RESUMO

Cervical-spine sensorimotor control is associated with chronicity and recurrence of neck pain (NP). Tests used to measure sensorimotor impairments lack consistency in studied parameters. Interpretation is often based on either a handful or numerous parameters, without considering their possible interrelation. Different aspects of motor-control could be studied with different parameters, but this has not yet been addressed. The aim of this study was to determine if different parameters of cervical position (JPE) and movement (Butterfly) sense tests represent distinct components of motor-control strategies in patients with chronic NP. Principal component analysis performed on 135 patients revealed three direction-specific (repositioning from flexion, extension or rotations) and one parameter-specific (variability of repositioning) component for JPE, two difficulty-specific (easy or medium and difficult trajectory) and one movement-specific (undershooting a target) component for Butterfly test. Here we report that these components could be related to central (neck repositioning and control of cervical movement) and peripheral sensorimotor adaptations (variability of repositioning) present in NP. New technologies allow extraction of greater number of parameters of which hand-picking could lead to information loss. This study adds towards better identification of diverse groups of parameters offering potentially clinically relevant information and improved functional diagnostics for patients with NP.

Despite commonly investigated predictable smooth-pursuit neck-torsion tasks (SPNT) in neck pain patients, unpredictable conditions have been seldom investigated but are indicative of preserved oculomotor functions during neck torsion. Although not previously studied, some speculations about compensatory cognitive mechanisms such as increased phasic alertness during unpredictable tasks were suggested. The aim of this study was to investigate eye movement accuracy and pupillometric responses during predictable and unpredictable SPNT test in neck pain patients and asymptomatic controls. Eye movements (gain and SPNT-difference) and pupillometry indicative of tonic (average and relative pupil diameter) and phasic (index of cognitive activity-ICA) alertness were measured in 28 idiopathic neck pain patients and 30 asymptomatic individuals using infrared video-oculography during predictable and unpredictable SPNT test. Gain in unpredictable SPNT test was lower as compared to predictable tasks and presented with similar levels in neutral and neck torsion positions, but not in the predictable SPNT test. ICA was lower during neutral position in all tasks in patients as compared to control group but increased during neck torsion positions in unpredictable tasks. Relative pupil diameters presented with no differences between the groups or neck positions, but the opposite was observed for average pupil diameter. Higher ICA indicates an increase in phasic alertness in neck pain patients despite no alterations in oculomotor control during SPNT test. This is the first study to indicate cognitive deficits in oculomotor task in neck pain patients. The latter could negatively affect other tasks where additional cognitive resources must be involved.

The sensory mismatch commonly observed in patients with neck pain disorders could alter intra-trial reliability in simple implicit smooth pursuit eye movement tasks. This could be more pronounced when neck is in torsioned position (SPNT). The aim of this study was to explore the effects of neck torsion, target movement velocity and amplitude on intra-trial reliability of smooth pursuit eye movements in patients with neck pain disorders and healthy individuals. SPNT test was evaluated in 32 chronic neck pain patients and 32 healthy controls. Ten cycles were performed using video-oculography at three different velocities (20° s-1, 30° s-1 and 40° s-1) and at three different amplitudes (30°, 40° and 50°) of target movement. Intra-trial reliability and differences between average gain and SPNT difference from the second to fifth cycle and from the sixth to ninth cycle were assessed using ICC3.1 and factorial analysis of variance, respectively. Intra-trial reliability for gain and SPNT difference at all target movement amplitudes and velocities proved to be good to excellent in both observed groups. Patients with neck pain disorders presented with a trend of inferior gain performance between the sixth and ninth cycle at 30° s-1 of target movement as compared to healthy individuals which was only evident when neck was in torsioned position. Although intra-trial reliability of smooth pursuit neck torsion test is good to excellent, the effects of learning are not as pronounced in patients with neck pain disorders.

Analyzing visual search strategies in tennis is primarily focused on studying relationships between visual behavior and tennis performance. However, diverse movement characteristics among different servers suggest the importance of adjusting the visual search strategies of an individual while playing against different opponents. The aim of this study was to analyze whether visual search strategies can be attributed to the individual server and the returning player during the tennis serve return or return performance. Seventeen tennis players were enrolled in this study (five international players and 12 national players) producing a sample of 1,020 returns measured with mobile eye trackers. The random forest machine learning model was used to analyze the ability to classify the returning player [area under the curve (AUC): 0.953], individual server (AUC: 0.686), and return performance category (AUC: 0.667) based on the location and duration of the focal vision fixation. In international tennis players, the higher predictability of the server was observed as compared with national level players (AUC: 0.901 and 0.834, respectively). More experienced tennis players presented with a higher ability to adjust their visual search strategies to different servers. International players also demonstrated anticipatory visual behavior during the tossing hand movement and superior information pickup during the final phases of the stroke of a server.

BACKGROUND: Research indicates that neck kinaesthetic awareness plays an important role in oculomotor and balance control, however the relationship has not been studied in athletes. As performance in ice hockey demands visual perception acuity during skating, while constantly shifting between unilateral and bilateral stances in sports specific posture more in-depth relationship should be studied. RESEARCH QUESTION: What is the relationship between neck kinaesthetic awareness, postural balance and eye movement control in professional ice hockey players and non-trained individuals? METHODS: In this observational study, centre-of-pressure was measured using force plate in twenty-eight hockey players and thirty non-trained participants during different stances in eyes-opened and eyes-closed conditions. Butterfly test and Head-to-Neutral Relocation test were performed to assess neck kinaesthesia. Horizontal smooth pursuit eye movements were measured using video-oculography. Linear regression was used to determine the relationship between neck kinaesthesia, body sway and oculomotor control. RESULTS: Time-on-target in Butterfly test was able to predict low to medium proportions of variance in amplitude and velocity parameters for single leg stances in hockey players (R2 = .220-.698). Head-to-Neutral Relocation test was able to predict low to medium proportion of variance in average eye movement velocity during first 100 milliseconds of smooth pursuit initiation for both groups (R2 = .262-.541). SIGNIFICANCE: Findings from our study suggest that cervical spine afferent input plays an important role in maintaining unilateral postural balance in hockey players, with less evidence presented in controls. Sports specific posture or upper body loading could lead to adaptations in neck proprioception, not frequently considered when searching for balance related injury risk factors or performance deficits. Our study suggests, that in addition to balance control, neck kinaesthesia can also affect oculomotor performance which was present in both groups. This is especially evident when initiating changes in eye movement direction.

Reports on body sway control following microdiscectomy lack reports on side-specific balance deficits as well as the effects of trunk balance control deficits on body sway during upright stances. About 3 weeks post microdiscectomy, the body sway of 27 patients and 25 controls was measured while standing in an upright quiet stance with feet positioned parallel on an unstable support surface, a tandem stance with the involved leg positioned in front or at the back, a single-leg stance with both legs, and sitting on an unstable surface. Velocity, average amplitude, and frequency-direction-specific parameters were analyzed from the center of pressure movement, measured by the force plate. Statistically significant differences between the 2 groups were observed for the medial-lateral body sway frequency in parallel stance on a stable and unstable support surface and for the sitting balance task in medial-lateral body sway parameters. Medium to high correlations were observed between body sway during sitting and the parallel stance, as well as between the tandem and single-legged stances. Following microdiscectomy, deficits in postural balance were side specific, as expected by the nature of the pathology. In addition, the results of this study confirmed the connection between proximal balance control deficits and balance during upright quiet balance tasks.

Bed rest has been shown to have detrimental effects on structural and functional characteristics of the trunk muscles, possibly affecting trunk and spinal stability. This is especially important in populations such as aging adults with often altered trunk stabilizing functions. This study examined the effects of a fourteen-day bed rest on anticipatory postural adjustments and postural reflex responses of the abdominal wall and back muscles in sixteen adult men. Postural activation of trunk muscles was measured using voluntary quick arm movement and sudden arm loading paradigm. Measurements were conducted prior to the bed rest, immediately after, and fourteen days after the bed rest. Immediately after the bed rest, latencies of anticipatory postural adjustments showed significant shortening, especially for the obliquus internus and externus muscles. After a fourteen-day recuperation period, anticipatory postural adjustments reached a near to complete recovery. On the contrary, reactive response latencies increased from pre-bed-rest to both post-bed-rest measurement sessions. Results indicate an important effect of bed rest on stabilizing functions of the trunk muscles in elderly adults. Moreover, there proved to be a significant deterioration of postural reactive responses that outlasted the 14-day post-bed-rest rehabilitation.

The purpose of this study was to use a holistic approach to investigate changes in jumping performance, kinaesthesia, static balance, isometric strength and fast stepping on spot during a 5-day recovery period, following an acute bout of damaging exercise consisted of drop jumps and leg curls, where specific emphasis was given on the hamstring muscles. Eleven young healthy subjects completed a series of highly intensive damaging exercises for their hamstring muscles. Prior to the exercise, and during the 5-day recovery period, the subjects were tested for biochemical markers (creatine kinase, aspartate aminotransferase, and lactate dehydrogenase), perceived pain sensation, physical performance (squat jump, counter movement jump, maximal frequency leg stamping, maximal isometric torque production and maximally explosive isometric torque production), kinaesthesia (active torque tracking) and static balance. We observed significant decreases in maximal isometric knee flexion torque production, the rate of torque production, and majority of the parameters for vertical jump performance. No alterations were found in kinaesthesia, static balance and fast stepping on spot. The highest drop in performance and increase in perceived pain sensation generally occurred 24 or 48 hours after the exercise. Damaging exercise substantially alters the neuromuscular functions of the hamstring muscles, which is specifically relevant for sports and rehabilitation experts, as the hamstrings are often stretched to significant lengths, in particular when the knee is extended and hip flexed. These findings are practically important for recovery after high-intensity trainings for hamstring muscles. This study explored the effects of a 14-day horizontal bed rest (BR) without countermeasures on postural sway, maximal voluntary torque and precision of voluntary torque matching. Sixteen subjects were tested before, immediately after and two weeks after BR. The increase in frequency and amplitude after BR was comparable for both sway subcomponents (rambling and trembling) in medial-lateral direction. But in anterior-posterior direction, rambling increased more in frequency (-7% vs. +31%, p < 0.05) while trembling increased more in amplitude (+35% vs. +84%, p < 0.05). The drop in maximal voluntary torque after BR was present for plantar flexion (p < 0.05) but not for dorsal flexion. After the BR, the subjects were less precise in the dorsal flexion torque matching task (p < 0.05). All the observed parameters, except the dorsal flexion torque matching error, returned back to the pre-BR values after the two weeks of re-conditioning. Results of this study indicate that body sway subcomponents responded differently to BR. Based on these findings, it was not possible to draw clear assumptions on the effects of neural and structural changes on body sway.

Literature confirms the effects of vision and stance on body sway and indicates possible interactions between the two. However, no attempts have been made to systematically compare the effect of vision on the different types of stance which are frequently used in clinical and research practice. The biomechanical changes that occur after changing shape and size of the support surface suggest possible sensory re-weighting might take place. The purpose of this study was to assess the effect of vision on body sway in relation to different stance configurations and width. Thirty-eight volunteers performed four quiet stance configurations (parallel, semi-tandem, tandem and single leg), repeating them with open and closed eyes. Traditional parameters, recurrence quantification analysis and sample entropy were analyzed from the CoP trajectory signal. Traditional and recurrence quantification analysis parameters were affected by vision removal and stance type. Exceptions were frequency of oscillation, entropy and trapping time. The most prominent effect of vision elimination on traditional parameters was observed for narrower stances. A significant interaction effect between vision removal and stance type was present for most of the parameters observed (p<0.05). The interaction effect between medio-lateral and antero-posterior traditional parameters differed in linearity between stances. The results confirm the effect of vision removal on the body sway. However, for the medio-lateral traditional parameters, the effects did not increase linearly with the change in width and stance type. This suggests that removal of vision could be more effectively compensated by other sensory systems in semi-tandem stance, tandem and single legged stance.

Body sway is usually described by center of foot pressure (COP)-derived parameters. Their sensitivity to vertical center of mass (COM) redistribution below its natural position has not yet been examined during quiet stance tasks. We examined the effects of both lowering and raising the COM on the following body sway parameters: cumulative, medial-lateral and anterior-posterior COP average velocity, amplitude and frequency. For this purpose, 13 healthy male subjects performed a quiet stance balance task with feet positioned in parallel stance (PS) at hip width apart and with hands holding a stick across the rear part of the shoulders. Each subject carried out five different modifications of the PS task in a randomized order: no additional load, an additional load of 10 kg and 30 kg suspended from the waist at mid-lower leg height, and an additional load of 10 kg and 30 kg across the rear of the shoulders. The studied body sway parameters proved to be sensitive to these manipulations. Specifically, lowering and raising the COM was mirrored in a systematic decrease/increase of the velocity, amplitude, and frequency parameters, indicating a larger effect in the anterior-posterior direction. These results suggest that the elevation of the body COM from a lower to a higher position systematically decreases the postural control during quiet standing, and consequently, increases the intensity of the balancing task. Thus, this type of physical manipulation could provide the basis for a simple progression in functional resistance training for persons with compromised balance.
