INTRODUCTION AND OBJECTIVES: Modular neck primary stems were introduced with the theoretical advantage of restoring the hip anatomy more precisely. However, the presence of a second junction has been associated with increased corrosion and release of metal debris. The objective of our study is to quantify of chromium and cobalt serum values, and to analyze their temporal evolution during five years. MATERIAL AND METHODS: We present a prospective series of 61 patients who underwent primary total hip arthroplasty by implantation of the HMAX-M® stem (Limacorporate, San Daniele, Italy). Serum chromium and cobalt determinations were performed at six months, two years and five years. RESULTS: Our series shows a progressive elevation in chromium levels with a significant difference between chromium values at six months (0.35±0.18) and five years (0.52±0.36), P=.01. Regarding cobalt, a statistically significant elevation is observed between six months and two years and a subsequent stabilization of values between two and five years, with a cobalt mean at six months (1.17±0.8) significantly lower than at two (2.63±1.76) and five years (2.84±2.1), P=.001. CONCLUSION: Elevated serum cobalt levels have been observed in patients who underwent modular neck stem implantation. The results obtained in this study have limited the use of stems with a modular neck in our clinical practice.

The aim of this study was to evaluate the nutritional profile and the physicochemical characteristics during storage of newly developed formulations of fermented milk drinks with added pineapple, mango and passion fruit pulp. The fermented drinks showed a high content of protein, iron, and calcium. The passion fruit milk drink had the lowest pH (4.13) and highest acidity (0.95%, expressed in % of lactic acid), which was significantly different (p<0.05) from the pineapple and mango drinks. As for syneresis and sedimentation, the pineapple milk drink had the highest rates at 14 days storage, with 34.33% and 6.50%, respectively and was significantly different (p≤0.05) when compared to the mango and passion fruit milk drinks. In conclusion, newly developed fermented milk drinks with added fruit pulp were a source of several nutrients. We observed physical-chemical characteristics suitable for a fermented milk product during storage.


El objetivo de este estudio fue el desarrollo de nuevas formulaciones de bebidas lácteas fermentadas adicionadas de piña, mango y maracuyá, para evaluar el perfil nutricional y las características fisicoquímicas durante el almacenamiento. Las bebidas fermentadas mostraron un alto contenido en proteínas, hierro y calcio. En cuanto a las características fisicoquímicas durante el almacenamiento, la bebida láctea de maracuyá presentó el pH más bajo (4,13) y la acidez más alta (0,95%, expresada en % de ácido láctico), con una diferencia significativa (p < 0,05), en comparación con las bebidas de piña y mango. En cuanto a sinéresis y sedimentación, la bebida láctea de piña presentó los mayores índices a los 14 días de almacenamiento, con 34,33% y 6,50%, respectivamente, y con diferencia significativa (p ≤ 0,05) al compararla con las bebidas lácteas de mango y maracuyá. En conclusión, las bebidas lácteas fermentadas con adición de pulpa de fruta son una fuente de varios nutrientes, y de características físico-químicas adecuadas para un producto lácteo fermentado durante el almacenamiento.

Hysteroscopy is an exploratory endoscopic technique that studies the interior of the uterine cavity and the endocervical canal. Various fluids, such as physiological saline, are used to optimise visualisation of the internal structures during this procedure. A rare complication of hysteroscopy is fluid overload, which can be associated with intravascular absorption syndrome, usually after lengthy procedures or tissue dissection. There are no data on the incidence and prevalence of this syndrome, and few cases involving physiological saline solution have been reported. We present a case of hysteroscopic myomectomy complicated by vascular absorption syndrome, which gave rise to acute pulmonary oedema that required admission to the intensive care unit.

Introduction: Introduction: there is a close relationship between obesity, gut health and immune system. A low-grade of inflammation, which could precede obesity, may have implications for the development of metabolic syndrome and insulin resistance. Objective: analyzing the anti-inflammatory capacity of several types of whey (cow, sheep, goat and a mixture of them). Methods: an in vitro model of intestinal inflammation employing a cell co-culture (Caco-2 and RAW 264.7) was performed after an in vitro digestion and fermentation (simulating mouth-to-colon conditions). Inflammatory markers such as IL-8 and TNF-α, as well as the transepithelial electrical resistance (TEER) of Caco-2 monolayer, were determined. Results: digested and fermented whey had a protective effect on cell permeability, being lower in the case of fermented goat whey and mixture. The anti-inflammatory activity of whey was greater the more digestion progressed. Fermented whey showed the greatest anti-inflammatory effect, inhibiting IL-8 and TNF-α secretion, probably due to its composition (protein degradation products such as peptides and amino acids, and SCFA). However, fermented goat whey did not show this degree of inhibition, perhaps due to its low SCFA concentration. Conclusion: milk whey, especially after being fermented in the colon, can be useful nutritional strategy to preserve the intestinal barrier and mitigate the low-grade of inflammation that characterizes metabolic disorders and obesity.


Introducción: Introducción: existe una estrecha relación entre obesidad, salud intestinal y sistema inmune. Un bajo grado de inflamación, que precedería a la obesidad, puede tener implicaciones en el desarrollo de síndrome metabólico y resistencia a la insulina. Objetivo: analizar el poder antiinflamatorio de varios tipos de lactosuero (vaca, oveja, cabra y mezcla de los anteriores). Metodología: se utilizó un modelo in vitro de inflamación intestinal, empleando un cocultivo celular (Caco-2 y RAW 264.7). Para ello, se realizó una digestión y fermentación in vitro (simulando las condiciones de boca a colon). Se estudiaron IL-8 y TNF-α como marcadores inflamatorios y la resistencia eléctrica transepitelial celular (RETE) de la monocapa celular Caco-2. Resultados: el suero digerido y fermentado tuvo un efecto protector sobre la permeabilidad celular que fue menor en el caso de lactosuero fermentado de cabra y mezcla. La actividad antiinflamatoria del suero fue mayor cuanto más progresaba la digestión. El lactosuero fermentado mostró el mayor efecto antiinflamatorio, inhibiendo la secreción de IL-8 y TNF-α, probablemente debido a su composición (productos de degradación proteica como péptidos y aminoácidos, y ácidos grasos de cadena corta [AGCC]). Sin embargo, el suero fermentado de cabra no mostró ese grado de inhibición, quizás debido a su baja concentración en AGCC. Conclusión: el lactosuero, sobre todo tras ser fermentado en colon, puede ser una estrategia nutricional útil para preservar la barrera intestinal y mitigar el bajo grado de inflamación que caracteriza a desordenes metabólicos y a la obesidad.

Resumen Presentamos el caso de una mujer de 38 años sin antecedentes personales relevantes, residente de Ciudad Autónoma de Buenos Aires, que consultó por fiebre, cefalea retroocular mialgias, ar tralgias y exantema maculopapular pruriginoso en dorso de manos y pies de 6 días de evolución. El laboratorio presentaba linfopenia, trombocitopenia grave y hepatitis anictérica. El cónyuge había cursado internación tres semanas antes por un cuadro de similares características sin diagnóstico etiológico. Posteriormente, la paciente evolucionó con metrorragia y petequias axilares asociados a fotofobia, somnolencia y temblor fino de la lengua, con líquido cefalorraquídeo normal, cumpliendo tratamiento con ceftriaxona 2 g/día intravenoso por 7 días. La tomografía computarizada de abdomen y pelvis evidenciaba un hematoma de pared abdominal izquierdo. Se derivaron muestras serológicas al Instituto Nacional de Enfermedades Virales Humanas Dr. Julio I. Maiztegui para virus dengue, leptospirosis y hantavirus con resultados no reactivos y RT-PCR de virus Junín que resultó positiva. Retrospectivamente se realizó el diagnóstico del cónyuge por detección de anticuerpos IgG para virus Junín por ELISA y prueba de neutralización. Ninguno de los dos casos presentaba un nexo epidemiológico claro. Nuestro objetivo es remarcar la importancia de la sospecha clínica fuera de áreas endémicas.


Abstract We present the case of a 38-year-old woman with no relevant medical history, resident of the City of Buenos Aires, who was admitted in hospital for presenting fever, retroocular headache, myalgia, arthralgia, and maculopapular pruritic rash on the back of the hands and feet of 6 days of evolution. Laboratory tests revealed lymphopenia, severe thrombocytopenia, and anicteric hepatitis. Her husband had been hospitalized three weeks earlier for a condition of similar characteristics without etiological diagnosis. Subsequently, it evolved with metrorrhagia and axillary petechiae associated with photophobia, drowsiness, and fine tremor of the tongue with normal cerebrospinal fluid, treated with intravenous ceftriaxone 2 g/day for 7 days. Computed tomography of abdomen and pelvis showed a left abdominal wall hematoma. Serological samples were sent to the National Institute of Human Viral Diseases Dr. Julio I. Maiztegui for dengue virus, leptospirosis and hantavirus with non-reactive results, and RT-PCR of Junín virus that was positive. Retrospectively, the spouse was diagnosed by detection of IgG antibodies to Junin virus by ELISA and neutralization tests. Neither of the two cases had a clear epidemiological link. Our aim is to highlight the importance of clinical suspicion outside of endemic areas.

Introduction: Introduction: the nutritional intervention has become a critical point to achieve the reduction of perioperative morbidity and mortality in cancer patients. There are different factors that will play a fundamental role in the evolution and prognosis of this pathology, being the state of nutrition and diet a cornerstone in these aspects. Objective: to evaluate the perioperative effect of whey protein isolate (WPI) and calcium caseinate (CaCNT) in cancer patients undergoing elective surgery. Methods: randomized controlled clinical trial with three groups: the control group (n = 15), consisting in conventional management by the oncology surgery services, and two intervention groups, one with calcium caseinate supplementation (n = 15) and another one with whey protein isolate supplementation (n = 15) for six weeks perioperatively. Handgrip strength, six-minute walk, and body composition were assessed pre and postoperatively. Results: those who were supplemented with WPI maintained their handgrip strength and showed less extracellular water (p < 0.02); also an increase in visceral mass was shown (p < 0.02). Finally, a correlation was found with variables associated with body composition and patient evolution when compared to the control group. Conclusions: nutritional supplementation needs to be approached from the functional and metabolic point of view to identify factors that have a favorable impact, as well as the distinction between carcinoma and the type of supplementation to be implemented.


Introducción: Introducción: la intervención nutricia se ha convertido en un punto crítico para lograr la disminución de la morbimortalidad perioperatoria en el paciente oncológico. Existen distintos factores que jugarán un papel fundamental en la evolución y el pronóstico de esta patología, siendo el estado de nutrición y la dieta una piedra angular en estos aspectos. Objetivos: evaluar el efecto perioperatorio de la proteína aislada de suero de leche (WPI) y el caseinato de calcio (CaCNT) en pacientes oncológicos sometidos a cirugía electiva. Métodos: ensayo clínico controlado aleatorizado con tres grupos: grupo control (n = 15), que consistió en manejo convencional por parte de los servicios de cirugía oncológica, y dos grupos de intervención, uno con suplementación de caseinato de calcio (n = 15) y otro con suplementación con proteína aislada de suero de leche (n = 15) durante seis semanas de forma perioperatoria. Se evaluaron en tiempos preoperatorio y postoperatorio la fuerza de prensión de mano, la caminata de seis minutos y la composición corporal. Resultados: aquellos que fueron suplementados con WPI mantuvieron la fuerza de prensión y mostraron menor cantidad de agua extracelular (p < 0,02); se mostró un incremento en la masa visceral (p < 0,02). Finalmente, se encontró correlación con variables asociadas a la composición corporal y variables de evolución del paciente en comparación con el grupo control. Conclusiones: la suplementación nutricional requiere ser abordada desde el punto de vista funcional y metabólico para identificar factores que impacten favorablemente, así como la distinción entre carcinoma y el tipo de suplementación a implementar.

INTRODUCTION: The most widely used marker for the diagnosis of invasive aspergillosis (IA) is the detection of galactomannan by ELISA. This study describes the evaluation of the results obtained by Euroimmun Aspergillus antigen ELISA (EIA-GM-E) in serum samples and bronchoalveolar lavage fluid (BAL) from patients at risk of IA, and compares these results with those obtained by Bio-Rad Galactomannan EIA (EIA-GM-BR). METHODS: Anonymous retrospective case-control comparative study in 64 serum samples and 28 BAL from 51 patients. RESULTS: Overall agreement of the results of the two assays was observed in 72 of 92 samples (78.3%). The sensitivity of EIA-GM-BR and EIA-GM-E in serum samples was 88.9% and 43.2%, respectively, and 100% and 88.9% for BAL. The specificity of EIA-GM-BR and EIA-GM-E in serum samples was 91.9% for both assays, and 68.4% and 84.2% in BAL. There were no statistically significant differences in the results of both assays. CONCLUSIONS: Both methods show good results for the discrimination of patients with IA when BAL is tested, or serum in case of EIA-GM-BR.

El suero lácteo y la moringa poseen alto valor nutricional; sin embargo, su uso en la elaboración de productos alimenticios es una alternativa viable pero poco utilizada. Objetivo. Elaborar una galleta fortificada por sustitución parcial de harina de moringa (HM) y polvo de suero lácteo (PSL). Materiales y métodos. Durante septiembre-diciembre 2021, se desarrollaron cuatro formulaciones con sustitución parcial de harina de trigo por HM y PSL (F0=100:0:0; F1=90:5:5; F2=80:10:10; F3=70:15:15). La aceptación del producto se determinó mediante una evaluación sensorial considerando los atributos sabor, olor, color, textura y aceptación general. Se realizó el análisis químico proximal a la HM, al PSL, a la formulación de mayor aceptación y a F0. Se compararon los distintos parámetros químico proximales y de aceptación mediante una anova de una vía, seguido de una prueba de comparación de medias de Tukey (p<0,05). Resultados. Conforme se incrementa el porcentaje de sustitución de harina de trigo por HM y PSL se obtiene menor aceptación de las galletas, siendo F1 la más aceptada. F1 registró un contenido proteico de 6,90±0,04%, significativamente mayor al observado en F0. Conclusiones. La buena aceptabilidad de F1 indica que la sustitución parcial por HM y PSL no influyen en su aceptación; además, F1 mostró un enriquecimiento de proteinas, lípidos y cenizas, así como contenido adecuado de humedad y carbohidratos(AU)


Introduction. Whey and moringa have high nutritional value; however, their use in the production of food products is a viable but rarely used alternative. Objective. To make a biscuit fortified by partial substitution of moringa flour (MF) and whey powder (WP). Materials and methods. During September-December 2021, four formulations were developed with different degrees of partial substitution of wheat flour for MF and WP (F0=100:0:0; F1=90:5:5; F2=80:10:10; F3=70:15:15). The acceptance of the product was determined through a sensory evaluation considering the attributes taste, odor, color, texture and general acceptance. A proximal chemical analysis was performed on the MF, WP, the most widely accepted formulation and F0. Proximal chemical parameters and acceptance attributes were compared using a one-way anova, followed by a Tukey mean comparison test (p<0.05). Results. As the percentage of substitution of wheat flour by MF and WP increases less acceptance of the biscuits is obtained, with F1 as the most accepted formulation. F1 registered a protein content of 6.90±0.04%, significantly higher than that observed in F0. Conclusions: The good acceptability of F1 indicates that the partial substitution of wheat flour for MF and WP do not influence its acceptance; in addition, F1, showed an enrichment of protein, lipid and ash, as well as adequate moisture and carbohydrate content(AU)

A pesquisa busca técnicas alternativas para expansão da vida de prateleira dos alimentos, isto tem impulsionado estudos sobre a utilização de conservantes naturais, tais como as bacteriocinas e óleos essenciais, que são considerados agentes antimicrobianos naturais. No entanto estes antimicrobianos naturais, não são adicionados diretamente em produtos alimentícios, devido a alterações sensoriais e em suas características físico e química. Com avanço tecnológico da microencapsulação, tem sido um potencial em fornecer sistemas que garantem estabilidade para os antimicrobianos naturais desta forma podendo compor a matriz de alimentos. Portanto, o objetivo desse trabalho foi microencapsular a enterocina produzida por Enterococcus durans MF5 e óleo de orégano usando leitelho. Para a microencapsulação, foram realizados três tratamentos: T1 controle leitelho, T2 leitelho/enterocina (LE), e T3 leitelho/enterocina/óleo (LEO). O material foi submetido ao processo de spray dryer e foram realizados ensaios para determinar a atividade antimicrobiana do material encapsulado contra as bactérias Listeria monocytogenes, Listeria innocua e Listeria ivanovi. O rendimento da microencapsulação foi de 13,01% e 11,63% para LE e LEO, respectivamente. Os resultados apresentados nos microencapsulados LE e LEO mostraram inibição contra todas as bactérias teste, foi constatado que a microencapsulação de enterocina e óleo de orégano mantiveram seu poder antimicrobiano. A efetividade da microencapsulação foi realizada por (FTIR), onde picos de intensidade entre as amostras na região 1000 a 930 cm-¹ e 1800 a 1500 cm-¹ foram observadas. Os resultados apontam para mudança no perfil químico das amostras encapsuladas, corroborando com a hipótese que o leitelho apresentou papel encapsulante da bactericiona e óleo de orégano.Portanto a microencapsulação aumenta a eficácia antimicrobiana dos antimicrobianos.


The research seeks alternative techniques for expanding the shelf life of foods, this has driven studies on the use of natural preservatives, such as bacteriocins and essential oils, which are considered natural antimicrobial agents. However, these natural antimicrobials are not directly added to food products due to sensory changes and their physical and chemical characteristics. With technological advancement of microencap- sulation, it has been a potential to provide systems that ensure stability for natural anti- microbials in this way can compose the food matrix. Therefore, this study has an objective microencapsulated the interocin and essencial oil, used buttermilk as a encapsulating ma- terial where, T1 Buttermilk Control, T2 buttermilk/enterocin (LE), e T3 Buttermilk/en- terocin/oil (LEO). The product has been submitted to spray drier process, were conducted trials to determine antimicrobial activity. Was observed with mass yield 13,01% e 11,63% para LE e LEO. These results the microencapsulate indicate then LE e LEO there was inihibiton against bacteria tests. Was observed that the microencapsulated between enter- ocin and essential oil oregano maintained antimicrobial power. The effectiveness of the microencapsulated was performed by Fourier transform infrared (FTIR) analysis, where a sample in the region 1000 to 930 cm-¹ and 1800 to 1500 cm-¹ was observed. Therefore microencapsulation increases antimicrobial efficacy of antimicrobials.


La investigación busca técnicas alternativas para ampliar la vida útil de los alimentos, esto ha impulsado estudios sobre el uso de conservantes naturales, como las bacteriocinas y los aceites esenciales, que se consideran agentes antimicrobianos naturales. Sin embargo, estos antimicrobianos naturales no se añaden directamente a los productos alimentarios debido a los cambios sensoriales y a sus características físicas y químicas. Con el avance tecnológico de la microencapsulación, ha sido un potencial para proporcionar sistemas que garanticen la estabilidad de los antimicrobianos naturales de esta manera puede componer la matriz alimentaria. Por lo tanto, este estudio tiene como objetivo microencapsular la interocina y el aceite esencial, utilizando suero de leche como material encapsulante donde, T1 Suero de leche Control, T2 Suero de leche/enterocina (LE), e T3 Suero de leche/enterocina/aceite (LEO). El producto ha sido sometido al proceso de secado por pulverización, se realizaron ensayos para determinar la actividad antimicrobiana. Se observó con rendimiento de masa 13,01% e 11,63% para LE e LEO. Estos resultados indican que el microencapsulado LE e LEO fue inhibido contra las pruebas bacterianas. Se observó que el microencapsulado entre enterocina y aceite esencial de orégano mantuvo el poder antimicrobiano. La eficacia del microencapsulado fue realizada por análisis de infrarrojo transformado de Fourier (FTIR), donde fue observada una muestra en la región de 1000 a 930 cm-¹ y de 1800 a 1500 cm-¹. Por lo tanto, la microencapsulación aumenta la eficacia antimicrobiana de los antimicrobianos. PALABRAS CLAVE: Bacteriocina; Enterococcus durans; Suero de Leche; Origanum vulgare; Spray Dryer.

A pesquisa busca técnicas alternativas para expansão da vida de prateleira dos alimentos, isto tem impulsionado estudos sobre a utilização de conservantes naturais, tais como as bacteriocinas e óleos essenciais, que são considerados agentes antimicrobianos naturais. No entanto estes antimicrobianos naturais, não são adicionados diretamente em produtos alimentícios, devido a alterações sensoriais e em suas características físico e química. Com avanço tecnológico da microencapsulação, tem sido um potencial em fornecer sistemas que garantem estabilidade para os antimicrobianos naturais desta forma podendo compor a matriz de alimentos. Portanto, o objetivo desse trabalho foi microencapsular a enterocina produzida por Enterococcus durans MF5 e óleo de orégano usando leitelho. Para a microencapsulação, foram realizados três tratamentos: T1 controle leitelho, T2 leitelho/enterocina (LE), e T3 leitelho/enterocina/óleo (LEO). O material foi submetido ao processo de spray dryer e foram realizados ensaios para determinar a atividade antimicrobiana do material encapsulado contra as bactérias Listeria monocytogenes, Listeria innocua e Listeria ivanovi. O rendimento da microencapsulação foi de 13,01% e 11,63% para LE e LEO, respectivamente. Os resultados apresentados nos microencapsulados LE e LEO mostraram inibição contra todas as bactérias teste, foi constatado que a microencapsulação de enterocina e óleo de orégano mantiveram seu poder antimicrobiano. A efetividade da microencapsulação foi realizada por (FTIR), onde picos de intensidade entre as amostras na região 1000 a 930 cm-¹ e 1800 a 1500 cm-¹ foram observadas. Os resultados apontam para mudança no perfil químico das amostras encapsuladas, corroborando com a hipótese que o leitelho apresentou papel encapsulante da bactericiona e óleo de orégano.Portanto a microencapsulação aumenta a eficácia antimicrobiana dos antimicrobianos.


The research seeks alternative techniques for expanding the shelf life of foods, this has driven studies on the use of natural preservatives, such as bacteriocins and essential oils, which are considered natural antimicrobial agents. With technological advancement of microencap- sulation, it has been a potential to provide systems that ensure stability for natural anti- microbials in this way can compose the food matrix. Therefore, this study has an objective microencapsulated the interocin and essencial oil, used buttermilk as a encapsulating ma- terial where, T1 Buttermilk Control, T2 buttermilk/enterocin (LE), e T3 Buttermilk/en- terocin/oil (LEO). The product has been submitted to spray drier process, were conducted trials to determine antimicrobial activity. Was observed with mass yield 13,01% e 11,63% para LE e LEO. These results the microencapsulate indicate then LE e LEO there was inihibiton against bacteria tests. Was observed that the microencapsulated between enter- ocin and essential oil oregano maintained antimicrobial power. The effectiveness of the microencapsulated was performed by Fourier transform infrared (FTIR) analysis, where a sample in the region 1000 to 930 cm-¹ and 1800 to 1500 cm-¹ was observed. Therefore microencapsulation increases antimicrobial efficacy of antimicrobials.


La investigación busca técnicas alternativas para ampliar la vida útil de los alimentos, esto ha impulsado estudios sobre el uso de conservantes naturales, como las bacteriocinas y los aceites esenciales, que se consideran agentes antimicrobianos naturales. Sin embargo, estos antimicrobianos naturales no se añaden directamente a los productos alimentarios debido a los cambios sensoriales y a sus características físicas y químicas. Con el avance tecnológico de la microencapsulación, ha sido un potencial para proporcionar sistemas que garanticen la estabilidad de los antimicrobianos naturales de esta manera puede componer la matriz alimentaria. Por lo tanto, este estudio tiene como objetivo microencapsular la interocina y el aceite esencial, utilizando suero de leche como material encapsulante donde, T1 Suero de leche Control, T2 Suero de leche/enterocina (LE), e T3 Suero de leche/enterocina/aceite (LEO). El producto ha sido sometido al proceso de secado por pulverización, se realizaron ensayos para determinar la actividad antimicrobiana. Se observó con rendimiento de masa 13,01% e 11,63% para LE e LEO. Estos resultados indican que el microencapsulado LE e LEO fue inhibido contra las pruebas bacterianas. Se observó que el microencapsulado entre enterocina y aceite esencial de orégano mantuvo el poder antimicrobiano. La eficacia del microencapsulado fue realizada por análisis de infrarrojo transformado de Fourier (FTIR), donde fue observada una muestra en la región de 1000 a 930 cm-¹ y de 1800 a 1500 cm-¹. INTRODUCTION: Nebulized hypertonic saline (HS) improves quality of life and reduces exacerbations in patients with cystic fibrosis. It is unknown if it would offer the same benefits in other hypersecretory pathologies. METHODS: Retrospective observational study. Patients who passed the tolerance test and started HS 5.8% with one year of follow-up were included. Clinical and healthcare parameters were quantified in the year before and after the start of treatment. RESULTS: 101 patients, 60.4% women, 65years (95%CI: 62.4-67.9): 82 (81.2%) bronchiectasis, 6 (5.9%) COPD, 2 (2%) asthma, 1 (1%) ILD, and 10 (9.9%) other causes. There was a reduction in bronchorrhea (91.1% vs 75.2%), recurrent infections (57.4% vs 22.8%) and cycles of antibiotic therapy (1.54 vs 0.55), as well as an increase in FEV1 (1881ml vs. 1942ml) and a decrease in visits to primary care (2.94 vs. 1.1), emergencies (0.36 vs. 0.17) and hospitalizations (0.17 vs. 0.17). 06). 73 patients (72.3%) presented an adequate tolerance. CONCLUSION: Nebulization of HS 5.8% in patients with bronchial hypersecretion is safe and has a remarkable clinical and healthcare impact.

ABSTRACT Introduction: Evidence indicates that whey protein supplementation may accelerate tissue repair and be useful for exercise-induced muscle injury (EIMD) by accelerating recovery and elevating protein synthesis and blood amino acids. However, the potential role of whey protein after EIMD in humans is inconsistent. Objective: Find the effective role of whey protein in post-exercise recovery from EIMD. Methods: Scopus, Medline, and Google scholar, were systematically searched until March 2022. To assess the risk of bias, the Cochrane collaboration tool was applied. Weighted mean differences (WMD), 95% confidence intervals (CI), and random effect models to calculate the total effect. Results: The result of the review indicated that the decreasing impact of whey protein intake is significant on creatine kinase (CK) [WMD = −19.11 IU.L-1, CI: −36.200, −2.036; P = 0.028]. The effect of whey protein supplementation on lactate dehydrogenase (LDH) concentration indicated that the impact of whey protein on changing LDH levels is significant. In addition, subgroup analysis showed significant decreases in CK and LDH based on post-exercise follow-up times, whey protein dosage, test duration, supplementation time, exercise types, and training status. Conclusion: The results showed the efficacy of whey protein in decreasing CK and LDH levels among adults in general and in subgroup analysis. Therefore, whey protein could have an effective role in the post-exercise recovery of EIMD. Level of evidence II; Therapeutic studies - review of results.

Entretanto, o papel potencial da proteína de soro de leite após a EIMD em humanos, é inconsistente. Objetivos: Encontrar a função efetiva da proteína de soro de leite na recuperação pós exercício de EIMD. Métodos: Scopus, Medline e Google scholar foram sistematicamente pesquisados até março de 2022. Para avaliar o risco de viés, foi aplicada a ferramenta de colaboração Cochrane. Diferenças médias ponderadas (WMD), intervalos de confiança de 95% (CI) e modelos de efeito aleatório foram utilizados para o cálculo do efeito total. Resultados: O resultado da revisão indicou que o impacto decrescente do consumo de proteína de soro de leite é significativo na creatina quinase (CK) [WMD = −19,11 IU.L-1, CI: −36,200, −2,036; P = 0,028]. O efeito da suplementação proteica do soro de leite na concentração de desidrogenase láctica (LDH) indicou que o impacto da proteína do soro de leite na mudança dos níveis de LDH é significativo. Além disso, a análise dos subgrupos mostrou diminuição significativa na CK e LDH, com base nos tempos de acompanhamento após o exercício, dosagem da proteína do soro de leite, duração dos testes, tempo de suplementação, tipos de exercício e status de treinamento. Conclusão: Os resultados mostraram a eficácia da proteína de soro de leite na diminuição dos níveis de CK e LDH entre adultos, em geral e na análise dos subgrupos. RESUMEN Introducción: Las pruebas indican que la suplementación con proteína de suero de leche puede acelerar la reparación de los tejidos y, por lo tanto, ser útil para las lesiones musculares inducidas por el ejercicio (EIMD) al acelerar la recuperación, así como elevar la síntesis de proteínas y los aminoácidos en sangre. Sin embargo, el papel potencial de la proteína de suero después de la EIMD en los seres humanos, es inconsistente. Objetivos: Encontrar el papel efectivo de la proteína de suero de leche en la recuperación posterior al ejercicio de EIMD. Métodos: Se realizaron búsquedas sistemáticas en Scopus, Medline y Google scholar hasta marzo de 2022. Para evaluar el riesgo de sesgo, se aplicó la herramienta de colaboración Cochrane. Para calcular el efecto total se utilizaron diferencias medias ponderadas (WMD), intervalos de confianza (CI) del 95% y modelos de efectos aleatorios. Resultados: El resultado de la revisión indicó que el impacto decreciente de la ingesta de proteína de suero es significativo en la creatina quinasa (CK) [WMD = −19,11 UI.L-1, IC: −36,200, −2,036; P = 0,028]. El efecto de la suplementación con proteína de suero en la concentración de lactato deshidrogenasa (LDH) indicó que el impacto de la proteína de suero en el cambio de los niveles de LDH es significativo. Además, el análisis de subgrupos mostró disminuciones significativas de la CK y la LDH en función de los tiempos de seguimiento tras el ejercicio, la dosis de proteína de suero, la duración de la prueba, el tiempo de suplementación, los tipos de ejercicio y el estado de entrenamiento. Resumen Introducción: La inmunoterapia pasiva es una alternativa terapéutica para los pacientes con COVID-19. Métodos: Se tomó la decisión de realizar una base de datos prospectiva de los pacientes con diagnóstico de neumonía por SARS-CoV-2, no hipoxémica, tratados de forma ambulatoria en el Hospital de Bolívar, Dr. Miguel Capredoni, provincia de Buenos Aires, Argentina, con el objetivo de evaluar la eficacia del tratamiento con suero equino hiperinmune en la reducción de casos graves e internaciones. Realizamos un análisis retrospectivo del período comprendido entre el 26/05/2021 al 28/08/2021, se incluyeron 151 pacientes. Las opciones fueron meprednisona y colchicina asociadas a dos infusiones de suero equino (n = 92) o me prednisona y colchicina durante 10 días por vía oral (n = 59). Resultados: No se observaron diferencias en las características poblacionales y comorbilidades entre ambos grupos. El 46% (69) de los pacientes había recibido por lo menos una dosis de vacuna contra COVID-19. Durante el seguimiento el 23% (35) requirió internación, sin diferencias entre el grupo suero equino y el grupo control (p = 0.89). Se observó una tendencia no significa tiva al riesgo de internación prolongada del 15.7%. (Grupo suero equino 38.1% vs. grupo control 53.8%, Fisher Exact test p = 0.41). La mortalidad en el grupo suero equino fue del 3.97% (4), sin observarse diferencias entre ambos grupos. Se observaron diferencias entre los pacientes vacunados y no vacunados, en puntos duros como necesidad de ARM (0% vs. 8% p = 0.001) y muerte (0% vs. 8% p = 0.001). Discusión: Si bien las tasas de internación y muerte fueron menores de lo esperado, la utilización de suero equino hiperinmune en el escenario ambulatorio impresiona no aportar beneficio clínico.


Abstract Introduction: Passive immunotherapy is a therapeutic alternative for patients with COVID-19. Methods: The decision was made to create a prospective database of patients diagnosed with SARS-CoV-2 pneumonia, non-hypoxemic, treated on an outpatient basis at the Hospital de Bolívar, Dr. Miguel Capredoni, province of Buenos Aires, Argentina, with the aim of evaluating the efficacy in reducing severe cases and hospitalizations of treatment with hyperimmune equine serum in this subgroup of patients. We performed a retrospective analysis of the period from 05/26/2021 to 08/28/2021, where a total of 151 patients were included. The options were meprednisone plus colchicine associated with two equine serum infusions (n = 92) or oral meprednisone and colchicine for 10 days (59). Results: No differences were observed between the population characteristics and comorbidities between both groups. A 46% (69) of the patients had received at least one dose of vaccine against COVID-19. During follow-up, 23% (35) required hospitalization, with no differences between the equine serum group and the control group (p = 0.89). A non-significant trend of 15.7% was observed for the risk of prolonged hospitalization. (Equine serum group 38.1% vs. control group 53.8%, Fisher Exact test p = 0.41). Mortality between the equine serum group was 3.97% (4), with no differences between the two groups. Differences were observed between vaccinated and unvaccinated patients in hard points such as the need for MRA (0% vs. 8% p = 0.001) and death (0% vs. 8% p = 0.001). Discussion: Although the rate of hospitalization and death were lower than expected, the use of hyperimmune equine serum in the outpatient setting impresses as not providing clinical benefit.

INTRODUCTION AND OBJECTIVES: To evaluate whether there is any difference between immediate postoperative instillation of intravesical chemotherapy (IPOIC) and continuous saline bladder irrigation (CSBI) in terms of bladder cancer (BC) recurrence in patients with primary low- or intermediate-risk non-muscle-invasive BC (NMIBC). MATERIALS AND METHODS: Medical records of 1482 patients who underwent transurethral resection of bladder tumor between March 1994 and August 2020 were reviewed retrospectively. Patients were divided into two groups according to IPOIC and/or CSBI administration status [Group-1 = CSBI alone; Group-2 = CSBI following IPOIC]. Low- and intermediate-risk NMIBC patients were also divided into subgroups according to IPOIC and/or CSBI administration status. RESULTS: A total of 594 patients with primary NMIBC were included. Of the patients, 86 (14.5%) were female and 508 (85.5%) were male with a median age of 69 (60-78) years. The frequency of patients in Group-1 and Group-2 were 361 (60.8%) and 233 (39.2%), respectively. Recurrent disease was observed in 213 (35.9%) patients. There was no difference between the groups when they were compared for recurrent disease frequency, median time to first recurrence and frequency of recurrence within first 12 months (P = .064, P = .671, and P = .145, respectively). Disease recurrence rates in low-risk NMIBC patients was lower when they were treated with "CSBI following IPOIC" when compared to "CSBI alone" (P = .042). However, no difference was observed in low-risk NMIBC subgroups when they were compared for pathological features of recurrent tumors such as number, size, grade, stage, and presence of carcinoma in situ (P > .05, for each). CONCLUSIONS: "CSBI following IPOIC" combination was not superior to "CSBI alone" for preventing adverse pathological outcomes in recurrent low- and intermediate-risk NMIBC.

RESUMEN Fundamento: La electroforesis de proteínas y las cadenas ligeras libres en suero son técnicas utilizadas en el diagnóstico del mieloma múltiple. Sin embargo, la utilidad diagnóstica de ambas pruebas puede variar según el método empleado y condiciones reales del medio donde se realicen. Objetivo: Determinar el valor diagnóstico de la electroforesis de proteínas y de las cadenas ligeras libres en suero en el mieloma múltiple. Metodología: Se realizó un estudio retrospectivo de los parámetros electroforesis de proteínas en suero y cadenas ligeras libres en suero a 43 pacientes con diagnóstico de mieloma múltiple por evaluación de la médula ósea. La electroforesis de proteínas se realizó por el método convencional de separación de proteínas sobre papel de acetato de celulosa y para las cadenas ligeras libres se aplicó un ensayo inmunoturbidimétrico en el que se usó un analizador químico (Cobas 311). Se calcularon 7 parámetros que evaluaron la exactitud diagnóstica. Resultados: Todos los parámetros que evaluaron la exactitud diagnóstica estuvieron dentro de los intervalos de confianza en ambas pruebas. Conclusiones: La electroforesis de proteínas y las cadenas ligeras libres en suero son ensayos de gran utilidad en el diagnóstico del mieloma múltiple y se deben utilizar en conjunto para la mayor captación posible de casos.


ABSTRACT Background: Protein electrophoresis and serum free light chains are techniques used in the diagnosis of multiple myeloma. However, the diagnostic utility of both tests may vary according to the method used and the actual conditions of the environment where they are performed. Objective: To determine the diagnostic value of protein electrophoresis and serum free light chains in multiple myeloma. Methodology: A retrospective study of serum protein electrophoresis parameters and serum free light chains was conducted in 43 patients diagnosed with multiple myeloma by bone marrow evaluation. Protein electrophoresis was completed by the conventional method of protein separation on cellulose acetate paper and for free light chains an immunoturbidimetric assay was applied in which a chemical analyzer (Cobas 311) was used. Seven parameters were calculated to evaluate diagnostic accuracy. Results: All parameters assessing diagnostic accuracy were within confidence intervals in both tests. Conclusions: Protein electrophoresis and serum free light chains are very useful assays in the diagnosis of multiple myeloma and should be used in conjunction for the highest possible approval of cases.

OBJECTIVE: The scope of this study was to assess salivary and serum osteoprotegerin (OPG) levels in knee osteoarthritis (OA). METHODS: Serum and saliva OPG levels of 30 knee OA and 30 matched healthy controls in this cross-sectional study was assessed by ELISA. Knee pain was assessed by WOMAC. Data were analyzed by Student's t-test, Spearman correlation test and ROC. RESULTS: The mean serum but not saliva OPG level was lower in knee OA than that of the healthy group. WOMAC negatively correlated with serum OPG (r = -0.501; P = 0.000). The serum OPG cutoff value was 237.5 pg/ml for the diagnosis of knee OA. CONCLUSIONS: As serum OPG was lower in knee OA and negatively correlated with WOMAC, it seems that detection of OPG in serum but not in saliva may be a probable marker to the diagnosis of knee OA. KEY MESSAGES: Osteoprotegerin decreases in knee osteoarthritis.

OBJECTIVE: To describe serum levels of calcium, copper, selenium, magnesium, iron and zinc and evaluate their relationship with maternal socio-demographic characteristics and dietary variables in women in the first trimester of pregnancy. METHOD: Cross-sectional study with 1279 participants from the INMA cohorts. RESULTS: The concentrations of the elements analyzed were within the normal range. Associations with higher levels of these metals were found for calcium with white meat intake (p=0.026), for copper with excess body weight (p <0.01), low social class (p=0.03) and being multipara (p <0.01), for magnesium with being over 35 years old (p=0.001), high social class (p=0.044), primiparous status (p=0.002) and low daily intake of bread (p=0.009) and legumes (p=0.020); for zinc with university education (p=0.039) and residence in Gipuzkoa (p <0.01), and for selenium with residence in Valencia (p <0.01), university education (p=0.001), vitamin B6 supplementation (p=0.006), fish intake (> 71g/day) (p=0.014) and having been born in Spain (p=0.001). Further, lower iron levels were associated with being overweight (p=0.021) or obese (p <0.001) and vitamin B12 supplementation (p=0.006). CONCLUSIONS: Our results suggest that trace elements in the analyzed cohorts are adequate for this stage of pregnancy. The variability in these elements is mainly linked to socio-demographic and anthropometric variables.

Contexto y objetivo: las anomalías hematológicas se encuentran entre las complicaciones más comunes de la infección por el VIH. También se han realizado bastantes estudios sobre las alteraciones en el perfil de lípidos, aunque los resultados en gran medida no han sido concluyentes. El presente estudio se llevó a cabo para evaluar el recuento de células CD4 y el perfil de lípidos en los pacientes infectados por el VIH y el SIDA en la población india y los correlaciona con los controles sero-negativos. Materiales y métodos: El presente estudio fue diseñado como un estudio transversal, con base en un hospital, para evaluar el recuento de células CD4 y el perfil de lípidos en los pacientes infectados por VIH y SIDA en la población india y los correlaciona con los controles sero-negativos. La evaluación del perfil de lípidos se realizó utilizando Erba EM 360, un analizador automático impulsado por un fotómetro de rejilla de difracción, mientras que los recuentos de células CD4 se evaluaron utilizando el Contador de ciclo de Partec. Análisis estadístico utilizado: Los datos se analizaron con SPSS versión 15.0 (SPSS Inc., Chicago, IL, EE. UU.). La comparación de dichos parámetros se realizó mediante el análisis de varianza (ANOVA) y la prueba post-hoc de Games-Howell. Se consideró estadísticamente significativo un valor de p <0,05. Resultados: Los niveles de colesterol total y lipoproteínas de baja densidad (LDL) disminuyeron significativamente, mientras que los triglicéridos y las lipoproteínas de muy baja densidad (VLDL) aumentaron significativamente en los pacientes infectados por VIH y SIDA en comparación con los controles sero-negativos. Conclusión: El colesterol total, las LDL, los triglicéridos y las VLDL se alteraron significativamente en los pacientes infectados por VIH y con SIDA en comparación con los controles sero-negativos


Context and Aim: Hematological abnormalities are amongst the most common complications of infection with HIV.There have been quite a few studies on the alterations in lipid profile, too, though the results have largely been inconclusive. The present study was carried-out to assess CD4 cell counts and lipid profile in the HIV infected and AIDS patients in the Indian population and correlates them with the sero-negative controls. Materials and Methods: The present study was designed as a cross-sectional, hospital-based study to assess CD4 cell counts and lipid profile in the HIV infected and AIDS patients in the Indian population and correlates them with the sero-negative controls. Evaluation of lipid profile was done using Erba EM 360, an automated analyzer powered by a diffraction grating photometer while CD4 cell counts were evaluated using Partec Cyflow Counter. Statistical analysis used: The data was analyzed using SPSS version 15.0 (SPSS Inc., Chicago, IL, USA). Comparison of the said parameters was done using Analysis of Variance (ANOVA) and post-hoc Games-Howell test. p-value of <0.05 was considered statistically significant. Results: The levels of total cholesterol and low-density lipoproteins (LDLs) were significantly decreased while triglycerides and very low density lipoproteins (VLDLs) were significantly increased in the HIV infected and AIDS patients when compared with the sero-negative controls. Conclusion: Total cholesterol, LDLs, triglycerides and VLDLs were significantly altered in the HIV infected and AIDS patients when compared with the sero-negative controls

Objetivo: Determinar la efectividad del suero autólogo rico en factores de crecimiento en la reparación de lesiones de la superficie ocular de evolución incierta con el tratamiento convencional. Materiales y métodos: Se trataron 46 unidades oculares con afecciones de la superficie ocular agrupadas en queratopatías por exposición, queratopatías por síndrome de ojo seco / neurotróficas, y traumas oculares. Las partes oculares afectadas fueron: conjuntiva, cornea (epitelio, estroma) y esclera. Se evaluaron de manera anatómica y funcional con la prueba de Schirmer, tinción con Fluoresceína y tomografía de coherencia óptica (TCO) entre marzo y diciembre del 2020. Resultados: Los síntomas mejoraron en el siguiente orden: dolor ocular, sensación de cuerpo extraño, blefaroespasmo, hiperemia y lagrimeo. Las lesiones evolucionaron favorablemente de la siguiente manera: en primer lugar las conjuntivales y del epitelio corneal, luego las del estroma corneal y finalmente las lesiones en la esclera. Se obtuvo una media de 15 días para recuperación inmediata de la superficie y de 21 días para recuperación tardía. Las lesiones con adelgazamiento parcial profundo de esclera tomaron alrededor de 2 meses. Conclusiones: Los hallazgos relacionados al umbral del dolor, tiempo de recuperación, remodelación cicatrizal del tejido afectado y recuperación de la agudeza visual son prometedores e importantes. La utilización de suero autólogo rico en factores de crecimiento puede ser una alternativa terapéutica para las lesiones de difícil resolución con el tratamiento convencional.


Objective: To determine the effectiveness of autologous serum rich in growth factors to repair ocular surface lesions which have uncertain progression with conventional treatment. Materials and methods: AForty-six (46) eyes with ocular surface disorders such as exposure keratopathy, keratopathy caused by dry eye syndrome, neurotrophic keratopathy and blunt eye injury were treated. The affected areas were the conjunctiva, cornea (epithelium, stroma) and sclera. Anatomical and functional evaluations were performed between March and December 2020 using Schirmer's test, fluorescein eye stain and optical coherence tomography (OCT). Results: The symptoms improved in the following order: eye pain, foreign body sensation, blepharospasm, hyperemia and epiphora. Additionally, the lesions progressed favorably as follows: first, those of the conjunctiva and corneal epithelium; then, those of the corneal stroma; and, finally, those of the sclera. An average of 15 days was required for immediate ocular surface recovery and 21 days for late recovery. The lesions with total scleral thinning healed in about two months. Conclusions: The findings related to pain threshold, recovery time, scar tissue remodeling of the affected tissue and visual acuity improvement are promising and important. Using autologous serum rich in growth factors may be a therapeutic alternative for those lesions that are difficult to resolve with conventional treatment.
