OBJECTIVES: The Nutrition Risk in the Critically Ill (NUTRIC) score has been advocated as a screening tool for nutrition risk assessment in critically ill patients. It was developed and validated to predict 28-day mortality using Acute Physiology and Chronic Health Evaluation II (APACHE II) score as one of its components. However, nowadays the Simplified Acute Physiology Score 3 (SAPS 3) demonstrates better performance. We aimed to test the performance of NUTRIC score in predicting 28-day mortality after replacement of APACHE II by SAPS 3, and the interaction between nutrition adequacy and mortality. METHODS: Adult patients who received nutrition therapy and remained >3 days in intensive care unit were retrospectively evaluated. In order to replace APACHE II component, we used ranges of SAPS 3 with similar predicted mortality. Discrimination between these tools in predicting 28-day mortality was assessed using the ROC curve, calibration was evaluated with calibration belt, and correlation with intraclass correlation. The relationship between nutritional adequacy and mortality was assessed in a subgroup with available data. RESULTS: 542 patients were analyzed (median age of 78 years old, 73.4% admitted for non-surgical reasons and 28-day mortality was 18.1%). Mortality prediction discrimination did not differ between tools (p>0.05), but showed a good agreement (intraclass correlation 0.86) with good calibration. In the subgroup analysis for nutritional adequacy (n = 99), no association with mortality was observed. CONCLUSION: Performance of NUTRIC score with SAPS 3 is similar to the original tool. Therefore, it might be used in settings where APACHE II is not available.

A desnutrição é frequentemente encontrada no ambiente hospitalar. Muitas vezes negligenciada, apesar de afetar desfavoravelmente a saúde da população, a desnutrição apresenta como principais complicações: pior resposta imunológica, atraso no processo de cicatrização, risco elevado de complicações cirúrgicas e infecciosas, maior probabilidade de desenvolvimento de lesões por pressão, aumento no tempo de internação e do risco de mortalidade. Fora isso, acarreta considerável aumento dos custos hospitalares. A taxa de desnutrição varia entre 20 e 50% em adultos hospitalizados e durante a hospitalização esta condição piora progressivamente principalmente em idosos e pacientes críticos. Em 1998, o inquérito brasileiro, conhecido como IBRANUTRI, avaliou 4 mil pacientes internados na rede pública hospitalar de vários estados brasileiros, confirmando a prevalência da desnutrição em 48,1% dos pacientes. Há 20 anos, estes dados foram publicados e o cenário permanece imutável até os dias atuais, pois, em 2016, outro estudo (com aproximadamente 30.000 pacientes) corroborou a manutenção da alta prevalência de desnutrição em pacientes hospitalizados. A identificação precoce da desnutrição, bem como o manejo, por meio de ferramentas recomendadas, possibilita estabelecer a conduta nutricional mais apropriada e melhora do desfecho nestes pacientes.O objetivo desta campanha é reduzir as taxas de desnutrição por meio de uma série de ações que incluem a triagem, o diagnóstico, o manejo e o tratamento da desnutrição. Para facilitar a maneira de difundir este conhecimento, foi desenvolvido um método mnemônico com a palavra "DESNUTRIÇÃO", abordando cada letra inicial de forma simples, desde o conceito até o tratamento da desnutrição. Desta forma, o método garante uma integração interdisciplinar, além de averiguar os principais aspectos do cuidado geral do paciente desnutrido.

OBJECTIVE: To evaluate the satisfaction of the intensive care unit staff with a computerized physician order entry and to compare the concept of the computerized physician order entry relevance among intensive care unit healthcare workers. METHODS: We performed a cross-sectional survey to assess the satisfaction of the intensive care unit staff with the computerized physician order entry in a 30-bed medical/surgical adult intensive care unit using a self-administered questionnaire. The questions used for grading satisfaction levels were answered according to a numerical scale that ranged from 1 point (low satisfaction) to 10 points (high satisfaction). RESULTS: The majority of the respondents (n=250) were female (66%) between the ages of 30 and 35 years of age (69%). The overall satisfaction with the computerized physician order entry scored 5.74±2.14 points. The satisfaction was lower among physicians (n=42) than among nurses, nurse technicians, respiratory therapists, clinical pharmacists and diet specialists (4.62±1.79 versus 5.97±2.14, p<0.001); satisfaction decreased with age (p<0.001). Physicians scored lower concerning the potential of the computerized physician order entry for improving patient safety (5.45±2.20 versus 8.09±2.21, p<0.001) and the ease of using the computerized physician order entry (3.83±1.88 versus 6.44±2.31, p<0.001). The characteristics independently associated with satisfaction were the system's user-friendliness, accuracy, capacity to provide clear information, and fast response time. CONCLUSION: Six months after its implementation, healthcare workers were satisfied, albeit not entirely, with the computerized physician order entry. The overall users' satisfaction with computerized physician order entry was lower among physicians compared to other healthcare professionals. The factors associated with satisfaction included the belief that digitalization decreased the workload and contributed to the intensive care unit quality with a user-friendly and accurate system and that digitalization provided concise information within a reasonable time frame.

Objetivo: Avaliar a satisfação da equipe da unidade de terapia intensiva com o prontuário eletrônico do paciente e comparar a relevância do conceito de registro eletrônico de ordens médicas entre os profissionais de saúde da unidade de terapia intensiva. Métodos: Estudo transversal de levantamento para avaliar a satisfação da equipe da unidade de terapia intensiva com o prontuário eletrônico do paciente em uma unidade de terapia intensiva clínica e cirúrgica para pacientes adultos com 30 leitos, utilizando um questionário de autopreenchimento. As questões utilizadas para graduar os níveis de satisfação foram respondidas segundo uma escala numérica, que variava de 1 (baixo grau de satisfação) a 10 pontos (elevado grau de satisfação). Resultados: As pessoas que responderam ao questionário (n=250) eram, em sua maioria, do gênero feminino (66%) com idades entre 30 e 35 anos (69%). O grau geral de satisfação com o prontuário eletrônico do paciente foi de 5,74±2,14 pontos. O grau de satisfação foi mais baixo entre os médicos (n=42) do que entre enfermeiros, técnicos de enfermagem, terapeutas respiratórios, farmacêuticos clínicos e nutricionistas (4,62±1,79 em comparação com 5,97±2,14; p<0,001); o grau de satisfação decresceu com a idade (p<0,001). Os médicos tiveram níveis mais baixos de satisfação com relação ao potencial do sistema de registro eletrônico de ordens médicas de melhorar a segurança do paciente (5,45±2,20 em comparação com 8,09±2,21; p<0,001) e facilidade de uso do sistema de registro eletrônico de ordens médicas (3,83±1,88 em comparação com 6,44±2,31; p<0,001). INTRODUCTION: Recent data have suggested that patient admission during intensive care unit (ICU) morning bedside rounds is associated with less favorable outcome. We undertook the present study to explore the association between morning round-time ICU admissions and hospital mortality in a large Canadian health region. METHODS: A multi-center retrospective cohort study was performed at five hospitals in Edmonton, Canada, between July 2002 and December 2009. Round-time ICU admission was defined as occurring between 8 and 11:59 a.m. Multivariable logistic regression analysis was used to explore the association between round-time admission and outcome. RESULTS: Of 18,857 unique ICU admissions, 2,055 (10.9%) occurred during round time. Round-time admissions were more frequent in community hospitals compared with tertiary hospitals (12.0% vs. 10.5%; odds ratio [OR] 1.16; 95% CI, 1.05-1.29, P < 0.004) and from the ward compared with the emergency department (ED) or operating theater (17.5% vs. 9.2%; OR 2.1; 95% CI, 1.9-2.3, P < 0.0001). Round-time admissions were more often medical than surgical (12.6% vs. 6.6%; OR 2.06; 95% CI, 1.83-2.31, P < 0.0001), had more comorbid illness (11.9% vs. 10.5%; OR 1.15; 95% CI, 1.04-1.27, P < 0.008) and higher APACHE II score (22.2 vs. 21.3, P < 0.001), and were more likely to have a primary diagnosis of respiratory failure (37.0% vs. 31.3%, P < 0.001) or sepsis (11.1% vs. 9.0%, P = 0.002). Crude ICU mortality (15.3% vs. 11.6%; OR 1.38; 95% CI, 1.21-1.57, P < 0.0001) and hospital mortality (23.9% vs. 20.6%; OR 1.21; 95% CI, 1.09-1.35, P < 0.001) were higher for round-time compared with non-round-time admissions. In multi-variable analysis, round-time admission was associated with increased ICU mortality (OR 1.19, 95% CI, 1.03-1.38, P = 0.017) but was not significantly associated with hospital mortality (OR 1.02; 95% CI, 0.90-1.16, P = 0.700). In the subgroup admitted from the ED, round-time admission showed significantly higher ICU mortality (OR 1.54; 95% CI, 1.21-1.95; P < 0.001) and a trend for higher hospital mortality (OR 1.22; 95% CI, 0.99-1.51, P = 0.057). CONCLUSIONS: Approximately 1 in 10 patients is admitted during morning rounds. These patients are more commonly admitted from the ward and are burdened by comorbidities, are non-operative, and have higher illness severity. These patients admitted during morning rounds have higher observed ICU mortality but no difference in hospital mortality.
