Mining activities can damage rock masses and easily induce ground collapse, which seriously threatens safe production in mining areas. Micro-seismic systems can monitor rock mass deformation signals in real time and provide more accurate data for rock mass deformation analysis. Therefore, in this study, the waveform characteristics of micro-seismic events induced by ground collapse in the Rongxing gypsum mine were analyzed; the occurrence of these events was introduced on the basis of Fast Fourier Transform, an established Frequency-Time-Amplitude model, in order to put forward the index of energy proportion of the main band. The results showed the following. (1) The seismic sequence type of ground collapse was foreshock-mainshock-aftershocks. The interval between the foreshock and mainshock was longer than that between the mainshock and aftershocks. (2) The deformation corresponding to the foreshock micro-seismic events was mainly that of a small-scale crack. The deformation corresponding to the micro-seismic events during the mainshock was characterized by the gradual development of small-scale cracks, and the development of large-scale cracks accelerated, accompanied by slight rock collapse. The deformation corresponding to the micro-seismic events during the aftershocks showed that almost no small-scale cracks developed, and the large-scale crack development was intense, and accompanied by numerous rock and soil mass collapses. (3) The observed decreasing frequency distribution and energy dispersion can be used as possible precursors of ground collapse.

In aquatic ecosystems, light penetrating the sediment surface in shallow lakes may regulate the internal phosphorus (P) release through benthic primary production, which subsequently affects oxidation, pH levels, and alkaline phosphatase activity in the upper sediment. To study the effects of light exposure on the P dynamics at the sediment-water interface under eutrophic conditions, a two-month mesocosm experiment was conducted in twelve cement tanks (1000 L each). The tanks were equipped with Light-Emitting Diode (LED) lights, and surface sediments collected from eutrophic Lake Nanhu (China) were exposed to four different light intensities (0, 50, 100, 200 µmol m-2 s-1). The results revealed that: 1) Both the total phosphorus concentration and the phosphorus release flux from the sediment were lower in the light treatments (mean value, 0.59-0.71 mg L-1 and 0.00-0.01 mg m-2 d-1, respectively) than in the control treatment (0.77 mg L-1 and 0.01 mg m-2 d-1, respectively), indicating that light supplement could decrease the internal P release. 2) Benthic primary production promoted by light directly absorbed soluble reactive phosphorus and decreased the internal P release. The resulting improved production could also increase dissolved oxygen concentrations at the sediment-water interface, thus indirectly inhibiting internal P release. 3) The relative contributions of direct absorption and indirect inhibition on the internal P release ranged between 23% to 69% and 31% to 77% depending on the light intensity.

The unprecedented global increase in the anthropogenic-derived nitrogen (N) input may have profound effects on phosphorus (P) dynamics and may potentially lead to enhanced eutrophication as demonstrated in short-term mesocosm experiments. However, the role of N-influenced P release is less well studied in large-scale ecosystems. To gain more insight into ecosystem effects, we conducted a five-year large-scale experiment in ten ponds (700-1000 m2 each) with two types of sediments and five targeted total N concentrations (TN) by adding NH4Cl fertilizer (0.5, 1, 5, 10, and 25 mg N L-1). The results showed that: (ⅰ) The sediment P release increased significantly when TN exceeded 10-25 mg N L-1. (ⅱ) The most pronounced sediment P release increase occurred in summer and from sediments rich in organic matter (OMSed). (ⅲ) TN, algal biomass, fish biomass, non-algal turbidity, sediment pH, and OMSed were the dominant factors explaining the sediment P release, as suggested by piecewise structural equation modeling. We propose several mechanisms that may have stimulated P release, i.e. high ammonium input causes a stoichiometric N:P imbalance and induce alkaline phosphatase production and dissolved P uptake by phytoplankton, leading to enhanced inorganic P diffusion gradient between sediment and water; higher pelagic fish production induced by the higher phytoplankton production may have led increased sediment P resuspension through disturbance; low oxygen level in the upper sediment caused by nitrification and organic decomposition of the settled phytoplankton and, finally, long-term N application-induced sediment acidification as a net effect of ammonium hydrolysis, nitrification, denitrification; The mechanisms revealed by this study shed new light on the complex processes underlying the N-stimulated sediment P release, with implications also for the strategies used for restoring eutrophicated lakes.

Nitrogen pollution, especially ammonia, and its impacts on aquatic ecosystems are always hot topics worldwide. Evaluating the toxicity effect of ammonia on aquatic organisms is the essential basis for nitrogen management. Benthic macroinvertebrates are widely used to evaluate ammonia toxicity based on acute and chronic lab tests. In comparison, responses of macroinvertebrates under field and controlled conditions were rarely studied. To explore the effect of ammonia on macroinvertebrate assemblages and the underlying mechanisms under field conditions, a 5-year fertilization experiment was conducted in 5 quasi-natural ponds located in the Yangtze River floodplain. One control (TN0, no artificial ammonia loading) and four treatments (TN2, TN10, TN20, TN100; ordered by artificial ammonia loading from low to high) were set. The results showed that (1) species number of macroinvertebrates differed little among the ponds, while total density and biomass were positively correlated with unionized ammonia concentration (NH3), indicating that increased ammonia loading had no adverse impact on macroinvertebrate abundance; (2) all ponds were dominated by gathering collectors and the biomass was higher in the ponds with higher ammonia loading resulting from the more phytoplankton promoted by ammonia loading and improved internal phosphorus release; (3) the biomass of predators also increased with the increasing NH3 which may be due to the bottom-up effect through their prey; (4) some species, such as Limnodrilus hoffmeisteri, survived and were dominant species in the ponds with higher NH3 compared with 96 h median lethal concentration from acute lab test. The results suggested that higher ammonia loading increased macroinvertebrate abundance, mainly contributed by gathering collectors and predators. Unlike previous acute and chronic lab tests, macroinvertebrates showed extremely high tolerance to NH3 in field conditions. This study supported that ammonia toxicity to aquatic organisms was scale-dependent and should be evaluated at multiple scales.

A new Qinghai-Tibet Plateau species, Rhyacodrilus tanggulaensis Jiang & Cui sp. nov. (Annelida, Clitellata, Naididae, Rhyacodrilinae), is described, and its phylogenetic relationships within the genus is assessed on the basis of both mitochondrial (16S rDNA, COⅠ gene) and nuclear (ITS2) markers. Data from 32 species of Naididae, representing 7 subfamilies and 25 genera are used. Ten species were chosen as outgroup taxa. The molecular data were analyzed by Bayesian inference. The new species is distinguished from other species of Rhyacodrilus by the following combination of characters: dorsal and ventral chaetae with distal tooth 2-3 times longer than proximal, spermathecal and penial chaetae present, atria long and tubular, atrial duct conspicuous. The analyses of the combined molecular data corroborate the close relationship between Naidinae and the rhyacodriline genera Rhyacodrilus, Monopylephorus, and Ainudrilus, and show that the new species is more closely related to R. falciformis, R pigueti, R. okamikae and R. subterraneus, than to R. sinicus, another Chinese species, R. hiemalis and R. coccineus.

Chest radiography is commonly performed as a diagnostic tool of neonatal diseases. Contact-based radiation personal protective equipment (RPPE) has been widely used for radiation protection, but it does not provide full body protection and it is often shared between users, which has become a major concern during the coronavirus disease 2019 (COVID-19) pandemic. To address these issues, we developed a novel trolley to protect radiographers against X-ray radiation by reducing scatter radiation during neonatal radiographic examinations. We measured the scatter radiation doses from a standard neonatal chest radiograph to the radiosensitive organs using a phantom operator in three protection scenarios (trolley, radiation personal protective equipment [RPPE], no protection) and at three distances. The results showed that the scatter radiation surface doses were significantly reduced when using the trolley compared with RPPE and with no protection at a short distance (P<0.05 for both scenarios in all radiosensitive organs). The novel protective trolley provides a non-contact protective tool for radiographers against the hazard of scatter radiation during neonatal radiography examinations.

The toxic effect of unionized ammonia (NH3) on aquatic organisms is receiving increasing attention due to the excessive nitrogen discharge to various surface waters. Researches have suggested the scale-dependence of NH3 toxicity, being lower in field than under lab conditions. Such scale-dependence of toxicity is a big challenge to water quality criteria setting as the results solely from lab tests might not apply to natural ecosystems. Therefore, it is necessary to explore the underlying mechanism to understand the difference of toxicity across various spatial scales. In this study, we used the widely distributed gastropod Bellamya aeruginosa as the test animal and performed two 192-h microcosm experiments. Each experiment included a control and an ammonia addition treatment: N0(LC50) & N+(LC50), N0(LC100) & N+(LC100) (96-h LC50 = 0.8 mg NH3N/L, 96-h LC100 = 18.1 mg NH3N/L). Besides water-only, three potential key components (food, sediment, and submersed macrophytes) were included in the various treatments to mimic different complexity levels of aquatic ecosystems (Water, Water + Food, Water + Sediment, Water + Sediment + Macrophytes). The results showed that: 1) food directly improved the survival and growth of gastropods under expected lethal concentration of ammonia (96-h concentration of NH3N = LC20 of the 96-h acute test); 2) sediment and macrophyte quickly decreased the ammonia concentration, mainly by sediment adsorption and macrophyte uptake, to alleviate the ammonia stress to gastropods and permitted them to survive and grow under expected lethal concentration of ammonia (96-h concentration of NH3N = LC10∼LC20 of the 96-h acute test); 3) sediment and macrophyte also provided additional food for gastropods; 4) under extremely high ammonia stress (i.e., 96-h LC100, food was left uneaten and macrophyte died, and gastropods could, therefore, not be released from ammonia stress. Our results demonstrate that under moderate ammonia stress, the introduction of extra ecosystem elements (food, sediment, and macrophytes) significantly improved the survival and growth of gastropods, mainly by enhancing their tolerance and by quickly decreasing the NH3 concentration and thus toxicity. However, these introduced elements had little effect at very high concentration of ammonia (i.e., 96-h LC100). Our findings add to the understanding of the reasons behind the previous observed scale-dependent toxicity of NH3 on aquatic organisms and contribute to better decisions on the role of NH3 in relation to water quality management.

Ammonium (NH4-N) produces a paradoxical effect on submersed macrophytes because it is not only the preferred nitrogen source for the growth of plants but also threatens the growth of plants at high concentration. Whether short-term and small-scale physiological toxicity experiments at an individual level can reflect the effects of high ammonium on populations of submersed macrophytes in natural conditions is still unclear. In this study, an 18-month experiment was conducted in six 600 m2 ponds subjected to different levels of ammonium loading. The effects of high ammonium on populations of canopy-forming Myriophyllum spicatum and rosette-forming Vallisneria natans were explored. The results showed that M. spicatum and V. natans populations can develop high cover and height at high ammonium concentration (7 mg/L) at short-term exposures, and V. natans may be tolerant to 18 mg/L ammonium concentration. However, the cover of M. spicatum and the height of both species were inhibited at 2.4 mg/L at long-term exposures. The height of M. spicatum was two to six times higher than that of V. natans across all treatments and control by the end of the experiment, and the cover of M. spicatum was 7-11 times higher than that of V. natans in most NH4-N loading treatments, except the cover of M. spicatum in the highest NH4-N loading treatment with 18 mg/L NH4-N. The rosette-forming V. natans resists ammonium stress by slow growth (shoot elongation) to reduce consumption, while canopy-forming species resist ammonium stress by shoot elongation and canopy development to capture light. Although increasing ammonium concentration may induce severe stress on M. spicatum, the morphological characteristics of this species may, to some extent, release the plants from this stress. Our present study indicates that the negative effects of ammonium stress on the development of populations increased with exposure duration, and the submersed macrophyte community with stronger ability for light capture and dispersal may resist high ammonium stress. Nevertheless, in strongly ammonium-enriched systems, competition and succession cannot be neglected.

An effective biological index should meet two criteria: (1) the selected parameters have clear relationships with ecosystem health and can be measured simply by standard methods and (2) reference conditions can be defined objectively and simply. Species richness is a widely used estimate of ecosystem condition, although it is increased by nutrient enrichment, a common disturbance. Based on macrobenthos data from 91 shallow Yangtze lakes disconnected from the mainstem, we constructed an observed species (SO)-area (A) model to predict expected species richness (SE), and then developed an observed to expected index (O/E-SA) by calculating the SO/SE ratio. We then compared O/E-SA with three other commonly used indices regarding their ability to discriminate cultivated and urban lakes: (1) River Invertebrate Prediction and Classification System (RIVPACS; O/E-RF), (2) Benthic Index of Biotic Integrity (B-IBI), and (3) Average Score Per Taxon (ASPT). O/E-SA showed significant positive linear relationships with O/E-RF, B-IBI and ASPT. Quantile regressions showed that O/E-SA and O/E-RF had hump-shape relationships with most eutrophication metrics, whereas B-IBI and ASPT had no obvious relationships. Only O/E-SA, O/E50 and B-IBI significantly discriminated cultivated from urban lakes. O/E-SA had comparable or higher performance with O/E-RF, B-IBI and ASPT, but was much simpler. Therefore, O/E-SA is a simple and reliable index for lake ecosystem health bioassessment. Finally, a framework was proposed for integrated biological assessment of Yangtze-disconnected lakes.

Urban lakes are important natural assets but are exposed to multiple stressors from human activities. Submersed macrophytes, a key plant group that helps to maintain clear-water conditions in lakes, tend to be scarce in urban lakes, particularly when they are eutrophic or hypertrophic, and their loss is linked, in part, to impaired underwater light climate. We tested if enhancing the underwater light conditions using light-emitting diodes (LEDs) could restore submersed macrophytes in urban lakes. Twelve mesocosms (1000 L each) were each planted with tape grass (Vallisneria natans) and monitored over three months (22 August-7 November), using a control and three artificial light intensity treatments (10, 50, and 100 µmol m-2 s-1). Compared with the control, the high light treatment (100 µmol m-2 s-1) had higher leaf number, maximum leaf length, and average leaf length (3.9, 5.8, and 2.8 times, respectively). Shoot number, leaf number, leaf dry mass, root dry mass, and photosynthetic photon flux density in the high-light treatment were significantly greater than the control, but root length and phytoplankton chlorophyll a were not related to plant growth variables and were low in all treatments. Periphyton chlorophyll a increased significantly with the plant growth variables (i.e., shoot number, leaf number, and maximum leaf length) and was high in the light treatments but did not hamper the growth of the macrophytes. These results indicate that LED light supplementation enables the growth of V. natans under eutrophic conditions, at least in the absence of fish as in our experiment, and that the method may have potential as a restoration method in urban lakes. Lake-scale studies are needed, however, to fully evaluate LED light supplementation under natural conditions where other stressors (e.g., fish grazing) may need to be controlled for successful restoration of urban lakes.

Exploring the effectiveness of DNA barcoding in species identification is a prerequisite for biodiversity conservation and environmental monitoring. Aquatic oligochaetes could serve as excellent indicators in aquatic monitoring programmes. However, few studies have examined the effectiveness of DNA barcoding in these specific organisms. The mitochondrial cytochrome C oxidase (COI) gene of 83 specimens belonging to 40 species of 18 genera were sequenced in this study. The results showed that there was a barcode gap between species of Naididae and the intraspecific genetic distances of each species were smaller than interspecific genetic distances. The classification results of ABGD (Automatic Barcode Gap Discovery) were consistent with those of morphological identification, except for Tubifextubifex and Lumbriculusvariegatus. All species were successfully distinguished in the phylogenetic tree, based on the ITS2 region, which was coincident with the morphological result. Our results provided evidence that DNA barcoding can be used as an effective and convenient tool for species identification of the family Naididae and even for other aquatic oligochaetes.

Along with a steady increasing use of artificial nitrogen fertilizer, concerns have been raised about the effects that high nitrogen loading may have on ecosystems. Due to the toxicity of unionized ammonia (NH3), tolerance criteria have been proposed for ambient water management in many countries; however, these are mainly based on acute or chronic tests carried out under lab conditions run with purified water. Aiming at understanding the responses of organisms to natural exposure to high ammonia concentrations, a Viviparidae gastropod, Bellamya aeruginosa, was tested at three experimental scales: standard 96-h lab test, one-month cage test in 6 experimental ponds with continuous nitrogen inputs, and intensive investigation of the B. aeruginosa from these ponds in spring and winter. The results were: 1) 96-h LC50 in the standard lab test was 0.56 mg NH3-N/L and 343.3 mg TAN/L (total ammonia expressed as N, standardized at pH 7 and 20 â„ƒ). 2) In the one-month cage test, the survival rate was 97% when NH3-N was 0.61 mg/L (i.e., a higher concentration than the lab 96-h LC50) and the body size of the gastropods actually increased with increasing NH3-N concentrations. 3) In the winter-spring investigation, little effect of ammonia on the standing crops of gastropods was found, and the body size of the gastropods tended to increase with increasing ammonia concentrations (NH3-N concentration range of 0.05 ~ 2.06 mg/L). Thus, B. aeruginosa showed higher tolerance to ammonia exposure (NH3-N concentration < 0.81 mg/L) in the field than under laboratory conditions. Our study points to the necessity of considering the relevant scale when determining criteria for ammonia toxicity in water management.

RESUMO

The uniformity of blue LED array in jaundice treatment box is improved. The mathematical model of illumination uniformity algorithm for inner and outer LED arrays layout is established. Taking the actual size of blue light board in jaundice treatment box as an example, the optimal illumination uniformity with best LED arrays layout are obtained through programming iteration and simulation verification. The uniformity of blue light LED improved 42.9 % comparing with tradition LED arrays.

Cervical cancer (CESC) is a gynecologic malignant tumor associated with high incidence and mortality rates because of its distinctive management complexity. Herein, we characterized the molecular features of CESC based on the metabolic gene expression profile by establishing a novel classification system and a scoring system termed as METAscore. Integrative analysis was performed on human CESC samples from TCGA dataset. Unsupervised clustering of RNA sequencing data on 2,752 formerly described metabolic genes identified three METAclusters. These METAclusters for overall survival time, immune characteristics, metabolic features, transcriptome features, and immunotherapeutic effectiveness existed distinct differences. Then we analyzed 207 DEGs among the three METAclusters and as well identified three geneclusters. Correspondingly, these three geneclusters also differently expressed among the aforementioned features, supporting the reliability of the metabolism-relevant molecular classification. Finally METAscore was constructed which emerged as an independent prognostic biomarker, related to CESC transcriptome features, metabolic features, immune characteristics, and linked to the sensitivity of immunotherapy for individual patient. These findings depicted a new classification and a scoring system in CESC based on the metabolic pattern, thereby furthering the understanding of CESC genetic signatures and aiding in the prediction of the effectiveness to anticancer immunotherapies.

Understanding the relative importance of multiple stressors is valuable to prioritize conservation and restoration measures. Yet, the effects of multiple stressors on ecosystem functioning remain largely unknown in many fresh waters. Here, we provided a methodology combining ecosystem modeling with linear regression to disentangle the effects of multiple stressors on matter flow, an important ecosystem function. Treating a shallow lake as the model ecosystem, we simulated matter flow dynamics during 1950s-2010s with different combinations of stressors by Ecopath with Ecosim (EwE) modeling and determined the relative importance of each stressor by generalized linear mixed models. We found that matter flow of the lake food web was highly dynamic, attributing to effects of multiple anthropogenic stressors. Biological invasion played the strongest role in driving the matter flow dynamics, followed by eutrophication, while biomanipulation (i.e., phytoplankton control by planktivorous fish stocking) was of little importance. Eutrophication had a stronger role on primary producers, pelagic food chain, and top predators, while biological invasion on consumers in the middle of food chains. The former was more important in driving the quantity of matter flow, while the latter on trophic transfer efficiencies. Scenario forecasting showed that reducing nutrients contents could largely shape the matter flow pattern, while biomanipulation had little effect. Our findings provided new insights into understanding the mechanistic links between anthropogenic stressors and ecosystem functioning by combining ecosystem modeling with linear regression.

Aging plays an important role in many diseases, including breast cancer. Aged mammary stem/progenitor cells are perceived to be the cells of origin in breast tumorigenesis; however, the extensive use of mice who have aged naturally for research is hampered by cost, time, disease complications, and high mortality. In this study, we characterized murine mammary stem/progenitor cells in a D-galactose-induced accelerated aging model and compared them with findings from our earlier study on mice from natural aging. Our results showed that mammary glands in the D-galactose-induced aging model mimic natural aging in terms of pathological changes, epithelial cell composition, and mammary stem/progenitor cell function. These changes are accompanied by elevated inflammatory responses both systemically in the blood and locally in the mammary glands, which is similar in mice who age naturally. Our study for the first time evaluated the mammary glands and mammary stem/progenitor function in a D-galactose-induced aging model in rodents, and our findings suggest that D-galactose treatment can be used as a surrogate to study the role aged stem/progenitor cells play in breast tumorigenesis.

RESUMO

Anesthetic management for patients with a giant emphysematous bulla (GEB) is challenging. This case report describes a patient who developed 95% pulmonary compression by a GEB. A 14-Ga indwelling catheter was placed in the GEB before surgery to allow for slow re-expansion of the collapsed lung tissue. This prevented rupture of the GEB during anesthesia. Additionally, positive-pressure ventilation was performed to reduce the risk of re-expansion pulmonary edema. This respiratory management strategy may be beneficial for patients with a GEB who develop pulmonary dysfunction during thoracic surgery.

Both tetrabromobisphenol A (TBBPA) and titanium dioxide nanoparticle (TiO2 NP) have widespread commercial applications, resulting in their ubiquitous co-presence in the environment and biota. Although environmental chemicals exist as mixtures, toxicity studies are nearly always conducted with single chemicals. Few studies explore potential interactions of different chemical mixtures. In this study, we employ the sensitive developing nerve system in zebrafish to assess the neurotoxicity of TBBPA/TiO2 NP mixtures. Specifically, zebrafish embryos were exposed to solvent control (0.1% DMSO), 2 µM TBBPA, 0.1 mg/L TiO2 NP, and their mixture from 8 to 120 h post fertilization (hpf), and motor/social behavioral assessments were conducted on embryos/larvae at different developmental stages. Our results showed that TBBPA/TiO2 NP single or co-exposures increased spontaneous movement, decreased touch response and swim speed, and affected social behaviors of light/dark preference, shoaling, mirror attack and social contact. In particular, many of these phenotypes were manifested with higher magnitude of changes from the mixture exposure. These behavioral deficits were also accompanied with increased cell death in olfactory region and neuromasts in the lateral line system, increased ROS in gallbladder, pancreas, liver, and intestine, as well as increased lipid peroxidation and decreased ATP levels in whole larval tissue homogenates. Further, genes coding for key cell apoptosis marker and antioxidant enzyme were significantly upregulated by these two chemicals, in particular to their mixture. Interestingly, the co-presence of TBBPA also increased the mean particle size of TiO2 NP in the exposure solutions and the TiO2 NP content in larval tissue. Together, our analysis suggests that TBBPA/TiO2 NP induced behavioral changes may be due to physical accumulation of these two chemicals in the target organs, and TiO2 NP may serve as carriers for increased accumulation of TBBPA. To conclude, we demonstrated that TBBPA/TiO2 NP together cause increased bioaccumulation of TiO2, and heightened responses in behavior, cell apoptosis and oxidative stress. Our findings also highlight the importance of toxicity assessment using chemical mixtures.

Phosphorus (P) release from sediment is a key process affecting the effectiveness of eutrophication mitigation. We hypothesized that high nitrate (NO3-) input may have dual effect on sediment P release: reduce the sediment P release by improving the oxidation of sediment or promote P release by stimulating the growth of phytoplankton and increase the decomposition rates and oxygen consumption at the sediment water interface. To test the effect of different NO3- concentrations, we conducted a three-month experiment in 15 cement tanks (1 m3), with five targeted concentrations of NO3-: control, 2 mg L-1, 5 mg L-1, 10 mg L-1, and 15 mg L-1. The results showed that: i) when NO3- was maintained at high levels: NO3-≥5-7 mg L-1 (range of median values), there was no effect of NO3- on net P release from the sediment, likely because the positive effects of NO3- (increasing oxidation) was counteracted by a promotion of phytoplankton growth. ii) after NO3- addition was terminated NO3- dropped sharply to a low level (NO3-≤0.4 mg L-1), followed by a minor P release in the low N treatments but a significant P release in the high N treatments, which likely reflect that the inhibition effect of NO3- on P release decreased, while the promotion effects at high NO3- concentrations continued. The results thus supported our hypotheses of a dual effect on sediment P release and suggest dose-dependent effect of NO3- loading on stimulating P release from the sediment, being clear at high NO3- exceeding 5-7 mg L-1.

OBJECTIVE: To investigate the postoperative analgesic efficacy of ultrasound-guided lumbar erector spinae plane (ESP) blocks in patients undergoing posterior lumbar spinal surgery for lumbar spinal fractures. METHODS: A total of 80 patients who were scheduled for posterior internal fixation for lumbar spinal fractures were divided into a patient-controlled analgesia (PCA) group or a combined ESP-PCA group. Numeric rating scale at rest and during movement, postoperative sufentanil consumption, and accumulative and effective bolus presses of PCA were recorded at 6, 12, 24, and 48 hours postoperatively. Numeric rating scale at rest and during movement was the primary outcome. Incidence of postoperative nausea and vomiting during the first 24-48 hours, pruritus and chronic postoperative pain, and dose of pethidine for rescue analgesia were also recorded. RESULTS: Numeric rating scale at rest and during movement at 6, 12, and 24 hours was lower in the ESP-PCA group (P < 0.001, P < 0.001, P = 0.0016 at rest; all P < 0.001 during movement). Lumbar ESP blocks diminished accumulative bolus presses and effective bolus presses of PCA at 6, 12, 24, and 48 hours postoperatively. Besides, patients in the ESP-PCA group had fewer demands for sufentanil and pethidine. The incidence of postoperative nausea and vomiting in the ESP-PCA group was lower than that in PCA group. CONCLUSIONS: PCA combined with lumbar ESP blocks provided superior postoperative analgesia for patients with lumbar spinal fractures treated with posterior internal fixation. Lumbar ESP blocks decreased postoperative opioid consumption and incidence of postoperative nausea and vomiting, thereby enhancing postoperative recovery.
