OBJECTIVES: Stent-assisted coiling prevents coil migration in broad-based intracranial aneurysms. So far, only permanent metal stents are approved for intracranial use. Bioresorbable stents allow a new therapeutic approach that may prevent the need for lifelong anticoagulation. We developed a neurovascular bioresorbable microstent (NBRS) and compared it in vitro to the commercial Neuroform EZ stent. MATERIALS AND METHODS: The self-expanding NBRS design is oriented on the Neuroform EZ stent. Poly L-lactic acid (PLLA) was used to manufacture semi-finished products in a dipping process. For the compensation of the inferior material properties of PLLA, design adjustments were made. The NBRS were cut by means of femtosecond (fs) laser and were morphologically and mechanically compared in vitro to the Neuroform EZ stent. In vitro implantation of an NBRS was performed using a complex patient-specific 3D-printed aneurysm model. In addition, an in vitro coiling procedure to assess the stent's ability to support a coil package was conducted. RESULTS: The NBRS could be reproducibly manufactured and had high quality regarding surface morphology. The radial force at the indicated vessel diameter of 3.0 mm was slightly higher for the Neuroform EZ stent compared to the NBRS. The self-expansion ability of the NBRS could be proven. The kink behavior of the NBRS was comparable to that of the Neuroform EZ stent, so no vessel lumen size reduction is expected. The stents showed identical deformation under local compression of 25 % based on the initial diameter, resulting in maximum forces of 24 ±â€Š5 mN (Neuroform EZ) and 8 ±â€Š2 mN (NBRS). The implanted NBRS expanded uniformly, and proper vessel wall adaptation was observed. The NBRS has the ability to retain a coil package. CONCLUSION: This study reported a reproducible manufacturing process for the developed NBRS as well as mechanical and morphological in vitro tests. Furthermore, successful NBRS implantation into a complex patient-specific vessel model was presented as proof of concept. The promising results of this study, also considering the commercial Neuroform EZ stent, support the idea of fully biodegradable microstents for intracranial aneurysm treatment. Currently, the effect of prenatal ultrasound on foetal development is intensively discussed and the guidelines for prenatal diagnostics have been changed. However, data supporting these concerns are scarce. Therefore, we used an established in ovo model of the chicken embryo to investigate cell proliferation and apoptosis within the retina. A total of 21 chicken eggs were fenestrated on Day 5 and allocated to either the control (n = 8) or exposition group (n = 13). The exposition group was treated with pulsed-wave Doppler ultrasound (PWD) for 10 min while controls remained without treatment. After subsequent incubation (6-48 h), the eggs were sacrificed, and chicken embryos were examined morphologically (HE-staining) and immunohistochemically. Counting of apoptotic and proliferating cells per retina was performed using antibodies specific for phospho-histone-H3 and active caspase-3 in combination with a biotin-labelled secondary antibody and peroxidase conjugated avidin-biotin complex for chromogenic detection. Due to a rather low number of specimens at each time point after ultrasound exposition, we neglected the effects of incubation time and focused on treatment effects. This approach revealed that the median number of proliferating cells is reduced after 10 min of exposure to PWD (569 vs. 766), while the number of apoptotic cells is fairly comparable between groups (5 vs. 6). Our data contribute to a better understanding of prenatal US on foetal development by suggesting that PWD could have an impact on the number of proliferating cells in the developing chicken retina and therefore justify further investigations.

We report the case of a 74-year-old female with abdominal pain, tarry stools, and tachycardia. Previous history included diabetes mellitus with micro- and macroangiopathy. Imaging revealed portal gas, left sided colitis, and emphysematous gastritis, besides severe atherosclerosis with subtotal celiac trunk occlusion and moderate stenosis of the inferior mesenteric artery. Upper endoscopy revealed findings consistent with focal necrotizing gastritis at the greater curvature and acute esophageal necrosis. Blood cultures immediately grew Clostridium perfringens. The patient was treated with broad spectrum antibiotics and was discharged after 21 days in the hospital. This case demonstrates the rare coincident occurrence of nontransmural ischemia of the left colon, the esophagus, and the stomach as a result of low-flow circulatory compromise, which then precipitated C. perfringens associated emphysematous gastritis and blood stream infection.

We aimed to evaluate electrocardiogram (ECG)-gated MR angiography (MRA) in the follow-up after surgery involving the ascending aorta regarding technical feasibility, image quality, spectrum of findings, and their implications for clinical management. We retrospectively analyzed a cohort of 19 patients (median age 59 years, range 38-79 years), who underwent MRA for follow-up imaging after surgery involving the ascending aorta. Our magnetic resonance imaging protocol consisted of a time-resolved, non-ECG-gated MRA and an ECG-gated MRA performed at 3T. Median examination duration was 25 minutes (range 11-41 minutes). All examinations were assessed by 2 readers in consensus for image quality on a 5-point scale ranging from 1 (non-diagnostic) to 5 (excellent). MRA examinations and patient charts were analyzed for diagnostic findings and their consequences for further management. Subjective image quality was rated as "sufficient" (score 3.1 ±â€…1.1) for the aortic root and as "good" to "excellent" for the ascending aorta (score 4.5 ±â€…0.7), aortic arch (4.5 ±â€…0.7), supra-aortic branches (4.5 ±â€…0.6) and descending aorta (4.6 ±â€…0.7). Abnormal findings were seen in 6 patients (32%) including progressive diameter of remaining aneurysm or dissection (3 patients, 16%) and suture aneurysms (3 patients, 16%). In all 6 of these patients, abnormal findings at MRA had consequences for clinical management. ECG-gated MR angiography at 3T yields good image quality for post-operative surveillance after aortic surgery involving the ascending aorta. This technique may serve as an alternative to computed tomography particularly in younger patients with repeated follow-up.

MR microscopy (MRM) is known as ultra-high-field (UHF) magnetic resonance imaging with an in-plane spatial resolution of <100 µm, yields highly resolved non-invasive anatomical imaging and allows longitudinal assessment of embryonic avian development. The aim of the present study was to evaluate the feasibility of in vivo anatomical MRI assessment of the developing upper extremity of the chicken. Thirty-eight fertilized chicken eggs were examined at 7 Tesla acquiring high-resolution T2-weighted images with an in-plane resolution of 74 × 74 µm. To reduce motion artefacts, the eggs were moderately cooled before and during MRI. Development of the upper extremity was anatomically and quantitatively assessed. Chondrification and ossification on MRI were correlated with histological examination. MRM allowed the identification of the embryo from stage D5 onwards. First chondrification of the upper extremity was visible at stage D7, and the differentiation of the forearm was possible from stage D9 throughout the developmental period with excellent correlation to histology. MRM also allowed the differentiation between cortical and medullary bone as well as the detection of chondrified areas. UHF MRM allows the in vivo and in ovo evaluation of the upper limb during embryonic development and provides non-invasive longitudinal anatomical information. This technique allows longitudinal studies of the same embryo during the developmental period and may therefore provide further insights into the development of the upper extremity. With improved coil technique and increasing availability of UHF MR systems, there is great potential regarding several research topics in experimental musculoskeletal radiology.

With its standardized MRI datasets of the entire spine, the German National Cohort (GNC) has the potential to deliver standardized biometric reference values for intervertebral discs (VD), vertebral bodies (VB) and spinal canal (SC). To handle such large-scale big data, artificial intelligence (AI) tools are needed. In this manuscript, we will present an AI software tool to analyze spine MRI and generate normative standard values. 330 representative GNC MRI datasets were randomly selected in equal distribution regarding parameters of age, sex and height. By using a 3D U-Net, an AI algorithm was trained, validated and tested. Finally, the machine learning algorithm explored the full dataset (n = 10,215). VB, VD and SC were successfully segmented and analyzed by using an AI-based algorithm. A software tool was developed to analyze spine-MRI and provide age, sex, and height-matched comparative biometric data. Using an AI algorithm, the reliable segmentation of MRI datasets of the entire spine from the GNC was possible and achieved an excellent agreement with manually segmented datasets. With the analysis of the total GNC MRI dataset with almost 30,000 subjects, it will be possible to generate real normative standard values in the future.

To evaluate the impact of a novel, deep-learning-based image reconstruction (DLIR) algorithm on image quality in CT angiography of the aorta, we retrospectively analyzed 51 consecutive patients who underwent ECG-gated chest CT angiography and non-gated acquisition for the abdomen on a 256-dectector-row CT. Images were reconstructed with adaptive statistical iterative reconstruction (ASIR-V) and DLIR. Intravascular image noise, the signal-to-noise ratio (SNR) and the contrast-to-noise ratio (CNR) were quantified for the ascending aorta, the descending thoracic aorta, the abdominal aorta and the iliac arteries. Two readers scored subjective image quality on a five-point scale. Compared to ASIR-V, DLIR reduced the median image noise by 51-54% for the ascending aorta and the descending thoracic aorta. Correspondingly, median CNR roughly doubled for the ascending aorta and descending thoracic aorta. There was a 38% reduction in image noise for the abdominal aorta and the iliac arteries, with a corresponding improvement in CNR. Median subjective image quality improved from good to excellent at all anatomical levels. In CT angiography of the aorta, DLIR substantially improved objective and subjective image quality beyond what can be achieved by state-of-the-art iterative reconstruction. This can pave the way for further radiation or contrast dose reductions.

BACKGROUND: In patients with peripheral artery disease (PAD), run-off MR-angiography (MRA) is a commonly performed diagnostic test to obtain high-resolution images for evaluation of the arterial system from the aorta through the distal run-off vessels. The aim of this study was to investigate the prevalence of visceral artery involvement (VAI) in patients with PAD and leg symptoms examined with run-off MRA. METHODS: We retrospectively analyzed 145 patients (median age 68 years, range 27-91) who underwent MRA due to known or suspected PAD at our institution between 2012 and 2018. MRA examinations were re-evaluated for visceral artery stenosis. Patient dossiers were reviewed to determine cardiovascular risk factors, kidney function and Fontaine stage of PAD. RESULTS: Involvement of at least one visceral artery with ≥ 50% diameter stenosis was found in 72 (50%) patients. There were no differences in age, gender, MRA indication, Fontaine stage, levels of C-reactive protein (CRP), cardiovascular risk factors or vascular comorbidities between patients with and without VAI. Renal artery (RA) involvement with ≥ 50% diameter stenosis was observed in 28 (20%) of patients. Patients with involvement of the RA were more likely to suffer from hypertension (79 vs. 54%, p = 0.019) and reduced renal function (glomerular filtration rate 70 vs. 88 mL/min/1.73m2, p = 0.014). CONCLUSION: Visceral artery stenosis can be seen in half of patients with known or suspected PAD and leg symptoms on run-off MRA. Investigating for RA stenosis in patients with PAD and hypertension and/or impaired renal function may have high diagnostic yield.

Purpose: To reveal age-related changes of the human crystalline lens by using high-spatial resolution T1-weighted brain magnetic resonance imaging of patients under general anesthesia. Methods: We retrospectively identified 47 children (2-17 years) and 30 adults (18-70 years) without diabetes or eye disease, who required brain magnetic resonance imaging examinations under general anesthesia between 2012 and 2019. Normalized signal intensity of the crystalline lens and vitreous body, as well as equatorial diameter and axial thickness of the lens were assessed by using a three-dimensional T1-weighted magnetization prepared rapid acquisition gradient echo sequence of the brain with 0.9-mm spatial resolution. Patient dossiers were reviewed to record indication for magnetic resonance imaging examination and hypertension. Results: Advancing age was significantly correlated with increasing equatorial diameter of the infantile lens (r = 0. 74; 95% confidence interval, 0.58-0.85; P < .0001) and increasing crystalline lens signal intensity of the adult lens (r = 0.38; 95% confidence interval, 0.02-0.65; P = .0382), which remained significant after accounting for potential confounding variables. There was no significant correlation between age and axial thickness or vitreous body signal intensity in the children and adult cohort. Conclusions: The present study demonstrated that advancing age was significantly correlated with an increasing equatorial diameter of the infantile lens and with increasing crystalline lens signal intensity of the adult lens. These normative data can contribute to our understanding of age-related changes in eye health and function, especially in regard to the emmetropization process and should also be taken into account when investigating lens pathologies.

PURPOSE: To review emerging techniques in cardiac magnetic resonance imaging (CMR) and their clinical applications with a special emphasis on new technologies, recent trials, and updated guidelines. TECHNOLOGICAL INNOVATIONS: The utility of CMR has expanded with the development of new MR sequences, postprocessing techniques, and artificial intelligence-based technologies, which have substantially increased the spectrum, quality, and reliability of information that can be obtained by CMR. ESTABLISHED AND EMERGING INDICATIONS: The CMR modality has become an irreplaceable tool for diagnosis, treatment guidance and follow-up of patients with ischemic heart disease, myocarditis, and cardiomyopathies. Its role has been further strengthened by recent trials and guidelines. Quantitative mapping techniques are increasingly used for tissue characterization and detection of diffuse myocardial changes including myocardial storage diseases. PRACTICAL RECOMMENDATIONS: With state-of-the-art CMR sequences, postprocessing techniques and understanding of their interpretation, CMR makes invaluable contributions to provide state-of-the-art diagnostics and care for cardiac patients in a multidisciplinary team.

OBJECTIVES: The clinical presentation of peripheral artery disease (PAD) and chronic venous insufficiency (CVI) can overlap and the conditions may co-exist. The purpose of our study was to investigate the prevalence and clinical significance of concomitant CVI in patients with PAD examined with run-off MR angiography (MRA). METHODS: We analysed 180 patients (median age 69 years, range 27 to 91) with known or suspected PAD who underwent MRA at our institution between 2012 and 2018. MRA datasets were re-evaluated for manifestations of CVI. Electronic charts were reviewed to analyse whether diagnosis of CVI was documented and to determine Fontaine stage of PAD. RESULTS: Evidence of possible CVI on MRA was found in 38 (21%) patients. Only seven (18%) of these patients had a documented diagnosis of CVI. Patients with co-existing PAD and CVI were more likely obese (median BMI 29.7 vs. 26.3 kg/m2, p = 0.001) and diabetic (55 vs. 35%, p = 0.039) than patients without CVI. The frequency of concomitant CVI manifestations decreased from distal to proximal with the lower leg affected in all 38 patients and the thigh in 17 patients (45%). Patients with co-existing PAD and CVI were more likely to have a clinical diagnosis of stage IV PAD than patients without co-existing CVI (57% vs. 34%, relative risk 1.68, p = 0.018). CONCLUSIONS: Signs of possible concomitant CVI can be seen in approximately one-fifth of patients with known or suspected PAD examined with run-off MRA. If present, these findings should be reported since CVI may mimic or contribute to symptoms attributed to PAD. KEY POINTS: • In total, 21% of patients with PAD patients examined with MR angiography show signs of possible co-existing CVI. • Patients with co-existing CVI were 1.7-fold more likely to have a clinical diagnosis of stage IV PAD. • Our data also showed that co-existing chronic venous insufficiency is under-diagnosed in patients with PAD.

Subvenous epicardial fat tissue (SEAT), which acts as an electrical insulation, and the venous diameter (VD) both constitute histomorphological challenges for optimal application and lead design in cardiac synchronization therapy (CRT). In this study, we characterized the morphology of human coronary veins to improve the technical design of future CRT systems and to optimize the application of CRT leads. We retrospectively analyzed data from cardiac computed tomography (CT) of 53 patients and did studies of 14 human hearts using the postmortem freeze section technique and micro CT. Morphometric parameters (tributary distances, offspring angles, luminal VD, and SEAT thickness) were assessed. The left posterior ventricular vein (VVSP) had a mean proximal VD of 4.0 ± 1.4 mm, the left marginal vein (VMS) of 3.2 ± 1.5 mm and the anterior interventricular vein (VIA) of 3.9 ± 1.3 mm. More distally (5 cm), VDs decreased to 2.4 ± 0.6 mm, 2.3 ± 0.7 mm, and 2.4 ± 0.6 mm, respectively. In their proximal portions (15 mm), veins possessed mean SEAT thicknesses of 3.2 ± 2.4 (VVSP), 3.4 ± 2.4 mm (VMS), and 4.2 ± 2.8 mm (VIA), respectively. More distally (20-70 mm), mean SEAT thicknesses decreased to alternating low levels of 1.3 ± 1.1 mm (VVSP), 1.7 ± 1.1 mm (VMS), and 4.3 ± 2.6 mm (VIA), respectively. In contrast to the VD, SEAT thicknesses alternated along the further distal vein course and did not display a continuous decrease. Besides the CRT responsiveness of different areas of the LV myocardium, SEAT is a relevant electrophysiological factor in CRT, potentially interfering with sensing and pacing. A sufficient VD is crucial for successful CRT lead placement. Measurements revealed a trend toward greater SEAT thickness for the VIA compared to VVSP and VMS, suggesting a superior signal-to-noise-ratio in VVSP and VMS.

PURPOSE: The aim of this study was to review established and emerging techniques of cardiac computed tomography (CT) and their clinical applications with a special emphasis on new techniques, recent trials, and guidelines. TECHNOLOGICAL INNOVATIONS: Cardiac CT has made great strides in recent years to become an ever more robust and safe imaging technique. The improvements in spatial and temporal resolution are equally important as the substantial reduction in radiation exposure, which has been achieved through prospective ECG-triggering, low tube voltage scanning, tube current modulation, and iterative reconstruction techniques. CT-derived fractional flow reserve and CT myocardial perfusion imaging are novel, investigational techniques to assess the hemodynamic significance of coronary stenosis. ESTABLISHED AND EMERGING INDICATIONS: In asymptomatic patients at risk for coronary artery disease, CT coronary artery calcium scoring is useful to assess cardiovascular risk and guide the intensity of risk factor modification. Coronary CT angiography is an excellent noninvasive test to rule out obstructive coronary artery disease in patients with stable chest pain. In acute chest pain with normal ECG and normal cardiac enzymes, cardiac CT can safely rule out acute coronary syndrome although its benefit and role in this indication remains controversial. Cardiac CT is the established standard for planning transcatheter aortic valve implantation and-increasingly-minimally invasive mitral valve procedures. PRACTICAL RECOMMENDATIONS: Our review makes practical recommendations on when and how to perform cardiac CT and provides templates for structured reporting of cardiac CT examinations.

PURPOSE: Ultrahigh-field MRI (UHF-MRI) with an in-plane spatial resolution of less than 100 µm is known as MR microscopy (MRM). MRM provides highly resolved anatomical images and allows quantitative assessment of different tissue types using diffusion-weighted imaging (DWI). The aim of the present study was to evaluate the feasibility of combined in vivo anatomical and quantitative assessment of the developing chicken eye in ovo. PROCEDURES: Thirty-eight fertilized chicken eggs were examined at 7.1 T (ClinScan, Bruker Biospin, Germany) acquiring a dataset comprising T2-weighted anatomical images, DWI, and diffusion tensor imaging. To reduce motion artifacts, the eggs were moderately cooled before and during MR imaging. Two eggs were imaged daily for the entire developmental period, and 36 eggs were examined pairwise at only one time point of the embryonic period. Development of the eye was anatomically and quantitatively assessed. RESULTS: From the D5 embryonic stage (116-124 h), MRM allowed differentiation between lens and vitreous body. The lens core and periphery were first identified at D9. DWI allowed quantification of lens maturation based on a significant decrease in apparent diffusion coefficient values and course of fractional anisotropy. Repeated moderate cooling had no influence on the development of the chicken embryo. CONCLUSIONS: MRM allows in vivo assessment of embryonic development of the chicken eye in ovo without affecting normal development. The method provides anatomical information supplemented by quantitative evaluation of lens development using DWI. With increasing availability of ultrahigh-field MR systems, this technique may provide a noninvasive complementary tool in the field of experimental ophthalmology.

RESUMO

Ultra-high-field MRI (UHF-MRI) is an outstanding technique for non-invasive and non-destructive imaging of soft tissues and can provide versatile contrasts and high resolution in the µm range. In vivo imaging of the embryonal chick eye with its filigree anatomical structures imposes these requirements. However, due to the short embryonal development cycle, chicken are a favourite animal model for embryonal research studies. Ultra-high-field MRI allows repeated and longitudinal in ovo investigations on the same embryo. In the present study, the limitations and opportunities of in ovo MR-imaging at 7 T were evaluated and the process of eye growth was described in detail.

The purposes of this study were (1) to characterize embryonic eye development during incubation in ovo and (2) to analyze the putative influence of repetitive ultrahigh-field MRI (UHF-MRI) measurements on this development. A population of 38 fertilized chicken eggs was divided into two sub-groups: two eggs (Group A) were examined repeatedly during the developmental period from embryonic day 1 (E1) to embryonic day 20 (E20) to evaluate the influence of daily MRI scanning. A second larger group of 36 eggs was examined pairwise on one day only, from E3 to E20, and the embryos were sacrificed immediately after MR imaging (Group B). Fast T2-weighted MR sequences provided biometric data on the eye with an in-plane resolution of 74 µm. The data show rapid growth of the eye with a steep increase in intraocular dimensions in all axis directions and in eyeball volume during initial development up to E10, followed by a phase of reduced growth rate in later developmental stages. Comparison of the two groups revealed no differences in ocular development.
