The paper describes the enzyme immunoassay system for detection of human immunodeficiency virus antigens, which is based on the use of rabbit anti-HIV antibodies and monoclonal antibodies to HIV-1 gene proteins gag. The system may be useful in the examination of laboratory and clinical samples to reveal both free and conjugated antigens in the composition of immune complexes. The sensitivity of the assay system under development is 0.5 ng/ml at 100% specificity.

A competitive time-resolved fluoroimmunoassay (TR-FIA) system for the detection of antibodies to protein p24 of HIV was developed on the basis of monoclonal antibodies. The advantages of this test system over analogous enzyme immunoassay system and commercial test system "Antigen" (USSR) were demonstrated. The newly developed test system of TR-FIA was used for examination of sera from HIV-infected persons.

Immunization of BALB/c mice with natural purified HIV antigen, fusion of spleen cells with myeloma cells and subsequent selection of hybrid clones using recombinant gag antigen of HIV gave hybridomas producing monoclonal antibodies (MCA) to HIV. The immune blotting method demonstrated that 3 clones interacted with protein p24 and 4 clones with protein p17 of HIV. Competitive EIA led to a conclusion that the resulting MCA detected at least 3 antigenic determinants in proteins, products of gag gene of HIV. The potentials of using these MCA for the detection of viral antigen in HIV-infected continuous cell lines were demonstrated.

Immunochemical study of blood sera from mice immunized with rabbit immunoglobulins (AT1) to herpes simplex virus type I (HSV-I) detected antiidiotypic antibodies AT2 in five of them (antiID). The affinity-purified antiID were shown to induce in syngeneic mice production of antibody (AT3) interacting with HSV-I antigen. In vitro, antiID (AT3) were shown to neutralize the virus activity. Analysis of the results suggests that the antiID imitate the antigenic properties of HSV-I and can induce specific immune response.

Central hemodynamic parameters under the effect of mildronate were examined in 62 patients suffering from coronary heart disease, 35 of these with acute myocardial infarction complicated by acute left-ventricular insufficiency and 20 with atherosclerotic cardiosclerosis with chronic cardiac insufficiency. The drug effect was assessed in two groups of patients after a single intravenous injection of 0.5-3 g. In group 1 (n = 53) mildronate effect on central hemodynamic parameters was assessed in spontaneous cardiac rhythm. Variously directed statistically unreliable hemodynamic shifts were revealed, related to heart rhythm changes. In group 2 (n = 9) the drug effect on heart rhythm was eliminated with the use of two-chamber electrocardiostimulation, and various hemodynamic regimens with hypo-, eu-, and hyperkinetic circulation types were artificially created by changing the A-V interval. Mildronate had no effect on the hemodynamics during two-chamber electrocardiostimulation.

The authors evaluate the efficacy of aortocoronary shunting in the treatment of 10 patients with progressive angina pectoris (unstable stenocardia) in follow-up periods of up to 12 months after the operation. In none of the patients did angina pectoris or myocardial infarction develop in the indicated postoperative periods.

Aortocoronary shunting was performed in 15 patients with unstable and early postinfarction angina pectoris. Coronarography revealed an isolated lesion of a coronary artery in all the patients. Two years after the operation the patients had no angina pectoris; no myocardial infarctions occurred.

Fourteen patients aged 34-54 years underwent aortocoronary bypass surgery for early postinfarction angina within 30 days following myocardial infarction. Crystal and molecular structure of D-11-aza-19-nortestosterone monohydrate C17H25NO2.H2O (a 8,288(2), b 12,433(2), c 7,570(2) A, beta 90,25(1) degrees; space group P2(1), R 8.3%) has been determined by X-ray analysis. Its comparison with the molecular structure of D-19-nortestosterone showed that the decrease in the hormonal activity upon 11-aza-substitution may be due to difference in chemical properties of imino and methylene groups.

Intravenous jet injections of 50 mg ethacizine (10 mg/min) are effective in 100% of cases of ventricular extrasystoles, and paroxysms of supraventricular and ventricular tachycardia, and in 59.5% of cases of atrial fibrillation paroxysms. A relationship has been demonstrated between the magnitude of therapeutic effect and the duration of atrial fibrillation as well as the type of underlying disease. The use of the drug is not justified in paroxysmal auricular flutter. Ethacizine is effective in 82.4% of patients with acute myocardial infarction, and in 65% of patients with other diseases. A similar effect was obtained with intravenous drip injections at the rate of 1.7 mg/min, while the incidence of side effects was reduced essentially. Therefore, drip injections of ethacizine are preferable for the treatment of heart rhythm disorders, such as extrasystoles, particularly in patients with acute myocardial infarction.

Basing on the data of the X-ray analysis of 2 beta-methylestr-4,9-dien-3-one-17 beta-ol (C19H26O2) and A-ring unsubstituted steroid 4,9-dien-3-ones, the noncomplanarity and flexibility of the conjugated dienone system in these transformed steroids has been demonstrated. The results have been also confirmed by UV spectroscopy. The ability of the dienone system to assume conformations with the out of- plane C3 = O3 and/or C9 = C10 bonds allows the AB-fragment of the steroid molecule to adopt the conformations required for interacting with various receptors. This property may also account for a simultaneous enhancement of several hormonal activities upon such a modification of steroids. The results of the X-ray analysis of 2 beta-methylestra-4,9-dien-3-one-17 beta-ol (space group P2(1)2(1)2(1), a 8,543(2), b 9,783(2), c 18,690(7) A, R 8,7%) are presented.

The efficiency of temporary atrial and ventricular electrocardiostimulation (ECS) used for the control and prevention of recurrent and drug-resistant arrhythmias was studied in 37 patients with coronary heart disease (CHD) and cardiosclerosis, including 21 patients with acute myocardial infarction (MI). The possibility of applying superfrequent ventricular ECS for controlling paroxysms of ventricular tachycardia in acute MI patients, and high efficiency of accelerating-suppressing ECS for the prevention of ventricular tachycardia and ventricular fibrillation paroxysms are demonstrated.

Relationship between structure and mode of action of 16 alpha, 17 alpha-cycloalkanoprogesterones (pregna-D'-pentaranes).

A new class of modified progesterones with an additional 16 alpha, 17 alpha-carbocycle (pregna-D'-pentaranes) is obtained. These compounds were found to exhibit a high progestational activity in the Clauberg-McPhail assay and to be active as well in the pregnancy maintenance test in ovariectomized rabbits. Some of the pentaranes displayed a remarkable contraceptive effect in combination with mestranol. X-Ray data were employed for the description of molecular conformations of D'-pentaranes. On this base, their biological responses are considered to be connected with the conformational changes of D and D' rings as well as 17 beta-acetyl side chain. Accordingly, the contraceptive effect of D'3-6-pentaranes is correlated with the value of C(15)-C(16)-C(17)-C(20) torsion angle of the corresponding molecules. The relative binding affinities of some D'6-pentaranes to the progestin receptor were determined and compared with their in vivo biological responses.
