Background: Deep Brain Stimulation (DBS) in the Subthalamic Nucleus (STN) or the Globus Pallidus Interna (GPI) is well-established as a surgical technique for improving global motor function in patients with idiopathic Parkinson's Disease (PD). Previous research has indicated speech deterioration in more than 30% of patients after STN-DBS implantation, whilst speech outcomes following GPI-DBS have received far less attention. Research comparing speech outcomes for patients with PD receiving STN-DBS and GPI-DBS can inform pre-surgical counseling and assist with clinician and patient decision-making when considering the neural targets selected for DBS-implantation. The aims of this pilot study were (1) to compare perceptual and acoustic speech outcomes for a group of patients with PD receiving bilateral DBS in the STN or the GPI with DBS stimulation both ON and OFF, and (2) examine associations between acoustic and perceptual speech measures and clinical characteristics. Methods: Ten individuals with PD receiving STN-DBS and eight individuals receiving GPI-DBS were audio-recorded reading a passage. Three listeners blinded to neural target and stimulation condition provided perceptual judgments of intelligibility and overall speech severity. Speech acoustic measures were obtained from the recordings. Acoustic and perceptual measures and clinical characteristics were compared for the two neural targets and stimulation conditions. Results: Intelligibility and speech severity were not significantly different across neural target or stimulation conditions. Generally, acoustic measures were also not statistically different for the two neural targets or stimulation conditions. Acoustic measures reflecting more varied speech prosody were associated with improved intelligibility and lessened severity. Convergent correlations were found between UPDRS-III speech scores and perceptual measures of intelligibility and severity. Conclusion: This study reports a systematic comparison of perceptual and acoustic speech outcomes following STN-DBS and GPI-DBS. Statistically significant differences in acoustic measures for the two neural targets were small in magnitude and did not yield group differences in perceptual measures. The absence of robust differences in speech outcomes for the two neural targets has implications for pre-surgical counseling. Results provide preliminary support for reliance on considerations other than speech when selecting the target for DBS in patients with PD.

INTRODUCTION: This study examined the utility of multiple second formant (F2) slope metrics to capture differences in speech production for individuals with dysarthria and healthy controls as a function of speaking rate. In addition, the utility of F2 slope metrics for predicting severity of intelligibility impairment in dysarthria was examined. METHODS: 23 speakers with Parkinson's disease and mild to moderate hypokinetic dysarthria (HD), 9 speakers with various neurological diseases and mild to severe ataxic dysarthria (AD), and 26 age-matched healthy control speakers (CON) participated in a sentence repetition task. Sentences were produced at habitual, fast, and slow speaking rate. A variety of metrics were derived from the rising second formant (F2) transition portion of the diphthong /ai/. To obtain measures of intelligibility for the two clinical speaker groups, 15 undergraduate SLP students participated in a transcription experiment. RESULTS: Significantly shallower slopes were found for the speakers with hypokinetic dysarthria compared to control speakers. Steeper F2 slopes were associated with increased speaking rate for all groups. Higher variability in F2 slope metrics was found for the speakers with ataxic dysarthria compared to the two other speaker groups. For both clinical speaker groups, there was a negative association between intelligibility and F2 slope variability metrics, indicating lower variability in speech production was associated with higher intelligibility. DISCUSSION: F2 slope metrics were sensitive to dysarthria presence, dysarthria type and speaking rate. The current study provided evidence that the use of F2 slope variability measures has additional value to F2 slope averaged measures for predicting severity of intelligibility impairment in dysarthria.

PURPOSE: Previous studies have found that typically developing (TD) children were able to compensate and adapt to auditory feedback perturbations to a similar or larger degree compared to young adults, while children with speech sound disorder (SSD) were found to produce predominantly following responses. However, large individual differences lie underneath the group-level results. This study investigates possible mechanisms in responses to formant shifts by modeling parameters of feedback and feedforward control of speech production based on behavioral data. METHOD: SimpleDIVA was used to model an existing dataset of compensation/adaptation behavior to auditory feedback perturbations collected from three groups of Dutch speakers: 50 young adults, twenty-three 4- to 8-year-old children with TD speech, and seven 4- to 8-year-old children with SSD. Between-groups and individual within-group differences in model outcome measures representing auditory and somatosensory feedback control gain and feedforward learning rate were assessed. RESULTS: Notable between-groups and within-group variation was found for all outcome measures. Data modeled for individual speakers yielded model fits with varying reliability. Auditory feedback control gain was negative in children with SSD and positive in both other groups. Somatosensory feedback control gain was negative for both groups of children and marginally negative for adults. Feedforward learning rate measures were highest in the children with TD speech followed by children with SSD, compared to adults. CONCLUSIONS: The SimpleDIVA model was able to account for responses to the perturbation of auditory feedback other than corrective, as negative auditory feedback control gains were associated with following responses to vowel shifts. These preliminary findings are suggestive of impaired auditory self-monitoring in children with complex SSD. Possible mechanisms underlying the nature of following responses are discussed.

BACKGROUND: Progressive ataxias frequently lead to speech disorders and consequently impact on communication participation and psychosocial wellbeing. Whilst recent studies demonstrate the potential for improvements in these areas, these treatments generally require intensive input which can reduce acceptability of the approach. A new model of care-ClearSpeechTogether-is proposed which maximises treatment intensity whilst minimising demands on clinician. This study aimed to establish feasibility and accessibility of this approach and at the same time determine the potential benefits and adverse effects on people with progressive ataxias. METHOD: This feasibility study targeted people with progressive ataxia and mild-moderate speech and gross motor impairment. ClearSpeechTogether consisted of four individual sessions over 2 weeks followed by 20 patient-led group sessions over 4 weeks. All sessions were provided online. Quantitative and qualitative data were collected for evaluation. RESULTS: Nine participants completed treatment. Feasibility and acceptability were high and no adverse effects were reported. Statistical tests found significantly reduced vocal strain, improved reading intelligibility and increased participation and confidence. Participant interviews highlighted the value of group support internalisation of speech strategies and psycho-social wellbeing. DISCUSSION: ClearSpeechTogether presented a feasible, acceptable intervention for a small cohort of people with progressive ataxia. It matched or exceeded the outcomes previously reported following individual therapy. Particularly notable was the fact that this could be achieved through patient led practice without the presence of a clinician. Pending confirmation of our results by larger, controlled trials, ClearSpeechTogether could represent an effective approach to manage speech problems in ataxia.

PURPOSE: This study investigated the effects of three clear speech variants on sentence intelligibility and speaking effort for speakers with Parkinson's disease (PD) and age- and sex-matched neurologically healthy controls. METHOD: Fourteen speakers with PD and 14 neurologically healthy speakers participated. Each speaker was recorded reading 18 sentences from the Speech Intelligibility Test in their habitual speaking style and for three clear speech variants: clear (SC; given instructions to speak clearly), hearing impaired (HI; given instructions to speak with someone with a hearing impairment), and overenunciate (OE; given instructions to overenunciate each word). Speakers rated the amount of physical and mental effort exerted during each speaking condition using visual analog scales (averaged to yield a metric of overall speaking effort). Sentence productions were orthographically transcribed by 50 naive listeners. Linear mixed-effects models were used to compare intelligibility and speaking effort across the clear speech variants. RESULTS: Intelligibility was reduced for the PD group in comparison to the control group only in the habitual condition. All clear speech variants significantly improved intelligibility above habitual levels for the PD group, with OE maximizing intelligibility, followed by the SC and HI conditions. Both groups rated speaking effort to be significantly higher for both the OE and HI conditions versus the SC and habitual conditions. DISCUSSION: For speakers with PD, all clear speech variants increased intelligibility to a level comparable to that of healthy controls. All clear speech variants were also associated with higher levels of speaking effort than habitual speech for the speakers with PD. Clinically, findings suggest that clear speech training programs consider using the instruction "overenunciate" for maximizing intelligibility. Future research is needed to identify if high levels of speaking effort elicited by the clear speech variants affect long-term sustainability of the intelligibility benefit.

OBJECTIVE: Reading a passage out loud is a commonly used task in the perceptual assessment of dysarthria. The extent to which perceptual characteristics remain unchanged or stable over the time course of a passage is largely unknown. This study investigated crowdsourced visual analogue scale (VAS) judgments of intelligibility across a reading passage as a function of cued speaking styles commonly used in treatment to maximize intelligibility. PATIENTS AND METHOD: The Hunter passage was read aloud in habitual, slow, loud, and clear speaking styles by 16 speakers with Parkinson's disease (PD), 30 speakers with multiple sclerosis (MS), and 32 control speakers. VAS judgments of intelligibility from three fragments representing the beginning, middle, and end of the reading passage were obtained from 540 crowdsourced online listeners. RESULTS: Overall passage intelligibility was reduced for the two clinical groups relative to the control group. All speaker groups exhibited intelligibility variation across the reading passage, with trends of increased intelligibility toward the end of the reading passage. For control speakers and speakers with PD, patterns of intelligibility variation across passage reading did not differ with speaking style. For the MS group, intelligibility variation across the passage was dependent on speaking style. CONCLUSIONS: The presence of intelligibility variation within a reading passage warrants careful selection of speech materials in research and clinical practice. Results further indicate that the crowdsourced VAS rating paradigm is useful to document intelligibility in a reading passage for different cued speaking styles commonly used in treatment for dysarthria.

Purpose This exploratory study sought to identify acoustic variables explaining rate-related variation in intelligibility for speakers with dysarthria secondary to multiple sclerosis. Method Seven speakers with dysarthria due to multiple sclerosis produced the same set of Harvard sentences at habitual and slow rates. Speakers were selected from a larger corpus on the basis of rate-related intelligibility characteristics. Four speakers demonstrated improved intelligibility and three speakers demonstrated reduced intelligibility when rate was slowed. A speech analysis resynthesis paradigm termed hybridization was used to create stimuli in which segmental (i.e., short-term spectral) and suprasegmental variables (i.e., sentence-level fundamental frequency, energy characteristics, and duration) of sentences produced at the slow rate were donated individually or in combination to habitually produced sentences. Online crowdsourced orthographic transcription was used to quantify intelligibility for six hybridized sentence types and the original habitual and slow productions. Results Sentence duration alone was not a contributing factor to improved intelligibility associated with slowed rate. Speakers whose intelligibility improved with slowed rate showed higher intelligibility scores for duration spectrum hybrids and energy hybrids compared to the original habitual rate sentences, suggesting these acoustic cues contributed to improved intelligibility for sentences produced with a slowed rate. Energy contour characteristics were also found to play a role in intelligibility losses for speakers with decreased intelligibility at slowed rate. The relative contribution of speech acoustic variables to intelligibility gains and losses varied considerably between speakers. Conclusions Hybridization can be used to identify acoustic correlates of intelligibility variation associated with slowed rate. This approach has further elucidated speaker-specific and individualized speech production adjustments when slowing rate.

Auditory feedback plays an important role in speech motor learning, yet, little is known about the strength of motor learning and feedback control in speech development. This study investigated compensatory and adaptive responses to auditory feedback perturbation in children (aged 4-9 years old) and young adults (aged 18-29 years old). Auditory feedback was perturbed by near-real-time shifting F1 and F2 of the vowel /ɪː/ during the production of consonant-vowel-consonant words. Children were able to compensate and adapt in a similar or larger degree compared to young adults. Higher token-to-token variability was found in children compared to adults but not disproportionately higher during the perturbation phases compared to the unperturbed baseline. The added challenge to auditory-motor integration did not influence production variability in children, and compensation and adaptation effects were found to be strong and sustainable. Significant group differences were absent in the proportions of speakers displaying a compensatory or adaptive response, an amplifying response, or no consistent response. Within these categories, children produced significantly stronger compensatory, adaptive, or amplifying responses, which could be explained by less-ingrained existing representations. The results are interpreted as both auditory-motor integration and learning capacities are stronger in young children compared to adults.

Background With respect to the clinical criteria for diagnosing childhood apraxia of speech (commonly defined as a disorder of speech motor planning and/or programming), research has made important progress in recent years. Three segmental and suprasegmental speech characteristics-error inconsistency, lengthened and disrupted coarticulation, and inappropriate prosody-have gained wide acceptance in the literature for purposes of participant selection. However, little research has sought to empirically test the diagnostic validity of these features. One major obstacle to such empirical study is the fact that none of these features is stated in operationalized terms. Purpose This tutorial provides a structured overview of perceptual, acoustic, and articulatory measurement procedures that have been used or could be used to operationalize and assess these 3 core characteristics. Methodological details are reviewed for each procedure, along with a short overview of research results reported in the literature. Conclusion The 3 types of measurement procedures should be seen as complementary. Some characteristics are better suited to be described at the perceptual level (especially phonemic errors and prosody), others at the acoustic level (especially phonetic distortions, coarticulation, and prosody), and still others at the kinematic level (especially coarticulation, stability, and gestural coordination). The type of data collected determines, to a large extent, the interpretation that can be given regarding the underlying deficit. Comprehensive studies are needed that include more than 1 diagnostic feature and more than 1 type of measurement procedure.

BACKGROUND/PURPOSE: Several studies indicate a close relation between auditory and speech motor functions in children with speech sound disorders (SSD). The aim of this study was to investigate the ability to compensate and adapt for perturbed auditory feedback in children with SSD compared to age-matched normally developing children. METHOD: 17 normally developing children aged 4.1-8.7 years (mean=5.5, SD=1.4), and 11 children with SSD aged 3.9-7.5 years (mean=5.1, SD=1.0) participated in the study. Auditory feedback was perturbed by real-time shifting the first and second formant of the vowel /e/ during the production of CVC words in a five-step paradigm (practice/familiarization; start/baseline; ramp; hold; end/release). RESULTS: At the group level, the normally developing children were better able to compensate and adapt, adjusting their formant frequencies in the direction opposite to the perturbation, while the group of children with SSD followed (amplifying) the perturbation. However, large individual differences lie underneath. Furthermore, strong correlations were found between the amount of compensation and performance on oral motor movement non-word repetition tasks. CONCLUSIONS: Results suggested that while most children with SSD can detect incongruencies in auditory feedback and can adapt their target representations, they are unable to compensate for perturbed auditory feedback. These findings suggest that impaired auditory-motor integration may play a key role in SSD. LEARNING OUTCOMES: The reader will be able to: (1) describe the potential role of auditory feedback control in developmental speech disorders (SSD); (2) identify the neural control subsystems involved in feedback based speech motor control; (3) describe the differences between compensation and adaptation for perturbed auditory feedback; (4) explain why auditory-motor integration may play a key role in SSD.

PURPOSE: Following recent attempts to quantify articulatory impairment in speech, the present study evaluates the usefulness of a novel measure of motor stability to characterize dysarthria. METHOD: The study included 8 speakers with ataxic dysarthria (AD), 16 speakers with hypokinetic dysarthria (HD) as a result of Parkinson's disease, and 24 unimpaired control participants. Each participant performed a series of sentence repetitions under habitual, fast, and slow speaking rate conditions. An algorithm to measure utterance-to-utterance spectro-temporal variation (UUV; Cummins, 2009) was used. Speech rate and intelligibility were also measured. RESULTS: UUV scores were significantly correlated with perceptually based intelligibility scores. There were significant differences in UUV between control speakers and the AD but not the HD groups, presumably because of differences in intelligibility in the samples used and not because of differences in pathology. Habitual speaking rate did not correlate with UUV scores. All speaker groups had greater UUV levels in the slow conditions compared with habitual and fast speaking rates. CONCLUSIONS: UUV results were consistent with those of other variability indices and thus appear to capture motor control issues in a similar way. The results suggest that the UUV could be developed into an easy-to-use clinical tool that could function as a valid and reliable assessment and outcome measure.

AIMS: This study aimed to investigate the effects of speech rate changes on kinematic characteristics and stability of speech movements in younger and older speakers using electromagnetic midsagittal articulography. PATIENTS AND METHODS: Eight young adults and 8 older adults engaged in a series of syllable repetition tasks of /pa/, /sa/ and /ta/ obtained at self-paced slow, habitual and fast speech rates, as well as in a series of metronome-guided speech rates, ranging from 2 to 4 syllables per second. The kinematic parameters duration, amplitude and peak velocity were obtained for opening and closing movements. RESULTS: Older speakers were able to increase speech rate to the same degree or higher compared to younger speakers in both pacing conditions. Kinematic data show that older adults increased duration and decreased peak velocity in closing movements of alveolar constrictions at slower rates more prominently than younger adults. The results on movement stability revealed no differences between age groups. CONCLUSIONS: The results suggest that an age-related difference in speed-accuracy trade-off can be ruled out. Differences in kinematic characteristics point towards the possibility that older adults aimed to facilitate a closed-loop control system to maintain movement stability at slower speech rates.
