ABSTRACT: Standard lead aprons do not protect the female breast adequately from radiation exposure, which has been associated with breast cancer in healthcare workers. A novel lead shield was designed to reduce radiation to the breast, axilla, and thyroid (BAT). A procedure room was simulated with an anthropomorphic phantom representing the operator. Dosimeters were positioned on the outer quadrant of each breast, the chest, the thyroid, and deep inside of a phantom acrylic female torso with neck and head. Standard lead vest plus a thyroid shield was used as control and compared to standard lead vest plus BAT shield. Three operator and two image receptor positions were tested. The reductions in radiation exposure were calculated. The standard vest plus BAT shield provided significant reductions in radiation exposure for all anatomic locations compared to control. When averaging all operator positions, the BAT provided reductions of 91% (p < 0.0001) for near breast. Reductions for far breast, chest, thyroid, and deep tissues were 76% (p = 0.016), 94% (p < 0.0001), 52% (p = 0.026), and 60% (p = 0.004). With operator 90° to the table using a cross-table lateral beam, the BAT provided a 97.7% reduction in radiation to the near breast and significant reduction in radiation to the chest, thyroid, and deep tissues. The BAT shield reduces radiation exposure to the breast, chest, thyroid and deep hematopoietic tissues. Such shields could benefit healthcare workers to reduce the risk of breast cancer and other radiation-associated cancers.

RESUMO

Sclerosing mesenteritis is a rare disorder characterized by fat necrosis, chronic inflammation, and fibrosis of the small bowel mesentery. With a paucity of published clinical trials on sclerosing mesenteritis, treatment is based on case reports and trials of other fibrosing diseases, such as idiopathic retroperitoneal fibrosis. We present a case of a 68-year-old woman with sclerosing mesenteritis who exhibited complete symptomatic and radiographic resolution with the use of tamoxifen monotherapy.

RESUMO

In a 24-hour period, three patients presented with acute aortic dissection involving the ascending aorta. Detailed analysis of these three patients was undertaken to compare preoperative factors, the emergency operative treatment, and postoperative outcomes, as well as pathological findings. A high-volume aortic referral center with specialized protocols and personnel can effectively manage this circumstance.

RESUMO

The quality of surgical assistance is an important factor affecting surgical performance and outcomes. This article gives a perspective on key principles of first assisting, specifically during cardiovascular operations. The principles start with operative techniques, then expand into concepts relating to the surgical field and operating room. By adhering to the described principles, the first assistant may enhance a surgical operation, benefiting the surgeon, the operating room team, and, most important, the patient.

The surgical treatment of breast cancer has rapidly evolved over the past 50 years, progressing from Halsted's radical mastectomy to a public campaign of surgical options, aesthetic reconstruction, and patient empowerment. Sparked by the research of Dr. Bernard Fisher and the first National Surgical Adjuvant Breast and Bowel Project trial in 1971, the field of breast surgery underwent significant growth over the next several decades, enabling general surgeons to limit their practices to the breast. High surgical volumes eventually led to the development of the first formal breast surgical oncology fellowship in a large community-based hospital at Baylor University Medical Center in 1982. The establishment of the American Society of Breast Surgeons, as well as several landmark clinical trials and public campaign efforts, further contributed to the advancement of breast surgery. In 2003, the Society of Surgical Oncology (SSO), in partnership with the American Society of Breast Surgeons and the American Society of Breast Disease, approved its first fellowship training program in breast surgical oncology. Since that time, the number of American fellowship programs has increased to approximately 60 programs, focusing not only on training in breast surgery, but also in medical oncology, radiation oncology, pathology, breast imaging, and plastic and reconstructive surgery. This article focuses on the happenings in the United States that led to the transition of breast surgery from a subset of general surgery to its own specialized field.

SCOPE OF SPECIALTY: Colorectal surgery encompasses any procedure performed on the colon and/or rectum. This section focuses on rectal surgery, specifically regarding surgery for rectal cancer and its technical complexity and disease complexity that often requires a multidisciplinary team-based approach. The emergence of rectal cancer surgical specialists exhibits improvements in rectal cancer outcomes. Colorectal surgery fellowships are available for general surgeons who desire specialization. Roughly 70 colorectal surgery fellowship programs exist, of which 39 are accredited by the National Accreditation Program for Rectal Cancer (NAPRC). PATIENT CARE AND QUALITY OUTCOMES: The OSTRiCh (Optimizing the Surgical Treatment of Rectal Cancer) Consortium (OC) and NAPRC developed guidelines for a standardized pathway for rectal cancer care and outline standards for patient care and performance indicators. These efforts focus on the recognition of disparities in rectal cancer outcomes, a system for improvement, delivery of quality care, and optimizing patient results by minimizing morbidity and mortality. REGULATORY REQUIREMENTS: In the United States, Board certification for surgeons performing colorectal surgery is granted by the American Board of Colon and Rectal Surgery (ABCRS). While the United States lags behind its European counterparts in development of rectal cancer quality measures, significant changes have been made to close the gap. Improvement initiatives launched by the OC implemented a myriad of quality metrics for longitudinal outcome tracking; surgical site infection, positive circumferential margin rate, rate of colostomy creation, recurrence rate, to name a few.

Cytokines secreted during pregnancy may influence immune development of the foetus. This study aimed to determine if maternal allergy alters patterns of systemic cytokine production throughout and after pregnancy. Maternal plasma cytokines and allergen-specific production of interleukin (IL)-10, IL-13 and interferon (IFN)-gamma were measured in allergic (n = 63) and non-allergic (n = 70) pregnant women who had a full set of sequential peripheral blood samples collected at 20-, 30-, 36-wk gestation and 6-wk post-partum. Maternal allergy was strictly defined by both allergen sensitization and doctor-diagnosed asthma, eczema or rhinitis. IL-13 responses to allergen were higher for allergic mothers at all time-points (20 wk: p < 0.001; 30 wk: p = 0.001; 36 wk: p < 0.001; post-partum: p < 0.001). For the non-allergic group, IL-13 levels to house dust mite decreased from 20- to 36-wk gestation (Friedman ANOVA p = 0.012) and were significantly lower at 36 wk compared with post-partum (p = 0.002). In contrast, IL-13 production by allergic mothers did not change from 20 wk through to post-partum. For both allergic and non-allergic mothers, in vitro IFN-gamma production was lower at all pregnancy time-points compared with post-partum levels. Allergic women had an increased propensity for peripheral blood allergen-specific T helper-2 responses during pregnancy, and failed to downregulate these responses in comparison with non-allergic women. This may be a factor that contributes to the increased risk of atopy in infants born to allergic mothers.
