron nanoparticles were uniformly embedded in the CMK-3 pores. The effect of factors affecting on the removal of BPA and TOC was evaluated by response surface methodology (RSM) with center composite design (CCD). The analysis of variance of the quadratic model showed that the model is significant (p value < .0001 and R2  > 99.4%) and can be used to optimize the removal efficiency of BPA. Optimization results showed that the highest removal efficiency of BPA (100%) and TOC (80.6%) was achieved in optimum conditions of pH 7.8, catalyst dose 0.33 g/L, PMS dose 3.35 mmol/L, BPA concentration 39.3 mg/L, and 78.5 min. In addition, statistical analysis of the data showed that, in the studied range, the initial concentration of BPA was the most influential factor, followed by pH and PMS dose. Highest catalytic stability of Fe/CMK-3 showed the potential applicability of catalyst in the treatment of BPA-containing solutions. The quenching test showed that sulfate radical was the main responsible for the removal of BPA. The decrease in IOUR value after the 75-min reaction time indicates that this process has a high ability for oxidation of the pollutant and its intermediates. Generally, the observed results suggest that the Fe-CMK-3/UV/US/PMS system can be a promising procedure for the removal of persistent pollutants such as BPA from aqueous media. PRACTITIONER POINTS: Fe/CMK-3 exhibited prominent catalytic activity and high stability for peroxymonosulfate activation. Effective degradation of bisphenol A was achieved in the Fe-CMK-3/UV/US/PMS system. The effect of five factors at five levels and their interactions during the removal of BPA was evaluated by RSM method coupled with central composite design (CCD). The analysis of variance of the quadratic model showed that the model is very significant (p value < .0001) and can be used to optimize the removal efficiency of BPA. The quenching test showed that sulfate radical was the main responsible for the removal of BPA. Reducing IOUR value after the 75-min reaction time indicates that toxicity of the solution was significantly decreased in this system.

INTRODUCTION: Due to the importance of correct interpretation of electrocardiogram (ECG) in the quality of healthcare delivery, new educational methods are increasingly being implemented and evaluated. In this regard, the purpose of this study was to compare the traditional and virtual teaching methods on the interpretation of cardiac dysrhythmia in nursing students. METHODS: The present study is a quasi-experimental research with pretest and posttest design. Sixty nursing students in the seventh semester from schools of nursing in Urmia and Khoy cities were randomly assigned to two virtual and traditional education groups. Students' knowledge was measured by a researcher-made 30-item test. RESULTS: The mean and standard deviation of the students' scores in the traditional education group was 11.20 ± 4.41 and 14.40 ± 4.62 and in the virtual group was 11.30 ± 2.74 and 18.43 ± 4.68, pre- and post-training, respectively. Paired t-test showed a significant difference between the mean score of pretest and posttest in both types of training (P < 0.001). According to the results of independent sample t-test, there were no significant differences between the two groups before the training (P > 0.05). However, in the posttraining period, there were significant differences between traditional and virtual education groups (P < 0.001). CONCLUSION: Considering the results of this study, which shows the positive effects of virtual education method on nursing students' knowledge about cardiac dysrhythmia, this method can be used as an alternative or complementary method to the traditional education.

This study aimed to compare the hematologic variables and pro-inflammatory biomarkers in urban adults living in Tabriz, Iran, facing various levels of outdoor volatile organic compounds (VOCs). Of all 219 people (212 male and 7 female), 71 were from the low traffic area and 148 were from high traffic and industrial areas. To validate the exposure levels, 93 air samples were taken to determine the target VOCs (benzene, toluene, xylenes, and styrene collectively called BTXS) concentrations in the studied areas. ANOVA and Tukey's tests were used for statistical analysis. Based on the results, significant differences were observed between the mean concentrations of BTXS with the following order of abundance: industrial > high traffic > low traffic. The Considerable decrease was observed in red blood cells (RBCs), hemoglobin, hematocrit, and eosinophils of 0.324 ( × 106/µL), 0.57 g/dL, 1.87%, and 0.17 ( × 103/µL), respectively in industrial area participants as compared to the low traffic area. However, a significant increase was observed in white blood cell count (WBC), neutrophils number, neutrophils percent, TNF-α and INF-γ of 0.88 ( × 103/µL), 0.80 ( × 103/µL), 3.53%, 34.2 ng/mL, and 40.06 ng/mL, respectively in the same groups. The comparison of low and high traffic areas showed significant differences in RBC (p = 0.034), tumor necrosis factor alpha (TNF-α) (p < 0.001), and interferon gamma (INF-γ) (p < 0.001). On the contrary, no significant difference was observed in TNF-α and INF-γ among the high traffic and industrial areas. In conclusion, the results showed that the samples from high traffic and industrial areas were regularly exposed to higher values of BTXS due to traffic and industrial pollutants as compared to the samples residing in low traffic regions. Based on the results living in both high traffic and industrial regions can increase adverse effects on hematologic parameters and pro-inflammatory cytokines.

BACKGROUND: The aim of the present study was to calculate and to assess the potential lifetime cancer risks for trihalomethanes from consuming chlorinated drinking water in Hamadan and Tuyserkan cities, western Iran in 2016-2017. STUDY DESIGN: A cross-sectional study. METHODS: Seventy-two water samples were collected from the distribution systems and from the outlet of water treatment plants (WTPs) and the experiments were carried out to determine the desired parameters. All the sampling and measurement methods were according to Standard Methods. The obtained data were analyzed using SPSS software. RESULTS: The mean concentration of total THMs in the summer and winter was 42.75 and 17.75 µg/L, respectively, below the WHO and Iranian standard. The positive correlation was observed between temperature and THMs levels. Moreover, THMs concentration in Shahid Beheshti's WTP was several times lower than in Ekbatan's WTP. Chloroform, the dominant species of THMs, was identified at different sampling points. The highest cancer risk in Hamadan was 1.4×10-5 and 4.8×10-5 for male and female, respectively; and the cancer risk was obtained to be 5.6×10-7-2.26×10-6 in Tuyserkan. CONCLUSION: The drinking water obtained from the studied area is safe in terms of THMs concentration. Nevertheless, the highest cancer risk was higher than the EPA's acceptable level of 10-6.

Breast milk is considered the best source of nutrition for all infants. However, exposure of newborns to toxic metals is of special interest due to their potential harmful effects. Thus, the primary aims of this study were to determine the concentration of toxic heavy metals including lead, mercury, cadmium, and barium in breast milk samples from Hamadan, Iran, in relation to some sociodemographic variables. A total of 100 breast milk samples were collected and their heavy metal contents were measured by inductively coupled plasma mass spectroscopy (ICP-MS). The median breast milk concentrations of Pb, Hg, and Ba were 41.9, 2.8, and 1.95 µg/L, respectively. Cd levels were < 1 µg/L in all samples. The Pb level in 94% of the samples was higher than the recommended Pb limit of < 5 µg/L in breast milk suggested by World Health Organization (WHO). Hg levels in 54% of the breast milk samples were higher than the normal mean concentration (1.7 µg/L) suggested by WHO. We found no correlation between Hg levels in breast milk and sociodemographic factors. Ba levels in all the breast milk samples were lower than the WHO's proposed health-based drinking water guideline (0.7 mg/L). Considering the results of the present study and the vulnerability of infants, along with the well-known toxicity of these metals, further studies are warranted to identify the main sources of exposure that contribute their concentration in breast milk, establish harmless intake values of toxic metals in breast milk, and develop preventive measures.

The present cross-sectional study is aimed at analyzing the breast milk of lactating mothers in Hamadan, Iran for aluminum and several minerals and trace elements. Ten governmental health care centers were utilized to facilitate collection of breast milk samples. The breast milk samples were collected at 1, 2, 6, 7, and 12 months postpartum from one hundred healthy lactating women, who delivered full-term newborns. Detection of sodium (Na), zinc (Zn), calcium (Ca), iron (Fe), copper (Cu), magnesium (Mg) and aluminum (Al) levels was conducted with the use of Inductively Coupled Plasma Mass Spectrometry (ICP-MS). This method has shown high accuracy, precision, sensitivity, and linearity for the wide range of concentrations. The accumulated data were not normally distributed; thus, the non-parametric Mann-Whitney U test was used in the statistical analysis of the results. Mean concentrations of Fe, Zn, Cu, Ca, Mg, and Na were 0.75, 1.38, 0.35, 255, 34.58, and 155.72 µg/mL, respectively. The mean level of Al, a well-known neurotoxic metal, was determined to be an alarming 0.191 µg/mL. Moreover, 95% of participants contained very harmful concentrations of Al in their milk. This study also revealed Zn deficiency in about 50% of milk samples. Further investigation is needed to elucidate sources of exposure and factors that may influence maternal and fetal exposure to aluminum.

Diazinon (DZN) is an organophosphate insecticide that has cytotoxic and pathological effects on the reproductive system. It causes a wide variety of pathological effects on the reproductive system such as testicular atrophy, disturbance in sex hormones, impaired spermatogenesis, low quality of sperm, and fertility problems. However, molecular and cellular mechanisms of its adverse effects are not well understood. General events such as testicular damage, inflammation, mitochondrial deficiency, DNA fragmentation, disintegration of sperm plasma membrane, apoptosis, and cell death are observed in DZN-exposed animals. Oxidative stress (OS) induced by reactive oxygen species may be a main mechanism, which can be associated with sperm DNA fragmentation, reduced integrity of sperm cell membrane, apoptosis, depletion of antioxidants, and subsequently poor sperm quality and male infertility. Therefore, identification of these pathways may provide valuable information regarding the mechanisms of DZN action on the male reproductive system. In this review, we aim to discuss the proposed cellular and molecular mechanisms of DZN action on male reproductive system, the importance of OS and mechanisms by which DZN induces OS and depletion of other antioxidants.
