Background: People usually spend most of their time indoors, so indoor fine particulate matter (PM2.5) concentrations are crucial for refining individual PM2.5 exposure evaluation. The development of indoor PM2.5 concentration prediction models is essential for the health risk assessment of PM2.5 in epidemiological studies involving large populations. Methods: In this study, based on the monitoring data of multiple types of places, the classical multiple linear regression (MLR) method and random forest regression (RFR) algorithm of machine learning were used to develop hourly average indoor PM2.5 concentration prediction models. Indoor PM2.5 concentration data, which included 11,712 records from five types of places, were obtained by on-site monitoring. Moreover, the potential predictor variable data were derived from outdoor monitoring stations and meteorological databases. A ten-fold cross-validation was conducted to examine the performance of all proposed models. Results: The final predictor variables incorporated in the MLR model were outdoor PM2.5 concentration, type of place, season, wind direction, surface wind speed, hour, precipitation, air pressure, and relative humidity. The ten-fold cross-validation results indicated that both models constructed had good predictive performance, with the determination coefficients (R2) of RFR and MLR were 72.20 and 60.35%, respectively. Generally, the RFR model had better predictive performance than the MLR model (RFR model developed using the same predictor variables as the MLR model, R2 = 71.86%). In terms of predictors, the importance results of predictor variables for both types of models suggested that outdoor PM2.5 concentration, type of place, season, hour, wind direction, and surface wind speed were the most important predictor variables. Conclusion: In this research, hourly average indoor PM2.5 concentration prediction models based on multiple types of places were developed for the first time. Both the MLR and RFR models based on easily accessible indicators displayed promising predictive performance, in which the machine learning domain RFR model outperformed the classical MLR model, and this result suggests the potential application of RFR algorithms for indoor air pollutant concentration prediction.

Background: Decabromodiphenyl ether (BDE209), an essential industrial flame retardant that is widely used, has recently been reported to be increasing in human serum. Due to the structural similarity between BDE209 and thyroid hormones, its toxic effects on the thyroid are of particular concern. Methods: Original articles in the PubMed database were collected using the terms "BDE209", "decabromodiphenyl ether", "endocrine disrupting", "thyroid", "carcinogenesis", "polybrominated diphenyl ethers", "PBDEs," and their synonyms from inception up to October of 2022. Results: Of the 748 studies initially identified, 45 were selected, which emphasized the adverse effects of BDE209 on endocrine system. BDE209 may have a toxic effect not only on thyroid function but also on thyroid cancer tumorigenesis at multiple levels, such as by directly interfering with the TR, hypothalamic-pituitary-thyroid (HPT) axis, enzyme activity, and methylation. However, it is impossible to draw a definitive conclusion on the exact pathway of thyroid toxicity from BDE209. Conclusions: Although the toxic effects of BDE209 on the thyroid have been well investigated, its tumorigenic effects remain unclear and further research is necessary.

Polybrominated diphenyl ethers (PBDEs) have been reported to possess endocrine-disrupting and tumour-promoting activity. However, the effects of long-term exposure to decabromodiphenyl ether (BDE209) on thyroid tumourigenesis of papillary thyroid carcinoma (PTC) and the underlying mechanisms remain poorly defined. In this study, functional assays in vitro and mouse models in vivo were used to evaluate the toxic effects of long-term exposure to environmental concentrations of BDE209 on the pathogenesis and progression of PTC. MTS, EdU and colony-forming assays confirmed the chronic toxicity of BDE209 on the proliferation of human normal follicular epithelial cell line (Nthy-ori 3-1) and PTC-derived cell lines (TPC-1 and BCPAP). Wound and Transwell assays showed that BDE209 exacerbated the aggressiveness of PTC cells. BDE209 significantly promoted cell proliferation during the S and G2/M phases of the cell cycle. Mechanistically, BDE209 altered the thyroid system by acting as a competitive inhibitor of thyroid receptor beta (TRß) expression and function, which was further proven by public databases and RNA-seq bioinformation analysis. Taken together, these results demonstrated that BDE209 has chronic toxicity and potential tumourigenic effects on the thyroid by inhibiting TRß.

The aim of the study was to analyze the temporal trends, pollution sources, and carcinogenic health risks associated with PM2.5-bound arsenic (As). A total of 588 PM2.5 samples were collected in Jinan during January 2014-December 2020. The content and distribution characteristics were determined for As and Al in PM2.5, and the pollution sources were identified based on enrichment factors (EFs). The health risk of inhalation exposure to As was estimated using the risk assessment methods recommended by the United States Environmental Protection Agency (US EPA). The annual average concentration of As in PM2.5 was 4.5-17.5 ng m-3, which was 0.8-2.9 times higher than the limit ruled by the European Union and China's Ambient Air Quality Standards (6 ng m-3). From 2014 to 2020, the As concentration gradually decreased from 17.5 to 4.9 ng m-3. After 2017, the concentration was close to the level required by the atmospheric quality standard (6 ng m-3). The PM2.5 and arsenic concentrations in the heating season were significantly higher than those in the non-heating season. The EF of As ranged from 144 to 607, which was higher than 10. The cancer risk of As in PM2.5 decreased to the lowest values (heating season 1.0 × 10-5 and non-heating season 7.1 × 10-6, respectively) in 2019. As in Jinan mainly came from anthropogenic pollution. The level of As pollution has been significantly reduced in recent years, but there is still a high risk of carcinogenesis. Air pollution control strategies and guidelines need to be implemented in urban areas, especially during the heating season in winter and spring.

BACKGROUND: Thyroid fine-needle aspiration biopsy (FNAB) is not well developed in most of the large academic hospitals in China, including Qilu Hospital of Shandong University. Ultrasound-guided FNAB (UG-FNAB) was initiated as a pre-surgical diagnostic method in 2015 in Qilu Hospital, and the current study evaluates its impact on the number of thyroid surgeries and the ratio of malignancy (ROM) in surgically resected cases, comparing the post and pre-UG-FNAB periods. METHODS: Thyroid surgery and UG-FNAB data for the study period (Jan 2013 to Dec 2018) were collected from the hospital information system (HIS). A natural experimental approach using segmented linear regression was performed on the interrupted time-series (ITS) data to estimate the level-change of the number and the trend-change of ROM of surgically resected cases following the implementation of UG-FNAB diagnosis. RESULTS: The number of UG-FNAB cases in Qilu Hospital increased rapidly from 1,367 in 2015 to 3,402 in 2018, with an average annual increase of 678 cases. Comparing the pre-UG-FNAB and post-UG-FNAB periods, the overall ROM of thyroid resections increased from 58.7% to 73.2% (P<0.01). Due to the implementation of UG-FNAB, the regression line of surgical cases showed an inflection point in March 2015, and the slope of the regression line of post-intervention was lower than that of pre-intervention. For each additional month, the ROM in thyroid surgical cases increased by 0.389% (P<0.01). After excluding the influence of time, the ROM in surgically resected thyroid cases increased by 4.781% due to the application of the UG-FNAB examination (P<0.01). CONCLUSIONS: This successful implementation of UG-FNAB further improved overall ROMs in surgically resected cases particularly as we acquired further experience with the procedure and reduced unnecessary diagnostic surgeries for patients with benign and borderline lesions.

PM2.5 samples were collected at the Wangsheren primary school site in Licheng District of Jinan, China, during 2016. Eleven metals and metalloids including Al, As, Be, Cd, Cr, Hg, Mn, Ni, Pb, Sb and Se in PM2.5 were measured by inductively coupled plasma-mass spectroscopy. The annual average mass concentration of PM2.5 was found to be 88.7 µg m-3. The highest PM2.5 concentrations were obtained during the heating seasons of winter and spring. The concentrations of metals and metalloids in PM2.5 were in a descending order of Al, Pb, Mn, As, Se, Cr, Sb, Ni, Cd, Hg and Be. The enrichment factors showed that Pb, Mn, As, Se, Cr, Sb, Ni, Cd, Hg originated from anthropogenic sources. Factor analysis indicated that the main sources of the metals were coal combustion dust, soil dust, metallurgical industry, brake abrasion of vehicles and other mixed sources. Coal combustion dust was the primary source of metal pollution in PM2.5. Non-carcinogenic risks associated with exposure through the respiratory system were between 6.30 × 10-4 and 7.62 × 10-1, which were lower than the safe limit (1). The carcinogenic risks of Cr, As and Cd were 3.17 × 10-5, 1.52 × 10-5, 2.22 × 10-6, respectively, which were higher than the precautionary criterion (10-6/year). This study indicates that the air pollution of PM2.5 is of public health concern in Licheng District of Jinan, particularly related to potential carcinogenic metals of As, Cr and Cd. Intervention action is needed to reduce the emission sources of these elements, especially coal combustion in winter heating season.

Polybrominated diphenyl ethers (PBDEs), which are persistent organic pollutants, affect thyroid function. Human exposure to decabromodiphenyl ethane (DBDPE), which has a similar structure to PBDEs, has recently increased, and the health effects of DBDPE have not been well studied. The objective of this study was to determine whether human exposure to DBDPE was associated with thyroid hormone levels in adults from a DBDPE manufacturing area. Three hundred-two blood samples were collected from two populations in the largest DBDPE manufacturing area located in North China: 133 DBDPE occupationally exposed workers from a DBDPE manufacturing plant and 169 non-DBDPE occupationally exposed residents from a nearby food processing plant. The levels of DBDPE, and thyroid function parameters [total thyroxine (TT4), free T4 (FT4), total triiodothyronine (TT3), free T3 (FT3), thyroid-stimulating-hormone (TSH), thyroglobulin antibody (TG-Ab), and thyroid peroxidase antibody (TPO-Ab)] were measured in serum samples. Serum concentrations of DBDPE ranged from 3.148 to 54,360 ng g-1 lipid weight (lw), with a geometric mean of 332.6 ng g-1 lw. A 10-fold increase in the DBDPE concentration was associated with increase of 4.73 nmol L-1 [95% confidence interval (CI): 2.75, 6.71] TT4 and 0.046 nmol L-1 TT3 [95% CI: 0.012, 0.081], corresponding to increases of approximately of 4.73% (95% CI: 2.75%-6.71%) and 2.38% (95% CI: 0.62%-4.20%), respectively. DBDPE in serum was also significantly and positively associated with the concentrations of TG-Ab and TPO-Ab. Our study found that exposure to DBDPE was associated with changes in thyroid activity in adults exposed to a high concentration of DBDPE, mainly increases of TT4, TT3, TPO-Ab, and TG-Ab. The association between DBDPE exposure and thyroid homeostasis requires further investigation because increasing DBDPE exposure has emerged in recent years.

Field investigations were conducted on a decabrominated diphenyl ether (BDE-209) manufacturing plant and a decabromodiphenyl ethane (DBDPE) manufacturing plant, and worker exposure to polybrominated diphenyl ethers (PBDEs) or DBDPE was assessed. Workshop air was collected and tested to measure levels of external exposure to corresponding chemicals via air inhalation. Paired human serum and urine samples taken from 202 workers were tested to assess levels of internal BFR exposure. Levels of BDE-209 in air for the BDE-209 plant ranged from 10.6 to 295 µg m-3, accounting for at least 99% of the total PBDEs in the workshop air, and those of DBDPE in air samples from the DBDPE plant ranged from 12.7 to 435 µg m-3. Maximum estimated daily intakes (EDIs) of BDE-209 and DBDPE accumulated via air inhalation exceeded the corresponding RfD level recommended by the U.S. Environmental Protection Agency, indicating that for some workers, occupational exposure is likely to create significant health problems. The levels of BDE-209 measured in serum taken from BDE-209 workers ranged from 0.202 to 57.1 µg g-1 lw, and those of DBDPE in serum taken from DBDPE workers ranged from 0.087 to 54.4 µg g-1 lw. These values are several orders of magnitude higher than those detected in general areas and e-waste recycling sites. BDE-209 and DBDPE were detected in all of the urine samples with median levels of 1.12 and 8.6 ng mL-1, respectively, and levels of BDE-209 and DBDPE in the urine were significantly and positively correlated with those observed in the serum.

Papillary thyroid carcinoma (PTC) is the most commonly diagnosed endocrine cancer, and those with BRAFV600E mutation have high recurrence rate and less favorable clinical behavior. Genistein having anti-carcinoma effects in various types of carcinomas as an estrogen analog, but the mechanism of Genistein in the progression of PTC remains unknown. Genistein significantly inhibits the proliferation and the invasion (P < 0.01), and the apoptosis (P < 0.001) of all tumor cell lines, which was probably due to the inducing of the arrest in G2/M phase of the cell cycle (P < 0.001). The anti-proliferation and apoptosis inducing effects are more obvious in BCPAP, IHH4 cell lines harboring BRAFV600E mutation. Genistein significantly decreased the invasion of PTC cell lines and partially reverses epithelial mesenchymal transition in PTC cell lines. Functional study indicated that small interfering RNA (siRNA) knockdown of ß-catenin significantly reverses the effect of genistein on EMT at protein levels. In conclusion, for the first time, our study suggested that genistein has anticarcinoma effect for PTC patients in the range of 2.5 and 80 µg/ml in thyroid carcinoma cells, which was probably through cytoplasmic translocation of ß-catenin. Further study will be needed to determine whether genistein could be used in clinical trial of high-risk PTC.

To evaluate the current diagnostic criteria in reporting nuclear features of noninvasive follicular thyroid neoplasm with papillary-like nuclear features (NIFTP), nine Asian pathologists with expertise in thyroid reviewed virtual slides of 30 noninvasive follicular patterned thyroid lesions according to the nuclear scoring system originally proposed by an international expert and a more detailed scoring system proposed by the Asian Working Group. The interobserver agreement for nuclear grading score was generally moderate (kappa value = 0.452). The best consistency fell on the chromatin features (kappa value = 0.658-1.000). A fair to moderate interobserver agreement was demonstrated in the evaluation of nuclear elongation, nuclear overlapping, membrane irregularities and distribution of papillary thyroid carcinoma (PTC) type nuclear features. A slight agreement was rendered for the evaluation of the nuclear enlargement. Intraobserver agreement was substantial to perfect when comparing results of both scoring systems. However, both scoring systems were not able to reliably separate NIFTP from an encapsulated follicular variant PTC with minimal lymph node metastasis or BRAFV600E mutation. Although the three-point nuclear scoring system for the diagnosis of NIFTP is widely used in Asian practice, interobserver variation was considerable. Ancillary immunohistochemical or molecular testing might be helpful in differentiating NIFTP from true PTC.

Columnar cell variant of papillary thyroid carcinoma (CCV-PTC) is an unusual neoplasm, the clinical behavior of which mainly depends on the encapsulation or infiltration. Patients with extensive extrathyroidal extension usually have an aggressive biological behavior. This study confirmed that beta-human chorionic gonadotropin (ß-hCG) secreting invasive CCV-PTC has good prognosis comparing with a cohort of follicular cell differentiated thyroid carcinoma. On the contrary, positive immunoreaction with ß-hCG was proved in three anaplastic thyroid carcinoma patients showing aggressive clinical courses. The clinicopathologic characteristics of CCV-PTC and the paraneoplastic syndromes in follicular cell differentiated thyroid carcinoma were further summarized using literature review.

Alcoholic liver disease (ALD) is a consequence of heavy and prolonged alcohol consumptions. We previously demonstrated a hepatic gamma-aminobutyric acid (GABA) signaling system that protects the liver from toxic injury. The present study was designed to investigate the role of the hepatic GABA signaling system in the process of acute ethanol exposure-induced liver injury. Our results showed that the expression of GABA synthesizing enzyme glutamic acid decarboxylase and type A GABA receptor (GABAA R) subunits was upregulated in ethanol-treated mice compared with saline-treated controls. Remarkably, pretreatment of mice with GABA (1.5 mg kg-1 body weight, intraperitoneal injection [i.p.]) or with the GABAA R agonist muscimol (1.2 mg kg-1 body weight, i.p.) protected the liver against ethanol toxicity and improved liver function, whereas pretreatment of mice with the GABAA R antagonist bicuculline (2.0 mg kg-1 body weight, i.p.) worsened the liver function. Further analyses suggest that GABAA R-mediated signaling protects the liver from ethanol injury by, at least partially, inhibiting the IRE1α-ASK1-JNK pro-apoptotic pathway in hepatocytes in the process of ethanol-induced endoplasmic reticulum stress response.

OBJECTIVE: Risk assessment and risk control for occupational exposure to chemical toxicants were performed on an isophorone nitrile device with an annual production of 5,000 tons, based on improved Singaporean semi-quantitative risk assessment method, with consideration of actual situation in China and in the present project. METHODS: With the use of engineering analysis and identification of occupational hazards in the improved Singaporean semi-quantitative risk assessment method, hazard rating (HR) and risk assessment were performed on chemical toxicants from an isophorone nitrile device with an annual production of 5,000 tons. RESULTS: The chemical toxicants in the isophorone nitrile device were mainly isophorone, hydrocyanic acid, methanol, phosphoric acid, sodium hydroxide, and sodium cyanide; the HR values were mild hazard (2), extreme hazard (5), mild hazard (2), mild hazard (2), moderate hazard (3), and extreme hazard (5), respectively, and the corresponding exposure rating (ER) values were 2.09, 2.72, 2.76, 1.68, 2.0, and 1.59, respectively. The risk of chemical toxicants in this project was assessed according to the formula Risk = [HR×ER](1/2). Hydrocyanic acid was determined as high risk, sodium hydroxide and sodium cyanide as medium risk, and isophorone, methanol, and phosphoric acid as low risk. Priority in handling of risks was determined by risk rating. The table of risk control measure was established for pre-assessment of occupational hazards. CONCLUSION: With risk assessment in this study, we concluded that the isophorone nitrile device with 5,000 ton annual production was a high-occupational hazard device. This device is a project of extreme occupational hazard. The improved Singaporean semi-quantitative risk assessment method is a scientific and applicable method, and is especially suitable for pre-evaluation of on-site project with no analogy.

OBJECTIVE: To investigate the occupational exposure levels of dust in new suspension preheated dry process (NSP) cement production line and put forward rectification measures for dust-exposed posts, and to provide ideas for the modern cement production enterprises in dust control and occupational health management. METHODS: Occupational health field investigation combined with field test were used to measure the time-weighted average concentration (C(TWA)) of the dust in the workplace. Rectification measures were taken for the dust-exposed posts with unqualified dust concentration, and the protective effects of dustproof facilities in the rectified workplace were evaluated. RESULTS: The field investigation revealed incompletely closed dustproof facilities, improperly set dust hoods, excess of dust leakage points, and other problems in the dust-exposed posts of an NSP cement production line before rectification, and the dustproof facilities could hardly exert dust removal effect. The field test showed that the vast majority of dust-exposed posts had the dust concentrations exceeding the occupational exposure limits (OELs), with a qualified rate as low as 31.8%. A series of rectification measures were taken for these posts. After the rectification, the dust-exposed posts demonstrated dramatically dropped C(TWA), and the qualified rate of dust concentration in the dust-exposed posts rose to 90.9%. CONCLUSION: The dust hazards in NSP cement production line cannot be ignored. Taking appropriate protective measures are critical for curbing dust hazards in modern cement production.
