RESUMO

This paper presents an artificial intelligence-based classification model for the detection of pulmonary embolism in computed tomography angiography. The proposed model, developed from public data and validated on a large dataset from a tertiary hospital, uses a two-dimensional approach that integrates temporal series to classify each slice of the examination and make predictions at both slice and examination levels. The training process consists of two stages: first using a convolutional neural network InceptionResNet V2 and then a recurrent neural network long short-term memory model. This approach achieved an accuracy of 93% at the slice level and 77% at the examination level. External validation using a hospital dataset resulted in a precision of 86% for positive pulmonary embolism cases and 69% for negative pulmonary embolism cases. Notably, the model excels in excluding pulmonary embolism, achieving a precision of 73% and a recall of 82%, emphasizing its clinical value in reducing unnecessary interventions. In addition, the diverse demographic distribution in the validation dataset strengthens the model's generalizability. Overall, this model offers promising potential for accurate detection and exclusion of pulmonary embolism, potentially streamlining diagnosis and improving patient outcomes.

Recent evidence has supported a pathogenic role for neuroinflammation in Parkinson's disease (PD). Inflammatory response has been associated with symptoms and subtypes of PD. However, it is unclear whether immune changes are involved in the initial pathogenesis of PD, leading to the non-motor symptoms (NMS) observed in its prodromal stage. The current study aimed to characterize the behavioral and cognitive changes in a toxin-induced model of prodromal PD-like syndrome. We also sought to investigate the role of neuroinflammation in prodromal PD-related NMS. Male mice were subjected to bilateral intranasal infusion with 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine (MPTP) or saline (control group), followed by comprehensive behavioral, pathological and neurochemical analysis. Intranasal MPTP infusion was able to cause the loss of dopaminergic neurons in the substantia nigra (SN). In parallel, it induced impairment in olfactory discrimination and social memory consolidation, compulsive and anxiety-like behaviors, but did not influence motor performance. Iba-1 and GFAP expressions were increased in the SN, suggesting an activated state of microglia and astrocytes. Consistent with this, MPTP mice had increased levels of IL-10 and IL-17A, and decreased levels of BDNF and TrkA mRNA in the SN. The striatum showed increased IL-17A, BDNF, and NFG levels compared to control mice. In conclusion, neuroinflammation may play an important role in the early stage of experimental PD-like syndrome, leading to cognitive and behavioral changes. Our results also indicate that intranasal administration of MPTP may represent a valuable mouse model for prodromal PD.

Substance abuse disorder is a chronic condition for which pharmacological treatment options remain limited. L-type calcium channels (LTCC) have been implicated in drug-related plasticity and behavior. Specifically, dopaminergic neurons in the mesocorticolimbic pathway express Cav1.2 and Cav1.3 channels, which may regulate dopaminergic activity associated with reward behavior. Therefore, this study aimed to investigate the hypothesis that pre-administration of the LTCC blocker, isradipine can mitigate the effects of cocaine by modulating central glutamatergic transmission. For that, we administered isradipine at varying concentrations (1, 7.5, and 15 µg/µL) via intracerebroventricular injection in male Swiss mice. This pretreatment was carried out prior to subjecting animals to behavioral assessments to evaluate cocaine-induced locomotor sensitization and conditioned place preference (CPP). The results revealed that isradipine administered at a concentration of 1 µg/µL effectively attenuated both the sensitization and CPP induced by cocaine (15 mg/kg, via i. p.). Moreover, mice treated with 1 µg/µL of isradipine showed decreased presynaptic levels of glutamate and calcium in the cortex and hippocampus as compared to control mice following cocaine exposure. Notably, the gene expression of ionotropic glutamate receptors, AMPA, and NMDA, remained unchanged, as did the expression of Cav1.2 and Cav1.3 channels. Importantly, these findings suggest that LTCC blockage may inhibit behavioral responses to cocaine, most likely by decreasing glutamatergic input in areas related to addiction.

The factor RasGEF1b is a Ras guanine exchange factor involved in immune responses. Studies have also implicated RasGEF1b in the CNS development. It is still limited the understanding of the role of RasGEF1b in CNS functioning. Using RasGEF1b deficient mice (RasGEF1b-cKO), we investigated the impact of this gene deletion in behavior, cognition, brain neurochemistry and microglia morphology. We showed that RasGEF1b-cKO mice display spontaneous hyperlocomotion and anhedonia. RasGEF1b-cKO mice also exhibited compulsive-like behavior that was restored after acute treatment with the selective serotonin reuptake inhibitor (SSRI) fluoxetine (5 mg/kg). A down-regulation of mRNA of dopamine receptor (Drd1, Drd2, Drd4 and Drd5) and serotonin receptor genes (5Htr1a, 5Htr1b and 5Htr1d) was observed in hippocampus of RasGEF1b-cKO mice. These mice also had reduction of Drd1 and Drd2 in prefrontal cortex and 5Htr1d in striatum. In addition, morphological alterations were observed in RasGEF1b deficient microglia along with decreased levels of hippocampal BDNF. We provided original evidence that the deletion of RasGEF1b leads to unique behavioral features, implicating this factor in CNS functioning.

Introdução: As atividades laborais de manicures e pedicures geram riscos de exposições acidentais aos patógenos vinculados ao sangue como vírus, fungos e bactérias, devido à falta de adesão às medidas de biossegurança, e procedimentos de esterilização acarretando riscos à saúde dos profissionais e clientes. Objetivo:Verificar os conhecimentos das manicures e pedicures sobre os métodos de limpeza e esterilização de materiais, o uso de equipamentos de proteção individual e doenças relacionadas à profissão. Metodologia:Estudo transversal de abordagem quantitativa, realizado entre Outubro de 2017 a Janeiro de 2018, com 22 participantes finais. Incluídos os maiores de 18 anos, ambos os sexos, experiência mínima de um ano e residir em Nova Floresta. A técnica para seleção da amostra foi snowball. Resultados: A limitação de conhecimento sobre os riscos laborais na exposição de agentes infectantes, a baixa adesão no uso de equipamentos de proteção individuale a falta de padronização no processo de descontaminação, limpeza, desinfecção e esterilização dos instrumentais estão associados ao potencial risco de contaminação por agentes infecciosos, e ao acometimento de doenças infectocontagiosas por manicures, pedicures e clientela. Conclusões:Existem fragilidades no conhecimento destes profissionais,a maioria expõe métodos de antissepsia precários, devido a falta de sistematização e padronização dos procedimentos de limpeza, descontaminação dos instrumentais e o uso de equipamentos de proteção individual, além do conhecimento da profissão ter sido adquirido informalmente (AU).


Introduction: The work activities of manicurists and pedicurists generate risks of accidental exposure to pathogens linked to the blood such as viruses, fungi and bacteria, due to the lack of adherence to biosafety measures, and sterilization procedures causing risks to the health of professionals and clients. Objective:To verify the knowledge of manicurists and pedicurists on methods of cleaning and sterilizing materials, the use of personal protective equipment and diseases related to the profession. Methodology: Cross-sectional study with a quantitative approach, carried out between October 2017 and January 2018, with 22 final participants. Included are those over 18 years of age, both sexes, with at least one year's experience and residing in Nova Floresta. The technique for sample selection was snowball.Results:Limited knowledge about occupational risks in the exposure of infectious agents, low adherence to the use of personal protective equipmentand the lack of standardization in the process of decontamination, cleaning, disinfection and sterilizationof instruments are associated with the potential risk of contamination by infectious agents, and the spread of infectious diseases by manicurists, pedicurists and clientele. Conclusions:There are weaknesses in the knowledge of these professionals, most expose precarious antisepsis methods, due to the lack of systematization and standardization of cleaning procedures, decontamination of instruments and the use of personal protective equipment, in addition to the knowledge of the profession having been acquired informally (AU).

Objetivo:Verificar los conocimientos de manicuras y pedicuras sobre métodos de limpieza y esterilización de materiales, el uso de equipos de protección personal y enfermedades relacionadas con la profesión. Metodología:Estudio transversal de abordaje cuantitativo, realizado entre octubre de 2017 y enero de 2018, con 22 participantes finales. Se incluyen los mayores de 18 años, de ambos sexos, con experiencia mínima de un año y vivir en Nova Floresta. La técnica para la selección de la muestra fue la de bola de nieve. Resultados:El conocimiento limitado sobre los riesgos laborales en la exposición de agentes infecciosos, la baja adherencia al uso de equipo de protección personal y la falta de estandarización en el proceso de descontaminación, limpieza, desinfección y esterilización de instrumentos se asocian con el riesgo potencial de contaminación por agentes infecciosos, y la propagación de enfermedades infecciosas entre manicuras, pedicuras y clientela. Conclusiones:Existen debilidades en el conocimiento de estos profesionales, la mayoría expone métodos de antisepsia precarios, debido a la falta de sistematización y estandarización de los procedimientos de limpieza, descontaminación de los instrumentos y el uso de equipo de protección Personal,además de haber adquirido los conocimientos de la profesión informalmente (AU).

Petiveria alliacea L. (Phytolaccaceae) holds significant importance in the Amazon region, where it has been traditionally utilized in folk medicine. In this study, we conducted a comprehensive bibliometric analysis using conventional metrics, combined with a critical content review of its pharmacological and toxicological properties, to identify gaps in the existing literature that require further investigation. Our investigation identified a total of 55 articles that met the inclusion criteria for this study. Remarkably, Brazil emerged as the primary contributor within the scope of this review, indicating a strong presence of research from this country. Furthermore, professional scientific societies have played a pivotal role in facilitating the dissemination of scientific findings through specialist journals, fostering the sharing of research work within the community. Analysis of keyword co-occurrence revealed that "Petiveria alliacea", "plant extract", and "guatemala" were the most frequently encountered terms, indicating their significance within the literature. In terms of study designs, in vivo and in vitro were the predominant types observed, highlighting their prevalence in this field of study. Our study also identified a lack in knowledge yet to be investigated.

Chronic consumption of hyperpalatable and hypercaloric foods has been pointed out as a factor associated with cognitive decline and memory impairment in obesity. In this context, the integration between peripheral and central inflammation may play a significant role in the negative effects of an obesogenic environment on memory. However, little is known about how obesity-related peripheral inflammation affects specific neurotransmission systems involved with memory regulation. Here, we test the hypothesis that chronic exposure to a highly palatable diet may cause neuroinflammation, glutamatergic dysfunction, and memory impairment. For that, we exposed C57BL/6J mice to a high sugar and butter diet (HSB) for 12 weeks, and we investigated its effects on behavior, glial reactivity, blood-brain barrier permeability, pro-inflammatory features, glutamatergic alterations, plasticity, and fractalkine-CX3CR1 axis. Our results revealed that HSB diet induced a decrease in memory reconsolidation and extinction, as well as an increase in hippocampal glutamate levels. Although our data indicated a peripheral pro-inflammatory profile, we did not observe hippocampal neuroinflammatory features. Furthermore, we also observed that the HSB diet increased hippocampal fractalkine levels, a key chemokine associated with neuroprotection and inflammatory regulation. Then, we hypothesized that the elevation on glutamate levels may saturate synaptic communication, partially limiting plasticity, whereas fractalkine levels increase as a strategy to decrease glutamatergic damage.

The involvement of innate immune mediators to the Zika virus (ZIKV)-induced neuroinflammation is not yet well known. Here, we investigated whether neutrophil extracellular traps (NETs), which are scaffolds of DNA associated with proteins, have the potential to injure peripheral nervous. The tissue lesions were evaluated after adding NETs to dorsal root ganglia (DRG) explants and to DRG constituent cells or injecting them into mouse sciatic nerves. Identification of NET harmful components was achieved by pharmacological inhibition of NET constituents. We found that ZIKV inoculation into sciatic nerves recruited neutrophils and elicited the production of the cytokines CXCL1 and IL-1ß, classical NET inducers, but did not trigger NET formation. ZIKV blocked PMA- and CXCL8-induced NET release, but, in contrast, the ZIKV nonstructural protein (NS)-1 induced NET formation. NET-enriched supernatants were toxic to DRG explants, decreasing neurite area, length, and arborization. NETs were toxic to DRG constituent cells and affected myelinating cells. Myeloperoxidase (MPO) and histones were identified as the harmful component of NETs. NS1 injection into mouse sciatic nerves recruited neutrophils and triggered NET release and caspase-3 activation, events that were also elicited by the injection of purified MPO. In summary, we found that ZIKV NS1 protein induces NET formation, which causes nervous tissue damages. Our findings reveal new mechanisms leading to neuroinflammation by ZIKV.

Introduction: aging is a natural process that has been occurring at an accelerated pace and that, due to the physiological decline of systems, can lead to the appearance of vulnerabilities in the elderly. Objective: to verify the socioeconomic factors, health conditions and lifestyle habits associated with the degree of Clinical-Functional Vulnerability of elderly people from the ESF São Vicente in Manhuaçu using the IVCF-20. Method: cross-sectional, quantitative study, carried out with elderly people enrolled in the ESF São Vicente. For data collection, the IVCF-20 instrument was used for the degree of vulnerability and the data collection form for the general profile, life habits and health conditions. A chi -square test was performed to verify the association of independent variables with clinical and functional vulnerability. Result: of the 255 seniors evaluated, 60% had low clinical and functional vulnerability, 22% moderate vulnerability and 18% high vulnerability. The mean age was 72 years ± 8 years, mostly female (62.5%), self-declared white (60%), married (56%), do not have a caregiver (89.5%), are retired (82%), have chronic diseases (78.5%), self-rated health as excellent/good (63.5%), practice religion (87%), leave home alone (82%) and 40% belong to the D/E social class. The variables associated with P >0.05 were low vulnerability, not having a caregiver, being black/brown, not being a pensioner, leaving home alone, not having a chronic disease, having an excellent/good perception of health . Conclusion: it is of great importance to identify socioeconomic factors, lifestyle habits and vulnerability of the elderly in order to develop effective actions that maintain functionality and quality of life.


Introdução: o envelhecimento é um processo natural que vem ocorrendo de maneira acelerada e que, pelo declínio fisiológico dos sistemas, pode propiciar ao aparecimento de vulnerabilidades no idoso. Objetivo: verificar os fatores socioeconômicos, condições de saúde e hábitos de vida associados ao grau de Vulnerabilidade Clínico-funcional de idosos da ESF São Vicente em Manhuaçu utilizando o IVCF-20. Método: estudo transversal, quantitativo, realizado com idosos cadastrados na ESF São Vicente. Para coleta de dados foi empregado o instrumento IVCF-20 para o grau de vulnerabilidade e a ficha de coleta de dados para o perfil geral, hábitos de vida e condições de saúde. Realizou-se teste do qui-quadrado para verificar associação das variáveis independentes à vulnerabilidade clínico funcional. Resultado: dos 255 idosos avaliados, 60% apresentaram baixa vulnerabilidade clínico funcional, 22% moderada vulnerabilidade e 18% alta vulnerabilidade. A média de idade foi de 72 anos ± 8 anos, em sua maioria, sexo feminino (62,5%), autodeclaram brancos (60%), casados (56%), não possuem cuidador (89,5%), são aposentados (82%), possuem doenças crônicas (78,5%), auto avaliam a saúde como ótima/bom (63,5%), a prática religiosa (87%), saem de casa sozinhos (82%) e 40% pertencem a classe social D/E. Comportaram-se como variáveis associadas p < 0,05 a baixa vulnerabilidade, não ter cuidador, ser negro/pardo, não ser pensionista, sair de casa sozinho, não ser doença crônica, ter uma ótima/boa percepção de saúde. Conclusão: é de grande importância a identificação dos fatores socioeconômicos, hábitos de vida e vulnerabilidade dos idosos para elaboração de ações efetivas que mantenham a funcionalidade e qualidade de vida.

Toxoplasma gondii chronic infection is characterized by the establishment of tissue cysts in the brain and increased levels of IFN-γ, which can lead to brain circuitry interference and consequently abnormal behaviour in mice. In this sense, the study presented here sought to investigate the impact of chronic infection by two T. gondii strains in the brain of infection-resistant mice, as a model for studying the involvement of chronic neuroinflammation with the development of behavioural alterations. For that, male BALB/c mice were divided into three groups: non-infected (Ni), infected with T. gondii ME49 clonal strain (ME49), and infected with TgCkBrRN2 atypical strain (CK2). Mice were monitored for 60 days to establish the chronic infection and then submitted to behavioural assessment. The enzyme-linked immunosorbent assay was used for measurement of specific IgG in the blood and levels of inflammatory cytokines and neurotrophic factors in the brain, and the cell's immunophenotype was determined by multiparametric flow cytometry. Mice infected with ME49 clonal strain displayed hyperlocomotor activity and memory deficit, although no signs of depressive- and/or anxiety-like behaviour were detected; on the other hand, chronic infection with CK2 atypical strain induced anxiety- and depressive-like behaviour. During chronic infection by CK2 atypical strain, mice displayed a higher number of T. gondii brain tissue cysts and inflammatory infiltrate, composed mainly of CD3+ T lymphocytes and Ly6Chi inflammatory monocytes, compared to mice infected with the ME49 clonal strain. Infected mice presented a marked decrease of microglia population compared to non-infected group. Chronic infection with CK2 strain produced elevated levels of IFN-γ and TNF-ɑ in the brain, decreased NGF levels in the prefrontal cortex and striatum, and altered levels of fractalkine (CX3CL1) in the prefrontal cortex and hippocampus. The persistent inflammation and the disturbance in the cerebral homeostasis may contribute to altered behaviour in mice, as the levels of IFN-γ were shown to be correlated with the behavioural parameters assessed here. Considering the high incidence and life-long persistence of T. gondii infection, this approach can be considered a suitable model for studying the impact of chronic infections in the brain and how it impacts in behavioural responses.

Some types of peripheral nerve injury lead to limb deafferentation, which leads to remodeling of body representation areas in different parts of the brain, such as in the primary motor cortex and primary sensory cortex. This plasticity is a consequence of several cellular events, such as the emergence and elimination of synapses in these areas. Beside neurons, microglial cells are intimately involved in synapse plasticity, especially in synaptic pruning. In this study, we investigated the transient changes in synaptic density in the primary motor and sensory cortex after different types of peripheral nerve injury, as well as the behavior of microglial cells in each scenario. Male C57/B6 mice were divided into a control group (no injury), sciatic-crush group, and sciatic-transection group, and treated with PBS or minocycline daily for different time points. Both types of sciatic lesion led to a significant decrease of synaptophysin and PSD-95 positive puncta counts compared to control animals 4 days after lesion (DAL), which recovered at 7 DAL and was sustained until 14 DAL. The changes in synaptic puncta density were concomitant with changes in the density and morphology of microglial cells, which were significantly more ramified in the primary motor cortex of injured animals at 1 and 4 DAL. Although the decreased synaptic puncta density overlapped with an increased number of microglial cells, the number of lysosomes per microglial cell did not increase on day 4 after lesion. Surprisingly, daily administration of minocycline increased microglial cell number and PSD-95 positive puncta density by 14 DAL. Taken together, we found evidence for transient changes in synaptic density in the primary motor, related to peripheral injury with possible participation of microglia in this plasticity process.

The use of psychostimulant drugs can modify brain function by inducing changes in the reward system, mainly due to alterations in dopaminergic and glutamatergic transmissions in the mesocorticolimbic pathway. However, the etiopathogenesis of addiction is a much more complex process. Previous data have suggested that microglia and other immune cells are involved in events associated with neuroplasticity and memory, which are phenomena that also occur in addiction. Nevertheless, how dependent is the development of addiction on the activity of these cells? Although the mechanisms are not known, some pathways may be involved. Recent data have shown psychoactive substances may act directly on immune cells, alter their functions and induce various inflammatory mediators that modulate synaptic activity. These could, in turn, be involved in the pathological alterations that occur in substance use disorder. Here, we extensively review the studies demonstrating how cocaine and amphetamines modulate microglial number, morphology, and function. We also describe the effect of these substances in the production of inflammatory mediators and a possible involvement of some molecular signaling pathways, such as the toll-like receptor 4. Although the literature in this field is scarce, this review compiles the knowledge on the neuroimmune axis that is involved in the pathogenesis of addiction, and suggests some pharmacological targets for the development of pharmacotherapy.

BACKGROUND: The phytocannabinoid cannabidiol (CBD) has previously shown to have anticonvulsant effects in preclinical and clinical studies. Recently, CBD has been approved to treat certain types of drug-resistant epileptic syndromes. However, the underlying mechanism of action remains unclear. The phosphatidylinositol 3-kinase (PI3K) signaling pathway has been proposed to modulate seizures and might be recruited by CBD. Thus, we tested the hypothesis that the anticonvulsant effect of CBD involves PI3K in a seizure model induced by pentylenetetrazole (PTZ). METHODS: We employed pharmacological and genetic approaches to inhibit PI3K and quantified its effects on seizure duration, latency, and number. RESULTS: PI3K genetic ablation increased the duration and number of seizures. CBD inhibited PTZ-induced seizures in mice. Genetic deletion of PI3K or pretreatment with the selective inhibitor LY294002 prevented CBD effects. CONCLUSION: Our data strengthen the hypothesis that the CBD anticonvulsant effect requires the PI3K signaling pathway.

OBJECTIVE: To verify the association of socioeconomic conditions, life habits, and health conditions to the family function of older adults assisted by a Family Health Team (FHT) in the city of Manhuaçu/Minas Gerais. METHOD: This is an analytical, cross-sectional, observational study carried out with 166 older adults assisted in an FHU, with data on their socioeconomic profile, lifestyle, and health conditions. The family function was assessed with the Apgar Family and classified as good family function or moderate/high dysfunction. The Chi-square test was applied. RESULTS: Good function was found in 89.7% of the families. Age between 60-69 years, being married, having 3-4 children, not being a pensioner, and being independent were associated with good family function (p<0.05). CONCLUSION: Families have good function and socioeconomic and health conditions were associated with family function, which reinforces the importance of studying families in order to promote better aging in the population.

OBJECTIVES: To verify the agreement between the Alberta Infant Motor Scale assessment and maternal perception of the motor development in full-term infants. METHODS: This is a cross-sectional study involving 161 infants and mothers. Children were assessed with the Alberta Infant Motor Scale (AIMS) for motor developmental classification. Mothers completed questionnaires aiming to identify maternal profiles and impressions about their children's development. The kappa test was used to analyze the concordance between AIMS and mother perceptions. RESULTS: A total of 83.2% of the sample was classified as typically developing and 16.8% as suspected or delayed development. The maternal impression indicates that 77% of infants are developing typically, 19.9% perceived their infants' development as advanced, and 3.1% delayed development. There was low agreement between the mothers' perceptions and AIMS classifications (kappa = 0.153). CONCLUSIONS: Maternal perception of their infant's development was unsatisfactory for evaluation of motor development because their perceptions did not agree with the findings of the AIMS.

The Zika epidemic brought to the fore the birth of children with unknown and unexpected needs that demand longitudinal follow-up, strengthening the bond, comprehensiveness and coordination of care in health, which are essential attributes of primary health care (PHC). This article aims to evaluate the orientation of PHC care, using the PCATool-Brasil. The data were collected between 2016 and 2019, by means of interviews with parents of children who were born in a public maternity hospital in Recife (state of Pernambuco) between October 2015 and February 2016, a critical period of the microcephaly outbreak, especially in the Northeast of Brazil. The parents of 109 children participated in the survey, 15.6% of which had a confirmed microcephaly diagnosis. The degree of affiliation and access was sufficient, but the aspects of coordination of care, longitudinality, comprehensiveness, family and community orientation were insufficient. In the context of congenital Zika syndrome, these attributes are essential for the care of children and families. The fostering of health from these domains depends on the orientation of the models of care and their management with state and federal investments. The defense of life depends on the ability to place life above all other rationalities.


A epidemia zika trouxe ao cenário o nascimento de crianças com necessidades desconhecidas e inesperadas que exigem acompanhamento longitudinal, fortalecimento do vínculo, integralidade e coordenação do cuidado, atributos essenciais da atenção primária em saúde (APS). Este artigo tem o objetivo de avaliar a orientação da APS utilizando o instrumento PCATool-Brasil. Os dados foram coletados entre 2016 e 2019 por meio de entrevistas com responsáveis de crianças que nasceram em uma maternidade pública do Recife-PE no período crítico do surto de microcefalia, sobretudo no Nordeste do Brasil. Participaram do inquérito 109 responsáveis, dos quais 15,6% tiveram diagnóstico confirmado de microcefalia para suas crianças. Evidenciou-se a suficiência do grau de afiliação e acesso, mas a insuficiência da coordenação de cuidado, longitudinalidade, integralidade, orientação familiar e comunitária. No contexto da síndrome congênita da zika, esses atributos são imprescindíveis para o cuidado integral de crianças e famílias. A produção de saúde a partir desses domínios depende da orientação dos modelos de atenção e gestão com fortes investimentos estaduais e federal. A defesa da vida depende da capacidade de colocar a vida acima de todas as outras racionalidades.
