RESUMO

The present study systematically reviewed the literature regarding the bond strength durability of zirconia ceramics to resin-based luting cements after application of different bonding protocols and aging conditions. Electronic searches in PubMed, Scopus, and Web of Science databases were performed for relevant literature published between January 1st 2015 and November 15th 2022. Ninety-three (93) English language in-vitro studies were included. The percentage of the mean bond strength change was recorded prior to and after artificial aging, and the weighted mean values and 95% confidence intervals were calculated. Bonding protocols were classified based on the combination of MDP/non-MDP containing cement/primer and surface pretreatment, as well as the level of artificial aging performed. Alumina sandblasting (SA) was identified as the most frequently used surface pre-treatment while an insufficient number of studies was identified for each category of alternative surface treatments. The combination of MDP cement with tribochemical silica coating (TSC) or SA yielded more durable results after aging, while the application of SA and TSC improved bond durability when a non-MDP cement and a non-MDP primer were used. TSC may lead to increased bond durability compared to SA, whereas MDP cements may act similarly when combined with SA or TSC.

STATEMENT OF PROBLEM: Screwmentable prostheses were developed to combine the benefits of screw retention and cement retention. However, data are limited on the clinical performance of this type of prosthesis. PURPOSE: The purpose of this systematic review was to collect scientific evidence on screwmentable prostheses and evaluate their long-term clinical behavior. MATERIAL AND METHODS: An electronic search was conducted by 2 independent reviewers for articles published in scientific dental journals in English from 2004 to April 2020. The search strategy followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. Inclusion criteria were scientific studies concerning the screwmentable type of prosthesis. RESULTS: The search provided 494 records. Of these, 24 studies fulfilled the inclusion criteria and were included in the review. The included articles presented significant heterogeneity concerning the manufacturing process and the materials used. One randomized clinical trial, 2 prospective clinical studies, 14 in vitro studies, 3 protocol descriptions, 1 case series, and 3 case reports were included. CONCLUSIONS: Based on the systematic search of the literature, it is concluded that the screwmentable prosthesis combines advantages of both cement-retained and screw-retained restorations, including passive fit, retrievability, excess cement control, tissue-friendly emergence profile, and improved esthetics. Nevertheless, data from well-designed clinical trials are limited, and further research is required to provide evidence on their long-term clinical behavior.

Advances of digital technology are rapidly adopted in dental practice. This systematic review aimed to collect evidence on the accuracy of fit of different types of fixed dental prostheses (FDPs) fabricated through digital, conventional, or combination impression techniques. Data collection was based on the guidelines of the preferred reporting items for systematic reviews and meta-analyses (PRISMA). Two databases (PubMed, Scopus) were searched for articles in English published between 2010 and 2021 resulting in 480 articles. Of those, 35 studies fulfilled the inclusion criteria. These articles referred to three groups of materials/techniques including all-ceramic (zirconia; lithium disilicate) and porcelain-fused-to-metal (PFM) restorations. Results showed clinically acceptable marginal fit (< 120 µm) for all materials and impression techniques. Α fully digital workflow appears more promising for the construction of short-span zirconia FDPs. Nevertheless, most articles evaluated marginal/internal fit of single crowns or short-span FDPs in vitro, while clinical data are limited for long-span FDPs. The necessity for gingival retraction remains a major drawback of all impression techniques, increasing procedural time and patient discomfort. Besides, factors related to the fabrication process, including milling and 3D printing of working models significantly influence the outcome. Overall, there still some way to go before digital technology can be incorporated in complex treatment plans in prosthodontics.

Non-steroidal, anti-inflammatory drugs and statins are two widely prescribed drug classes that affect bone formation. The aim of this study was to elucidate the effect of diclofenac and simvastatin in artificial bone defect healing. One hundred and forty-four male Wistar rats were used, and the specimens were divided into groups, with respect to the route of drug administration and the type of defect healing (with or without collagen membrane), and subgroups, with respect to the study duration (2, 4 or 8 weeks). Diclofenac was intramuscularly administered while simvastatin was administered both systemically and locally. Animals were euthanized and specimens were histomorphometrically analyzed to evaluate the percentage of new bone formation (%). Bone healing that occurred without any intervention developed more steadily than that of all other groups. Diclofenac exerted a clear, direct inhibitory effect on bone healing and its systemic administration should be avoided. The systemic administration of simvastatin was related to severe myopathy, while the solvent for the local administration of simvastatin seemed to play significant role in bone growth, as simvastatin, when it is administered intraperitoneally in a DMSO solution, appeared to promote bone healing. Local administration may have a significant impact on bone healing and it should be further investigated with the type of solvent or carrier that is used, which both may play a significant role in bone repair induction.

PURPOSE: The aim of this systematic review was to identify studies with a minimum of 5-years follow-up, reporting on the management of periodontally compromised teeth with either extraction and subsequent implant placement or teeth preservation with conventional periodontal treatment and application of regenerative procedures. The outcomes of these two approaches, based on clinical and radiographic data and the incidence of tooth- and implant-loss, were also investigated. MATERIAL AND METHODS: A systematic search for studies reporting on clinical and radiographic outcomes of periodontal treatment or replacement of periodontally compromised teeth with implants was conducted in 3 electronic databases, followed by a hand-search in 8 journals. Only randomized controlled trials (RCTs), cohort studies, and case series with prospective design were included. RESULTS: The initial search resulted in 1080 papers. After the first two screenings, 24 publications were selected for inclusion in this systematic review. The treatment protocols for the teeth preservation group contained nonsurgical and/or surgical periodontal treatment with or without regeneration procedures. The implant studies included extraction of periodontally involved teeth and implant placement with or without bone and soft tissue augmentation, followed by restoration with fixed dental prostheses (FDPs). Survival rates ranged between 81.8% and 100% in the tooth retention group, and between 94.8% and 100% in the implant group. In the extraction group, no complications were reported for 76.09% of the implants. Similarly, no complications were reported for 86.83% of the tooth retention group. The lack of standardized comparable studies prohibited conduction of a metaanalysis. CONCLUSION: Both treatment approaches, treatment of periodontally compromised teeth, or tooth extraction followed by implant placement, present high survival rates. The application of bone regeneration techniques improves the long-term prognosis of periodontally involved teeth. Hence, treatment of periodontally involved teeth with subsequent application of a rigorous maintenance protocol can be a viable alternative for a number of years, before proceeding to extraction and replacement with dental implants. More well-designed randomized controlled trials are needed in order to draw definite conclusions on the subject.

PURPOSE: The investigation of zirconia core surface pretreatments on the flexural strength of bilayered zirconia ceramics before and after artificial accelerating aging. METHODS: Ninety bar-shaped specimens were manufactured from Yttria Stabilized Tetragonal Zirconia Polycrystal (Y-TZP) and divided in three groups depending on zirconia surface pretreatment before veneering: layering with liner, pretreatment with silane-containing gas flame (SGF) with the Silano-Pen device and alumina air-abrasion. Half of the veneered specimens in each group (n=15) underwent artificial accelerating aging. A 4-point bending test was performed to determine flexural strength. Three specimens from each group were further analyzed using Scanning Electron Microscopy (SEM), X-ray Diffraction (XRD) and Fourier Transform Infrared Spectroscopy (FTIR) before veneering and after fracture (aged and non-aged subgroups). RESULTS: Alumina air-abrasion was correlated to increased phase transformation from tetragonal to monoclinic zirconia phase. Qualitative analysis revealed that with the majority of the specimens pretreated with the silane-containing gas flame, areas of the veneering material remained firmly attached to the zirconia core after flexural strength testing. There was no statistically significant difference on the flexural strength among the groups before or after aging. Artificial accelerating aging resulted in statistically significant higher flexural strength of the specimens after aging. CONCLUSION: SGF pretreatment can be an acceptable and feasible alternative method before the veneering of Y-TZP zirconia as it presented slightly higher bond strength compared with alumina air-abrasion which was associated with higher tetragonal to monoclinic (t→m) phase transformation. Accelerating aging leads to an increase of the mechanical properties under in vitro conditions.

Aim of this in vitro study was to investigate the effect of artificial ageing and differential abutment support on the load-bearing capacity of zirconia posterior four-unit fixed dental prostheses (FDPs). Thirty-six FDPs were fabricated using CAD/CAM technology and divided into three groups. Specimens in the first group were cemented onto tooth analogues with simulated periodontal resilience, in the second group onto a dental implant and a tooth analogue, but in the third group only onto implants. Half of the samples in each group underwent artificial ageing. Afterwards, all FDPs were loaded until bulk fracture in a universal testing machine. Load-displacement curves and forces at fracture were recorded and results were statistically analysed using ANOVA. Load-bearing capacities within the different test groups averaged as follows (control/artificially aged): tooth-tooth supported (2,009/1,751 N), tooth-implant supported (2,144/1,935 N) and implant-implant supported (2,689/2,484 N). Artificial ageing as well as differential abutment support did have a significant influence on the fracture strength of the zirconia FDPs. Implant-retained prostheses demonstrated the highest load-bearing capacity, while resilient support was demonstrated to be unfavourable. According to these in vitro results, zirconia four-unit prostheses may be promising for application in posterior areas with all three support scenarios (implant-assisted, tooth-retained, or implant-tooth-interconnected prostheses). However, the restorations' mechanical strength may expected to be significantly influenced in situ by ageing of the material on the long term.

AIM: The aim of this in vitro study was to investigate the influence of heterogeneous abutment supports on the load-bearing capacity of zirconia posterior four-unit fixed dental prostheses. METHODS: A total of 18 specimens were fabricated and divided into 3 groups. Specimens in the first group were supported by teeth with simulated periodontal resilience, in the second group by the combination of a tooth with simulated periodontal resilience and an implant, whereas in the last group by implants only. The bridgeworks were loaded in a universal testing machine until fracture, and the load-bearing capacity was recorded. Statistical analysis was performed by one-way analysis of variance, with the level of significance set at 0.05. RESULTS: Implant-supported zirconia restorations exhibited the highest loads at fracture (2689.3 N), followed by combined restorations (2144.2 N) and tooth-retained prostheses (2009.4 N). Statistical analysis revealed a significant influence of abutment support on the load-bearing capacity (P = 0.014). Visual inspection of the tested specimens showed different fracture patterns in the 3 groups. CONCLUSION: For all support scenarios, the recorded loads were much higher than maximum natural bite forces, so that all the tested prosthesis designs could be recommended for application in posterior areas. The long-term prognosis of such restorations should however be further evaluated within in vivo studies to recommend their use in the clinic.
