RESUMO

Solitary fibrous tumor (SFT) rarely occurs in the kidneys, and only one reported case of renal SFT has shown distant metastasis. We report the second case of renal SFT exhibiting distant metastasis. A 48-year-old man was referred to our hospital because of a right renal mass. An abdominal CT scan detected a large renal tumor, which was suspected to be a renal cell carcinoma. Right radical nephrectomy was performed, and the tumor was found to measure 28 × 18 × 10 cm. The pathological diagnosis was benign solitary fibrous tumor of the kidney. Eight years after the operation, lung and liver metastases developed. Pulmonary segmentectomy and partial hepatectomy were performed. The pathological diagnoses of these resected tissue specimens were compatible with benign SFT.

Purpose. To evaluate the educational effect of the Japanese Society of Interventional Radiology 7th Academic Summer Seminar from a technical perspective. Materials and Methods. Nineteen trainees participated in the seminar. The seminar consisted of vertebroplasty trainings using swine with the single-plane landmark method and with the ISOcenter Puncture (ISOP) method. All trainees were advised by an instructor as they operated the instruments and punctured the vertebra. For each trainee, the accuracy in the final position of the needle tip of the initial puncture in each swine training was evaluated. Results. Error in the final position of the needle tip of ≥5 mm from the target puncture site occurred in the lateral direction in 42% (8/19) of trainees with the landmark method and 5% (1/19) with the ISOP method. No error ≥5 mm occurred in the vertical or anteroposterior directions. In terms of puncture accuracy, error in the lateral direction was significantly lower with the ISOP method than with the landmark method (2.2 ± 1.5 mm versus 5.6 ± 3.2 mm). Conclusion. This seminar was effective training for trocar placement for beginners. The puncture was more accurate with the ISOP method than with the landmark method.

PURPOSE: Pulmonary vascular alterations are not exclusive to pulmonary vascular disorders; they are also present in patients with mild chronic obstructive pulmonary disease (COPD) with normal pulmonary function. Moreover, vascular alteration is closely related to lung ventilation. We hypothesized that pulmonary vascular alterations might occur even in nonsmokers with subtle airflow limitation, and there could be a significant correlation between pulmonary vascular alterations and airflow impairment. The purpose of this study was to evaluate the correlation between the vascular alterations measured by cross-sectional area (CSA) and airflow impairment in nonsmokers with normal pulmonary function. MATERIALS AND METHODS: Thirty patients (1 man, 29 women; mean age 55 ± 14 years, range 33-81 years) underwent pulmonary function tests (PFTs) within 2 weeks of undergoing multidetector computed tomography (CT) scans. Total vessel CSA < 5 mm(2) was calculated by a threshold technique using imaging software. Forced expiratory volume in 1 s (FEV(1)), the ratio of FEV(1) to FVC (FEV(1)/FVC), and the mid-expiratory phase of the forced expiratory flow (FEF(25%-75%)) were obtained from PFTs. We calculated Spearman correlation coefficients between %CSA < 5 and the results of PFTs. RESULTS: %CSA < 5 had a significant positive correlation with FEV(1)/FVC (r = 0.651, P < 0.0001) and FEF(25%-75%) (r = 0.627, P = 0.0002), whereas CSA < 5% had no significant correlation with FEV(1) (r = 0.172, P = 0.362). CONCLUSION: There was a significant correlation between pulmonary small vascular alteration and airflow impairment even in nonsmokers without airway obstruction.
