RESUMO

The effect of ispaghula husk on colonic motility of the right and left side was examined in 10 patients with left sided diverticular disease using an untethered pressure sensitive radiotelemetry capsule. After treatment, ispaghula husk reduced mouth to rectum transit by a median of 8.8 hours and the time to midtransverse colon by five hours. In the right colon there was an increase in the median percentage activity of 7% and the median number of pressure waves greater than 5 mm Hg/hour rose by 35.3. Motility changes in the left colon were less pronounced. Five of the seven patients with abdominal pain and six of the nine patients with altered bowel habit responded to treatment. These results suggest that it is ispaghula husk's action on the right unaffected colon which alleviates the symptoms of left sided diverticular disease.

RESUMO

A new radiotelemetry receiver type 7060 for use with radiopills is described. Its performance under laboratory conditions was assessed and its use for clinical colonic pressure measurements evaluated. The receiver, which resembles a small microcomputer, was easy to set up and worked reasonably well under ward conditions. Difficulties were experienced with the belt aerial supplied with the receiver, which did not reliably pick up signals from a freely moving radiopill. The receiver is not suitable for ambulatory applications. Its main clinical use is in situations where frequent sampling of the radiopill's signal over a long period of time is essential (e.g. colonic pressures). The large quantity of data generated can be fed from the receiver's analogue or digital output on line to a recorder.

Little is known about congenital viral and toxoplasmal infections in the developing countries. This study attempts to provide preliminary data on such infections among Arab mothers and infants in Kuwait. Babies born with an elevated IgM in cord blood were selected at birth and then studied prospectively during the first year of life for serological and clinical evidence of viral and toxoplasmal infections. There appeared to be a significant association between the elevation of total cord blood IgM and the selection of cases with cytomegalovirus infection. Demonstration of specific IgM in cord sera was attempted, whenever possible, to provide further evidence of congenital infection and to exclude early postnatal infections. In 18.5% of cases there was an association between serological evidence of infection and certain clinical abnormalities. Apart from cytomegalovirus and rubella virus, no evidence was found for congenital infection by other viruses or toxoplasma in this study.

Sixty-six formalin fixed, paraffin-embedded reactive and neoplastic lymph node biopsies were examined by the immunoperoxidase (PAP) procedure to localize toxoplasma antigen. 13:16 (81.25%) of the histologically suggestive toxoplasmic lymphadenitis and 4:8 (50%) of Hodgkin's lymphoma with histiocytic (epitheloid) cell infiltrates were positive. All other reactive and neoplastic biopsies were negative. Our data prove the usefulness of the PAP procedure as a specific and sensitive test to confirm the diagnosis of histologically suspicious toxoplasmosis. This study, however, is the first of its kind that demonstrates immunohistologically the association of toxoplasmosis with Hodgkin's lymphoma in the same lymph nodes. It also calls in question the nature of the histiocytes in lymphoid neoplasms and suggests the possibility that, at least in some lymphomas especially Hodgkin's, the histiocystic component may be due to the association of other conditions which are worth of further investigation.

Five hundred and seventy three of 606 (94.6%), 583 of 604 (96.5%) and 575 of 595 (96.6%) women were found to have antibody to rubella virus, cytomegalovirus and herpes simplex virus, respectively. The Geometric Mean Titre (GMT) of haemagglutination-inhibition antibody to rubella virus was 60.97, while the GMT of complement-fixation antibody to cytomegalovirus and herpes simplex virus were 28.49 and 25.01, respectively. Furthermore, 301 of 517 (58.2%) women were also found to possess antibody to toxoplasma (GMT = 10.12) as detected by the passive-haemagglutination test. No significant differences in antibody prevalence or GMT could be found among the various nationalities studied except among Bedouins who showed a significantly higher rate and GMT of antibody to rubella virus and for the Mediterranean Arabs who showed significantly higher rate of immunity to and GMT of toxoplasma antibody when compared to other nationalities. Finally, no significant differences in antibody prevalence or GMT could be found among the various age groups.

RESUMO

A survey of brown rats (Rattus norvegicus) was made in Kuwait in 1979 for the presence of Leptospira organisms. Kidney tissue from 49 rats, trapped mostly from various Kuwait City districts, were cultured on E.M.J.H. and Stuart media. Eight leptospira strains were isolated; all strains were identical and belonged to Leptospira interrogans Canicola serogroup; they were later identified as a new serovar kuwait. The frequency of L. interrogans group Canicola carriers among the local rats in Kuwait was 16.3%, which is higher than reported so far. A summary of published data on the isolation of Leptospira other than L. interrogans serogroup Icterohaemorrhagiae from rats, particularly the brown rat, is presented with emphasis on the Middle East.
