Purpose Since 2020, UHC2030 has undertaken a project to review the progress of UN member states in fulfilling their commitments toward achieving universal health coverage (UHC) as outlined in the 2019 UHC Political Declaration. This involves identifying countries where UHC progress is of particular concern and engaging with multi-stakeholders. This article aims to provide a concise overview and widespread introduction to the State of UHC Commitment project for public health experts in Japan, with particular emphasis on the key findings from a pilot project on voluntary national reviews.Method In order to gauge the evidence-based accountability actions of governments toward UHC, we conducted a comprehensive review of initial five-year Voluntary National Review (VNR) reports (2016-2020, 187 reports) published at the United Nations High-level Political Forum (HLPF). This included all descriptions (quantitative and qualitative information) regarding UHC and health systems. We also compared the descriptions in the latest VNR reports (40 reports) available as of February 2021. We checked whether there have been any improvements in the evidence-based accountability of each country.Research Results We compared the 2021 VNR reports and the first five-year VNR reports and observed an improvement in evidence-based accountability. However, considering the wealth of data released by the United Nations Statistics on UHC and health systems, these indicators have not yet been fully utilized for accountability purposes.Conclusion Despite the UHC Political Declaration follow-up meeting and the SDGs Mid-term Review held at the United Nations General Assembly in September 2023, some UHC targets set in 2019 have been postponed to 2025. The current status of UHC progress poses challenges to achieving the 2030 goal. There is an urgent need to strengthen governments' evidence-based accountability using UN statistics and promote UHC progress by implementing the agreed Political Declaration.

UN member states have committed to universal health coverage (UHC) to ensure all individuals and communities receive the health services they need without suffering financial hardship. Although the pursuit of UHC should unify disparate global health challenges, it is too commonly seen as another standalone initiative with a singular focus on the health sector. Despite constituting the cornerstone of the health-related Sustainable Development Goals, UHC-related commitments, actions, and metrics do not engage with the major drivers and determinants of health, such as poverty, gender inequality, discriminatory laws and policies, environment, housing, education, sanitation, and employment. Given that all countries already face multiple competing health priorities, the global UHC agenda should be used to reconcile, rationalise, prioritise, and integrate investments and multisectoral actions that influence health. In this paper, we call for greater coordination and coherence using a UHC+ lens to suggest new approaches to funding that can extend beyond biomedical health services to include the cross-cutting determinants of health. The proposed intersectoral co-financing mechanisms aim to support the advancement of health for all, regardless of countries' income.

In the transition to the post-2015 agenda, many countries are striving towards universal health coverage (UHC). Achieving this, governments need to shift from curative care to promotion and prevention services. This research analyses Thailand's financing system for health promotion and prevention, and assesses policy options for health financing reforms. The study employed a mixed-methods approach and integrates multiple sources of evidence, including scientific and grey literature, expenditure data, and semi-structured interviews with key stakeholders in Thailand. The analysis was underpinned by the use of a well-known health financing framework. In Thailand, three agencies plus local governments share major funding roles for health promotion and prevention services: the Ministry of Public Health (MOPH), the National Health Security Office, the Thai Health Promotion Foundation and Tambon Health Insurance Funds. The total expenditure on prevention and public health in 2010 was 10.8% of the total health expenditure, greater than many middle-income countries that average 7.0-9.2%. MOPH was the largest contributor at 32.9%, the Universal Coverage scheme was the second at 23.1%, followed by the local governments and ThaiHealth at 22.8 and 7.3%, respectively. Thailand's health financing system for promotion and prevention is strategic and innovative due to the three complementary mechanisms in operation. There are several methodological limitations to determine the adequate level of spending. The health financing reforms in Thailand could usefully inform policymakers on ways to increase spending on promotion and prevention. Further comparative policy research is needed to generate evidence to support efforts towards UHC.
