RESUMO

Cervical cancer remains a significant health burden in China, characterized by high incidence and mortality rates, which are exacerbated by low Human Papillomavirus (HPV) vaccination coverage, leading to substantial loss of productivity, emotional suffering, and family strain. Understanding factors that influence HPV awareness and knowledge is crucial for developing effective educational strategies. This cross-sectional study, conducted from September to October 2022, involved 2,679 college students from various educational institutions in Jiangsu Province, China. Data were collected via an online questionnaire covering demographics, HPV knowledge, and vaccination behaviors. Statistical analyses, including Chi-square tests and multifactorial logistic regression, were used to identify factors influencing HPV knowledge. The study revealed that while over 90% of students correctly identified HPV's transmission and risks, significant knowledge gaps and misconceptions persist, particularly regarding HPV's association with HIV/AIDS and its treatment. Factors significantly associated with better HPV knowledge included age (22-24 years), female gender, being a medical major, being in a relationship, familiarity with HPV, and participation in sexual education programs. Despite a high willingness to receive the HPV vaccine (91.64%), actual vaccination rates remained low. These findings suggest that while Chinese college students were generally aware of HPV, targeted educational interventions are essential to address knowledge gaps and promote HPV vaccination effectively.

RESUMO

Background Although favorable outcomes have been reported with radiofrequency ablation (RFA) for secondary hyperparathyroidism (SHPT), the long-term efficacy remains insufficiently investigated. Purpose To evaluate the long-term efficacy and safety of US-guided percutaneous RFA in patients with SHPT undergoing dialysis and to identify possible predictors associated with treatment failure. Materials and Methods This retrospective study included consecutive patients with SHPT with at least one enlarged parathyroid gland accessible for RFA who were undergoing dialysis at seven tertiary centers from May 2013 to July 2022. The primary end point was the proportion of patients with parathyroid hormone (PTH) levels less than or equal to 585 pg/mL at the end of follow-up. Secondary end points were the proportion of patients with normal calcium and phosphorus levels, the technical success rate, procedure-related complications, and improvement in self-rated hyperparathyroidism-related symptoms (0-3 ranking scale). The Wilcoxon signed rank test and generalized estimating equation model were used to evaluate treatment outcomes. Univariable and multivariable regression analyses identified variables associated with treatment failure (recurrent or persistent hyperparathyroidism). Results This study included 165 patients (median age, 51 years [IQR, 44-60 years]; 92 female) and 582 glands. RFA effectively reduced PTH, calcium, and phosphorus levels, with targeted ranges achieved in 78.2% (129 of 165), 72.7% (120 of 165), and 60.0% (99 of 165) of patients, respectively, at the end of follow-up (mean, 51 months). For the RFA sessions, the technical success rate was 100% (214 of 214). Median symptom scores (ostealgia, arthralgia, pruritus) decreased (all P < .001). Regarding complications, only hypocalcemia (45.8%, 98 of 214) was common. Treatment failure occurred in 36 patients (recurrent [n = 5] or persistent [n = 31] hyperparathyroidism). The only potential independent predictor of treatment failure was having less than four treated glands (odds ratio, 17.18; 95% CI: 4.34, 67.95; P < .001). Conclusion US-guided percutaneous RFA was effective and safe in the long term as a nonsurgical alternative for patients with SHPT undergoing dialysis; the only potential independent predictor of treatment failure was a lower number (<4) of treated glands. © RSNA, 2024 Supplemental material is available for this article.

RESUMO

OBJECTIVE: To prospectively evaluate the outcomes of ultrasound (US)-guided radiofrequency ablation (RFA) in tertiary hyperparathyroidism (THPT). MATERIALS AND METHODS: Patients with THPT underwent RFA between September 2017 and January 2022. Laboratory parameters, including serum intact parathyroid hormone (iPTH) levels, were monitored for 48 months after RFA and compared with the levels at baseline. Complications related to RFA and changes in hyperparathyroidism-related clinical symptoms were recorded before and after RFA. RESULTS: A total of 42 patients with THPT were recruited for this study. Ultimately, 36 patients with renal failure and 2 patients who underwent successful renal transplantation (male:female, 17:21; median age, 54.5 years) were enrolled. The follow-up time was 21.5 ± 19.0 months in the 36 patients with renal failure. In these 36 patients, iPTH levels were significantly decreased to 261.1 pg/mL at 48 months compared with the baseline value of 1284.9 pg/mL (P = 0.012). Persistent hyperparathyroidism, defined as iPTH levels maintained at > 585.0 pg/mL for 6 months after treatment, occurred in 4.0% of patients (1/25). Recurrent hyperparathyroidism, defined as iPTH levels > 585.0 pg/mL after 6 months, were 4.0% (1/25) and 0.0% (0/9) at 6 months and 4 years after treatment, respectively. In two patients with THPT after successful renal transplantation, iPTH decreased from the baseline value of 242.5 and 115.9 pg/mL to 171.0 and 62.0 pg/mL at 6 months after treatment. All complications resolved within 6 months of ablation without medical intervention, except in 10.5% (4/38) patients with permanent hypocalcemia. The overall symptom recovery rate was 58.8% (10/17). The severity scores for bone pain, arthralgia, and itchy skin associated with hyperparathyroidism improved after treatment (P < 0.05). CONCLUSION: US-guided RFA is an effective and safe alternative to surgery in the treatment of patients with TPTH and improves hyperparathyroidism-related clinical symptoms.

RESUMO

BACKGROUND: Reoperation for recurrent or persistent secondary hyperparathyroidism (SHPT) after parathyroidectomy is challenging due to surgical scars and postoperative adhesions. Therefore, there is an increasing need to develop a new minimally invasive therapy. OBJECTIVE: To analyze the efficacy of ultrasound (US)-guided radiofrequency ablation (RFA) in patients with recurrent or persistent SHPT after parathyroidectomy. PATIENTS AND METHODS: From March 2013 to January 2022, 20 enlarged parathyroid glands in 10 patients with recurrent or persistent SHPT were treated with US-guided RFA. The levels of serum intact parathyroid hormone (iPTH), calcium (Ca), phosphorus (P), and alkaline phosphatase (ALP), as well as clinical symptoms, were compared before and after RFA. The ablation procedure-related complications were also evaluated. The target range for iPTH levels was approximately 2-9 times the upper limit of normal (130-585 pg/mL). RESULTS: The mean follow-up time was 49.6 ± 34.5 months (range from 6 to101 months). The levels of serum iPTH, Ca, and P decreased significantly one day post-ablation. Six months after RFA, 70% of patients reached the targets for iPTH, and 50% of patients reached targets at the end of follow-up. Two patients underwent repeat ablation at 9 months and 6 years after RFA, respectively, due to persistently elevated iPTH levels, and both had serum iPTH concentrations in the recommended range at the recent follow-up visit. The patients' clinical symptoms significantly improved after ablation. Major complications after RFA included hoarseness (2/10) and permanent hypoparathyroidism (1/10). Severe hypocalcemia occurred in four patients (4/10) after ablation. CONCLUSION: US-guided percutaneous RFA for recurrent or persistent SHPT is safe, efficacious, and repeatable, and can significantly improve hyperparathyroidism-related symptoms.

RESUMO

PURPOSE: Secondary hyperparathyroidism (SHPT) is a frequently encountered problem in patients with end-stage renal disease (ESRD) prior to renal transplantation (RTP), and the successful management of SPHP currently is challenging. In this study, we aimed to investigate the effectiveness of radiofrequency ablation (RFA) for SHPT as a bridge to RTP and to evaluate post-transplantation outcomes. METHODS: Patients with SHPT receiving RFA treatment were retrospectively reviewed, and those underwent RTP after ablation were enrolled. Serum parathyroid hormone (PTH), calcium, and phosphate levels were collected before ablation and at follow-up periods. The primary endpoints are PTH values at time of transplantation and at the final follow-up. The secondary endpoints were RFA-related complications, serum calcium and phosphate concentrations, and allograft function. RESULTS: Eleven patients with 43 enlarged parathyroid glands were treated with 16 RFA sessions and enrolled in the study. Complete ablation was achieved in all glands with transient hoarseness and hypocalcemia occurring in two and five of the treatments, respectively. At time of transplantation, serum PTH levels (246.7 ± 182.6 pg/mL) were significantly lower than that before RFA (1666.55 ± 874.48 pg/mL, p < 0.001) and were all within guideline-oriented range. The median follow-up period was 57.2 months. At last visit, all patients were alive, with normal PTH values and functioning grafts. CONCLUSIONS: Ultrasound-guided RFA is effective for destroying hyperplastic parathyroid tissues in SHPT patients, whose PTH values fall within the guideline-oriented range both pre-and post-transplantation. Percutaneous RFA acts as an effective bridge to RTP and might provide a new management paradigm designed to improve post-transplant outcomes.

RESUMO

Xinfengjiang River, the largest tributary of Dongjiang River, plays a key role in the water supply of Heyuan, Huizhou, Guangzhou and even the Pearl River urban agglomeration. It is crucial to determine the pollution status, potential ecological risk degree of heavy metals in Xinfengjiang river sediment and their influence on the abundance of fish species. In this paper, seven heavy metal concentrations in sediment from the Heyuan section of the Xinfengjiang river were investigated. The order of average concentration was: As > Zn > Pb > Cr > Cu > Cd > Hg. The average concentrations of Cd, Zn and Cu in the upper reaches of the Xinfengjiang Reservoir were significantly higher than those in the reservoir. The mean value order of Igeo was: Cd > Zn > Pb > As > Cu > Cr > Hg. Cd and As had the highest ecological risk index and the greatest threat to the ecological environment. Pearson correlation analysis and principal component analysis demonstrated that the pollution source of heavy metals such as Cu and Cd are much more likely to originate from the mine fields located in the northeast of the sampling sites. In addition, agriculture, electronic industry and domestic sewage also contributed to the concentration of heavy metals in different degrees. Redundancy analysis showed that the abundance of Cypriniformes was negatively correlated with Cu and Cd concentrations, suggesting that mining activities might indirectly affect the abundance of fish species.

RESUMO

This work examined the potential of bioethanol production from expired cookies (EC) by the separate hydrolysis and fermentation process. EC was hydrolyzed by glucoamylase with different enzyme addition (3.5 U/g to 140 U/g) to produce the EC hydrolysate. The glucose concentration increased with enzyme addition from 3.5 U/g to 14 U/g and the highest glucose concentration of 21.2 g/L was obtained. The EC hydrolysate was used by Saccharomyces cerevisiae for bioethanol production. The optimal ethanol production obtained from this study was 40.1 g/L in term of economics and efficiency. According to the mass balance, the highest ethanol yield from EC was 0.4 g/g. Techno-economic analysis of the plant with capacity of 5 tons EC/day was also assessed in this study. The total capital cost and annual operation cost were US$540400.7 and US$144543.9/y, respectively. The revenue of the plant was US$390522/y with the sales of 660 t/y ethanol and 412.5 t/y oils. The plant should feed the EC more than 1.04 t/d (334.2 t/y) to avoid the shutdown point. This is the first study to demonstrate the bioethanol production from EC and assess the economic feasibility for industrial application.

RESUMO

Studying the spatial management scope of the river ecological corridor is a crucial step in effectively managing river health problems. For various purposes and needs, human beings intervene excessively in the river, resulting in the problems of unclear spatial scope, unclear ownership, and unreasonable functional utilization of the river ecological corridor. However, there is scarce research on the management scope of the river ecological corridor at present, and on the coordination relationship with territorial spatial protection planning. Therefore, in order to solve this key problem, this paper reviews and summarizes the current research status and development trends in terms of the concept, components, and other basic theories of the river ecological corridor, as well as relevant policy regulations. The relationship between the spatial scope of the river ecological corridor and the territorial spatial control line is analyzed, including the relationship with the river shoreline, aquatic ecological redline, "three control lines" and other control lines. Accordingly, this study reviewed the spatial management and control scope of the river ecological corridor. It also determined that the boundary line of the river shoreline management is the minimum line, the aquatic ecological redline, and the "three control lines" are the outermost boundary lines, in which the aquatic ecological redline has priority over other control lines. It also points out the thinking of determining the management scope in the protection and restoration of the river ecological corridor in the future. Our findings can provide a decision-making basis for the management of river ecological space.

RESUMO

BACKGROUND: At present, both 16G and 18G needles are used for percutaneous renal biopsy in China. This study aimed to compare the efficacy and safety of biopsy performed with the 18G needle vs. the 16G needle. METHODS: The data of patients who underwent percutaneous renal biopsy at our hospital between January 2015 and December 2019 were retrospectively analyzed. The number of glomeruli obtained by puncture and postoperative complications were compared between patients undergoing biopsy with the 16G and 18G needles. Continuous variables were compared by the t test or the Mann-Whitney U test, and categorical variables by the chi-square test. Correlation analysis was used to examine the relationship of different variables with hematoma size. RESULTS: Of the total 3138 kidney biopsies, 2526 were performed with the18G needle and 612 with the 16G needle. The number of glomeruli obtained was not significantly different between the two groups (P = 0.078). Large hematomas were significantly more common the 16G group than in the 18G group (9.31% vs. 5.98%, P = 0.003). Arteriovenous fistula was also more common in the 16G group (1.14% vs. 0.23%, P = 0.005). Other complications were rare, with similar incidence in the two groups. CONCLUSION: The 18G needle is as effective as the 16G needle for percutaneous renal biopsy. The risk of large hematoma and arteriovenous fistula appear to be lower with the 18G needle.

RESUMO

The damming of the river changes the structure of the original river ecosystem, and although fish passage plays an important role in maintaining the connectivity of the river ecosystem, the fish have difficulty finding the fish passage entrance during the upstream process. This paper studied the rheotaxis of fish under three different water flow conditions experimentally through recirculating water tanks. To better understand the response of Crucian carp (Carassius auratus) to water flow stimulation, the representative swimming trajectory, sensing success rate, attraction success rate, reaction time, and attraction time of the fish were analyzed by using a video monitoring system. The experimental results showed that fish responded differently to single-peak and lateral bimodal outflow conditions: (1) the single-peak outflow condition had a much better attraction effect than the lateral bimodal outflow condition, both in terms of sensing success rate and attraction success rate; (2) the fish swam mainly in the middle area of the lateral bimodal outflow condition, while the fish swam more evenly in the single-peak outflow condition. Therefore, setting the attraction current at the right time and near the entrance of the fish passage may help to improve the effect of fish attraction.

RESUMO

OBJECTIVE: To investigate the efficacy of radiofrequency ablation (RFA) as a treatment option for primary hyperparathyroidism (pHPT) and risk factors for postablative eucalcemic parathyroid hormone elevation (ePTH). METHODS: This retrospective study included 51 patients with pHPT who underwent RFA. The patients were divided into the ePTH and normal PTH groups, based on the serum intact parathyroid hormone (iPTH) level one month after ablation. Serum iPTH, calcium, and phosphorus levels, and the volume reduction rates (VRR) of the parathyroid glands were compared between the groups at each follow-up point. Risk factors for ePTH at one month after ablation were examined. RESULTS: After RFA, one (2%) patient had persistent pHPT, and 50 (98%) patients were cured. The incidence rates of ePTH at 1, 3, 6, and 12 months were 48%, 30%, 20%, and 16%, respectively. Serum iPTH levels in the ePTH group were higher than those in the normal PTH group at each follow-up point (all p < 0.05), except 1 day after ablation (p > 0.05). Serum calcium and phosphorus levels, and the VRR of the glands were comparable in both groups at each follow-up point (all p > 0.05), except for calcium levels 3 days after RFA (p < 0.05). Baseline iPTH (odds ratio, 1.067; p = 0.045) and calcium (odds ratio, 3.923; p = 0.038) levels were independent risk factors for ePTH 1 month after RFA. CONCLUSIONS: RFA is safe and effective for the treatment of pHPT. Moreover, ePTH occurrence after RFA was associated with baseline iPTH and calcium levels and did not increase the risk of recurrent pHPT.

RESUMO

Lake health assessment (LHA), a powerful tool for lake ecological protection, provides the foundation for sustainable water environment management. However, existing methods have not yet considered the effects of fuzziness and randomness on LHA. In addition, most of the current studies on LHA focus on the plain areas, lack of quantitative studies in mountain areas, such as the Qinghai-Tibet Plateau. The Pythagorean fuzzy cloud (PFC) integration algorithm drawing on the advantages of Pythagorean fuzzy sets (PFS) and cloud model was proposed. A novel hybrid decision-making framework combining PFC integration algorithm and TOPSIS model was developed to determine the lake health levels with fuzziness and randomness. An indicator system incorporating ecosystem integrity (physical habitat, water quantity and quality, aquatic life) and non-ecological performance (social services) was established. To comprehensively investigate the lake health level in the Qinghai-Tibet Plateau, the Nam Co was selected as study area. Our results confirm that the developed framework in this study can overcome the shortcomings of existing methods and provide a more effective approach for LHA with fuzziness and randomness. In Nam Co, the non-ecological performance was significantly better than the ecosystem integrity. Health levels exhibited a remarkable spatial variation influenced by tourism and grazing, with decreasing health status from the northwestern to southeastern Nam Co. Approximately 85% of the sampling sites were at excellent or healthy levels, 15% were subhealthy, and no sampling sites were unhealthy and sick. Our results highlight that tourism has affected health levels at Nam Co, and effective measures are needed to minimize the impact in ecological fragile areas.

RESUMO

This study aimed to utilize the enzymatic hydrolysis of leftover cooked rice (LCR) for fermentative ethanol production. Effect of glucoamylase volumes (V1: 5 U/g, V2: 25 U/g, and V3: 50 U/g) on the performance of LCR hydrolysis was also evaluated. It was found that the highest chemical oxygen demand (COD) of 77.5 g/L and reducing sugar (RS) of 34.6 g/L were achieved at V3. The LCR hydrolyzate obtained from enzymatic hydrolysis was then used as feedstock for ethanol fermentation. Higher ethanol production was obtained when RS increased from 18.7 g/L (V1) to 23.2 g/L (V2). However, lower ethanol production was found when RS further increased to 34.6 g/L (V3) probably because too high RS concentration led to the inhibition on the yeast. The maximum ethanol production and yield were 21.1 g/L and 0.3 g ethanol/g LCR, respectively. The LCR could be a promising substrate for fermentative ethanol production for industrial application.

RESUMO

Managed aquifer recharge (MAR) is an important approach to address water security, water quality decline, ground subsidence, and aquifer degradation. In this study, the large-scale recharge experiments were conducted in a natural river with multiple water sources. The MAR with multi-source water was investigated by developing an improved matter-element model under a limited recharged quantity and period in Jinan, China. Results showed that the background levels (BL) of groundwater quality before recharge was relatively good. However, the use of different water sources would cause a significant increase in the content of some groundwater quality indexes, which might further induce deterioration of regional groundwater quality. And the water quality in porous and karst aquifer displayed deteriorating trends during different water source recharge. Additionally, the adverse effects of recharge water sources on regional groundwater quality in turn was South-to-North Water Diversion Project (SN) > Yellow River (YR) > Wohushan Reservoir (WR). Meanwhile, the high-risk indexes in groundwater quality were presented during different water source recharge. Accordingly, relevant suggestions and measures were then put forward to optimize the MAR with multi-source water and explore the high-efficiency and low-risk recharge mode.

RESUMO

Investigation of the coupling coordination between urbanization and geological hazards is an important component in fostering environmentally sustainable development. This study constructed the comprehensive index system and developed the improved coupling coordination degree (CCD) model, Moran's I and projection pursuit model based on genetic algorithm to quantitatively investigate the dynamic mechanism and spatiotemporal distribution of coupling coordination between two subsystems within the context of the Five-Year Plan. Results showed that the comprehensive level of urbanization subsystem followed a continuous upward trend, and the geological hazard subsystem changed from a decreasing (2000-2010) to a rising (2011-2017) trend. A U-shaped relationship existed between two subsystems. And the CCD between two subsystems was higher in northeastern than that in southwestern China. Moreover, the significant spatial dependency existed in CCD. Driving by terrain, rainfall, vegetation cover, and human activities, the provinces in southwestern China exhibited low-low (L-L) clustering, while that in northeastern China showed high-high (H-H) clustering. In the past 17 years, the provinces in transitional and balanced development increased by 14.7% and 8.8%, and that in unbalanced development decreased by 23.5%. Overall, the provinces with low CCD were shrinking and the provinces with high CCD were extending from northeastern to southwestern China, meaning that the state of coupling coordination was changing from unbalanced to transitional development.

RESUMO

BACKGROUND: The purpose of this study was to determine if ultrasound-guided radiofrequency ablation (RFA) of hyperplastic parathyroid glands could be used to treat secondary hyperparathyroidism (HPT) in patients with chronic kidney disease. METHODS: RFA of the hyperplastic parathyroid glands was performed in 34 patients with secondary HPT. Intact parathyroid hormone (iPTH), calcium, and phosphorus were measured. The outcome was based on the ablation extent (ie, 4, 3, and 1-2 glands). RESULTS: The iPTH, calcium, and phosphorus levels decreased in all groups after RFA. One year after ablation, these parameters remained significantly lower in the 4-gland ablation group compared with the 3-gland and 1 to 2-gland groups. The same tendency was observed for the symptom score. The iPTH levels of <272 pg/mL on the day after ablation was the best predictor for maintaining parathyroid hormone (PTH) levels in a reasonable range 1 year after ablation. CONCLUSIONS: RFA of hyperplastic parathyroid glands for treating secondary HPT is feasible in selected patients. © 2016 Wiley Periodicals, Inc. Head Neck 39: 564-571, 2017.

RESUMO

OBJECTIVE: To assess the accuracy of detection by automated breast volume scanner (ABVS) in diagnosis of high-risk and small breast lesions. METHODS: One hundred and twelve patients with solid high-risk and small breast lesions were identified by ABVS. The patients were divided into benign lesion group and cancer group after pathological examination. The clinicopathological findings and ultrasonographic features of the lesions were compared. RESULTS: Among the 112 lesions there were 49 benign and 63 malignant lesions. The mean size on ABVS and pathology were (1.59 ± 0.52) cm and (1.52 ± 0.58) cm. There was no significant difference in tumor sizes determined by ABVS and pathology (P = 0.194). The mean age of patients with benign lesions was (38.5 ± 7.4) years and that of malignant lesions was (52.4 ± 13.6) years, showing a significant difference between the two groups (P < 0.001) . The mass shape, orientation, margin, lesion boundary, echo pattern, calcification, BI-RADS category and retraction phenomenon were significantly different of the malignant and benign masses (P < 0.05). But there was no significant difference in the location of lesions and posterior acoustic features (P > 0.05) . Retraction phenomenon was significantly associated with pathological type and histologic grade of the breast cancer (P < 0.01). The specificity, sensitivity and accuracy of retraction phenomenon were 100% (46/46), 73.0% (46/63), and 84.8% (95/112), respectively. CONCLUSIONS: ABVS provides advantages of better size prediction of high-risk and small breast lesions. Furthermore, the retraction phenomenon in coronal plane shows high specificity and sensitivity in detecting breast cancer.

RESUMO

This study is to investigate the tumor invasion and metastasis inhibition effects of the immunoconjugate composed of lidamycin and anti-type IV collagenase monoclonal antibody Fab' fragment. Boyden chamber assay was used to evaluate the influence of Fab'-LDM on HT-1080 cells invasion ability, gelatinase spectrum was used to measure the change of invasion factor MMP-2 and MMP-9's secretion, and RT-PCR was adopted to determine TIMP-1 mRNA expression level. The immunoconjugate inhibition of tumor in situ metastasis was also tested in nude mice. The Fab'-LDM conjugates had dose-dependent inhibition effect on HT-1080 cells' invasion. At the concentrations of 5 and 10 nmol L(-1), the Fab'-LDM inhibited the invasion by (60 +/- 12) % and (79 +/- 11) % respectively. At the concentration of 5 and 10 nmol L(-1), the Fab'-LDM inhibited the secretion of MMP-2 by (42 +/- 8) % and (54 +/- 6) % and that of MMP-9 by (57 +/- 3) % and (87 +/- 1) %, respectively. RT-PCR indicated that conjugates increased the anti-invasion factor TIMP-1 level. The in vivo experiment showed that, compared with the control group, the tumor inhibition rate in Fab', Fab'-LDM, and LDM group equaled to (30 +/- 13) %, (86 +/- 26) %, (74 +/- 22) % respectively. In conclusion, Fab'-LDM could inhibit the invasion and metastasis of tumor and it might be a new tumor biotherapy agent.

RESUMO

Pre-B cell leukemia transcription factor 1 (PBX1) encodes a homeodomain containing protein that is essential for pancreatic development and interacts with insulin promoter factor 1 to regulate insulin secretion. PBX1 maps to chromosome 1q22, a region with replicated linkage to type 2 diabetes (T2DM). We screened for sequence variation in nine exons, intronic regions flanking the exons, the 3' untranslated region (3' UTR), as well as 1-kb upstream of exon 1 in 16 Caucasians and 16 African American individuals with T2DM. We evaluated 18 variants including the nonsynonymous substitution G21S in exon 1, one 4 bp insertion/deletion, and one 7 bp insertion/deletion. We typed 10 variants on the basis of frequency and linkage disequilibrium patterns unrelated Caucasian subjects with T2DM and controls, and nine common variants in 129 Caucasian individuals for whom we had detailed assessments of insulin action and insulin secretion. We typed four common variants in African Americans individuals and additional SNPs in pooled DNA samples from both populations. No coding variant was associated with diabetes and no association was found among African American subjects. However, three variants in Caucasians (78287, 91227, and 252050 bp) were associated with T2DM (p<0.05), as were four marker haplotypes that included intron 2 variants. Additionally, three variants including G21S (61 bp) and the diabetes associated SNP at 78287 were significant determinants of insulin sensitivity (S(I)) in interaction with body mass index (p<0.02). Sequence variants in different locations of the PBX1 gene may have modest pleiotropic effects on T2DM susceptibility in Caucasians.
