RESUMO

Light-based technologies are applied in various fields of medicine: for example optical diagnostics, light-activated therapy and surgery. Although light-based surgical procedures had hardly been a novelty, the revolutionizing moment for surgery came with the first use of light to cut tissue. Nowadays, surgical lasers are routinely used across numerous medical specialties, including gynaecology and urology. They are a part of the surgical treatment of benign prostatic hyperplasia, prostate carcinoma, penis carcinoma, genital skin lesions and orchidectomy. While in human urology lasers continue to establish their position as one of the standard surgical tools, veterinary patients are rarely treated with what here is still considered a technical innovation. However, through research on laser treatment of the prostate hyperplasia conducted on a canine model, veterinary medicine has gained a massive portion of data. It may prove beneficial for our clinical patients. In this review, we introduce the very principles of laser surgery as well as its current and future applications in oncologic surgery of the canine prostate gland and the male reproductive system.

OBJECTIVE: To describe the use of a caudal superficial epigastric flap in combination with a full-thickness oral mucosal/submucosal graft for single-stage reconstruction of extensive preputial defects in dogs. MATERIALS AND METHODS: Medical records of dogs with extensive preputial defects either of traumatic origin or derived from tumour excision were reviewed. In all dogs, the prepuce was reconstructed using a full-thickness oral mucosal/submucosal graft combined with a caudal superficial epigastric axial pattern flap during a single surgical procedure. Outcome was assessed by routine clinical examinations for 6 months postoperatively, and through telephone follow-up thereafter. RESULTS: Six dogs were included. The caudal superficial epigastric axial pattern flap healed without complications in all dogs, while the full-thickness oral mucosal/submucosal graft failed in one dog. In this individual the skin flap underwent contracture 30 days after surgery and preputial advancement was required. One dog showed postoperative discomfort during urination, which was successfully managed with a Foley catheter and analgesic administration. Three dogs developed paraphimosis at 30, 80 and 90 days, respectively, and required further surgery. Long-term results were good in all dogs. CLINICAL SIGNIFICANCE: The use of a full-thickness oral mucosal/submucosal graft combined with a caudal superficial epigastric axial pattern flap is feasible for single-stage preputial reconstruction in dogs. Attention should be paid to create a sufficiently large preputial opening, in order to prevent paraphimosis.

INTRODUCTION: Within the veterinary world, data regarding the surgical management of hypospadias is lacking. Reports within equines have documented resective phallectomy procedures rather than urethral reconstruction. This case report documents the first ever urethroplasty for an equine hypospadias, performed by a consultant paediatric surgeon. The urethroplasty was achieved by applying the same surgical principles mastered from paediatric urology to a horse. The indication for surgery was contact dermatitis of the hind-leg, which impaired the thoroughbred foal's racing potential. OPERATION: A single stage urethroplasty was performed, during which the wide urethral plate was tubularised in two layers and the urethral meatus was successfully relocated distally to open upon the glans. The urethroplasty was covered with dartos fascia and the penile shaft skin and prepuce were reconstructed. Post-operatively, urine flowed cranially and the contact dermatitis resolved. Minor superficial wound dehiscence was observed which was managed conservatively and self-resolved. Follow-up at 3 years confirmed the horse continues to void from the re-sited meatus without complication and went on to have an illustrious career within horseracing. CONCLUSION: We have documented the first reconstructive urethroplasty for hypospadias within a horse, achieved by adopting the surgical techniques normally implemented within paediatric urology with good functionality.

RESUMO

Reproductive surgical techniques are considered by practitioners of theriogenology to be the best method to manage infertility-causing conditions or diseases of the bull. Injury or diseases of the reproductive tract may cause abnormalities that may result in substantial losses to the producers of beef and dairy cattle. The most cost-effective method of dealing with reproductive conditions or diseases of the bull is culling and replacement. Some injuries, diseases, or conditions are amenable to surgical management. Surgical management may preserve the genetic potential of the bull and economic productivity. The surgical procedures described are commonly used methods to restore fertility in bulls.

A pubectomy was carried out to relieve obstruction of the pelvic canal in a 6-year-old dog diagnosed with sacral osteosarcoma. Two days after surgery, the dog was ambulatory with normal urination and defecation. Pubectomy is a viable option to relieve clinical signs in patients with pelvic canal obstruction due to a non-resectable tumor.


Pubectomie et radiothérapie stéréotactique pour le traitement d'un ostéosarcome sacral non réséquable causant un blocage du canal pelvien chez un chien. Une pubectomie a été réalisée afin de soulager un blocage du canal pelvien chez une chienne âgée de 6 ans avec un diagnostic d'ostéosarcome sacral. Deux jours après la chirurgie, la chienne était ambulatoire et avait une miction et une défécation normales. La pubectomie représente une option viable pour soulager les signes cliniques chez les patients ayant un blocage pelvien causé par une tumeur non réséquable.(Traduit par Isabelle Vallières).

Medical and surgical management can be used to restore a bull that has suffered a reproductive tract malady. The economic cost of treatment weighed against the bull's replacement value as well as prognosis for recovery is of prime consideration. In turn, early recognition of a treatable condition and immediate initiation of action are factors that impact both treatment cost and prognosis in many cases. Common problems are penile hair rings, fibropapillomas, vesicular adenitis, penile hematoma, and traumatic injury to the prepuce. Less frequent problems are injuries that lead to denervation of the penis, penile shunts, and penile deviation.

The objective of this study was to evaluate the clinical usefulness, in terms of analgesic efficacy and safety, of ultrasound-guided pudendal nerve block performed with bupivacaine in cats undergoing perineal urethrostomy. Eighteen client-owned male cats scheduled for perineal urethrostomy were enrolled in the study and assigned to one of two treatment groups. The pudendal nerve block was performed under general anaesthesia, as described elsewhere, with 0.3 ml/kg of either saline (group C) or 0.5% bupivacaine (group B) - the total injection volume being split equally between the two sites of injection (left and right). Intra-operatively, assessment of nociception was based on the rescue analgesics requirement, as well as on the evaluation of changes in physiological parameters in comparison with the baseline values. Postoperative pain assessment was performed using three different pain scales at recovery and then 1, 2 and 3 h after recovery. Cats in group B showed lower heart rates and required fewer analgesics during surgery than group C. Postoperatively, group B had lower pain scores and needed less rescue buprenorphine than group C. Iatrogenic block-related complications were not observed. In conclusion, the ultrasound-guided pudendal nerve block can be considered clinically useful in feline medicine as it provides reliable analgesia in cats undergoing perineal urethrostomy.

OBJECTIVE: To examine if the tension at the site of a urethral anastomosis can be relieved by performing either a crural release technique or an ischial osteotomy technique. STUDY DESIGN: Cadaveric study and 2 case reports. ANIMALS: Adult male cat cadavers (n = 18). METHODS: Cats were divided into 2 groups; crural release (n = 9) and ischial osteotomy (n = 9). In each group, 20%, 25%, and 30% of the pelvic urethra was excised in 3 cats. The length of the urethral defect was measured after excision of the urethral segment, and after approximation, before and subsequent to the tension relieving technique performed. Two clinical cases are described. RESULTS: Both crural release and ischial osteotomy were effective in relieving the tension encountered at the urethral anastomosis after removal of 20% of the urethral length. In the ischial osteotomy group, apposition without tension after removing up to 30% of the intrapelvic urethral length was easily achieved. A similar technique was successfully used in 2 clinical cases. CONCLUSION: Crural release and ischial osteotomy techniques allow approximation and tension free anastomosis of large segmental defects of the pelvic urethra in cats.

OBJECTIVE: To report a modified procedure for perineal urethrostomy (PU) in goats. STUDY DESIGN: Case series. ANIMALS: Adult castrated male pet goats (n = 11) that had recurrence of urinary tract obstruction after one or more surgical procedures for obstructive urolithiasis. METHODS: Medical records (May 2008-February 2011) of goats that had a modified proximal PU were reviewed for history, signalment, and intraoperative and postoperative complications. Follow up was obtained through clinical examination or by telephone interview of owners or referring veterinarians. RESULTS: Ten goats survived to discharge. Postoperative complications included hemorrhage (n = 7), misdirected urine stream (3), obstructive urolithiasis (2), and bladder atony, dysuria, dehiscence, or delayed healing (1 each). One goat died acutely 15 hours after surgery from peritonitis, pleuritis, hemorrhagic cystitis, and hepatic lipidosis, and 1 goat was euthanatized 14 months after surgery because of renal failure and persistent cystitis. Urethrostomy sites were patent and functional in 9 goats available for long-term follow-up (>12 months). CONCLUSIONS: Transection of penile body attachments from the pelvis and careful mucocutaneous apposition may decrease the risk of postoperative urethral stricture formation in goats after PU. Hemorrhage is common after the procedure but does not affect clinical outcome.

OBJECTIVE: To report a technique for improving surgical access to the bladder for removal of cystic calculi in male horses. STUDY DESIGN: Retrospective case series. ANIMALS OR SAMPLE POPULATION: Geldings (8) with cystic calculi. METHODS: After catheterization of the bladder through the penis, 100 mL 2% lidocaine hydrochloride solution was instilled. After 10 minutes, the bladder was distended with sterile, warmed Hartmann's solution to a pressure of ≈ 40 cm H(2) O, using gravity feed. This was left in place until abdominal access was gained at surgery, then the fluid siphoned off via the catheter. RESULTS: Calculi were 3-11 cm in diameter (median, 6 cm). Bladder capacity ranged from 1.4 to 2.5 L (median, 1.8 L). Exteriorization time from placing a hand in the abdomen to having the bladder in a surgically accessible position was <5 minutes. CONCLUSION: The described technique facilitates exteriorization of the bladder for removal of cystic calculi.

RESUMO

A three-and-a-half-year-old male neutered Siamese cat presented with idiopathic feline lower urinary tract inflammation and dysuria, which appeared to be caused by stricture of the urethral meatus. Wedge meatoplasty was performed, which relieved the cat's dysuria and restored a normal urine stream. To my knowledge, this is the first report of meatoplasty as a treatment for stricture of the urethral meatus in a cat.

Bilateral enlargement of both epididymes was observed in a 6-year-old German shepherd dog following a pre-scrotal urethrostomy. Testicular parenchyma showed regular structure, and the spermatogenesis and the steroidogenic functions were not modified. However, macroscopic examination of the tail and the body of both epididymes exhibited multiple white and well-delimited foci. Histopathological study of the epididymes confirmed the development of granulomas associated with extravasated spermatozoa. Urethrostomy caused a severe stenosis of the penile urethra, favouring the retention of urine at the urinary bladder. The retrograde pressure exerted by the distension of the urinary bladder could have allowed the urine to reach the prostatic urethra and the deferent ducts and, finally, the epididymes, causing irritation and rupture of the mucous layer of the epididymal duct, the consequent sperm extravasation and the development of sperm granulomas. We speculate that the inadequate surgical resolution of the urethral calculi caused the bladder distension, the subsequent retrograde flow of urine and the development of the lesions.

A 4-yr-old, 300-kg male Bongo antelope presented a fluctuant swelling on the ventral abdomen surrounding the penis, which was confirmed later as compatible with urethral rupture. Initial treatment included broad-spectrum antibiotic and radical surgical debridement. Total prepuce and partial penile amputation with orchidectomy was performed and normal micturition was restored. At a later stage, when the extent of the lesion was controlled, a permanent scrotal urethrostomy was performed. Complete healing by second intention was achieved 2 mo after the last surgery. Although the outcome was very good, considering the extent and severity of the tissue damage, prompt permanent urethrostomy when urethral rupture was first suspected would have prevented deterioration of the condition and prevented the need for extensive surgery.

CASE DESCRIPTION: A 1-year-old castrated male German Shepherd Dog was evaluated because of a history of hematuria and stranguria secondary to recurrent urinary tract infections. CLINICAL FINDINGS: Physical examination revealed hypospadias with penile and preputial aplasia. The urethral orifice was just ventral to the ventral aspect of the anocutaneous junction. Ascending urinary tract infections, secondary to fecal contamination of the urethral orifice, were the presumed source of recurrent bouts of cystitis that developed despite periodic antimicrobial treatment. TREATMENT AND OUTCOME: A 1-cm-diameter urethral extension was constructed from the urethral mucosal remnant located along the midline of the perineum (urethral trough). Two parallel 4-cm incisions (3 cm apart) were made lateral to that urethral trough. The borders were sutured to form an inverted, epithelium-lined tube (bipedicled flap) attached to the dorsal urethral orifice. The lateral skin margins were sutured over the reconstructed urethral extension, completing the procedure. Postoperative swelling necessitated temporary catheterization of the urinary bladder. After closure of a small fistula from the reconstructed urethral segment, the dog subsequently had only 2 episodes of cystitis during a 3-year period. To minimize skin irritation secondary to urine exposure, the dog's owner regularly trimmed the hair around the new urethral orifice. CLINICAL RELEVANCE: In dogs, correction of perineal (subanal) hypospadias via urethral reconstruction should be considered among treatment options. By use of an inverse tubed urethral extension, direct fecal contamination to the lower urinary tract may be effectively eliminated, dramatically reducing the incidence of ascending urinary tract infections in dogs with hypospadias.

Dezoito novilhos mestiços, com idade entre 16 e 20 meses, foram submetidos à remoção do ligamento apical do pênis para provocar o seu desvio. Decorridos 60 dias os animais foram colocados em presença de fêmeas em estro, sendo verificado formação de desvios ventral e lateral direito do pênis, com incapacidade de cópula. Os bovinos foram submetidos ao transplante autólogo de um segmento do tendão da porção superficial do músculo flexor superficial dos dedos, sobre a túnica albugínea, substituindo o ligamento apical do pênis removido. Observou-se a presença de poucas células mononucleares, polimorfonucleares e células gigantes e formação de tecido conjuntivo fibroso, ocasionando aderências resistentes e capacidade de sustentar a extremidade livre do pênis. Os bovinos, quando em presença de fêmeas em estro, não apresentaram desvio de pênis e modificações do comportamento sexual. A técnica mostrou ser eficaz, podendo ser indicada para corrigir desvio de pênis ventral e lateral de bovinos.


In this experiment, 18 hybrid steers, from the ages between 16 and 20 months, were submitted to the removal of the penis apical ligament to provoke a penis deviation. After passed 30 days, to be tested, the animals were put together to the females that were in heat. It was verified the formation of the ventral and right lateral penis deviation with incapacity of the copula. The bovines were submitted to the autologous implant from the one following superficial portion tendon from the superficial flexor fingers muscle, on the top of albuginea tunic, replacing the apical penis ligament removed. The material implanted was well tolerated, appearance a few inflammatory cells and formation of the fibrous conjunctive tissue, causing resistant adherence and capacity of sustaining the free penis extremity. The bovines, when were among the females that were in heat, did not show a penis deviation nor sexual behavior modification. The method revealed itself satisfactory and it can be indicated to correct ventral and right lateral penis deviation in bovines.

A chimpanzee (Pan troglodytes) with traumatic loss of the distal penis developed a gradually enlarging ventral urethral swelling and progressive dysuria. Endoscopy identified a urethral diverticulum, and endoscopic resection of the diverticulum wall was performed. Postoperative infection caused extensive necrosis of the penis, which necessitated a perineal urethrostomy. Complications of the urethrostomy included urethral obstruction from recurrent urethral stricture. The stricture was managed by regular dilatation using urethral bougies. Because of considerable postoperative swelling, catheterization was required to allow micturition following both the diverticular resection and polyp debulking. A shortened catheter sutured to the skin was tolerated for up to 10 days. Four yr after the urethrostomy, the animal is healthy and asymptomatic with endoscopic examinations performed at 12 mo intervals. This case demonstrates that with appropriate aftercare, perineal urethrostomy is an effective technique in the treatment of chronic distal urethral obstruction in the chimpanzee and probably other primate species.

A próstata é a única glândula sexual nos cães e, embora seja encontrada em todos os mamíferos, sua importância clínica é maior no homem e nesta espécie animal devido à quantidade de afecções que os acometem. Diversas técnicas cirúrgicas têm sido utilizadas para o tratamento de cistos e abscessos prostáticos em cães, e há alguns anos foi relatado o primeiro uso da técnica de omentalização prostática para o tratamento de cistos e abscessos, com sucesso efetivo, e até o momento, não há informações de seu emprego no Brasil. Sendo assim, este trabalho teve como objetivo avaliar a recuperação e período pós-operatório de animais submetidos a esta técnica, durante período de 2002 a 2004. Foram estudados 17 machos, sendo 11 com cistos prostáticos, 4 com abscesso e 2 com cisto paraprostático. Quinze se recuperaram sem complicações, enquanto um apresentou incontinência urinária por dois dias após a cirurgia. Um animal veio a óbito em decorrência de septicemia preexistente. A baixa incidência de complicações e o curto período de hospitalização fazem da omentalização a cirurgia de escolha para o tratamento de abscessos e cistos prostáticos em cães.


The prostate is the only acessory sex gland in the male dog and, although the prostate is found in all mammals, it has a great importance in men and dogs due to the frequency of disorders. Several techniques have been employed for the treatment of prostatic cysts and abscesses and, a few years ago, it was successfully described the use of prostatic omentalisation for this purpose; until now, there are no data about this surgery in Brazil. So, the aim of this study was to evaluate the animal recovery, as well as the post operative period of dogs that underwent prostatic omentalisation, from 2002 to 2004. It was evaluated 11 dogs with prostatic cysts, 4 dogs with prostatic abscesses and 2 with paraprostatic cysts. Fifteen dogs had an uneventful recovery while one dog had a minor incontinence for two days. One dog died due to a previous long term sepsis. The low incidence of post operative complications and brief hospitalization period make omentalisation the surgery of choice for the treatment of prostatic cysts and abscess.

OBJECTIVE: To describe the use of buccal mucosa as a urethral graft for reversal of perineal urethrostomy (PU) in a castrated male goat. STUDY DESIGN: Clinical case report. SAMPLE POPULATION: One client-owned castrated male goat. RESULTS: An 8-month-old Pygmy cross wether was admitted for urolithiasis and PU stricture. After repeated unsuccessful attempts at urethrostomy reversal and urethral reconstruction, a buccal mucosal graft was used for urethroplasty. Buccal mucosa proved to be an excellent graft for urethral repair. Normal urination has occurred through the reconstructed region for at least 24 months. CONCLUSIONS: Buccal mucosa worked well as a graft for urethroplasty and PU reversal. CLINICAL RELEVANCE: Grafting with buccal mucosa may also prove useful in other urethral repair techniques in small ruminants.

OBJECTIVE: To report a surgical technique for penile amputation and sheath ablation in horses. Study Design-Retrospective study. Animals or Sample Population-Twenty-five adult geldings of various breeds with conditions requiring penile amputation and sheath ablation. METHODS: The medical records of horses that had penile amputation and sheath ablation were reviewed. Briefly, the technique involved en bloc resection of the skin and subcutaneous tissues of the inner and outer lamina of the prepuce and penile shaft. Resection of the penile shaft, proximal to diseased segments, and spatulation of the urethral mucosa was followed by fixation of the penis to the ventral abdomen and closure of the peripreputial skin over the ventral aspect of the penis and creation of a new mucocutaneous junction at the urethral orifice. RESULTS: Mild to moderate postoperative hemorrhage occurred in all horses. Partial dehiscence of the mucocutaneous junction occurred in 8 horses; all healed by second intention. Postoperative pain lasting 24 to 48 hours occurred in 7 horses; 1 required epidural analgesia for pain relief. Long-term follow-up was available for 8 horses; none had any long-term complications. CLINICAL RELEVANCE: Partial amputation of the penis with pexy to the ventral abdomen and revision of the peripreputial skin can be used as an alternative to en bloc resection and penile retroversion for lesions involving the free part of the penis and the prepuce. It requires smaller incisions and results in less alteration to the normal appearance of the external genitalia than retroversion techniques, while still allowing the surgeon to remove extensive portions of the penis and extirpate the regional lymph nodes.
