RESUMO

In this paper, we report ecological and environmental investigations on Pteris vittata in the As-Pb-Hg-polluted Bone River area, Gorontalo Province, Indonesia. The density distribution of P. vittata decreases from around the artisanal and small-scale gold mining (ASGM) site to the lower reaches of the Bone River, and it is rarely found near Gorontalo City. The maximum concentrations of As, Hg, and Pb recorded in the soil samples were 401, 36, and 159 mg kg-1, respectively, with their maximum concentrations in P. vittata recorded as 17,700, 5.2, and 39 mg kg-1, respectively. Around the ASGM sites, the concentrations of As, Pb, and Hg in P. vittata were highest in the study area. These data suggest that P. vittata, a hyperaccumulator of As, may be useful as a bioindicator for assessing environmental pollution by Pb and Hg.

We conducted this study to evaluate the effect of radiofrequency (RF) stimulation with suture repair on the healing of tears in the meniscal white-white zone. Fifty-four New Zealand white rabbits underwent surgically induced meniscal injuries within the white-white region. RF was applied using a 0.8-mm TOPAZ MicroDebrider RF wand (ArthroCare) at level 4 for 500 milliseconds. Rabbits were sacrificed at 28 and 84 days for gross and histologic analysis by 3 blinded observers and at 9, 28, and 84 days for biochemical examination. Biochemical analyses included evaluation of cell proliferation (3H-thymidine), as well as mitogenic (IGF-1, bFGF) and angiogenic (VEGF, αV) factors. Of specimens repaired with RF combined with suture, 19 (58%) showed a degree of gross morphologic and histologic healing. No significant healing was seen in specimens with either no repair or repair with suture alone. We observed a 40% increase in cellular proliferation when RF supplementation was used (P<.05). With regards to mitogenic and angiogenic markers (IGF-1, bFGF, VEGF, and αV), there was a significant increase in groups treated with RF at 9 and 28 days (P>0.05). RF supplementation of avascular zone meniscal repairs may lead to an increased healing response.

This study was conducted to evaluate the phytoremediation and phytomining potential of 10 plant species growing naturally at one of the largest lead-zinc mines in Northern Vietnam. Total concentrations of heavy metals and arsenic were determined in the plant and in associated soil and water in and outside of the mine area. The results indicate that hyperaccumulation levels (mg kg(-1) dry weight) were obtained in Houttuynia cordata Thunb. (1140) and Pteris vittata L. (3750) for arsenic, and in Ageratum houstonianum Mill. (1130), Potamogeton oxyphyllus Miq. (4210), and P. vittata (1020) for lead. To the best of our knowledge, the present paper is the first report on metal accumulation and hyperaccumulation by H. cordata, A. houstonianum, and P. oxyphyllus. Based on the obtained concentrations of metals, bioconcentration and translocation factors, as well as the biomass of these plants, the two latter species and P. vittata are good candidates for phytoremediation of sites contaminated with arsenic and multi-metals. None of the collected plants was suitable for phytomining, given their low concentrations of useful metals (e.g., silver, gallium, and indium).

Eleocharis acicularis was exposed to different concentrations of In, Ag, Pb, Cu, Cd, and Zn in the laboratory to assess its capability in accumulating these metals. After 15 days, 477 mg/kg dry wt. of In was accumulated by the roots; concentrations of Ag, Pb, Cu, Cd, and Zn in the shoots were 326, 1120, 575, 195, and 213 mg/kg dry wt., respectively. The results indicate that E. acicularis has the ability to accumulate these metals from water, making it a good candidate species for phytoremediation and phytomining.

This retrospective study includes 6 patients (average age, 8.7 years) with a dislocation of the radial head and ulnar plastic deformation. All were Monteggia fractures, Bado type I equivalents. The maximum ulnar bow was near the midulna. Five patients underwent an ulnar osteotomy, with elongation and reduction of the angulation within the middle third of the ulna, and open reduction of the radial head. One patient underwent an ulnar osteotomy with only elongation. The osteotomy sites were stabilized by a plate and screws or Kirschner wires. Mean follow-up was 3.4 years. Postoperatively, the average elbow range of motion was extension to 0 degrees, flexion to 138 degrees, forearm supination to 90 degrees, and forearm pronation to 88 degrees. Results in all patients were rated as excellent. One nonunion occurred. An osteotomy performed within the middle third of the ulna, combined with open reduction of the radial head, resulted in excellent clinical outcomes.

RESUMO

OBJECTIVE: The purpose of this study was to further our understanding of the normal appearance of the popliteomeniscal fasciculi (PMF) on MRI after the determination of finely tuned imaging parameters. For this purpose we performed the study in two stages. Stage I was to determine suitable parameters for depicting the popliteomeniscal fasciculi. Stage II was to classify the "normal" image. CONCLUSION: The findings presented in this article will contribute to the understanding of the normal appearance of the popliteomeniscal fasciculi on MRI, and of the degree of variation of this structure among the population.

To investigate the mechano-transduction system of cells, we identified genes responsive to a cyclic mechanical stimulus. MC3T3.E1 cells were cultured on a computer-controlled vacuum-pump-operated device designed to provide a cyclic mechanical stimulus. A maximum elongation of 15% of membrane at 10 cycles/min (3 s extension followed by 3 s relax per cycle) was repeated for 48 h. By means of a differential display, the gene expression pattern of cells exposed to the stimulus was compared with that of unexposed cells. As a result, a gene fragment that was exclusively expressed in mechanically stressed cells was identified. By using expressed sequence tag walking together with the oligo-capping method, this gene was identified as phosphatidylinositol 4-phosphate 5-kinase type II beta (initially known as Pip5k2beta but now reclassified as Pip4k2beta). The specific up-regulation of Pip4k2beta upon mechanical stimulus was also confirmed by using another apparatus, viz. a computer-controlled linearized-stepping motor system. To examine the involvement of the cyclic mechanical stimulus in the regulation of Pip4k2beta expression in musculoskeletal tissue, we created an Achilles tendon transection model in rabbits. The temporal expression of Pip4k2beta was assessed by means of a quantitative reverse-transcribed polymerase chain reaction. In the gastrocnemius muscle, expression of Pip4k2beta rapidly decreased 1 week after transection but was restored to normal levels at 4 weeks. In the Achilles tendon, however, expression remained decreased until 4 weeks after transection. We suggest that the expression of Pip4k2beta can be used as a marker for cells receiving a suitable mechanical stimulus.

From 1993 to 2003, 6 patients with displaced fractures of the humeral capitellum were treated by open reduction and internal fixation of the capitellar fragments with Herbert bone screws. By use of the criteria of Grantham et al, there were 2 type II-A fractures, 1 type II-B fracture, 1 type II-C fracture, and 2 type III-A fractures. A lateral approach was used in 4 patients and a posterior approach with olecranon osteotomy in 2. The elbows were immobilized postoperatively for 4 to 28 days (mean, 13.5 days). We evaluated the range of motion, stability, and pain using the criteria of Grantham et al. The follow-up period ranged from 2.5 to 9.3 years (mean, 5.6 years). All patients had a stable, pain-free elbow with good range of motion at follow-up. All fractures healed, and there was no evidence of avascular necrosis or degenerative change.

PURPOSE: To evaluate the clinical outcomes of a new arthroscopic procedure, arthroscopic posteromedial release (PMR), and its potential use as a treatment option for medial-type osteoarthritic (OA) knees. TYPE OF STUDY: Retrospective analysis of clinical outcomes of a case series. METHODS: Knees with medial-type OA and flexion contracture were treated with PMR. They were classified using the Kellgren and Lawrence (K/L) radiographic grading system and classified using magnetic resonance imaging (MRI) into smooth (S) or irregular (IR) groups, based on the subchondral contour of the medial femoral condyle. Clinical outcome was evaluated using the Japanese Orthopaedic Association knee score (JOA score), verbal rating scale (VRS), and patient satisfaction. RESULTS: Fifty-two patients with 58 OA knees were included in the study. The mean age of the patients at the time of surgery was 71.6 years, the average ROM was from 13 degrees to 129 degrees , and the average follow-up period was 3.3 years. Most of the knees were classified as K/L grade III or IV. Overall, the average JOA score improved to 71.6 points from 56.3 points preoperatively. VRS scores decreased in most patients, and 76% of patients were satisfied at their last follow-up. The JOA score of the K/L grade III knees improved to 76.9 from 60.4 points preoperatively and that of the K/L grade IV knees improved to 69.5 from 55.3 points. The improvement in JOA score was less for the IR group, from 54.5 to 66.2 points, than for the S group, from 62.3 to 79.6 points. Five knees from the IR group and 1 from the S group were converted to total knee arthroplasty. CONCLUSIONS: Knees with relatively advanced OA, for which arthroscopic debridement has conventionally been contraindicated, can be treated with PMR if they are selected properly based on MRI findings.
