RESUMO
A correção de má oclusão Classe II de Angle(1) sem extrações dentais normalmente requer a distalização dos molares. Os aparelhos distalizadores intraorais, tiveram seu inicio nos anos 80, com o Distal-jet(2), Jones-jig(3), molas de NiTi(4), Pendulum/Pendex(5), para distalizar os molares sem o mínimo de cooperação dos pacientes e com estética. Os aparelhos distalizadores intraorais foram desenvolvidos para introduzir uma força contínua aos molares superiores. E toda ação para mover os molares distalmente, terá uma reação de mesma intensidade sobre os dentes de ancoragem. Como consequência, se os pré-molares ou incisivos forem os dentes de ancoragem, eles mesializarão e os incisivos vestibularizarão, aumentando a sobremordida. Esse efeito é contra indicado na correção da Classe II. Então, atualmente, os implantes ósseos estão sendo usados como mecanismo de ancoragem absoluta. O caso a seguir será tratado com aparelho distalizador Pendex, Ortodontia Lingual e miniimplantes.
ABSTRACT
Treatment of Angle(1) Class II malocclusion without extration usually requires distalization of maxillary molars. Beginning in the 1980s, intraoral appliances, such as repelling Distal-jet(2), Jones-jig(3), superelastic NiTi coil springs(4), Pendulum/Pendex(5), and have been introduced to distalize molars with minimal patient compliance and a good aesthetic. Intraoral distalization appliances have been designed to deliver a continuous reciprocal force on the maxillary first molars. Any action to move molars distally produces a mesial reaction force on the anchoring teeth. As a consequence, if the premolars or incisors (or both) are the anchoring teeth, they move mesially, the incisors protrude, and overjet increases. However, this effect is in contradiction with the main objective of Class II treatment. Then, nowadays, microscrew implants are using like rigid anchorage mecanisms. The following case report is highlighting the use of microscrew implants as an anchorage aid for the Pendex in lingual orthodontics.
