RESUMO
Rectal leiomyosarcoma is a very rare entity. Surgery is the main treatment, but the place of radiation therapy remains unclear. A 67-year-old woman was referred for a few-weeks' history of bleeding and anal pain intensified during defecation. Pelvic magnetic resonance imaging (MRI) showed a rectal lesion and biopsies revealed a leiomyosarcoma of the lower rectum. She was free of metastasis on computed tomography imaging. The patient refused radical surgery. After discussion by a multidisciplinary team, the patient received pre-operative long-course radiotherapy followed by surgery. The tumor was treated with 50Gy delivered in 25 fractions, within 5 weeks. The aim of radiotherapy was local control, allowing organ-preservation. Four weeks after radiation therapy, organ-preservation surgery could be performed. She had no adjuvant treatment. At 38-months follow-up, she had no local recurrence. However, distant recurrence (lung, liver, and bone) was detected 38 months after the resection and was managed by intra-venous doxorubicin 60mg/m2 and dacarbazine 800mg/m2 every 3 weeks. The patient was in a stable condition for nearly 8 months. The patient died 4 years and 3 months after the diagnosis.

Ocular manifestations of rheumatoid arthritis are mainly dry eye syndrome, scleritis, and keratitis. The occurrence of corneal ulceration in the course of this disease is a rare complication but can lead to ocular perforation. We report the case of a woman followed for rheumatoid arthritis who presented a bilateral sterile paracentral ulcer that responded well to medical treatment.

We report a case of severe keratitis due to Fusarium solani in a young man in the Sahara in Morocco where the climate is arid. This patient reported had a grain of sand in his right eye for a week after a sandstorm. On admission he had a corneal abscess. Despite rapid diagnosis and initiation of treatment with available antifungal drugs: amphotericin B and natamycin eye drops, the prognosis worsened and led to the enucleation of the right eye. Faced with a suspected eye infection after a microtrauma caused by grains of sand carried by a sandstorm, it is important to take biological samples to search for fungal infections among other. It is also important to have new triazole antifungal drugs available to treat ocular mycosis rapidly and effectively.

Isolated cilioretinal artery occlusion is a rarely reported initial manifestation of polycythemia vera. In this study, we reported a case of a 65-year-old man with polycythemia vera with cilioretinal artery occlusion as an initial manifestation.
RESUMO
Idiopathic inflammatory orbital myosites represent 10% of the orbital pseudotumors and 8% of the orbital pathologic processes in children. They are rarely described although they raise real diagnostic problems. The authors report the case of a 10-year old little girl who presented sudden proptosis. The clinical and radiological work up and the therapeutic test with corticosteroids were in favor of an inflammatory orbital myositis. The authors remind the clinical, radiological, and therapeutical aspects of this entity that remains very unfrequent in children and needs to be differenciated from rhabdomyosarcoma.

Kaposi's sarcoma is a proliferative disease, probably induced by human herpes virus type 8 (HHV8). Its expression is cutaneous and visceral, with four clinical forms. An orbital location of Kaposi's sarcoma remains exceptional. In order to evaluate the epidemiological, clinical, and therapeutic objectives of this tumor location, we report a case of a 58-year-old patient hospitalized in the ophthalmology department for a surgically treated right orbital tumor.

Severe restrictive disorders of ocular motility are rarely described in high myopia. Their cause is still unknown. Several theories have been suggested: structural changes in the ocular muscles, sixth-nerve palsy or an elongation of the ocular globe. We report the case of a 44 years old man with high myopia of the left eye and disturbed ocular motility. Medical imaging techniques (echography, CT, MRI) allowed us to objectify the cause of ocular motility problem.

INTRODUCTION: The orbital hydatid cyst is a rare (1 to 2%) localization of the Echinococcus granulosus parasite. OBSERVATIONS: An intra-orbital localization of hydatidosis was revealed in two patients by the occurrence of unilateral exophthalmia. After tumoral resection the diagnosis was confirmed by histological examination. COMMENTS: The principle clinical sign of an orbital hydatid cyst is exophthalmia. Imaging is essential for pre-operative diagnosis. Serology is insufficient and treatment is surgical.

Esthesioneuroblastoma (ENB) is a rare malignant tumor, which develops from the olfactory neuroepithelium and is one of the rarest tumors of the nasal cavity. Revelation by ocular signs is unusual. We report the original observation of a case of ENB of the olfactive placodes in a 28-year-old adult discovered during exophthalmia. The authors studied the clinical, radiological, pathological, and therapeutic aspects. ENB should not be forgotten in case of unilateral tumorous exophthalmia associating rhinological signs.
