RESUMO

We report the case of a 65-year-old man who presented with a right testicular mass and synchronous involvement of skin and Waldeyer's ring. These facts led us to the working diagnosis of malignant primary testicular lymphoma. MATERIAL/RESULTS: We present the case with comments and make a bibliographic review of the disease. CONCLUSIONS: Primary testicular lymphoma is an uncommon testicular tumour that accounts for not more that 9% of all testicular tumours in the series with higher incidence. Testicular lymphomas are also rare among haematopoietic tumours, accounting for just 1% of all lymphomas, but due to their highly malignant histopathology they may become highly aggressive tumours. Patient age at presentation is over 60 years which makes it the most frequent tumour for this age group. 70% of recently diagnosed patients show Ann Arbor stages I and II. Tumours in advanced-stage have a predilection for spreading to extranodal sites such as central nervous system, skin, Waldeyer's ring and lungs.

Acute bacterial prostatitis (ABP) (NIH Category I), has not undergone any modification in the update of prostatitis classification. ABP was diagnosed in 614 patients in our centre over 9 years (1993-2001). We analyse the clinical pattern of ABP and the role of bladder outlet obstruction in its etiology, as well as whether two different ABP sub-categories could be defined as a function of a history of previous manipulation of the lower urinary tract. The results of the study show that the clinical pattern of a patient suffering from ABP does not differ from the statements of previous publications. On the other hand, patients with ABP have been shown to present with no bladder outlet obstruction. Finally, this study has disclosed the fact that the cases of ABP elicited by previous manipulation of the lower urinary tract (10%) show a different pattern from those cases where no previous manipulation has occurred (90%). The patients with ABP secondary to manipulation are older, have a higher risk of prostate abscess and higher frequency of multiple infections and also infections by pathogens other than Escherichia coli. Due to all of these reasons, it would be advisable to subdivide category I within the classification of prostatitis.

OBJECTIVES: To assess the feasibility and a validity of the Spanish version of the FACT-P and its relation to urinary symptoms. METHODS: The Spanish version of the FACT-P was self-administered to 60 Prostate Cancer patients (pts) waiting to undergo radical treatment and 20 age-matched controls (con) with a negative biopsy. Clinical stage for patients was T1c in 34 and T2 in 26. FACT-P was analysed for internal consistency (Chronbach's a) and relation to symptom scales (IPSS, IPSS bother score and incontinence questionnaire (ICI-SF)). RESULTS: Feasibility (evaluable questionnaires) was 80.9 for con and 95% for pts. Internal consistency was high for all sub-scales (alpha > 0.7). Emotional well-being subscale and IPSS were significantly higher in pts. (worse emotional wellbeing and more lower urinary tract symptoms). There were no statistically significant differences between pts and con. in any of the other FACT-P sub-scores nor in symptom scores. Correlations were found between some FACT-P sub-scores and bother score. CONCLUSIONS: The Spanish version of the FACTP is feasible and psychometrically valid. Pts undergoing treatment for localized disease have similar QoL that a control group, but higher scores for emotional distress and IPSS.
