RESUMO

Testicular cancer is one of the most curable cancers. However, the course of the disease largely depends on the clinical stage at diagnosis, and there are still cases where the tumor size is large, which makes surgical treatment challenging. A 30-year-old man presented with painless, extremely enlarged scrotum. A CT scan revealed a tumor of the right testis of 21.5 × 15 × 18cm in size. The patient underwent a right orchiectomy and histologic examination revealed teratocarcinoma. Suspicion of hydrocele testis should prompt meticulous differential diagnosis including malignancies. There is a strong need to increase public awareness in terms of symptoms of testicular cancer.

A solitary fibrous tumor develops from mesenchymal cells as a pleural neoplasm, but it is also occasionally reported in extrapleural sites. Retroperitoneal tumors are a group of neoplasms located between muscles and the fascia of the posterior abdominal wall and the parietal peritoneum. Their cytology differs from that of urinary tract organs or adrenals. This case report presents a rare solitary fibrous tumor incidentally found during an ultrasound examination. A 54-year-old male underwent urgent surgery for a tumor located in the left retroperitoneal space. The histologic examination confirmed a solitary fibrous tumor with a 5% Ki67 proliferation index, a 1 MF/10 HPF mitotic activity, and CD34-positive immunostains. A solitary fibrous tumor is a rare retroperitoneal tumor. Its symptoms and signs might resemble those of the classical triad of renal cell carcinoma, although the tumor's growth phase is typically asymptomatic. Intraoperative diagnosis of a solitary fibrous tumor strongly recommends radical excision.

Premature ejaculation is the most common sexual dysfunction in men. Its prevalence rate in Europe and in United States is estimated to be between 20% and 30%. The diagnosis of premature ejaculation is based on three main criteria: increased intravaginal ejaculatory latency time (IELT), lack of control over ejaculation and interpersonal psychological disturbances. Premature ejaculation is classified as lifelong (primary) or acquired (secondary) and might be facilitated by chronic prostatitis, diabetes mellitus, hyperthyroidism, obesity. The exact etiology of the disease remains unclear, although 5-HT (5-hydroxytryptamine) receptors are known to have a significant role. The use of SSRIs (selective serotonine reuptake inhibitors) is old and efficient form of therapy for premature ejaculation. Other drugs like tramadol, clomipramine, local anaesthetics and PDE-5 (phosphodiesterase 5) inhibitors also have some efficacy in the treatment of premature ejaculation. To minimize adverse effects the "on demand" therapy is preferred to the daily treatment. Simple questionnaires for patients are used to assess treatment effects.

UNLABELLED: Defunctioning stomas (ileostomy and transversostomy) are the most common methods of the decompression of distal digestive tract. The aim of this study was to assess the results of transverse loop colostomy in colorectal cancer patients. MATERIAL AND METHOD: Between 1998 and 2007, 119 patients with colorectal cancer underwent transversostomy. In 97 patients the colostomy was formed during the primary operation as a planned prevention from further complications (1st group) and in 22 previously operated patients due to postoperative complications as a second procedure (2nd group). RESULTS: In the study group, there were 2 deaths in the perioperative period due to the anastomosis leakage and the intraabdominal sepsis. Three patients required reconstructive operation because of the colostomy stenosis and further 3 due to colostomy prolapse. There were also several cases of superficial wound infection after termination of transversostomy. Almost 78 patients in total had the restoration of digestive tract continuity performed, prevalently from the 1st group. CONCLUSIONS: 1. In our opinion transversostomy is an effective solution for the proximal faecal diversion especially at high risk of anastomosis insufficiency or with present anastomosis leak. 2. In our study group, the incidence of complications (such as: chyme loss or dyselectrolytemia) in patients with performed transversostomy was low, thus we find it recommendable for the colon decompression.
