INTRODUCTION AND OBJECTIVES: Patients with a single syncopal episode (SSE) and complete bundle branch block (cBBB) are frequently managed more conservatively than patients with recurrent episodes (RSE). The objective of this study was to analyze if there are differences between patients with single or recurrent unexplained syncope and cBBB in arrhythmic risk, the diagnostic yield of tests, and clinical outcomes. METHODS: Cohort study of consecutive patients with unexplained syncope and cBBB with a median follow-up time of 3 years. The patients were evaluated via a stepwise workup protocol based on electrophysiological study (EPS) and long-term follow-up with an implantable cardiac monitor. RESULTS: Of the 503 patients included in the study, 238 (47.3%) had had only 1 syncopal episode. The risk of an arrhythmic syncope was similar in both groups (58.8% in SSE vs 57.0% in RSE; P=.68), also after adjustment for possible confounding variables (HR, 1.06; 95%CI, 0.81-1.38; P=.674). No significant differences between the groups were found in the EPS results and implantable cardiac monitor diagnostic yield. A total of 141 (59.2%) patients with SSE and 154 (58.1%) patients with RSE required cardiac device implantation (P=.797). After appropriate treatment, 35 (7%) patients had recurrence of syncope. The recurrence rate and mortality were also similar in both groups. CONCLUSIONS: Patients with cBBB and unexplained syncope are at high risk of an arrhythmic etiology, even after the first syncopal episode. Patients with SSE and RSE have a similar arrhythmic risk and similar outcomes, and therefore there is no clinical justification for not managing them in the same manner.

INTRODUCTION AND OBJECTIVES: Left bundle branch block (LBBB)-induced cardiomyopathy occurs in patients with long-standing LBBB. These patients characteristically exhibit hyperresponsiveness to cardiac resynchronization therapies (CRT). However, there is scarce information on their response to medical treatment. The aim of this study was to assess the change in left ventricular ejection fraction (LVEF) after a 3-month period following titration of guideline-directed medical therapy for heart failure. METHODS: This retrospective analysis included all patients assessed in the heart failure unit of a Spanish University Hospital between 2020 and 2021, who presented with de novo ventricular dysfunction (LVEF <40%) and had a history of long-standing LBBB with no other possible causes of cardiomyopathy. RESULTS: A total of 1497 patients were analyzed, of which 21 were finally eligible. Mean time from first diagnosis of LBBB to first consultation was 4.05± 4.1 years. Mean LVEF from first consultation to end of titration improved from 29.5±5.7% to 32.7±8.6% (P = .172), but none had recovered ventricular function at the end of follow-up. New York Heart Association functional class improved from 1.91±0.46 to 1.81±0.53 (P=.542). After CRT device implantation in 8 patients, LVEF improved by 18.1±6.4% (P=.003). CONCLUSIONS: Guideline-directed medical therapy seems to be ineffective in improving LVEF and functional class in patients with de novo heart failure and LBBB-induced cardiomyopathy. Based on a positive response to CRT on LVEF improvement, early CRT implantation could be a reasonable strategy for these patients.

RESUMEN Introducción: Existe poca literatura sobre la asociación entre el bloqueo de rama derecha y factores de riesgo como la hipertensión y la diabetes en pacientes de Latinoamérica y Perú. Objetivo: Determinar los factores asociados al bloqueo de rama derecha en adultos mayores. Métodos: Estudio transversal que incluyó 376 pacientes adultos mayores atendidos en el Hospital Almanzor Aguinaga Asenjo, Lambayeque-Perú. La variable dependiente fue la presencia de bloqueo de rama derecha y las variables independientes fueron edad, sexo, antecedente de hipertensión arterial y diabetes. Se estimaron razones de prevalencia e intervalos de confianza al 95 %. Resultados: De 376 pacientes, la mayoría eran varones (55,9 %), la edad media fue de 75,6 años, el 11,4 % tuvo antecedente de hipertensión y el 9 % era diabético. El 41 % tuvo diagnóstico de bloqueo de rama derecha. En la regresión múltiple; el sexo femenino (RP=1,53, IC 95 %: 1,21 - 1,95), antecedente de hipertensión (RP=1,55; IC 95 %: 1,21 - 2,00) y diabetes mellitus (RP=1,49, IC 95 %: 1,12 - 2,00) se asociaron de forma positiva a presentar bloqueo de rama derecha. El modelo anidado seleccionó las variables: sexo femenino (RP=1,54, IC 95 %: 1,21 - 1,96) y antecedente de hipertensión arterial (RP=1,61, IC 95 %: 1,25 - 2,08). Conclusión: El sexo femenino, antecedente de hipertensión arterial y de diabetes mellitus se asocian positivamente a presentar bloqueo de rama derecha.


ABSTRACT Introduction: There is little literature on the association between right bundle branch block and risk factors such as hypertension and diabetes in patients in Latin America and Peru. Objective: To determine the factors associated with right bundle branch block in older adults. Methods: Analytical cross-sectional study completed by 376 elderly patients treated at Almanzor Aguinaga Asenjo Hospital, Lambayeque-Peru. The dependent variable was the presence of right bundle branch block, and the independent variables were age, sex, history of hypertension, and diabetes. Prevalence ratios and 95 % confidence intervals were estimated. Results: Of 376 patients, the majority were male (55,9 %), the mean age was 75,6 years, 11,4 % had a history of hypertension and 9 % were diabetic; 41 % had a diagnosis of right bundle branch block. In simple regression, the frequency of right bundle branch block was 56 % (PR= 1,56; 95 % CI: 1,22-1,99), 65 % (PR=1,65; 95 % CI: 1,26 - 2,15) and 59 % (PR= 1,59; CI 95 %: 1,18 - 2,14) higher in women, hypertensive and diabetics; respectively. In the multiple regression; female sex (PR= 1,53; 95 % CI: 1,21 - 1,95), history of hypertension (PR= 1,55; 95 % CI: 1,21 - 2,00) and diabetes mellitus (PR= 1,49; 95 % CI: 1,12 - 2,00) were positively associated with right bundle branch block. The nested model selected the variables: female sex (PR= 1,54; 95 % CI: 1,21 - 1,96) and history of arterial hypertension (PR= 1,61; 95 % CI: 1,25 - 2,08). Conclusion: Female sex, history of arterial hypertension and diabetes mellitus were positively associated with presenting right bundle branch block in older adults.

OBJECTIVES: This work aims to determine the prevalence, characteristics, and impact on prognosis of right bundle branch block (RBBB) in a cohort of acute heart failure (AHF) patients. METHODS: We prospectively analyzed 3,638 AHF patients included in the RICA registry (National Heart Failure Registry of the Spanish Internal Medicine Society). We independently analyzed the relationship between baseline and clinical characteristics and the presence of RBBB as well as the potential impact of RBBB on 1-year all-cause mortality and a composite endpoint of 90-day post-discharge hospitalization or death. RESULTS: The prevalence of RBBB was 10.9%. Patients with RBBB were older, a higher proportion were male, had more pulmonary comorbidities, had higher left ventricular ejection fraction values, and had worse functional status. There were no differences in risk for patients with RBBB, with an adjusted hazard ratio (95% confidence interval) for 1-year mortality of 1.05 (0.83-1.32), and for the composite endpoint of 90-day post-discharge hospitalization or death of 0.97 (0.74-1.25). These results were consistent on the sensitivity analyses. CONCLUSIONS: Few patients with AHF present with RBBB, which is consistently associated with advanced age, male sex, pulmonary comorbidities, preserved left ventricular ejection fraction, and worse functional status. Nonetheless, after considering these factors, RBBB in AHF patients is not associated with worse outcomes.

RESUMEN Los trastornos de la repolarización ventricular son manifestaciones comunes de una amplia variedad de situaciones, entre las que se incluye la memoria cardíaca; un fenómeno no reconocido frecuentemente en la práctica diaria. La gravedad de cada una de estas causas es muy variable; sin embargo, el diagnóstico definitivo de cada una de ellas no siempre es evidente. Se presenta el caso de un paciente que acude al servicio de urgencias con dolor torácico y ondas T negativas profundas en el electrocardiograma, que simulan una isquemia miocárdica grave, y que fue definido como memoria eléctrica cardíaca.


ABSTRACT The abnormalities in ventricular repolarization are common manifestations of several conditions, among these, we can include cardiac memory, a frequently unrecognized phenomenon in medical practice. The severity of each of these causes is variable; nonetheless, a definitive diagnosis of each of them is not always evident. We present the case of a patient admitted at the emergency room with chest pain and deeply inverted T waves in the electrocardiogram, mimicking a severe myocardial ischemia, which was defined as cardiac electrical memory.

La terapia de resincronización cardiaca mediante estimulación hisiana ha demostrado ser efectiva en pacientes con bloqueo de rama izquierda del haz de His e insuficiencia cardiaca. Paciente masculino, con 47 años de edad, con insuficiencia cardiaca, fracción de expulsión del 17% y miocardio dilatada idiopática, electrocardiograma en ritmo sinusal, bloqueo auriculoventricular de 1.er grado, intervalo PR 400 ms, bloqueo completo de rama derecha del haz de His, bloqueo del fascículo anterior de la rama izquierda del haz de His, duración del QRS 200 ms. Se decidió realizar estimulación selectiva del haz de His. La resincronización cardiaca biventricular convencional en pacientes con presencia de bloqueo completo de la rama derecha del haz de His no está indicada debido a la pobre respuesta al tratamiento. La estimulación hisiana permite reclutar la rama bloqueada y reestablecer la conducción a través de ella, de tal forma que, en ausencia de necrosis, se logre sincronía biventricular. En el caso presentado el reclutamiento de la rama derecha mediante estimulación hisiana se reflejó en el restablecimiento de la sincronía biventricular, medida por rastreo de marcas (speckle tracking) e incremento significativo de la fracción de expulsión del ventrículo izquierdo del 17 al 36.6%, con un incremento absoluto del 19.6%.Cardiac resynchronization therapy has proven to be an effective therapy in patients with left bundle branch block and heart failure. His bundle pacing allows recruiting the blocked branch and restoring conduction throughout it, therefore, in the absence of necrosis the biventricular synchrony is achieved. We presented a case of His bundle pacing with recruitment of the right bundle branch, which reestablish biventricular synchrony measured by speckle tracking, and with a significant increase of the left ventricle ejection fraction from 17 to 36.6%, with an absolute increase of 19.6%.

RESUMEN El bloqueo de rama enmascarado es una forma poco frecuente de bloqueo bifascicular de la cual se desconoce su mecanismo con exactitud. Es más frecuente encontrarlo en pacientes de edad avanzada o aquellos con una cardiopatía estructural como: enfermedad coronaria, hipertrofia ventricular, miocardiopatías, miocarditis chagásica y degeneración idiopática del sistema excitoconductor del corazón. Su diagnóstico electrocardiográfico se obtiene por la presencia de ondas R altas y anchas en V1 (patrón de bloqueo de rama derecha), desviación del eje eléctrico a la izquierda (entre -80 y -120 grados) y una onda S inferior a 1 mm o ausente en DI y aVL. Su presencia denota un factor de mal pronóstico en los pacientes.


ABSTRACT Masquerading bundle branch block is a rare form of bifascicular block, whose exact mechanism is unknown. It is more frequently found in elderly patients or those with structural heart disease such as: coronary artery disease, ventricular hypertrophy, cardiomyopathies, Chagas myocarditis and idiopathic degeneration of the cardiac conduction system. Its electrocardiographic diagnosis is obtained by the presence of high and wide R waves in V1 (right bundle branch block pattern), left axis deviation (between -80 and -120 degrees) and an S wave of less than 1 mm or absent in I and aVL leads. Its presence denotes a poor prognostic factor in patients.

OBJECTIVE: Left ventricular hypertrophy is the most common marker of target organ damage in arterial hypertension. Electrocardiograms are typically performed to identify left ventricular hypertrophy. The aim of this study was to analyse the prognostic utility of other electrocardiographic abnormalities in patients with arterial hypertension, beyond ventricular hypertrophy. MATERIALS AND METHODS: The study included 1003 patients older than 65years with arterial hypertension. We recorded risk factors, previous cardiovascular history and medical treatment and analysed various electrocardiographic abnormalities including the Sokolow-Lyon index, the Cornell index, ventricular overload and branch blocks. The study conducted a 2-year follow-up, recording the major cardiovascular events (mortality, myocardial infarction, stroke and hospitalisation for heart failure). RESULTS: The study population's mean age was 72.9±5.8years, 47.5% of whom were men. During the follow-up, 13.9% of the patients experienced a major cardiovascular event. These patients were older, more often smokers and engaged in less physical exercise, without presenting differences in the antihypertensive therapy or blood pressure control. The ventricular overload pattern (HR: 1.93; 95%CI: 1.160-3.196; P=.011) and the complete left bundle branch block (HR: 2.27; 95%CI: 1.040-4.956; P=.040) behaved as independent electrocardiographic predictors of major cardiovascular events; however, left ventricular hypertrophy using the Sokolow and/or Cornell index did not behave as such. CONCLUSIONS: For patients with hypertension, the presence in the baseline electrocardiogram of complete left bundle branch block or a pattern of ventricular overload identifies a population at increased cardiovascular risk.

RESUMO

A new or presumably new left bundle branch block along with ischemic symptoms has traditionally been considered an electrocardiographic equivalent of ST-segment elevation myocardial infarction, which should be brought to emergent reperfusion. However, several criteria have been proposed for its definition, but none has reached out an optimal diagnostic yield. Below we detail these criteria, their main problems and the advantages they have shown.


La presencia de un bloqueo de rama izquierda del haz de His nuevo o presumiblemente nuevo junto con síntomas isquémicos se ha considerado tradicionalmente un equivalente electrocardiográfico de infarto agudo de miocardio con elevación del segmento ST, el cual debe ser llevado a reperfusión emergente. Para su definición se han propuesto varios criterios, pero ninguno ha alcanzado un rendimiento diagnóstico óptimo. A continuación detallaremos dichos criterios, sus principales problemas y las ventajas que han demostrado.

El bloqueo de rama funcional durante una taquicardia supraventricular puede ser observado con longitudes de ciclo cortas y representa una respuesta fisiológica del sistema de conducción intraventricular por la existencia de conducción nodal auriculo ventricular acelerada. Presentamos el caso de un paciente joven con taquicardia reentrante aurículo-ventricular ortodrómica y bloqueo de rama alternante. Esta respuesta infrecuente se explica por el hallazgo obtenido durante el estudio electrofisiológico. Una conducción nodal aurículo-ventricular acelerada produce un frente de onda que despolariza las ramas durante sus períodos refractarios. Una vez que ocurrió el bloqueo en una de las ramas, dicho bloqueo persistió debido al fenómeno de linking, que es por conducción oculta retrógrada repetitiva de la rama contralateral. Después de la ablación transcatéter de una vía accesoria oculta lateral izquierda, el marcapaseo auricular rápido a la misma longitud de ciclo de la taquicardia, reprodujo la misma aberrancia observada durante la taquicardia. Este procedimiento demostró que el bloqueo de rama funcional fue debido a la longitud de ciclo corto y no a la presencia de una vía accesoria aurículo-ventricular.


Functional bundle branch block during a supraventricular tachycardia can be observed with shorter cycle lengths and represent a physiologic response by the specialized intraventricular conduction system to accelerated AV nodal conduction. The present case corresponds to a young patient with exercise induced orthodromic A-V reentrant tachycardia and alternating bundle branch block. This unusual response is explained by the finding obtained during the electrophysiology study. An accelerated AV nodal conduction made the depolarizing wave front reach the bundle branches during their refractory period. Once block in one bundle was stablished, block persisted due to the linking phenomenon that is repetitive retrograde concealed conduction from the contralateral bundle. After catheter ablation of a concealed left-sided accessory A-V pathway, rapid atrial pacing at the same cycle length of the tachycardia reproduced the same aberrancies observed during tachycardia. This response proved that functional bundle branch block is due to the short cycle length and not the presence of an accessory A-V pathway.

Para su definición se han propuesto varios criterios, pero ninguno ha alcanzado un rendimiento diagnóstico óptimo. A continuación detallaremos dichos criterios, sus principales problemas y las ventajas que han demostrado.


Abstract A new or presumably new left bundle branch block along with ischemic symptoms has traditionally been considered an electrocardiographic equivalent of ST-segment elevation myocardial infarction, which should be brought to emergent reperfusion. However, several criteria have been proposed for its definition, but none has reached out an optimal diagnostic yield. Below we detail these criteria, their main problems and the advantages they have shown.

The clinical significance of a right bundle branch block (RBBB) in an asymptomatic adult without evidence of cardiovascular disease is controversial. To establish the relationship between the appearance of the RBBB and the increase of cardiovascular morbidity and mortality in healthy patients, we have carried out a literature review of documents available until September 2017 through a systematic search on the Pubmed database, Cochrane library and a manual search of the mentioned literature and related articles. From the 29 articles included in the study sample, eight showed mortality and 16 morbidity outcomes. An increase of risk of death is observed is eight articles and an increase of cardiovascular events is observed in 11 articles. The most recent publications suggest that the appearance of an RBBB in healthy individuals should not be underestimated, thus further studies are needed to analyse the type of follow-up that should be carried out in these patients.

INTRODUCTION: Exercise-induced left bundle branch block (EI-LBBB) is a rare circumstance of unknown significance. The purpose of this paper is to describe the scintigraphic features and the prognostic value of this finding. MATERIAL AND METHODS: We reviewed the features of 1,885 patients who had visited our department to undergo GATED-SPECT ergometry to diagnose ischaemic heart disease. Seven patients showed EI-LBBB throughout the exercise testing. Coronary angiography was performed in 4 of them. Patients were followed-up over an average period of time of 30±8 months. The onset of major cardiovascular events was recorded during the follow-up period. RESULTS: The prevalence of EI-LBBB was 0.37%. Six out of 7 patients were women. Myocardial function and perfusion were normal in 3 patients. Three patients had fixed perfusion defects and one patient had a reversible defect. Two out of the 4 patients showing perfusion defects presented a moderate-severe decrease of the left ventricular ejection fraction. None of the 4 patients with perfusion defects were found to have coronary disease on coronary angiography. CONCLUSIONS: The prevalence of EI-LBBB among the patients that came to undergo GATED-SPECT ergometry was very low. The finding was more frequent in women. In our series, 2 patients presented non-ischaemic structural heart disease, but no patient was diagnosed with coronary artery disease. In our patients the presence of EI-LBBB did not relate to a greater risk of experiencing a major cardiovascular event.

Introducción: El bloqueo de rama izquierda del haz de His dependiente de la frecuencia cardíaca no es una entidad común en la práctica clínica del anestesiólogo, sino que se presenta como consecuencia de enfermedades cardiacas o no que ocurren por lentitud de la frecuencia ventricular sobre todo, con enfermedad de las arterias coronarias planteadas solo como diagnóstico después de haberlas descartado. Objetivo: Describir el manejo clínico-anestésico de una paciente que presentó bloqueo de rama izquierda del haz de His para cirugía torácica electiva por tumor de mediastino. Desarrollo: Se presenta un caso clínico de una paciente con bloqueo de rama izquierda dependiente de la frecuencia cardiaca que se anunció para cirugía del tórax, sin historia previa de coronariopatía. La cirugía trascurrió sin complicaciones y con buenos resultados. Conclusiones: el bloque de rama izquierda dependiente de la frecuencia cardiaca es una entidad rara y un diagnóstico excluyente de otras entidades cardiovasculares o no como la isquemia miocárdica. Un adecuado manejo intraoperatorio del paciente proporciona excelentes resultados sin traducción clínica ni complicaciones(AU)


Introduction: Rate-dependent left bundle branch block is not a rare entity for the clinical practice in anesthesiology, but appears as a consequence of heart diseases or not always due to slowness in atrial frequency above all, with coronary artery diseases diagnosed only after their being ruled out. Objective: To describe the clinical-anesthetic management of a patient who presented left bundle branch block for elective thoracic surgery due to mediastinal tumor. Findings: We present a clinical case of a female patient with rate-dependent left bundle branch block who was announced for thoracic surgery, without a previous history of coronary artery disease. The surgery proceeded without complications and with positive outcome. Conclusions: Rate-dependent left-branch block is a rare entity and a diagnosis ruling out other cardiovascular entities or not, such as myocardial ischemia. An adequate intraoperative management of the patient provides excellent outcome without clinical translation or complications(AU)

INTRODUCTION AND OBJECTIVES: Little is known about the timing of onset and outcome of conduction abnormalities (CA) following balloon-expandable transcatheter aortic valve implantation. The aim of this study was to examine the timing of CA and determine the impact of balloon aortic valvuloplasty (BAV) on the persistence of these abnormalities. METHODS: A total of 347 patients were included. Of these, 75 had a continuous electrocardiogram recording and a 6-lead electrocardiogram at each step of the procedure. RESULTS: In the transcatheter aortic valve implantation population undergoing continuous electrocardiogram monitoring, new-onset left bundle branch block (LBBB) or third-degree atrioventricular block occurred in 48 (64%) and 16 (21.3%) patients, with 51.5% of CA occurring before valve implantation. Left bundle branch block persisted more frequently at hospital discharge (53.8 vs 22.7%; P=.028) and at 1-month follow-up (38.5 vs 13.6%; P=.054) when occurring before valve implantation. Balloon aortic valvuloplasty prior to valve implantation was used in 264 (76.1%) patients, and 78 (22.5%) had persistent LBBB or complete atrioventricular block requiring pacemaker implantation. Persistent LBBB or unresolved atrioventricular block at 1 month occurred more frequently in the BAV group (76.1 vs 47.6%; P=.021), and the use of BAV was associated with a lack of CA resolution (OR, 3.5; 95%CI, 1.17-10.43; P=.021). CONCLUSIONS: In patients undergoing a balloon-expandable transcatheter aortic valve implantation, more than half of CA occurred before valve implantation. Early occurrence of CA was associated with a higher rate of persistence at 1-month follow-up. The use of BAV was associated with an increased risk of CA persistence.

Resumen La afectación cardíaca en pacientes con sarcoidosis está siendo cada vez más reconocida y se asocia con mal pronóstico. Aunque en su patogénesis están implicados los factores ambientales y genéticos, la etiología de la sarcoidosis cardíaca no es clara. Las manifestaciones clínicas incluyen alteraciones de la conducción aurículo-ventricular, arritmias e insuficiencia cardíaca congestiva. Es una entidad extremadamente difícil de diagnosticar debido a que las manifestaciones clínicas son inespecíficas, y la sensibilidad y la especificidad de las modalidades de diagnóstico son limitadas. El tratamiento cardíaco óptimo no ha sido bien definido, y aunque los corticoides siguen siendo el pilar del manejo, hay poca evidencia de la dosis o la duración de la terapia. Se expone el caso de una paciente con sarcoidosis cardíaca aislada, que debutó con extrasístoles ventriculares y progresó a falla cardiaca y arritmias ventriculares sostenidas.


Abstract Cardiac involvement is increasingly being observed in patients with sarcoidosis and is associated with a poor prognosis. Although environmental and genetic factors play a part in its pathogenesis, the aetiology of cardiac sarcoidosis is still not clear. The clinical signs include anomalies in atrial-ventricular conduction, arrhythmias, and congestive cardiac failure. It is an extremely difficult condition to diagnose as the clinical signs are vague, and the sensitivity and specificity of the diagnostic models are limited. There is no well-defined optimum cardiac treatment, and although corticosteroids continue to be the mainstay of its management, there is little evidence on the dose or duration of the treatment. It is presented a case of a patient with isolated cardiac sarcoidosis that debuted with ventricular extrasystoles and progressed to cardiac failure and sustained ventricular arrhythmias.

La terapia de resincronización cardíaca (TRC) mejora los síntomas, reduce la morbilidad y mortalidad de los pacientes con insuficiencia cardíaca avanzada asociada a disfunción sistólica del ventrículo izquierdo, bloqueo de rama izquierda y disincronía mecánica, refractaria al tratamiento farmacológico óptimo. Los efectos beneficiosos de la TRC han sido demostrados de manera consistente en grandes ensayos clínicos en pacientes que cumplen los criterios de selección recomendados por las guías actuales. Una población, que no ha sido ampliamente representada en los ensayos, son los pacientes con enfermedad de Chagas, donde el trastorno de la conducción más frecuente es el bloqueo de rama derecha (BRD), aunque debemos considerar otros trastornos intraventriculares asociados al BRD, en cuyo caso se describen beneficios con la TRC.


Cardiac resynchronization therapy (CRT) improves symptoms, reduces morbidity and mortality in patients with advanced heart failure associated with left ventricular systolic dysfunction, left bundle branch block and mechanical dyssynchrony, refractory to optimal pharmacological treatment. The beneficial effects of CRT have been consistently demonstrated in large clinical trials in patients meeting the selection criteria recommended by current guidelines. A population that has not been widely represented in the trials are patients with Chagas' disease, where the most common conduction disorder is right bundle branch block (BRD), although other intraventricular disorders associated with BRD are consistent with benefits of CRT.


A terapia de ressincronização (TRC) melhora os sintomas, reduz a morbidade e mortalidade em pacientes com insuficiência cardíaca avançada associada com disfunção ventricular esquerda sistólica, bloqueio de ramo esquerdo e dissincronismo mecânica, refratário ao tratamento farmacológico ideal. Os efeitos benéficos do TRC foram demonstrados de forma consistente em grandes ensaios clínicos em doentes que satisfazem os critérios de seleção recomendado pelas diretrizes atuais. Uma população que não tem sido amplamente representada nos ensaios são os pacientes com doença de Chagas, onde a distúrbio de condução mais comum é bloqueio de ramo direito (BRD), embora devessem considerar outros distúrbios intraventriculares associados ao BRD como potenciais candidatos para os benefícios descritos com TRC.

Coronary sinus mapping is commonly used to evaluate left atrial activation. Herein, we propose to use it to assess which right ventricular pacing modality produces the shortest left ventricular activation times (R-LVtime) and the narrowest QRS widths. Three study groups were defined: 54 controls without intraventricular conduction disturbances; 15 patients with left bundle branch block, and other 15 with right bundle branch block. Left ventricular activation times and QRS widths were evaluated among groups under sinus rhythm, right ventricular apex, right ventricular outflow tract and high output septal zone (SEPHO). Left ventricular activation time was measured as the time elapsed from the surface QRS onset to the most distal left ventricular deflection recorded at coronary sinus. During the above stimulation modalities, coronary sinus mapping reproduced electrical differences that followed mechanical differences measured by tissue doppler imaging. Surprisingly, 33% of the patients with left bundle branch block displayed an early left ventricular activation time, suggesting that these patients would not benefit from resynchronization therapy. SEPHO improved QRS widths and left ventricular activation times in all groups, especially in patients with left bundle branch block, in whom these variables became similar to controls. Left ventricular activation time could be useful to search the optimum pacing site and would also enable detection of non-responders to cardiac resynchronization therapy. Finally, SEPHO resulted the best pacing modality, because it narrowed QRS-complexes and shortened left ventricular activations of patients with left bundle branch block and preserved the physiological depolarization of controls.


El mapeo del seno coronario se utiliza comúnmente para evaluar la activación de la aurícula izquierda. Aquí, investigamos su utilidad para evaluar qué modalidad de estimulación ventricular derecha produce los menores tiempos de activación ventricular izquierda (R-LVtime). Se definieron tres grupos: 54 controles; 15 pacientes con bloqueo de rama izquierda y 15 con bloqueo de rama derecha. El ancho de QRS y los tiempos de activación fueron evaluados en cada grupo bajo las siguientes modalidades: ritmo sinusal, ápex del ventrículo derecho, tracto de salida del ventrículo derecho y alta salida en septum (SEPHO). El R-LVtime se midió como el tiempo transcurrido desde el inicio del QRS de superficie y la deflexión ventricular izquierda más distal del seno coronario. Durante las distintas modalidades de estimulación, el mapeo del seno coronario reprodujo diferencias eléctricas acompañadas por diferencias mecánicas que fueron evaluadas mediante Tissue Doppler Imaging. El 33% de los pacientes con bloqueo de rama izquierda mostró R-LVtime tempranos, lo que sugiere que estos pacientes no se beneficiarían con terapia de resincronización. SEPHO mejoró el ancho de QRS y el R-LVtime de todos los grupos, especialmente en los pacientes con bloqueo de rama izquierda. En conclusión, el R-LVtime serviría para identificar el sitio óptimo de estimulación y permitiría detectar ciertos no respondedores a la terapia de resincronización. Además, el SEPHO resultó la mejor modalidad de estimulación porque estrechó el QRS y acortó el R-LVtime de los pacientes con bloqueo de rama izquierda pero no alteró la despolarización fisiológica de los controles.
