Abstract Tapeworms of zoonotic importance have been described as a leading public health problem. Current research was aim to assess the prevalence of tapeworms among 5-12years school children residing in district Lower Dir, Pakistan from January 2019-December 2019. The wet mount preparation in saline/iodine/methods were used for stool examination. Data was analyzed using appropriate descriptive, static methods. Of the 400 children studied 71.7% were infected with one or more species of intestinal parasites. Single infection of cestode species was found in 69 individuals with 17.2% prevalence and multiple parasitic infections were identified in 19.7% (n=79/400) individuals. The multiple infection were comprised as 10% (n=40) double, 6.75% (n=27) triple and 3% (n=12) quadruple. A total of 9 species of helminths and one species of protozoan infection. Among the helminths Ascaris lumbricoides was the most prevalent 33.1% (n=95), Taenia saginata 22.6% (n=65), hookworm 19.8% (n=57), Hymenolepis nana 18.8% (n=54), Enterobius vermicularis and Hymenolepis diminuta 1.39% (n=4each), Trichuris trichura 1.04% (n=3), Toxocara spp 0.69% (n=2) and Schistosoma japonicum 0.34% (n=1) were reported. One protozoan species was Cryptosporidium spp 0.69% (n=2) in current study. In case of A.lumbricoides, hookworm, E.vermicularis, T.trichura, T.saginata, H.nana and H.diminuta the male children of below 8 years of age were highly infected. Other infections are reported in the same prevalence with slight difference if any. We conclude that there is a need for mass scale campaigns to create awareness regarding health and hygiene in children and the need for development of effective poverty control programs because deworming alone is not adequate to control parasitic infections.


Resumo As tênias de importância zoonótica têm sido descritas como um dos principais problemas de saúde pública. A pesquisa atual teve como objetivo avaliar a prevalência de tênias entre crianças em idade escolar de 5 a 12 anos que residem no distrito de Lower Dir, Paquistão, de janeiro de 2019 a dezembro de 2019. Os métodos de preparação para montagem úmida em solução salina/iodo foram usados ​​para exame de fezes. Os dados foram analisados ​​usando métodos descritivos e estáticos apropriados. No caso de A.lumbricoides, ancilostomíase, E.vermicularis, T.trichura, T.saginata, H.nana e H.diminuta, as crianças do sexo masculino com menos de 8 anos de idade estavam altamente infectadas. Outras infecções são relatadas na mesma prevalência, com ligeira diferença, se houver. Concluímos que há uma necessidade de campanhas em massa para criar consciência sobre saúde e higiene em crianças e a necessidade de desenvolvimento de programas eficazes de controle da pobreza, porque a desparasitação por si só não é adequada para controlar infecções parasitárias.

The article describes the effectiveness of the treatment of chronic apical periodontitis in one visit compared to multiple visits. A systematic review of the literature was conducted analyzing articles published on PubMed, Google scholar, ResearchGate and ScienceDirect between 2011 and 2021, treatment effectiveness was the selected outcome variable. A total of 21 articles were selected, 9 of which were selected for detailed review. Chief question in this article was: which treatment approach is more effective? Cleaning, Shaping, disinfecting and obturating in one visit? Or multiple (two) visit protocol? Chronic Apical Periodontitis is treatable in one visit, if done properly. Chronic Apical Periodontitis is treatable in one visit, if done properly, with proper shaping, disinfection, and proper hermetic obturation of the root canal. Bioceramic sealers showed the highest success rate out of all sealers used, the usage of 3-5.25% sodium hydrochloride and 17% ethylenediaminetetraacetic acid showed favorable results, although photodynamic therapy and ultrasonic activated irrigation showed outstanding results.

In this manuscript, a new nonlinear model for the rapidly spreading Corona virus disease (COVID-19) is developed. We incorporate an additional class of vaccinated humans which ascertains the impact of vaccination strategy for susceptible humans. A complete mathematical analysis of this model is conducted to predict the dynamics of Corona virus in the population. The analysis proves the effectiveness of vaccination strategy employed and helps public health services to control or to reduce the burden of corona virus pandemic. We first prove the existence and uniqueness and then boundedness and positivity of solutions. Threshold parameter for the vaccination model is computed analytically. Stability of the proposed model at fixed points is investigated analytically with the help of threshold parameter to examine epidemiological relevance of the pandemic. We apply LaSalle's invariance principle from the theory of Lyapunov function to prove the global stability of both the equilibria. Two well known numerical techniques namely Runge-Kutta method of order 4 (RK4), and the Non-Standard Finite Difference (NSFD) method are employed to solve the system of ODE's and to validate our obtained theoretical results. For different coverage levels of voluntary vaccination, we explored a complete quantitative analysis of the model. To draw our conclusions, the effect of proposed vaccination on threshold parameter is studied numerically. It is claimed that Corona virus disease could be eradicated faster if a human community selfishly adopts mandatory vaccination measures at various coverage levels with proper awareness. Finally, we have executed the joint variability of all classes to understand the effect of vaccination strategy on a disease dynamics.

RESUMO

We perform a theoretical study of an atomically thin, two-dimensional layer obtained by positioning atoms at the vertices of the classical Pythagorean tiling. This leads to an unusual geometrical pattern that is only stable for the three halogens Cl, Br, and I. In this Pythagorean structure, halogen atoms are arranged in strongly bound diatomic units that bind together by weaker electrostatic bonds. The energy of these phases is competitive with those of the low-temperature phase of the halogens and the two-dimensional layer obtained by exfoliating it. The Pythagorean layers are semiconducting, with an unusual band structure composed of very mobile holes and extremely heavy electrons. They are also soft, exhibiting small values of the elastic constants and a very low energy flexural mode. Analysis of the allowed Raman transitions reveals breathing-like modes that might be used to fingerprint, experimentally, the Pythagorean structure. Finally, we present a series of substrates that, due to lattice matching and compatible symmetry, can be used to stabilize these peculiar two-dimensional layers.

RESUMO

The excessive use of nodes and communication links in a wireless control system (WCS) causes unnecessary utilization of resources. In this article, a strategic topological formation is studied for a WCS, where a previously proposed topology consisting of a plant system, a controller system, and an intermediate network system is further developed. More specifically, this article presents a modeling framework and a design procedure for the topology that results in the utilization of a reduced number of nodes and communication links. It also discusses several conditions for the connectivity of the nodes under different topological scenarios. This article uses a four-tank process system as an application example to demonstrate the strategic topological formation of its WCS.

BACKGROUND: Gender differences in the thrombotic and bleeding risk have been suggested to condition the benefits of antithrombotic therapies in Acute Coronary Syndrome (ACS) patients, and mainly among those undergoing percutaneous coronary interventions with drug eluting stents (DES). The impact of gender on the optimal duration of dual antiplatelet therapy (DAPT) in ACS patients is still unexplored and was, therefore, the aim of the present sub-study. METHODS: REDUCE was a prospective, multicenter, randomized investigator-initiated study designed to enroll 1500 ACS patients after treatment with the COMBO Dual Stent Therapy, based on a noninferiority design. Patients were randomized in a 1:1 fashion to either 3 or 12 months of DAPT. Primary study endpoint was a composite of all-cause mortality, myocardial infarction, definite/probable stent thrombosis (ST), stroke, target-vessel revascularization (TVR) and bleedings (BARC II, III, V) at 12 months. Secondary endpoints were cardiovascular mortality and the individual components of the primary endpoint within 24 months. RESULTS: From June 2014 to May 2016 300 women and 1196 men were included in the study. Among them, 43.7% of females and 51.9% of males were assigned to the 3 months DAPT treatment. Baseline characteristics were well matched between the two arms, with the exception of a lower rate of TIMI flow < 3 (p = 0.04), lower systolic blood pressure (p = 0.05) and use of spironolactone (p = 0.006) among women and a more advanced age (p = 0.05) among men receiving a short-term DAPT. At a mean follow-up of 525 (± 198) days, no difference in the primary endpoint was observed according to DAPT duration in both females [6.9% vs 5.9%, HR (95% CI) = 1.19 (0.48-2.9), p = 0.71] and males [8.2% vs 9%, HR (95% CI) = 0.92 (0.63-1.35), p = 0.67; p INT = 0.20]. Results were confirmed after correction for baseline differences [females: adjusted HR (95% CI) = 1.12 (0.45-2.78), p = 0.81; males: adjusted HR (95% CI) = 0.90 (0.61-1.32), p = 0.60]. Comparable rates of survival, thrombotic (MI, stent thrombosis, TVR, stroke) and bleeding events were observed with the two DAPT strategies, with no impact of gender. CONCLUSIONS: The present study shows that among ACS patients randomized in the REDUCE trial, a 3 months DAPT strategy offers comparable results as compared to a standard 12 months DAPT at 2-years follow-up in both male and female gender.

Metabolic syndrome (MS) is comprised of a cluster of abnormalities in glucose, lipid, and vascular homeostasis, which is most commonly linked to abdominal obesity. MS heralds increased risk for development of diabetes and is linked to impairment in insulin signaling. Insulin-degrading enzyme (IDE) is one of the mechanisms through which insulin blood levels are maintained. It has been previously suggested that controlling IDE levels could provide yet another potential therapeutic approach in diabetes. Here we aim to investigate whether changes in serum IDE levels correlate with the severity of MS. Using a highly sensitive ELISA assay of active IDE in human serum, we found a strong correlation between circulating IDE levels and circulating levels of triglycerides, insulin, and c-peptide and an inverse correlation with HDL cholesterol (HDLc). Serum IDE levels were higher in MS subjects than in control subjects. Hence, circulating IDE may serve as a tool to identify subjects with abnormal insulin metabolism, possibly those with MS that are at risk to develop diabetes.

Food animal welfare is an issue of great concern, as society has a responsibility for animals under human care. Pork is the most consumed meat worldwide, with more than a billion pigs being slaughtered globally every year. Still, in most countries, sows are restrained in farrowing crates throughout lactation. In these crates, sows are confined with bars to an area that is just slightly larger than their body. Thus, moving and turning around, grooming, or expressing other natural behaviors are typically impossible. In this study, we utilized a simple and practical modification of conventional farrowing crates to designed farrowing pens, by removable confinement bars, which provide the flexibility to change the housing system from one to another. Our objective was to examine the parameters of production and hair cortisol concentrations after different restraint periods during lactation. Analyses included data from 77 sows and their 997 piglets. Sows were housed in farrowing crates, but the confinement bars were removed after different periods, from 3 days post-farrowing to full restraint. For certain analyses, sows were grouped into Short or Long Restraint groups (3-10 days vs 13-24 days, respectively). Multiple linear regression revealed that for any additional day in restraint of the sows, piglets' weaning rate decreases by 0.4% (P < 0.05). Moreover, the total number of weaned piglets per litter was higher in the Short Restraint group as compared to the Long Restraint group (10.4 ±â€¯0.3 vs 9.7 ±â€¯0.3, respectively; P < 0.05). Accordingly, total litter weight on the weaning day tended to be higher in the Short Restraint group (68.8 ±â€¯2.2 vs 64.9 ±â€¯1.8 kg; P = 0.1210). The requirement for medical treatments during lactation (e.g., antibiotics, NSAID) tended to be less frequent in the Short Restraint group (Sows: 21.9% vs 40%; P = 0.1219. Piglets: 2.4% vs 17.1%; P = 0.0609). Hair cortisol as a marker for chronic stress during lactation decreased when the restraint period was shortened in both sows and piglets. Our analysis revealed that sows' hair cortisol is a significant mediator between the restraint of the sow and its piglets' hair cortisol (Sobel test; P < 0.05). For every day of sows' restraint, sows' hair cortisol increased by 0.5 pg/mg, and for any additional unit of sows' hair cortisol, piglets' hair cortisol increased by 0.36 pg/mg. In conclusion, sustainable swine farming management can be beneficial for both animals and farmers; limiting sow restraint during lactation is expected to reduce stress, enhance welfare and production, and potentially improve the economics of swine operations.

We describe a case of delayed onset, acute demyelinating neuropathy secondary to novel SARS-CoV-2 infection. A previously healthy 46-year-old man presented with bilateral leg pain and loss of sensation in his feet 53 days after having COVID-19 pneumonitis. He developed painful sensory symptoms followed by a rapidly progressive lower motor neuron weakness involving all limbs, face and respiratory muscles, needing ventilatory support. In keeping with a diagnosis of Guillain-Barré syndrome, cerebrospinal fluid examination showed albuminocytologic dissociation and nerve conduction studies supported the diagnosis of an acute inflammatory demyelinating polyradiculoneuropathy. The delayed neurological dysfunction seen in our patient following SARS-CoV-2 infection may indicate a novel mechanism of disease that is part of the emerging 'long COVID-19 syndrome'.

With the global pandemic due to coronavirus disease 2019 (COVID-19), there has been a significant strain on healthcare facilities. The infectivity rate, as well as the rate of healthcare workers who have fallen ill to the disease, has raised concerns globally on the proper management of patients as well as the role of safe healthcare provision utilizing personal protective equipment (PPE). Furthermore, the limited supply of PPEs has mandated rationing their use to achieve maximum utility and preservation. Multiple gastroenterology associations have issued guidance and statements that would help healthcare providers in navigating these unprecedented and difficult times, and the Saudi Gastroenterology Association has provided this statement in an effort to bring the most up to date information for the management of endoscopy units in terms of resources, manpower planning, scheduling, as well as infection control policies and leadership.

The semimetallic bandstructure of graphene and silicene limit their use in functional devices. Mixing silicon and carbon offers a rather unexplored pathway to build semiconducting sheets compatible with current Si-based electronics. We present here a complete theoretical study of the phase diagram of two-dimensional silicon-carbon binaries. To scan the composition range, we employ an ab initio global structural prediction method, complemented by exhaustive enumeration of two-dimensional structure prototypes. We find a wealth of two-dimensional low-energy structures, from standard honeycomb single- and double-layers, passing by dumbbell geometries, to carbon nanosheets bridged by Si atoms. Many of these phases depart from planarity, either through buckling, or by germinating three-dimensional networks with a mixture of sp2 and sp3 bonds. We further characterize the most interesting crystal structures, unveiling a large variety of electronic properties, that could be exploited to develop high-performance electronic devices at the nanoscale.

Polo-like kinases (Plks) are key cell cycle regulators. They contain a kinase domain followed by a polo-box domain that recognizes phosphorylated substrates and enhances their phosphorylation. The regulatory subunit of the Dbf4-dependent kinase complex interacts with the polo-box domain of Cdc5 (the sole Plk in Saccharomyces cerevisiae) in a phosphorylation-independent manner. We have solved the crystal structures of the polo-box domain of Cdc5 on its own and in the presence of peptides derived from Dbf4 and a canonical phosphorylated substrate. The structure bound to the Dbf4-peptide reveals an additional density on the surface opposite to the phospho-peptide binding site that allowed us to propose a model for the interaction. We found that the two peptides can bind simultaneously and non-competitively to the polo-box domain in solution. Furthermore, point mutations on the surface opposite to the phosphopeptide binding site of the polo-box domain disrupt the interaction with the Dbf4 peptide in solution and cause an early anaphase arrest phenotype distinct from the mitotic exit defect typically observed in cdc5 mutants. Collectively, our data illustrates the importance of non-canonical interactions mediated by the polo-box domain and provide key mechanistic insights into the combinatorial recognition of substrates by Polo-like kinases.

The current study explores the use of quercetin for developing a highly selective spectrofluorimetric methodology for trace determination, speciation and thermodynamic characterization of tungstate (WO42-) species in water. The study relies on the principle of chelate formation between WO42- and quercetin with subsequent increase in the emission intensity. The developed method could be applied successfully in a wide linear range (1.0-400.0 µg L-1) with a detection limit of 0.28 µg L-1 and quantification limit of 0.92 µg L-1 at λex/em = 400/492 nm. The developed method was successfully applied in real tap and waste water samples. The suitability of the proposed method was further validated by inductively coupled plasma-optical emission spectrometry (ICP-OES) in terms of student's t and F tests at 95% confidence. Characterization (NMR, FTIR and electronic spectra), stoichiometry, stability constant, fluorescence mechanism and thermodynamic parameters (ΔH, ΔS, and ΔG) of the produced complex species were evaluated and properly assigned. The fluorescence quenching mechanism of tungstate quercetin complex by Triton X-100 was also evaluated for computing Stern-Volmer quenching constant and approximating quenching sphere. The method showed a clear significance over most of the reported methods for tungsten in literature in terms of good accuracy, robustness, ruggedness, short analytical time and cost-effectiveness.

The syndrome presents prominent clinical features, including alopecia, neuroendocrine defects, neurological findings and progressive hearing loss. The condition results from mutations in the DCAF17 gene. AIMS: To search for the underlying genetic defect in a Pakistani family with WSS phenotypes. METHODOLOGY: Whole exome sequencing was used to search for the disease-causing variant. RESULTS: Analysis of the exome data revealed a start loss sequence variant (c.1A>G, p.M1?) in DCAF17. CONCLUSION: This variant is predicted to abolish translation of the DCAF17 polypeptide. To our knowledge, this is the first start loss variant identified in the DCAF17.

PURPOSE: To present the level of knowledge, attitude, and practice (KAP) for glaucoma management and their determinants among adult Saudi patients. METHODS: The study was conducted between January and June 2017. Glaucoma patients from the ophthalmology clinic were interviewed. Demographics and disease-related information were noted. The questionnaire included four K questions, seven A questions, and six P squestions. The correct response of questions were summed for K, A, and P, and the percentile was calculated. K and P scores were graded as excellent (more than >75%), good (51%-75%), poor (26%-50%), and very poor (≤25%). If A score was >50%, it was termed as positive. RESULTS: We interviewed 263 adult glaucoma patients. Their mean age was 61.6 ± 14.2 years. The excellent grade of knowledge about glaucoma and its management was 63 (24% [95% confidence interval (CI) 18.8-29.1]). Poor grade of knowledge was 95 (36.1% [95% CI 30.3-41.9]). The attitude toward glaucoma and its management was positive in 89 (33.8% [95% CI 28.1-39.6]). The practice to manage glaucoma was of excellent grade in 10 (3.8% [95% CI 1.5-6.1]). The overall excellent grade of KAP glaucoma patients was 17.9% (13.2-22.5). Good practice for glaucoma management was associated with a history of glaucoma surgery (P < 0.001), longer duration of glaucoma (P = 0.02), and young age (P = 0.004). CONCLUSIONS: Low KAP about glaucoma disease and medication among Saudi glaucoma patients is a matter of concern. Strategies are needed to improve the KAP of glaucoma patients.

Toxoplasmosis is a zoonotic disease caused by Toxoplasma gondii that is prevalent in humans and animals. This study was aimed to determine the seroprevalence of T. gondii infection among hemato-oncology patients and its association with sociodemographic and behavioural characteristics. This cross-sectional study was conducted at the Hospital Universiti Sains Malaysia (USM) involving 56 blood samples from hemato-oncology patients. Anti-T. gondii IgG and IgM antibodies and IgG avidity were determined using enzyme-linked immunosorbent assays (ELISA). The association of T. gondii exposure, sociodemographic, and behavioural characteristics were assessed by a questionnaire and face-to-face interviews. Twenty-eight (50%) patients were seropositive for T. gondii antibodies, where 27 (48.21%) patients were IgG+/IgM- and one patient (1.79%) was IgG+/IgM+ with high avidity index, indicating infection of more than 20 weeks. A univariate analysis showed that age, gender, ethnicity, marital status, educational level, employment status, stem cell transplant, blood transfusion, close contact with cats, water supply, and consumption of undercooked meat were not significantly associated with Toxoplasma seropositivity (p < 0.05). Our study has demonstrated, for the first time, the serological evidence of T. gondii exposure among hemato-oncology patients in Hospital USM. Our findings indicated that latent toxoplasmosis was relatively prevalence among our patients. Therefore, serological screening tests should be considered for immunocompromised patients as well as the implementation of health education programmes to encourage a healthy lifestyle and the consumption of healthy food among them.

Two new species of the genus Coomansinema Ahmad and Jairajpuri, 1989 are described and illustrated. C. japonicum n. sp. is characterized by having medium size body (L= 1.40 - 1.45 mm); lip region truncate with completely amalgamated lips; amphideal fovea goblet - shaped; 16 - 20 µm long odontostyle; 23 - 25 µm long odontophore; comparatively anterior position of the second pair of pharyngeal glands; amphidelphic female genital system; longitudinal vulva; males with 48 - 54 µm long spicules; 7 - 8 spaced ventromedian supplements and tail long filiform in female and short conoid in male. C. longicaudatum n. sp. is characterized by having medium size body (L= 1.1 - 1.3 mm); lip region truncate, continuous with completely amalgamated lips; amphideal fovea cup - shaped; 16 - 17 µm long odontostyle; 19 - 20 µm long odontophore; comparatively anterior position of the second pair of pharyngeal glands; amphidelphic female genital system; transverse vulva, intestinal - prerectum junction with a tongue - like structure and 210 - 269 µm long filiform tail. A key to its seven valid species is provided.

We compile a large data set designed for the efficient benchmarking of exchange-correlation functionals for the calculation of electronic band gaps. The data set comprises information on the experimental structure and band gap of 472 nonmagnetic materials and includes a diverse group of covalent-, ionic-, and van der Waals-bonded solids. We used it to benchmark 12 functionals, ranging from standard local and semilocal functionals, passing through meta-generalized-gradient approximations, and several hybrids. We included both general purpose functionals, like the Perdew-Burke-Ernzerhof approximation, and functionals specifically crafted for the determination of band gaps. The comparison of experimental and theoretical band gaps shows that the modified Becke-Johnson is at the moment the best available density functional, closely followed by the Heyd-Scuseria-Ernzerhof screened hybrid from 2006 and the high-local-exchange generalized-gradient approximation.

The ß-clamp is a protein hub central to DNA replication and fork management. Proteins interacting with the ß-clamp harbor a conserved clamp-binding motif that is often found in extended regions. Therefore, clamp interactions have -almost exclusively- been studied using short peptides recapitulating the binding motif. This approach has revealed the molecular determinants that mediate the binding but cannot describe how proteins with clamp-binding motifs embedded in structured domains are recognized. The mismatch repair protein MutL has an internal clamp-binding motif, but its interaction with the ß-clamp has different roles depending on the organism. In Bacillus subtilis, the interaction stimulates the endonuclease activity of MutL and it is critical for DNA mismatch repair. Conversely, disrupting the interaction between Escherichia coli MutL and the ß-clamp only causes a mild mutator phenotype. Here, we determined the structures of the regulatory domains of E. coli and B. subtilis MutL bound to their respective ß-clamps. The structures reveal different binding modes consistent with the binding to the ß-clamp being a two-step process. Functional characterization indicates that, within the regulatory domain, only the clamp binding motif is required for the interaction between the two proteins. However, additional motifs beyond the regulatory domain may stabilize the interaction. We propose a model for the activation of the endonuclease activity of MutL in organisms lacking methyl-directed mismatch repair.
