RESUMO

Brain aneurysm ruptured subarachnoid hemorrhages (SAH) are extremely rare except in humans. This study described a SAH caused by a ruptured anterior communication artery aneurysm and concurrent myocardial infarction, along with pneumonia and intestinal obstruction in a rhesus monkey, which is rather rare in animal experiments.

A 12 yr old male neutered beagle was presented on transfer to the intensive care unit with severe anemia, thrombocytopenia, and bruising detected 1 day after undergoing tibial plateau leveling osteotomy surgery. The patient had undergone extra-capsular stifle stabilization surgery 14 wk prior to treat ligament disease in the same knee. Laboratory testing and treatment for anemia, presumptive immune-mediated thrombocytopenia, and possible hemostatic disorder was initiated. A persistent anemia, progressive thrombocytopenia, and the development of a firm swelling and neurologic impairment in the limb raised concerns for compartment syndrome (CS). A musculoskeletal ultrasound revealed a large aneurysm in the caudal thigh surrounded by abnormal muscle tissue. The patient underwent amputation of the limb and recovered without further complication. Pathology findings were consistent with the development of femoral CS secondary to a ruptured peripheral arterial aneurysm or a pseudoaneurysm. A consumptive thrombocytopenia and regenerative anemia were attributed to periodic or progressive thrombosis of the vessel and regional hemorrhage. Postoperative CS can develop in combination with peripheral arterial aneurysm or pseudoaneurysm, and screening for vascular abnormalities as well as CS should be considered in complicated recovery from orthopedic surgery with compatible clinical signs including progressive soft tissue swelling, persistent anemia, and thrombocytopenia.

A 19.5-y-old, male domestic yak ( Bos grunniens) with a history of sudden unexpected death was submitted for autopsy. The yak had hemoabdomen, and a large blood clot was attached to the liver and forestomachs. The hepatic artery had a saccular aneurysm with a 1-cm tear. The arterial wall at the site of the rupture was thin, and the luminal surface was roughened with yellow streaks. The arterial wall adjacent to the rupture was thickened, white, firm, and less elastic than normal arterial walls. Cause of death was concluded to be acute exsanguination. Similar cases with sudden death have been reported in domestic cattle and humans. No risk factors, such as nutritional deficiencies, genetic predisposition, or blunt trauma, were identified in this case, and there was no gross or histologic evidence of generalized vascular disease.

In this retrospective study, the authors describe the gross and histologic changes associated with rupture of an abdominal artery aneurysm in 33 mature female Holstein cattle between January 1980 and June 2005 from 29 farms in upstate New York and northern Pennsylvania. Over this period, there was an increase in the number of cases submitted for necropsy per year, and a seasonal trend did not exist. Affected animals ranged from 2.5 to 5.5 years of age. Grossly affected cattle exhibited marked hemoabdomen. There was marked dilation and rupture of the abdominal aorta or one of its branches, including the mesenteric, left gastric, celiac artery, right ruminal artery, or left ruminal artery. Histologically, the tunica media of the affected arteries was often thin and irregular in width with disrupted, fragmented, and coiled elastin. Granulation tissue and hemorrhage was localized at the site of rupture. There was mild-to-marked hyperplasia of the tunica intima that was irregular and disorderly with adjacent smooth-muscle hyperplasia within the tunica media. In addition, in a fraction of cases, there was mild-to-moderate mucinous change (48%) and mineralization (30%) within the tunica media. To the authors' knowledge, this is the first description of the gross and histologic changes in Holstein cows with abdominal artery aneurysm and rupture.

BACKGROUND: Small aneurysms (lesser than 2 mm) in humans called sessile, baby aneurysms, or microaneurysms, generally are not able to be clipped or to be coil-packed through endovascular route. Among the modalities of treatment that have been used for treating microaneurysms, bipolar coagulation and wrapping of the lesion are outstanding. Nevertheless, demonstration of the efficacy of these treatments is difficult because most reported experimental models for inducting aneurysms are complex and difficult to be reproduced. This study aimed to develop a simple and reproducible model for inducing microaneurysms. METHODS: Microaneurysms were induced using a mechanical lesion of the bifurcation of the aorta in 72 rats. Three groups of 10 animals were sacrificed 7, 14, and 21 days after the lesion and 2 groups (35 and 7 animals) after 30 days. The aortic bifurcation was macro/microscopically analyzed in the first 4 groups and a resistance test was applied in the fifth group. RESULTS: Microaneurysms occurred in 77.8% of cases. Microscopically, degenerative changes were observed in the intima, media, and adventitia and in the internal elastic lamina. The bursting pressure ranged from 368 to 1,472 mm Hg during the resistance test in the fifth group. CONCLUSIONS: The presented model of experimental microaneurysm induction is simple, reproducible and gives a high rate of positivity.

Exploratory laparotomy of an adult dairy cow, examined because of acute signs of persistent abdominal pain, revealed a firm pulsatile mass with associated fremitus just distal to the origin of the cranial mesenteric artery. The cow died acutely 2.5 days after surgery. A dilated, thin-walled, sacculated aneurysm, which had ruptured, was located along the proximal portion of the cranial mesenteric artery. It was postulated that the aneurysm developed secondary to structural defects in the arterial wall, but caused no clinical signs until enlargement and local tissue stretching or circulatory disturbances caused intestinal ischemia, resulting in abdominal pain. Aneurysms of visceral arteries in cattle should be considered as another differential diagnosis for signs of abdominal pain after more common causes such as severe bloat, mesenteric root volvulus, intussusception, cecal dilatation/volvulus, and uterine torsion have been excluded.

RESUMO

An exploratory laparotomy of a four-year-old red Holstein cow with signs of abdominal discomfort revealed a painful pulsating mass in the cranial part of the abdomen. The cow died suddenly. A post mortem examination showed that a ruptured aneurysm of the cranial mesenteric artery had been the cause of death.
