Fatty acid translocase (FAT/CD36) is a transmembrane glycoprotein that plays an important role in transporting long-chain fatty acids. In the current study, a full-length cDNA of FAT/CD36 was first cloned from the intestine of White King pigeon by rapid amplification of cDNA ends (RACE) method. The full-length cDNA of pigeon FAT/CD36 was 2,282 bp, including a 5'-untranslated region of 224 bp, a 3'-untranslated region of 642 bp, and an open reading frame of 1,416 bp encoding a protein of 471 amino acids with the predicted molecular weight of 52.7 kDa. Sequence comparison indicated that FAT/CD36 of pigeon had high identity with other avian FAT/CD36. Using quantitative real-time PCR, expression of FAT/CD36 was the greatest in the duodenum at 28 d posthatch, and in the jejunum, the expression of FAT/CD36 at 14 d posthatch was greater than at 8 d but the same as 28 d posthatch. However, in the ileum, expression of FAT/CD36 peaked at embryonic d 15 and 8 d posthatch. The effects of long-chain fatty acids on pigeon FAT/CD36 and peroxisome proliferator activated receptor γ (PPARγ) mRNA expression were also investigated in vitro. It showed that a low concentration (5 µM) of oleic acid, palmitic acid, and linoleic acid can significantly increase FAT/CD36 and PPARγ mRNA level in pigeon jejunum. However, for linolenic acid or arachidonic acid, the induction of both gene expressions needed a higher concentration (50 µM or 250 µM). Two hundred and 50 µM palmitic acid was shown to suppress FAT/CD36 gene expression. The results suggest that FAT/CD36 may be a representative of intestine development in pigeon, and it could be regulated by long-chain fatty acids via PPARγ pathway.

There are suggestions that duodenal ulcer protects individuals from gastric cancer and that rice is ulcerogenic while wheat is gastro-protective. We aimed to examine the relationship of gastric cancer, duodenal and gastric ulcers in different geographical regions in China and identified dietary risk factors for duodenal ulcer and gastric cancer. The prevalence of peptic ulcer and gastric cancer among symptomatic patients in eight major cities, four each from the north and the south representing all the six defined regions of China were studied. Endoscopy and case records over a 10 year period were reviewed and cases of confirmed duodenal and gastric ulcer and gastric cancer, together with the total number of endoscopies performed per year, were recorded. Rates were expressed as cases/1000 endoscopies. Results were compared to another epidemiological study on diet and mortality in the same regions in China conducted at the same time. Duodenal ulcer rates were 2.4-fold higher in southern China than northern China, whereas gastric cancer rates were 1.6-fold higher in the north than in the south. Correlation studies showed for the first time an inverse linear relationship between the gastric cancer rates and the duodenal ulcer rates (r=-0.8076, P=0.015), as well as the duodenal ulcer: gastric ulcer ratios (r=-0.9133, P=0.002). Gastric ulcer rates were higher in southern China but did not correlate with the gastric cancer rates (r=0.1455, P=0.731). Duodenal ulcer rates were found to be related to daily rice intake (r=0.8554, P=0.029) and inversely related to daily wheat flour intake (r=-0.8472, P=0.033). Gastric cancer rates were not related to any dietary risk factors tested. We concluded there was an inverse relationship between gastric cancer rates and duodenal ulcer rates. Although duodenal ulceration and gastric cancer are both linked to Helicobacter pylori infection, the findings of this study indicate independent additional aetiological factors for the pathogenesis of these conditions. Dietary factors such as rice or wheat intake may play a role.

We studied 128 patients with active peptic ulcer diagnosed by gastro-endoscopy four weeks after treatment with Xi Lei powder, PGE2 levels in both serum and gastro-duodenal mucosa were significantly higher than that before the treatment, the difference was significant (P < 0.01). The rate of negative conversion of Helicobacter pylori (HP) showed in 63.3% of cases. The distributed density of HP significantly reduced, the difference was significant (P < 0.01). These results indicated that Xi Lei powder could heal ulcer probably owing to the increasing of the PGE2 level in gastroduodenal mucosa, it inhibited the growth and reproduction of HP, and eliminated the HP, but did not inhibit the secretion of gastric acid and release the gastrin.

Fifty-three cases of atypical tuberculous peritonitis were diagnosed by Machida FLA-8 fibrolaparoscope and direct-vision peritoneal biopsy in our hospital during the last few years. The misdiagnosis rate of this disease is very high. The rate of accurate clinical diagnosis was only 39.6% in patients of this study, while 60.4% was misdiagnosed as other diseases, such as cirrhosis, chronic hepatitis, hepatic carcinoma ovarian cyst etc. In addition, many patients with other diseases were misdiagnosed as tuberculous peritonitis by clinical consideration, for instance, 56 cases who were diagnosed or doubted as tuberculous peritonitis by clinical consideration were diagnosed as other diseases by laparoscopy and liver and peritoneal biopsy under direct-vision. Among them chronic hepatitis accounted for 32 cases, peritoneal carcinoma 11 cases, cirrhosis 7 cases, normal peritoneum, liver, gall bladder and spleen 6 cases. Therefore, the patient who is presumptively diagnosed as tuberculous peritonitis by clinical consideration should have laparoscopy and direct-vision peritoneal biopsy performed.
