PURPOSE: This study analyzes the initial experience with minimally invasive mitral valve surgery through a right minithoracotomy in a public teaching hospital in Australia and evaluates early surgical outcomes. METHODS: A retrospective review of patients who underwent minimally invasive mitral valve surgery between November 2006 and March 2009 was performed. RESULTS: A total of 60 patients included 47 (78%) patients who had mitral valve plasty and 13 (22%) who had mitral valve replacement. The mean age was 61 ± 15 years; 33 (55%) patients were male; and 6 (10%) had had previous cardiac operations. The mean cardiopulmonary bypass and aortic cross-clamp times were 140 ± 46 and 93 ± 35 min, respectively. All patients who underwent mitral valve plasty left the operation room with no more than trivial residual mitral regurgitation. There was no operative mortality. Reoperation for bleeding and stroke occurred in 2 patients each. The mean intensive care unit and hospital stays were 3.1 ± 5.8 and 10.6 ± 8.9 days, respectively. Among the 47 patients with mitral valve plasty, 46 (98%) had mild or less mitral regurgitation on transthoracic echocardiography at discharge. There was one late death. No reoperation for the mitral valve has been observed so far. An echocardiography report was obtained for 34 of the 47 who had had mitral valve plasty at 12.1 ± 7.9 months postoperatively, and 27 (79%) of them had mild or less mitral regurgitation. CONCLUSION: Minimally invasive mitral valve surgery through a right minithoracotomy was safely performed with no early mortality.

RESUMO

Severe bacterial aortitis without an aneurysmal component is a rare but life-threatening problem that requires aggressive treatment to eliminate the infection and prevent recurrence. Herein, we present the case of a 58-year-old man who underwent patch repair of a nonaneurysmal aorta that had ruptured due to Staphylococcus aureus infection. Postoperatively, he experienced a recurrent rupture that required reoperation. We successfully performed wide-margin débridement followed by aortic arch replacement with a prosthetic vascular graft and omental flap.

BACKGROUND: Increased fish or fish-oil consumption is associated with reduced risk of cardiac mortality, especially sudden death. This benefit putatively arises from the incorporation of the long-chain n-3 fatty acids eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA) into cardiomyocyte phospholipids. OBJECTIVE: The study examined the kinetics of incorporation of n-3 fatty acids into human myocardial membrane phospholipids during supplementation with fish oil and alpha-linolenic acid-rich flaxseed oil. DESIGN: Patients with low self-reported fish intake (<1 fish meal/wk and no oil supplements) accepted for elective cardiac surgery involving cardiopulmonary bypass were randomly allocated to 1 of 6 groups: no supplement; fish oil (6 g EPA+DHA/d) for either 7, 14, or 21 d before surgery; flaxseed oil; or olive oil (both 10 mL/d for 21 d before surgery). Right atrial appendage tissue removed during surgery and blood collected at enrollment and before surgery were analyzed for phospholipid fatty acids. RESULTS: Surgery rescheduling resulted in a range of treatment times from 7 to 118 d. In the fish-oil-treated subjects, accumulation of EPA and DHA in the right atrium was curvilinear with time and reached a maximum at approximately 30 d of treatment and displaced mainly arachidonic acid. Flaxseed oil supplementation yielded a small increase in atrial EPA but not DHA, whereas olive oil did not significantly change atrial n-3 fatty acids. CONCLUSION: The results of the present study show that dietary n-3 fatty acids are rapidly incorporated into human myocardial phospholipids at the expense of arachidonic acid during high-dose fish-oil supplementation.

RESUMO

Bronchopleural fistula, one of the most serious complications following pneumonectomy, has a complicated treatment protocol and carries a high mortality rate. We present a case report of a 75-year-old female with squamous cell carcinoma of the lower lobe with positive peribronchial hilar nodes who underwent a right pneumonectomy. She represented with a large bronchopleural fistula one month postoperatively, eventually treated by a novel tracheobronchial stenting procedure.

OBJECTIVE: The objective of this study was to examine the psychological risk factors for cardiac-related readmission within 6 months of coronary artery bypass graft surgery (CABG). METHODS: Consecutive patients awaiting elective CABG (N=119; 100 males and 19 females), with a mean age of 63.3 years, completed a battery of psychosocial measures in a three-stage repeated-measures design. Relevant medical data were also extracted from patients' medical records 6 months postoperatively to allow for the examination of potential covariates. RESULTS: Two psychological variables, increased postoperative anxiety and increased preoperative depression, were identified as risk factors for cardiac-related readmission independent of the only significant covariate identified, cardiopulmonary bypass time. CONCLUSION: Anxiety in the immediate postoperative period and, to a lesser extent, preoperative depression are important determinants of health care utilization postdischarge. Further research to clarify the psychological factors that are predictive of readmission, and that attempt to determine both the underlying cause of readmissions and potential mechanisms through which psychological factors act is recommended. Such research may highlight potential factors to target in interventions and the best time at which to intervene.

To date, researchers have examined the role of psychological factors in longer-term adaptation to coronary artery bypass graft surgery (CABG), but few have investigated the role of such factors in the immediate post-operative period. Thus, the current study examined psychological risk factors for increased post-operative length of hospital stay in 119 consecutive elective CABG patients (100 men and 19 women). When controlling for operative/post-operative factors, medical factors accounted for 24.5% of the variance. However, pre-operative psychological factors accounted for a further 4.4% of the variance, with increased depression and lower PTSD symptomatology identified as significant independent risk factors for longer post-operative length of hospital stay. Hence, while post-operative length of hospital stay is largely determined by medical factors, psychological factors also influence this outcome. Further research is required to replicate the current findings and to determine the mechanisms through which these variables may act.

RESUMO

A 57-year-old Caucasian woman presented with nonproductive cough. Computed tomography revealed a peripheral solid mass in the upper lobe of the left lung. She underwent thoracotomy and upper lobectomy. Histology of the excised tumor demonstrated lymphoepithelioma-like carcinoma of the lung, with no associated Epstein-Barr virus activity. Being a rare entity and mostly seen in Asians, very few cases have been described previously.

RESUMO

We present an extremely rare case of an immunocompromised patient with a T-cell acute lymphocytic leukemia relapse presenting as a right atrial tumor. Problems in diagnosis, vulnerability due to previous immunosuppression and bone marrow transplant, and successful surgical excision are highlighted. Cardiac involvement with hematologic neoplasms should be taken with more than academic interest, as it may be amenable to treatment.
