RESUMO

OBJECTIVES: This study aims to evaluate the diagnostic value of complete blood count (CBC) parameters in patients with peripheral nerve sheath tumors (PNSTs). PATIENTS AND METHODS: A total of 181 patients (83 males, 98 females; median age: 44 years; range, 15 to 83 years) who underwent surgical treatment for PNSTs in our tertiary oncology center between January 2010 and December 2019 were retrospectively analyzed. Eighty-two patients were diagnosed with a neurofibroma, 79 with a schwannoma, and 20 with a malignant PNST (MPNST). The patient group was evaluated as malignant (n=20) and benign (n=161). Age- and sex-matched patients admitted to our outpatient clinic of orthopedic and traumatology with non-specific symptoms other than tumor, infection, fracture, and rheumatological or hematological diseases were included as the control group (n=165). Data including age, sex, definitive histopathological diagnosis, and pre-treatment CBC values were obtained from the hospital records. Pre-treatment CBC values such as neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), and lymphocyte-to-monocyte ratio (LMR) were calculated for both malignant and benign groups and control groups. Diagnostic values of NLR, PLR, and LMR between PNST groups were assessed using the receiver operating characteristic (ROC) curve analysis. RESULTS: Neurofibroma, schwannoma, and MNPST groups had significantly higher median NLR, compared to the control group (p<0.001), while the median LMR was significantly lower in these groups (p<0.05). However, the median PLR was higher only in the MPNST group, compared to the control group (p<0.001). Post-hoc analyses revealed that median NLR, PLR, and LMR ratios were similar in PNST groups, compared to the control group. In addition, the median NLR, PLR, and LMR ratios were similar between malignant and benign patient groups. The highest area under the curve (AUC) was found for NLR (AUC=0.756) and LMR (AUC=0.716) in the MPNST group. CONCLUSION: Our study results suggest that NLR, PLR, and LMR may have an added value in the early diagnosis of PNSTs and are valuable for differentiating patients from healthy individuals, although their value in differential diagnosis is still unclear.

Neurofibromatosis type 1 (NF1) is an autosomal dominant syndrome whose characteristic manifestations include benign neurofibromas, yet NF1 is also associated with a high risk of cancer. Measurements of circulating free plasma DNA (cfDNA) are gaining wider applicability in cancer diagnostics, targeting of therapy, and monitoring of therapeutic response. Individuals with NF1 are likely to be followed up using this method, but the effects of NF1 and neurofibromas on cfDNA levels are not known. We studied peripheral blood samples from 19 adults with NF1 and 12 healthy controls. The cfDNA was isolated from plasma with QIAamp Circulating Nucleic Acid Kit and quantified using the Qubit 2.0 Fluorometer. The cfDNA concentration of each sample was normalized relative to the plasma protein concentration. The normalized median concentration of cfDNA in plasma was 19.3 ng/ml (range 6.6-78.6) among individuals with NF1 and 15.9 ng/ml (range 4.8-47.0) among controls (p = .369). Individuals with NF1 who also had plexiform neurofibroma (pNF) showed non-significantly elevated cfDNA concentration compared to individuals with NF1 and without known pNF (median 25.4 vs. 18.8 ng/ml, p = .122). The effect of NF1 on cfDNA seems to be relatively small and NF1 is therefore unlikely to hamper the use of cfDNA-based assays.

The aim of this study was to discover the clinical value of the tumour marker AMDL DR-70 in a group of patients with cancer of the tongue. Serum concentrations of AMDL DR-70 were estimated by enzyme linked immuno-sorbent assay in 52 patients with carcinoma of the tongue and compared with 40 controls and 42 patients with benign lesions in the tongue. Thirty-nine patients with carcinoma of the tongue had results above 6 mg/L (75%), compared with 3/40 (7%) in healthy controls and 4/42 (10%) in those with benign tumours. The concentration of AMDL DR-70 in serum correlated significantly with 3-year survival.

An unusual population of leukocytes was observed in the peripheral blood of a cow with a large tumor burden, using flow microfluorimetry. This new population accounted for 50% of the total cells in the peripheral blood of this animal. These cells expressed the p150,95 molecule (bovine CD11c equivalent), identified by the monoclonal antibody C5B6, a molecule found on myeloid cells and activated lymphocytes. The new population did not express the pan T molecules BoCD2 (the bovine T11 equivalent), BoCD5 (the bovine CD5 equivalent) or surface IgM. Isolated peripheral blood mononuclear cells maintained in bulk culture were able to kill autologous tumor cells and BHV-1 infected A549 in an NK-like assay. In vitro cytotoxicity by cells cultured from the peripheral blood of this animal was augmented 2- to 4-fold by the addition of IL-2.

Through the analyses of 22 cases of neurofibromatosis (NF) patient ranging from 3 months to 63 yrs. old. We found that 4 cases had bilateral hearing loss and 2 of them were excluded from acoustic neuroma. The ocular appearance showed lid neurofibroma in 12 cases, orbital bone abnormalities in 4 cases, iris nodules in 20 cases (90%), congenital glaucoma (4 eyes) in 3 cases which showed abnormal gonioscopy in 3 eyes, lens opacity in 2 cases, mild and moderate myopia in 8 eyes, and high myopia in 7 eyes, occupying 16% of the total. 11 cases belong to 5 pedigrees showing autosomal dominant inheritance. The ABO and MN blood grouping of the NF cases and families in the pedigree showed that NF was more related to ABO than MN blood group. The ocular appearance, especially the shape and number of iris nodules in relation to age and diagnosis is discussed. The significance of 1 case of retinitis pigmentosa, 7 cases of high myopia in relation with NF should be further studied.

RESUMO

Under observation were 140 children with retroperitoneal neurogenic tumors. In 69 of 125 patients with malignant neoplasms metastases were detected. Hemographic and myelographic findings were analysed depending on the degree of malignancy and tumor spread. Anemia usually moderate and normochromatic was observed only in neuroblastomas. This group of patients is also characterized by frequent shifts to the left leucograms and high sedimentation test, contrary to benigh tumors. The investigation of bone marrow allows one to reveal the suppression, and more rarely irritation of various blood stems.
