RESUMO

CASE DESCRIPTION: A 9-month-old miniature Hereford heifer was evaluated for a mass on the right inferior eyelid that had progressed in size over 3 months. CLINICAL FINDINGS: Physical examination revealed a firm, ulcerated, pedunculated mass on the right inferior eyelid that extended from the medial quarter to beyond the lateral canthus of the eye. The base of the mass measured 7.4 × 6.7 cm, and the dorsal margin of the base of the mass was approximately 3 mm ventral to the inferior eyelid margin. Histologic evaluation of incisional biopsy specimens from the mass was consistent with fibrosarcoma. TREATMENT AND OUTCOME: The mass was surgically resected with care taken to preserve the eyelid margin. The resulting 10 × 8.5-cm surgical wound was treated with adjunct CO2 laser therapy and closed by primary closure at its medial and central aspects and placement of a 4.6 × 2.6-cm lyophilized equine amnion multilayer graft and 2 sheets of 4-ply porcine small intestinal submucosa at its lateral aspect. The grafts were kept moist by alternating topical antimicrobial and artificial tear ointments for 3 weeks. The wound healed without complications, resulting in a functional and aesthetically pleasing outcome despite the development of moderate ectropion at the lateral aspect of the inferior eyelid. CLINICAL RELEVANCE: Results suggested that a combination of extracellular matrix scaffolds may be an alternative to extensive skin flaps for management of large dermal wounds, particularly wounds resulting from blepharoplasty where preservation of an eyelid margin is desired.

A 3-year-old Thoroughbred filly presented to a referral equine hospital for surgical correction of a severe cicatricial lateral lower eyelid ectropion OD, with secondary exposure keratitis. The severity of the ectropion deemed that conventional ectropion repairs would be unsuccessful. Therefore, a soft tissue expansion device was used to create sufficient local tissue for a rotational graft with tension-relieving horizontal incisions to be performed to facilitate closure and acceptable eyelid apposition. The keratitis had resolved by two months postoperatively. There were no long-term complications and only mild recurrence of the keratitis observed 14-years postoperatively.

A 1-year-old Dutch Warmblood gelding was presented to the University of Florida's Large Animal Hospital (UF-LAH) for correction of ectropion of the right lower eyelid. The ectropion was the result of a lower eyelid laceration. A primary repair was performed by the referring veterinarian; however, the horse prematurely removed the sutures and the wound healed with inversion of the eyelid margin. Surgical correction of the entropion, with removal of tissue from the lower eyelid, resulted in cicatricial ectropion. During the initial evaluation at UF-LAH, a corneal ulcer was noted in the right eye because of exposure from the anatomically nonfunctional lower eyelid. A modified Kuhnt-Szymanowski procedure was performed to correct the ectropion and repair the eyelid margin. The surgical site healed appropriately with an acceptable cosmetic and functional result. No recurrences of corneal ulcers in the right eye were reported in the 18 months following surgical correction of the ectropion.

RESUMO

Two dogs were presented with severe dermatopathies leading to cicatricial ectropion and severe surface ocular pathology. In one case, idiopathic sterile pyogranuloma was diagnosed on histopathology and managed with systemic immunosuppressive therapy. In the second case, demodicosis was diagnosed on skin scrapings and successfully treated with acaricidal medication. Following control of the underlying dermatopathies, both cases developed cicatricial ectropion as a result of scarring and contracture of eyelid tissue. Wharton-Jones (V-Y) blepharoplasties were performed on the affected eyelids and resulted in improved eyelid function and reduced corneal exposure in both cases.

In this study we described four cases of bilateral ectropion in pigeons that were investigated in Greece. Anemia, leukocytosis, and increased levels of enzymes lactate dehydrogenase, aspartate aminotransferase, creatine phosphokinase, and total serum proteins were found. Chlamydial elementary bodies were observed by modified Ziehl-Neelsen stain in direct smears of conjunctiva, liver, and spleen as well as in yolk sac samples after egg inoculation with eyelid material. Histologically, significant hyperplasia of the conjunctival epithelium was observed. Using immunohistochemical methods, chlamydial antigen was revealed in eyelid, liver, and spleen paraffin sections. This study suggests that Chlamydia spp. was the causative agent that induced ectropion.

A three-month-old chow chow mixed-breed puppy was presented with severe cicatricial ectropion of the upper eyelids after being burned maliciously. The burn wounds healed by second intention with contracture, causing eversion, elevation, and immobilization of the upper eyelids. The puppy was unable to blink or close the upper eyelids due to cicatrix (i.e., scar) formation. Surgical repair using tissue-relaxing procedures was successful in reducing corneal exposure and improving the puppy's appearance.

RESUMO

An experimental study was made of 20 dogs in order to compare various surgical techniques used to correct eversion of the third eyelid, namely resection of most of the cartilage, resection of the central portion of the cartilage, and cartilage homotransplantation. An analysis was made of histological results obtained 45 days after operation, the most satisfactory result being recorded for homotransplantation of the third eyelid cartilage.
