RESUMO

The ability to initiate volitional action is fundamental to human behaviour. Loss of dopaminergic neurons in Parkinson's disease is associated with impaired action initiation, also termed akinesia. Both dopamine and subthalamic deep brain stimulation (DBS) can alleviate akinesia, but the underlying mechanisms are unknown. An important question is whether dopamine and DBS facilitate de novo build-up of neural dynamics for motor execution or accelerate existing cortical movement initiation signals through shared modulatory circuit effects. Answering these questions can provide the foundation for new closed-loop neurotherapies with adaptive DBS, but the objectification of neural processing delays prior to performance of volitional action remains a significant challenge. To overcome this challenge, we studied readiness potentials and trained brain signal decoders on invasive neurophysiology signals in 25 DBS patients (12 female) with Parkinson's disease during performance of self-initiated movements. Combined sensorimotor cortex electrocorticography (ECoG) and subthalamic local field potential (LFP) recordings were performed OFF therapy (N = 22), ON dopaminergic medication (N = 18) and ON subthalamic deep brain stimulation (N = 8). This allowed us to compare their therapeutic effects on neural latencies between the earliest cortical representation of movement intention as decoded by linear discriminant analysis classifiers and onset of muscle activation recorded with electromyography (EMG). In the hypodopaminergic OFF state, we observed long latencies between motor intention and motor execution for readiness potentials and machine learning classifications. Both, dopamine and DBS significantly shortened these latencies, hinting towards a shared therapeutic mechanism for alleviation of akinesia. To investigate this further, we analysed directional cortico-subthalamic oscillatory communication with multivariate granger causality. Strikingly, we found that both therapies independently shifted cortico-subthalamic oscillatory information flow from antikinetic beta (13-35 Hz) to prokinetic theta (4-10 Hz) rhythms, which was correlated with latencies in motor execution. Our study reveals a shared brain network modulation pattern of dopamine and DBS that may underlie the acceleration of neural dynamics for augmentation of movement initiation in Parkinson's disease. Instead of producing or increasing preparatory brain signals, both therapies modulate oscillatory communication. These insights provide a link between the pathophysiology of akinesia and its' therapeutic alleviation with oscillatory network changes in other non-motor and motor domains, e.g. related to hyperkinesia or effort and reward perception. In the future, our study may inspire the development of clinical brain computer interfaces based on brain signal decoders to provide temporally precise support for action initiation in patients with brain disorders.

Movement-related cortical potential (MRCP) is observed in EEG recordings prior to a voluntary movement. It has been used for e.g., quantifying motor learning and for brain-computer interfacing (BCIs). The MRCP amplitude is affected by various factors, but the effect of caffeine is underexplored. The aim of this study was to investigate if a cup of coffee with 85 mg caffeine modulated the MRCP amplitude and the classification of MRCPs versus idle activity, which estimates BCI performance. Twenty-six healthy participants performed 2 × 100 ankle dorsiflexion separated by a 10-min break before a cup of coffee was consumed, followed by another 100 movements. EEG was recorded during the movements and divided into epochs, which were averaged to extract three average MRCPs that were compared. Also, idle activity epochs were extracted. Features were extracted from the epochs and classified using random forest analysis. The MRCP amplitude did not change after consuming caffeine. There was a slight increase of two percentage points in the classification accuracy after consuming caffeine. In conclusion, a cup of coffee with 85 mg caffeine does not affect the MRCP amplitude, and improves MRCP-based BCI performance slightly. The findings suggest that drinking coffee is only a minor confounder in MRCP-related studies.

Covert behavior is defined as behavior that is not directly visible and is thus comparable to a type of behavioral silence that requires modern psychophysiological techniques to reveal. Goal-directed behavior is teleologically purposive. Fundamentally, there are two approaches to accounting for purposeful behavior. One is the cybernetic approach, which views behavior as homeostatic and largely reflexive. The other one views behavior as a cognitive process that involves an interaction between neural events representing the previous experience, the present state of the individual, and the occurrence of particular features in the environment. This review, based on published data, presents a non-invasive psychophysiological method for investigating the electrical brain activity associated with those "silent" behaviors such as intention, evaluation of results, and memorization. Movement-related potentials (MRPs) are ideal for studying these processes. The MRPs are recorded during the execution of the skilled performance task (SPT). This task requires the execution of fast ballistic movements with the thumbs of both hands, learning a precise and short time interval between the two thumb presses, and scoring the highest number of target performances. The subject receives real-time feedback about the results of his performance. The MRPs associated with this task and present during covert behavior are the Bereitschaftspotential (BP) present before the onset of movement and the Skilled Performance Positivity (SPP) after movement, which coincides with the subject's awareness of the success or failure of his performance. These potentials show a maturational trend, reaching the adult form around the age of 10 when formal and abstract thinking progress. SPT and MRPs are particularly suitable to study neurodevelopmental disorders. Children with developmental dyslexia show abnormal MRPs, both in latency and amplitude, in different brain areas.

OBJECTIVE: To investigate the subjective phenomenon and the neural underpinnings of tics compared with voluntary movements in patients with tic disorders. METHODS: We recorded electroencephalographic and electromyographic data while subjects completed a Libet clock paradigm. Patients and healthy volunteers reported the times of W (willing to move) and M (movement occurrence) while performing voluntary movements. This was repeated only for the patients for the tics. RESULTS: In the patients, W and M times preceding voluntary movements and tics did not significantly differ from voluntary movements of healthy volunteers. The Bereitschaftspotentials in the patients were similar to healthy volunteers. Tics were only assessable for 7 patients due to artifacts. Two subjects did not show Bereitschaftspotentials, and they reported the lowest levels of tic voluntariness. 5 subjects did not show beta band event-related desynchronization before tics. CONCLUSIONS: For patients, the sense of volition for tics is similar to that of their voluntary movements which is similar to normal. Patients showed dissociations between the Bereitschaftspotential and beta desynchronization for tics, with 5/7 showing normal Bereitschaftspotentials and 2/7 showing desynchronization. The absence of desynchronization may suggest attempts to suppress tics. SIGNIFICANCE: This physiology shows a difference for most tics compared with normal movements.

In 1983 Benjamin Libet and colleagues published a paper apparently challenging the view that the conscious intention to move precedes the brain's preparation for movement. The experiment initiated debates about the nature of intention, the neurophysiology of movement, and philosophical and legal understanding of free will and moral responsibility. Here we review the concept of "conscious intention" and attempts to measure its timing. Scalp electroencephalographic activity prior to movement, the Bereitschaftspotential, clearly begins prior to the reported onset of conscious intent. However, the interpretation of this finding remains controversial. Numerous studies show that the Libet method for determining intent, W time, is not accurate and may be misleading. We conclude that intention has many different aspects, and although we now understand much more about how the brain makes movements, identifying the time of conscious intention is still elusive.

Background: Functional movement disorders (FMD) are a diagnostic and therapeutic challenge, both to the neurologist and psychiatrists. The phenomenology is varied and can present as tremors, dystonia, jerks/myoclonus, gait disorder, other abnormal movements or a combination. There has been an increase in the use of electrophysiological studies that are an important tool in the evaluation of FMDs. Methods: We searched the database platforms of MEDLINE, Google scholar, Web of Sciences, Scopus using the Medical Subject Heading terms (MeSH) for all the articles from 1st January 1970 till November 2022. A total of 658 articles were obtained by the search mechanism. A total of 79 relevant articles were reviewed thoroughly, of which 26 articles that had electrophysiological data were included in the present review. Results: Variability, distractibility and entertainability can be demonstrated in functional tremors by using multichannel surface electromyography. Voluntary ballistic movements tend to decrease the tremor, while loading the tremulous limb with weight causes the tremor amplitude to increase in functional tremor. Presence of Bereitschaftspotential demonstrates the functional nature of palatal tremor and myoclonus. Co-contraction testing may be helpful in differentiating functional from organic dystonia. The R2 blink reflex recovery cycle has been found to be abnormally enhanced in organic blepharospasm, whereas it is normal in presumed functional blepharospasm. Plasticity is found to be abnormally high in organic dystonia and normal in functional dystonia, in addition to enhanced facilitation in patients with organic dystonia. Conclusions: Electrophysiological tests supplement clinical examination and helps in differentiating FMD from organic movement disorders.

Mechanical ventilation is a highly utilized life-saving tool, particularly in the current era. The use of EEG in a brain-ventilator interface (BVI) to detect respiratory discomfort (due to sub-optimal ventilator settings) would improve treatment in mechanically ventilated patients. This concept has been realized via development of an EEG covariance-based classifier that detects respiratory-related cortical activity associated with respiratory discomfort. The aim of this study was to determine if head movement, detected by an accelerometer, can detect and/or improve the detection of respiratory-related cortical activity compared to EEG alone. In 25 healthy participants, EEG and acceleration of the head were recorded during loaded and quiet breathing in the seated and lying postures. Detection of respiratory-related cortical activity using an EEG covariance-based classifier was improved by inclusion of data from an Accelerometer-based classifier, i.e. classifier 'Fusion'. In addition, 'smoothed' data over 50s, rather than one 5 s window of EEG/Accelerometer signals, improved detection. Waveform averages of EEG and head acceleration showed the incidence of pre-inspiratory potentials did not differ between loaded and quiet breathing, but head movement was greater in loaded breathing. This study confirms that compared to event-related analysis with >5 min of signal acquisition, an EEG-based classifier is a clinically valuable tool with rapid processing, detection times, and accuracy. Data smoothing would introduce a small delay (<1 min) but improves detection results. As head acceleration improved detection compared to EEG alone, the number of EEG signals required to detect respiratory discomfort with future BVIs could be reduced if head acceleration is included.

The relationship between intelligence and brain electrical activity has long aroused the interest of many researchers. Quantified EEG and event-related potentials have contributed significantly to the development of this topic. Conversely, movement-related potentials have contributed little for both adults and children. This study analyses possible relationships between intelligence and movement-related potentials. One hundred and ten normally developing boys performed a skilled performance task (SPT) that required adaptive programming, learning proper timing and performance improvement by providing real time knowledge of results and feedback. The EEG was recorded with Ag/AgCl electrodes over Fpz, Cz, Pz, right and left precentral (RPC, LPC), P3 and P4. Intelligence was tested using the Wechsler Intelligence Scale for Children-Revised (WISC-III-R). Linear and multiple linear regressions were used to test the association between intelligence quotients (IQ) and readiness potential or Bereitschaftspotential (BP) and skilled performance positivity (SPP). BP onset was significantly and positively related to verbal IQ, and BP amplitude was significantly related to all three IQs (verbal, performance and full) in all cerebral areas. Children with higher IQ had a greater BP amplitude. SPP latency was significantly and negatively related to age but not to IQ, while SPP amplitude was significantly and negatively related to all three IQs and smaller in children with higher IQ. BP and SPP amplitudes seem to reflect the efficiency of cognitive processes associated with the task.

OBJECTIVES: Multiple sclerosis (MS) is a chronic inflammatory disease of the central nervous system, characterized by the accumulation of demyelinating lesions and axonal loss over its course. This study aimed to increase current knowledge of motor preparation in this condition, by assessing the two components of the Bereitschaftspotential (BP1 and BP2), also known as the readiness potential. METHODS: Twelve patients with MS and ten age- and gender-matched healthy controls (HC) were included. Patients' demographic and clinical data were collected. Participants were asked to perform two different tasks, a simple index extension and a Luria sequence. BP1 and BP2 values were obtained from 18 central electroencephalography electrodes and were compared between the two groups. RESULTS: Compared to HC, patients with MS showed earlier BP1 onset (i.e., longer latency) in almost all the analyzed scalp regions during index extension. This was also observed during the Luria sequence, but only in the centro-parietal regions. As for BP2 latency, no significant difference was noted between groups during either task. With regard to amplitudes, patients with MS had larger BP1 amplitudes in the right fronto-central area during index extension and greater BP1 and BP2 amplitudes in bilateral centro-parietal and left central regions during the Luria task. BP1 latency was also found to be significantly correlated with disease duration and performance on executive function tests (Trail Making Test). CONCLUSIONS: This study showed, for the first time, changes in the Bereitschaftspotential in patients with MS. These data reflect prolonged movement preparation in this population and may suggest global alteration of the premotor scheme.

The movement-related cortical potential (MRCP) is a brain signal that can be recorded using surface electroencephalography (EEG) and represents the cortical processes involved in movement preparation. The MRCP has been widely researched in simple, single-joint movements, however, these movements often lack ecological validity. Ecological validity refers to the generalizability of the findings to real-world situations, such as neurological rehabilitation. This scoping review aimed to synthesize the research evidence investigating the MRCP in ecologically valid movement tasks. A search of six electronic databases identified 102 studies that investigated the MRCP during multi-joint movements; 59 of these studies investigated ecologically valid movement tasks and were included in the review. The included studies investigated 15 different movement tasks that were applicable to everyday situations, but these were largely carried out in healthy populations. The synthesized findings suggest that the recording and analysis of MRCP signals is possible in ecologically valid movements, however the characteristics of the signal appear to vary across different movement tasks (i.e., those with greater complexity, increased cognitive load, or a secondary motor task) and different populations (i.e., expert performers, people with Parkinson's Disease, and older adults). The scarcity of research in clinical populations highlights the need for further research in people with neurological and age-related conditions to progress our understanding of the MRCPs characteristics and to determine its potential as a measure of neurological recovery and intervention efficacy. MRCP-based neuromodulatory interventions applied during ecologically valid movements were only represented in one study in this review as these have been largely delivered during simple joint movements. No studies were identified that used ecologically valid movements to control BCI-driven external devices; this may reflect the technical challenges associated with accurately classifying functional movements from MRCPs. Future research investigating MRCP-based interventions should use movement tasks that are functionally relevant to everyday situations. This will facilitate the application of this knowledge into the rehabilitation setting.

This is the second chapter of the series on the use of clinical neurophysiology for the study of movement disorders. It focusses on methods that can be used to probe neural circuits in brain and spinal cord. These include use of spinal and supraspinal reflexes to probe the integrity of transmission in specific pathways; transcranial methods of brain stimulation such as transcranial magnetic stimulation and transcranial direct current stimulation, which activate or modulate (respectively) the activity of populations of central neurones; EEG methods, both in conjunction with brain stimulation or with behavioural measures that record the activity of populations of central neurones; and pure behavioural measures that allow us to build conceptual models of motor control. The methods are discussed mainly in relation to work on healthy individuals. Later chapters will focus specifically on changes caused by pathology.

Is consciousness-the subjective awareness of the sensations, perceptions, beliefs, desires, and intentions of mental life-a genuine cause of human action or a mere impotent epiphenomenon accompanying the brain's physical activity but utterly incapable of making anything actually happen? This article will review the history and current status of experiments and commentary related to Libet's influential paper (Brain 106:623-664, 1983) whose conclusion "that cerebral initiation even of a spontaneous voluntary act …can and usually does begin unconsciously" has had a huge effect on debate about the efficacy of conscious intentions. Early (up to 2008) and more recent (2008 on) experiments replicating and criticizing Libet's conclusions and especially his methods will be discussed, focusing especially on recent observations that the readiness potential (RP) may only be an "artifact of averaging" and that, when intention is measured using "tone probes," the onset of intention is found much earlier and often before the onset of the RP. Based on these findings, Libet's methodology was flawed and his results are no longer valid reasons for rejecting Fodor's "good old commonsense belief/desire psychology" that "my wanting is causally responsible for my reaching.".

Objective: Multiple system atrophy (MSA) is a neurodegenerative disorder manifesting as parkinsonism, cerebellar ataxia, and autonomic dysfunction. It is categorized into MSA with predominant parkinsonism (MSA-P) and into MSA with predominant cerebellar ataxia (MSA-C). The pathophysiology of motor control circuitry involvement in MSA subtype is unclear. Bereitschaftspotential (BP) is a feasible clinical tool to measure electroencephalographic activity prior to volitional motions. We recorded BP in patients with MSA-P and MSA-C to investigate their motor cortical preparation and activation for volitional movement. Methods: We included eight patients with MSA-P, eight patients with MSA-C, and eight age-matched healthy controls. BP was recorded during self-paced rapid wrist extension movements. The electroencephalographic epochs were time-locked to the electromyography onset of the voluntary wrist movements. The three groups were compared with respect to the mean amplitudes of early (1,500-500 ms before movement onset) and late (500-0 ms before movement onset) BP. Results: Mean early BP amplitude was non-significantly different between the three groups. Mean late BP amplitude in the two patient groups was significantly reduced in the parietal area contralateral to the movement side compared with that in the healthy control group. In addition, the late BP of the MSA-C group but not the MSA-P group was significantly reduced at the central parietal area compared with that of the healthy control group. Conclusions: Our findings suggest that patients with MSA exhibit motor cortical dysfunction in voluntary movement preparation and activation. The dysfunction can be practicably evaluated using late BP, which represents the cerebello-dentato-thalamo-cortical pathway.

INTRODUCTION: As we vocalize, our brains generate predictions of the sounds we produce to enable suppression of neural responses when intentions match vocalizations and to make adjustments when they do not. This may be instantiated by efference copy and corollary discharge mechanisms, which are impaired in people with schizophrenia (SZ). Although innate, these mechanisms can be affected by intentions. We asked if attending to pitch during vocalizations would take these mechanisms "off-line" and reduce suppression. METHODS: Event-related potentials (ERP) were recorded from 96 SZ and 92 healthy controls (HC) as they vocalized triplets in monotone (Phrase) or sang triplets in ascending thirds (Pitch). Pre-vocalization activity (Bereitschaftspotential, BP), N1, and P2 ERP components to sounds were compared during vocalization and playback. RESULTS: N1 was not as suppressed during Pitch as during Phrase. N1 suppression was not affected by SZ in either task when all data were collapsed across pitches (Pitch) and positions (Phrase). However, when binned according to vocalization performance, SZ showed less N1 suppression than HC at longer (>2 s) inter-stimulus intervals (Phrase) and inconsistent suppression across pitches (Pitch). Unlike N1, P2 was more suppressed during Pitch than Phrase and not affected by SZ. BP was greater during vocalization than playback but did not contribute to N1 or P2 effects. Pitch variability was inversely related to negative symptoms. CONCLUSIONS: Neural processing is not suppressed when patients and controls sing, and corollary discharge abnormalities in schizophrenia are only seen at long vocalization intervals.

The anticipation of upcoming events is a key-feature of cognition. Previous investigations on anticipatory visuospatial attention mainly adopted transient and-more rarely-sustained tasks, whose main difference consists in the presence of transient or sustained cue stimuli and different involvement of top-down or bottom-up forms of attention. In particular, while top-down control has been suggested to drive sustained attention, it is not clear whether both endogenous and exogenous controls are recruited in transient attention task, or whether the cue-evoked attention may be interpreted as a mainly bottom-up guided process. To solve this issue, the present study focused on the preparatory brain activity of participants performing a sustained and a transient attention task. To this aim, the focus was on pre-stimulus event-related potential (ERP) components, i.e., the prefrontal negativity (pN) and the visual negativity (vN), associated with cognitive and sensorial preparation, emerging from prefrontal and visual areas, respectively. Results indicated that the pN was specific for the sustained task, while the vN emerged for both tasks, although smaller in the transient task, with a hemispheric lateralization contralateral to the attended hemifield. The present findings support the interpretation of the vN as a modality-specific index of attentional preparation, and suggest the presence of cognitive endogenous control in sustained tasks only, as revealed by the presence of a prefrontal activity that was interpreted as the locus of the top-down attentional modulation during the stimulus expectancy stage.

The ST involved a self-paced tapping task in which the participants extended their right index finger. In the dual task, the participants performed the ST and a visual number counting task simultaneously. [Results] The amplitude and integral value of MRCP from electroencephalography electrode C3 was significantly higher in the S-DT than in the ST, whereas they were similar between the C-DT and the ST. Medium-load divided attention (i.e., S-DT) led to significantly more changes in the MRCP magnitude than did low-load divided attention (i.e., ST). However, the MRCP of high-load divided attention (i.e., C-DT) was similar to that of low-load divided attention. [Conclusion] These results suggest that MRCP reflects the function of or network between the supplementary motor area and the dorsolateral prefrontal cortex, and may serve as a marker for screening the capacity of individuals to perform dual tasks.

Schizophrenic patients often do not have the sense that they direct their own movements or author their own thoughts (passivity phenomena). As willing must precede movement to be causal and thus generate the sense of agency, it is possible that the timing between the senses of willing and movement is shortened in schizophrenia. We tested the subjective perception of this time interval in patients with schizophrenia using a method based on Libet's paradigm, in which subjects specify a time W - the time of willing a movement - and a time M - the time that movement occurred. Patients with schizophrenia and healthy volunteers made voluntary movements at times of their own choice while looking at a fast-rotating clock on a computer screen and reported when their movements were willed and made. We recorded surface electromyography to determine the time of actual movement, and electroencephalography to record brain potentials associated with movement. Results showed a significantly reduced interval between the reported M and W in patients with respect to the healthy volunteers (p < 0.05). Specifically, patients did not report a significant difference in the timing of W at 19 ms prior to movement onset and M at 7.4 ms prior to movement onset (p > 0.05), while the control group experienced a time W at 100 ms prior to movement onset and this differed significantly from their time M at 19 ms prior to movement onset (p < 0.01). These results suggest that patients with schizophrenia do have an altered timing of awareness of action - or an impaired judgment of the sequence of events - and that this might be etiologic in the development of the abnormal sense of agency.

OBJECTIVE: The current study sought to determine whether there is a Bereitschaftspotential (BP) before uninstructed, spontaneous movements. METHODS: 14 participants were seated on a comfortable armchair for one hour without any instruction except not to fall asleep and to keep their eyes open. Electroencephalography (EEG) and electromyography (EMG) activity were recorded during the whole session. EEG activity was analyzed before spontaneous movements and compared with EEG activity before repetitive, instructed movements in a separate session. RESULTS: BPs were identified in most participants with the spontaneous movements. The BPs with spontaneous movements were mostly localized in the medial frontocentral regions. The BPs with the instructed movements were localized primarily in the central regions and had larger amplitude. CONCLUSION: Presence of a BP before movement does not depend on instruction and may be independent of conscious volition. The amplitude of the BP may depend on the amount of attention. SIGNIFICANCE: This study shows that the presence of a BP before movement is not an "artifact" of the experimental instructions.

Previous event-related potential (ERP) studies mainly from the present research group showed a novel component, that is, the prefrontal negativity (pN), recorded in visual-motor discriminative tasks during the pre-stimulus phase. This component is concomitant to activity related to motor preparation, that is, the Bereitschaftspotential (BP). The pN component has been reported in experiments based on the visual modality only; for other modalities (acoustic and/or somatosensory) the presence of the pN warrants further investigation. This study represents a first step toward this direction; indeed, we aimed at describing the pN and the BP components in discriminative response tasks (DRTs) for three sensory modalities. In experiment 1 ERPs were recorded in 29 adults in visual and auditory DRT; an additional group of 15 adults participated to a somatosensory DRT (experiment 2). In line with previous results both the pN and the BP were clearly detectable in the visual modality. In the auditory modality the prefrontal pN was not detectable directly; however, the pN could be derived by subtraction of separate EEG traces recorded in a "passive" version of the same auditory task, in which motor responses were not required. In the somatosensory modality both the pN and the BP were detectable, although with lower amplitudes with respect to other two sensory modalities. Overall, regardless of the sensory modality, anticipatory task-related pN and BP components could be detected (or derived by subtraction) over both the prefrontal and motor cortices. These results support the view that anticipatory processes share common components among sensory modalities.

RESUMO

Background: There is little published work describing the electrophysiological characteristics of essential palatal tremor, a condition now believed by many to be a functional (psychogenic) movement disorder. Case Report: Here we combine electroencephalography and electromyography with time-locked video recordings to document two cases of essential palatal tremor in which a definitive diagnosis is achieved using these electrophysiological tools. Discussion: We believe that sharing how these objective tools can be used to diagnose a functional movement disorder, as well as providing more published evidence to support the functional origin of essential palatal myoclonus, will help to diagnose this condition in the future.
