RESUMO

We performed a case-control study to investigate the urinary steroid excretions in ovarian cancer patients. The use of gas-liquid chromatography permitted the estimation of 14 urinary steroids. Data on 139 urine specimens from 36 cancer patients were compared with those on age-matched controls. A general depression of adreno-corticosteroid excretions was consistently observed in the cancer patients as compared with the control women. The possible role of observed steroidal disorders in the genesis of ovarian neoplasia is discussed in light of the epidemiological and biological aspects of this tumor.

Clinical-hormone studied were done on 34 patients with germinal tumors of the testis. The gonadotropin titres varied from normal to highly elevated values. Only in 4 cases (11.77%), however, were the gonadotropin titres above 10,000 mouse units/24 hours. These 4 were among the 9 cases (26.47%) with positive pregnancy tests. Gynaecomastia was present in 4 patients (11.77%). Total estrogens were assayed in these 4 cases and were found to be high. Gynaecomastia is attributed to excess estrogens. Elevated gonadotropin titres were found predominantly in cases of embryonal carcinoma, teratocarcinoma and choriocarcinoma, as well as in the most advanced clinical stages. As regards the correlation between gonadotropin titres and course of the tumor, it was found that the higher the titre the worse the prognosis. The results suggest that hormonal assays are of value in the clinical follow-up of testicular germinal tumors and should therefore be carried out in association with other clinical parameters.

Serial samples of plasma and serum were collected in the morning and afternoon from cancer patients, along with 24-h urine specimens. Values for serum and plasma samples taken at the same time from the same patient differed little, suggesting that either procedure is acceptable for polyamine analysis. Increases in concentrations of putrescine and spermidine in serum and plasma correlate well with such increases in 24-h urine samples. Spermidine concentrations in sera were consistently about 10-fold lower than corresponding urine values. Putrescine concentrations were 10- to 100-fold different. The variation in putrescine values may be due to its more active metabolism by diamine oxidase, an enzyme known to be present in serum. Concentrations of polyamines in serum and urine increased in response to effective chemotherapy.

RESUMO

A case of intrasellar teratoma with a germinal structure in a 10-year-old girl is described. A few months after intracranial surgery the tumor differentiated into a choriocarcinoma and finally spread to multiple cerebral, pulmonary, and renal metastases. In the course of choriocarcinomatous evolution, very high urinary levels of luteinizing gonadotropin (HCG) developed, but there was no clinical or anatomical evidence of precocious puberty.

Ninety-three cases of germinal testicular tumors with urinary gonadotropin (HCG) values, estimated by biologic or radioimmunoassay techniques, were reviewed. Preoperative values of HCG and subsequent postoperative values were related to response to treatment and prognosis. Twenty-five patients (26.9 per cent) comprised the seminoma group, and 68 patients (73.1 per cent) the nonseminomatous groups of germinal neoplasms. High HCG values were observed in the nonseminomatous group pre- and postoperatively by both assay procedures. Most significant was the high HCG values postoperatively (p smaller than 0.05, x2 equals 5.21 and p smaller than 0.05, x2 equals 5.23) for the biologic assay and radioimmunoassay, respectively, with high HCG values being associated with a poor cumulative 24 per cent two-year survival rate. Urinary HCG assay is of prognostic value in the ongoing management of testicular neoplasms. In the future, serum HCG assays may be of additional benefit.

RESUMO

Normal ranges for urinary polyamines have been obtained. Non-malignant genitourinary tract disease is not associated with elevated urinary polyamines. A high correlation exists between the presence of active urologic cancer and elevated urinary excretion of polyamines. Post-treatment polyamines correlate with the continued presence or absence of genitourinary tract malignancy.

RESUMO

Systematic investigations of the chorionic gonadotropin excretion were performed in 79 patients with testicular tumours. The influence of the hormonal activity in diagnosis and prognosis of metastatic testicular tumours was investigated. It was discovered, that the part of hormonal active testicular tumours is relatively small and that the evidence of the chorionic gonadotropin excretion is only possible in a few cases before the clinical and radiological evidence of metastases. The hormonal activity of testicular tumours is without influence on the survival time of tumour carriers.
