BACKGROUND: The dengue virus is present throughout the tropics. Thrombocytopenia is one of the severe manifestations of the dengue virus. We studied the association of thrombocytopenia with serum transaminase level, leucopenia, and nonstructural protein one antigen (Ns1Ag) level. METHODS: Data were taken retrospectively from hospital records after obtaining ethical committee approval. In the study, we included 102 patients with acute febrile illness with clinical features suggestive of dengue fever (dengue Ns1Ag positive, dengue IgM positive, or both). We excluded patients with thrombocytopenia due to other causes. Patients' demographic, clinical, and laboratory parameters were collected. We also noted episodes of bleeding or the need for a platelet transfusion. We did a statistical analysis to find out the correlation between age, sex, leucopenia, transaminitis, Ns1Ag level, and thrombocytopenia and its severity. RESULTS: Multiple regression analysis was used to find thrombocytopenia predictors among aspartate transaminase (AST), alanine transaminase (ALT), Ns1Ag level, and leucopenia. AST and ALT correlated inversely with thrombocytopenia, with p-values of 0.012 and 0.027, respectively. Ns1Ag and leucopenia were not associated with thrombocytopenia, with p-values of 0.802 and 0.532, respectively (p-values significant at 0.01<= p<=0.05). CONCLUSION: Serum AST and ALT levels correlate with thrombocytopenia in dengue fever.

RESUMO

The COVID pandemic has affected the world in a drastic manner taking a toll of not only human lives but also the economy and lifestyle. Of all the population suffering, the underprivileged and vulnerable groups have faced the maximum economic burden. Within India, an ample quota of people migrates annually for elementary occupations in service, sales, building and domestic industries. Generally, they are exposed to discrimination, work-rights exploitation and job insecurity. The onset of COVID 19 has accentuated these issues in unprecedented ways. The Apex Court of the country took note of this plight and gave directions to the governments to take care of the immediate needs of the migrant workers. This article attempts to reflect the mental health concerns of the migrant workers who were temporarily sheltered at relief camps across Bengaluru city in the state of Karnataka, during the ongoing COVID pandemic. The article ends with giving recommendations.

BACKGROUND: Mild to moderate association between childhood abuse (CA) and intimate partner violence (IPV) is reported among women from community samples. CA and IPV in clinical samples have shown strong association with adult psychopathology. METHODS: We investigated the association between CA and IPV among women with mood disorders (MD), in comparison with healthy women (HW), at a tertiary mental health centre in India. Women diagnosed with mental health disorders (n=609) and healthy volunteers (n=100), between the ages of 18 to 50 years, were assessed as part of a larger study. For the purpose of this analysis, we have taken a sample of women with MD (n=251) this includes 121 women with unipolar depression, 130 with Bipolar disorder and HW (n=72), with intimate partners. RESULTS: Incidences of childhood emotional abuse (χ2 =4.200, p=<0.05) and IPV [Severe combine abuse (t=3.924, p<0.01), Emotional abuse (t=3.895, p<0.01), Physical abuse (t=2.333, p<0.05)] were higher in women with MD as compared to HW. We noted a positive correlation between CA and IPV, in women with MD. And also CA came out as a predictor for IPV in later life among women with MD on regression analysis. LIMITATIONS: No information about the total number of depressive and manic episodes, and also the mood ratings at the entry point. CONCLUSIONS: CA and IPV have an additive effect which might lead to the expression of severe mental disorders like MD and these factors might also have an impact on course and outcome of mood disorder which needs further studies.

RESUMO

Homelessness is considered a serious public health concern. Bidirectional relationship has been established between homelessness and mental illness by research studies and its association with compromised well-being, poor quality of life and low productivity. Recent legislative enactments in India have necessitated on the part of the state to address issues related to the rights of persons with mental illness including shelter and housing. Therefore, it becomes imperative to discuss opportunities and prospects in India towards rehabilitating homeless mentally ill in context of existing programs, policies and legislations.

The present study was conducted to ascertain the effectiveness of Project ECHO, a Hub and Spokes tele-mentoring model to bridge the urban-rural divide in mental health and addiction care in the context of a developing country like India. The Counsellors from 11 rural and underserved districts of Chhattisgarh were periodically connected to NIMHANS multidisciplinary specialists by smartphone app and underwent virtual mentoring to learn and translate "best practices" in Mental health and Addiction by using "patient-centric learning", a core component of NIMHANS ECHO model. The outcome evaluation was modelled on Moore's evaluation framework focusing on participant engagement, satisfaction, learning, competence and performance. Over the period of 6 months i.e. 12 tele-ECHO clinics, 41 patients case summaries were discussed by the Counsellors with NIMHANS Hub Specialists. Half of the counsellors could join >80% clinics and overall there were no drop-outs. There was a significant increase in learning and self-confidence after six months. The participants liked "relevance of the courses to clinical practices". "group based discussions" and "a reduction in professionals isolation". The results indicate promise of the NIMHANS ECHO tele-mentoring model as one with potential for capacity-building in mental health and addiction for remote and rural areas by leveraging technology.
