RESUMO

The recognition-directed host-guest interaction is recognized as a valuable tool for creating supramolecular polymers. Functional hydrogels constructed through the dynamic and reversible host-guest complexation are endowed with a great many appealing features, such as superior self-healing, injectability, flexibility, stimuli-responsiveness and biocompatibility, which are crucial for biological and medicinal applications. With numerous topological structures and host-guest combinations established previously, recent breakthroughs in this area mostly focus on further improvement and fine-tuning of various properties for practical utilizations. The current contribution provides a comprehensive overview of the latest developments in host-guest supramolecular hydrogels, with a particular emphasis on the innovative molecular-level design strategies and hydrogel formation methodologies targeting at a wide range of active biomedical domains, including drug delivery, 3D printing, wound healing, tissue engineering, artificial actuators, biosensors, etc. Furthermore, a brief conclusion and discussion on the steps forward to bring these smart hydrogels to clinical practice is also presented.

INTRODUCTION: Cancers that originate from the upper aerodigestive tract are collectively known as head and neck cancer. The most common are squamous cell carcinomas of the oropharynx, larynx, and oral cavity. Head and neck cancer patients experience significant physical and psychological changes because of the disease and treatment. There is a substantial strain on family carers who have extensive responsibilities over most aspects of the patient's life. The aim of the study was to understand the perspectives of being an HNC carer and their perceived expectations of the role. METHODS: The study adopted a qualitative research design with a social constructionist epistemology. Interviews were conducted with 20 carers who were currently caring for someone diagnosed with head and neck cancer. RESULTS: Three overarching themes were identified: taking responsibility for the patient's nutrition, navigating a new and different relationship, and "my life's not my own." Participants felt responsible for ensuring the patient was eating and became increasingly frustrated when the patient was unable to intake food. Carers prioritised the patients' needs at the expense of their own, and several came to resent the role. CONCLUSION: Carers' expectations of their role informed how they approached giving care. Carers need to be supported from diagnosis and encouraged to prioritise their own wellbeing.

RESUMO

Nanogels are robust nanoparticles that could be used to deliver active drug compounds in controlled drug delivery applications. This review discusses the design, synthesis, loading, and release of local anesthetics using polymeric nanoparticles produced via various types of polymerization techniques. The strategy of using layer-by-layer approach to control the burst release of procaine hydrochloride (PrHy; a local anesthetic drug of the amino ester group) is described and discussed.

Improvements in treatment for acute burn edema have stalled in comparison with other areas of burn care. Designing acute edema treatment studies in humans is hindered by the lack of objective, sensitive methods of measurement in the burn population. Bioimpedence spectroscopy (BIS) is a noninvasive method of measuring fluid volumes in the body. The aim of this study was to examine the reliability and sensitivity of BIS in the measurement of acute edema shifts in burn survivors, including assessment across different wound conditions. BIS measurements were collected in triplicate from 21 burn patients in total. Phase I (n = 13) examined BIS under three different dressing conditions. Phase II (n = 8) considered only patients with dressings intact. Sensitivity (minimum detectable difference [MDD]) was examined with total body water, extracellular fluid (ECF), and intracellular fluid volume measurements. BIS demonstrated excellent reliability across all dressings conditions, including when open wounds were present (intraclass correlation coefficient = 0.975-1.00, 95% confidence intervals = 0.938-1.00). Clinically useful levels of sensitivity, or MDD, were calculated. BIS MDD for total body water (open wounds) = 360 ml, ECF (open wounds) = < or = 10 ml, ECF (new dressings) = 540 ml, and intracellular fluid (open wounds) = 310 ml. BIS analysis is clinically applicable for real-time, noninvasive monitoring of whole-body fluid shifts in acute burn survivors with < or = 30% TBSA.
