RESUMO

In this study, a pilot-scale anaerobic membrane bioreactor (AnMBR) was developed to continuously produce volatile fatty acids (VFAs) from kitchen waste slurry under an alkaline condition. The alkaline fermentation effectively suppressed methanogenesis, thus achieving high VFAs production of 60.3 g/L. Acetic acid, propionic acid, and butyric acid accounted for over 95.0 % of the total VFAs. The VFAs yield, productivity, and chemical oxygen demand (COD) recovery efficiency reached 0.5 g/g-CODinfluent, 6.0 kg/m3/d, and 62.8 %, respectively. Moreover, the CODVFAs/CODeffluent ratio exceeded 96.0 %, and the CODVFAs/NH3-N ratio through ammonia distillation reached up to 192.5. The microbial community was reshaped during the alkaline fermentation with increasing salinity. The membrane fouling of the AnMBR was alleviated by chemical cleaning and sludge discharge, and membrane modules displayed a sustained filtration performance. In conclusion, this study demonstrated that high-quality VFAs could be efficiently produced from kitchen waste slurry using an AnMBR process via alkaline fermentation.

A rare case of coronary artery involvement in a child with Systemic Juvenile Idiopathic Arthritis (sJIA) complicated by Macrophage Activation Syndrome (MAS) is reported. The patient initially received an inaccurate diagnosis of Kawasaki Disease, sepsis, and mycoplasma infection and showed no improvement after Intravenous Immune Globulin (IVIG) treatment. Upon admission, symptoms included diffuse red rash, swelling of the limbs, lymph node enlargement, and hepatosplenomegaly. Post investigations, a diagnosis of sJIA and MAS was confirmed, and treatment involved a combination of hormones (methylprednisolone) and immunosuppressive drugs (methotrexate). The revealed widened coronary artery diameter was managed with a disease-specific treatment plan and prophylactic plus low-dose aspirin anti-coagulation therapy. Under this management, MAS was well controlled, and follow-ups showed normalization of the child's coronary artery structure and function. This case and the associated literature review underscore the importance of early recognition, diagnosis, treatment, and long-term monitoring for children presenting with sJIA and MAS complicated by coronary artery involvement.

Background: Klinefelter's syndrome (KS) was once considered infertile due to congenital chromosomal abnormalities, but the presence of focal spermatozoa changed this. The key to predict and promote spermatogenesis is to find targets that regulate focal spermatogenesis. Objective: To explore the trend of fertility changes in KS patients at different ages and identify potential therapeutic targets. Methods: Bibliometric analysis was used to collect clinical research data on KS from the Web of Science Core Collection (WoSCC) from 1992 to 2022. A cross-sectional study was conducted on 75 KS patients who underwent microscopic testicular sperm extraction (mTESE) from 2017 to 2022 in the real world. The reproductive hormones, testicular histopathology, androgen receptors, insulin-like factor 3 (INSL3) receptors and sperm recovery rate (SRR) were analyzed. Results: Male infertility, dysplasia, Sertoli cells, Leydig cells, testosterone and spermatogenesis were the research focuses related to KS. Luteinizing hormone (LH), testosterone, and INSL3 were evaluation indicators of Leydig cell function that fluctuate with age. Testosterone and LH peaked at ages 13-19 and 30-45, while INSL3 only peaked at ages 13-19. 27 patients (27/75) recovered sperm through mTESE and experienced SRR peaks at the ages of 20, 28, 34, and 37. The SRR of fibrosis patients was 46.15%, fatty degeneration was 7.14%, and melanosis was 40.00%. The INSL3 and androgen receptors were highly expressed and roughly balanced in focal spermatogenesis. Conclusion: Abnormal metabolism of Leydig cells led to imbalanced expression of INSL3 and androgen receptors, which might be a potential target for spermatogenesis in KS.

RESUMO

Cardiovascular involvement is an uncommon but severe complication of Epstein-Barr virus (EBV) infection caused by direct damage and immune injury. Recently, it has drawn increasing attention due to its dismal prognosis. It can manifest in various ways, including coronary artery dilation (CAD), coronary artery aneurysm (CAA), myocarditis, arrhythmias, and heart failure, among others. If not treated promptly, cardiovascular damage can progress over time and even lead to death, which poses a challenge to clinicians. Early diagnosis and treatment can improve the prognosis and reduce mortality. However, there is a lack of reliable large-scale data and evidence-based guidance for the management of cardiovascular damage. Consequently, in this review, we attempt to synthesize the present knowledge of cardiovascular damage associated with EBV and to provide an overview of the pathogenesis, classification, treatment, and prognosis, which may enhance the recognition of cardiovascular complications related to EBV and may be valuable to their clinical management.

BACKGROUND: Oxidative stress and neurocyte apoptosis are crucial pathophysiological process in early brain injury (EBI) after subarachnoid hemorrhage (SAH). Geniposide (GNP) has been reported to exert neuroprotective effects by reducing oxidative injury and neurocyte apoptosis. However, the effect of GNP has not been clarified in EBI after SAH. The study was performed to evaluate the neuroprotective effects and mechanisms of GNP in EBI after SAH. METHODS AND RESULTS: A total of 60 male Wistar rats were randomly divided into five groups. The prechiasmatic cistern SAH model was used in this study. SAH grade was evaluated using a grading system. Neurological function was evaluated using the Garcia scores. Brain edema was measured by the wet-dry method. Blood-brain barrier (BBB) permeability was measured by the extravasation of Evans Blue (EB). The neurocyte apoptosis was observed using TUNEL assay. The levels of malondialdehyde (MDA) and superoxide dismutase (SOD), as well as the expressions of nuclear factor erythroid 2-related factor 2 (Nrf2), hemeoxygenase-1 (HO-1), glutathione S-transferase (GST) and quinone oxidoreductase-1 (NQO-1) were performed. The results showed that GNP reduced brain edema, attenuated BBB permeability, inhibited neurocyte apoptosis and improved neurological function. Moreover, GNP also decreased the levels of ROS and MDA, elevated Nrf2 expression in the temporal cortex and up-regulated the expression of NQO-1, HO-1 and GST after SAH. CONCLUSIONS: GNP could ameliorate oxidative stress and neurocyte apoptosis to exert neuroprotective effects by Nrf2 pathway.

Si3N4 ceramic is generally recognized as being difficult to machine due to its hardness and brittleness. It is necessary to control the normal load applied and the machined depth of the abrasive particles in order to eliminate surface/subsurface damage and defects during the grinding or polishing. In this study, scratch experiments were conducted on the polished surface of Si3N4 specimens to investigate the brittle-ductile transformation and the evolution of material removal mechanisms. In addition, the cracking behaviour of Si3N4 ceramic was characterized by indentation tests. The Vickers indentation produced cracks that exhibited good developmental integrity and geometric symmetry. The results indicate that the scratch track can be divided into three stages: the ductile regime, the brittle-ductile coexisting stage, and the brittle fracture regime. The critical loads and the corresponding penetration depths of cracking occurrence in Si3N4 were recorded. The material removal of Si3N4 ceramic was primary attributed to ductile regime removal when the load was less than 9.8 N. Microcrack initiation on the subsurface was observed when the penetration depth of the scratch tip reached 8 µm or the depth of the indentation tip reached 3.2 µm. Microcracks expanded rapidly as the load was further increased, resulting in a brittle fracture of the Si3N4 ceramic.

In this study, a novel finishing method, entitled clustered magnetorheological finish (CMRF), was proposed to improve surface finish of the silicon nitride ( Si 3 N 4 ) balls with ultra fine precision. The effects of different polishing parameters including rotation speeds, eccentricities and the machining gaps on surface finish of Si 3 N 4 balls were investigated by analyzing the roughness, sphericity and the micro morphology of the machined surface. The experimental results showed that the polishing parameters significantly influenced the surface finish. The best surface finish was obtained by using the polishing parameters: the machining gap of 0.8 mm, the eccentricity of 10 mm and the rotation ratio of 3/4. To further investigate the influence of the polishing parameters on the surface finish, an analytical model was also developed to analyze the kinematics of the ceramic ball during CMRF process. The resulting surface finish, as a function of different polishing parameters employed, was evaluated by analyzing the visualized finishing trace and the distribution of the contact points. The simulative results showed that the distribution and trace of the contact points changed with different polishing parameters, which was in accordance with the results of experiments.

To systemically evaluate the safety and efficacy of Xiyanping injection combined with azithromycin in the treatment of mycoplasmal pneumonia in children. PubMed, Wanfang Data, CNKI, VIP and CBM were used to search for the clinical randomized controlled trials on Xiyanping injection combined with azithromycin in the treatment of mycoplasmal pneumonia in children from database establishment to July, 2017. The papers were screened according to the established inclusion and exclusion criteria, and then the quality of the included studies was evaluated to extract valid data for Meta-analysis by using Revman 5.3 software. A total of 31 clinical randomized controlled trials were included, involving 2 881 patients. Meta-analysis showed that as compared with azithromycin alone, the combination of azithromycin with azithromycin had obvious advantages in the total effective rate(OR=5.42,95%CI[3.98,7.38],P<0.000 01), fever clearance time(MD=-1.29,95%CI[-1.51,-1.08],P<0.000 01), cough disappearance time (MD=-1.72,95%CI[-1.99,-1.46],P<0.000 01), lung wet rales disappearance time(MD=-1.51,95%CI[-1.88,-1.14],P<0.000 01), chest X-ray recovery time(MD=-2.72,95%CI[-3.82,-1.63],P<0.000 01), shortening the hospital stay(MD=-1.88,95%CI[-2.26,-1.50],P<0.000 01), reducing the incidence of adverse reactions(MD=0.51,95%CI[0.33,0.79],P=0.002), and other aspects, with statistically significant difference. Based on the existing evidences, Xiyanping injection combined with azithromycin in the treatment of mycoplasmal pneumonia in children can significantly improve the overall clinical efficiency, shorten the hospital stay and reduce the incidence of adverse reactions. However, the clinical trials of existing small randomized controlled trials have low quality of methodology and require a large sample of high quality clinical trials for further validation.

This study was to test the hypothesis that tetramethylpyrazine (TMP) protected against early brain injury after subarachnoid hemorrhage (SAH) by affecting the mitochondrial-dependent caspase-3 apoptotic pathway. TMP was administrated after the rats' prechiasmatic SAH mode. Animal neurobehavioral functions were assessed and the mitochondrial morphology, mitochondrial and cytoplasmic calcium, and mitochondrial membrane potential changes (Δψm) of the brain tissues were measured. The expressions of cytoplasmic cytochrome c (cyt c), second mitochondria-derived activator of caspases (Smac), and cleaved caspase-3 B-cell lymphoma 2 (bcl-2) in cells were determined and cellular apoptosis was detected. The treatment of TMP resulted in less apoptotic cells and milder mitochondrial injury and potentially performed better in the neurobehavioral outcome compared to those with saline. Also, TMP ameliorated calcium overload in mitochondria and cytoplasm and alleviated the decrease of Δψm. In addition, TMP inhibited the expression of cytoplasmic cyt c, Smac, and cleaved caspase-3, yet it upregulated the expression of bcl-2. These findings suggest that TMP exerts an antiapoptosis property in the SAH rat model and this is probably mediated by the caspase-3 apoptotic pathway triggered by mitochondrial calcium overload. The finding offers a new therapeutic candidate for early brain injury after SAH.

The anaerobic digestion of high-strength kitchen waste slurry via a pilot-scale anaerobic membrane bioreactor (AnMBR) was investigated at two different operational modes, including no sludge discharge and daily sludge discharge of 20 L. The AnMBR provided excellent and reliable permeate quality with high COD removal efficiencies over 99%. The obvious accumulations of long chain fatty acids (LCFAs) and Ca(2+) were found in the anaerobic digester by precipitation and agglomeration. Though the physicochemical process contributed to attenuating the free LCFAs toxicity on anaerobic digestion, the digestion efficiency was partly influenced for the low bioavailability of those precipitates. Moreover, higher organic loading rate (OLR) of 5.8 kg COD/(m(3) d) and digestion efficiency of 78% were achieved as the AnMBR was stably operated with sludge discharge, where the membrane fouling propensity was also alleviated, indicating the crucial significance of SRT control on the treatment of high-strength kitchen waste slurry via AnMBRs.

OBJECTIVE: To investigate the impact of anxiety-depression symptoms on the healing of burn wounds and the serum tumor necrosis factor a (TNF-alpha) and interleukin-6 (IL-6) in the patients with moderate and severe burns. METHODS: The patients with moderate and severe burns were divided into anxiety-depression group (24 patients) and non-anxiety-depression group (27 patients)according to the scores of anxiety-depression scores three days after burn measured by Hamilton Rating Scale for Depression (HAMD) and Hamilton Rating Scale for Anxiety (HAMA). The routine therapy was applied for both groups for 30 days. Then the scores of anxiety and depression and the degrees of wound healing were examined. The serum TNF-alpha IL-6 were measured with enzyme linked immunosorbent assay (ELISA). RESULTS: No significant decrease of the scores of depression was found before and after treatments in the anxiety-depression group (P > 0.05). The days that were needed for healing the wounds and the serum TNF-alpha and IL-6 increased in the patients with depression compared with the control group (P < 0.05). CONCLUSION: There are anxious and depressive reactions to burns by the patients. The continuous negative emotion can induce the increase of serum TNF-alpha and IL-6 and lead to delay of the healing of the burn wounds.

BACKGROUND & OBJECTIVE: Garlic and the organosulfer compound from garlic have antitumor effects, but the mechanisms still remain unclear. This study was to investigate the changes and significance of caspase-3 activity in diallyl trisulfide (DATS)-induced apoptosis of human gastric cancer cell line MGC803. METHODS: Effects of DATS on the apoptosis of MGC803 cells and the change of activated caspase-3 were observed under a fluorescent and an electron microscopy, and detected by flow cytometry and Western blot. RESULTS: After incubation with DATS, MGC803 cells showed typical apoptotic morphologic changes, and the apoptosis rate increased significantly. DATS activated caspase-3 in a time-dependent manner: the positive rates of activated caspase-3 were 1.9%, 3.0%, 7.3%, 14.4%, and 27.6% respectively in MGC-803 cells treated with 12 mg/L DATS for 0, 4, 8, 12 and 24 h. When treated with 12 mg/L DATS for 24 h, the apoptosis rate was significantly lower in the cells with pretreatment of Ac-DEVD-CHO (a caspase-3 inhibitor) than in the cells without pretreatment (5.1% vs. 23.0%, P<0.01), indicating that Ac-DEVD-CHO efficiently attenuated DATS-induced apoptosis of MGC803 cells. Moreover, pro-caspase-3 was hydrolyzed and activated in DATS-treated MGC803 cells. CONCLUSION: DATS induces apoptosis in MGC803 cells which may be through the activation of caspase-3 pathway.

BACKGROUND & OBJECTIVE: Diallyl disulfide (DADS) can inhibit growth of various cancer cell lines in vitro, but little is known about its in vivo antitumor effect. This study was designed to investigate effect of DADS on growth of human gastric carcinoma MGC803 cells xenograft in BALB/C nude mice. METHODS: MGC803 cells, with or without 1-day treatment of DADS (30 mg/L), were subcutaneously transplanted into the right axial regions of nude mice. The xenograft tumor growth in mice was observed after in vitro treatment or intraperitoneal injection of DADS. Proliferating cell nuclear antigen (PCNA) expression was detected by Western blot. RESULTS: No xenograft tumor was observed in the nude mice inoculated with DADS-treated MGC803 cells. When injected with 50, 100, and 200 mg/kg of DADS, the inhibition rate of tumor growth in the nude mice inoculated with untreated MGC803 cells were 27.8%, 66.1%, and 73.0%; the expression of PCNA was inhibited. CONCLUSION: DADS could suppress incidence of human gastric carcinoma xenograft in BALB/C nude mice, and inhibit growth of transplanted tumor.
