Resumo

Background: Malignant pleural mesothelioma (MPM) is a neoplasm with low incidence in small animals, and the possible causes are poorly elucidated but may be related to contact with asbestos. In the thoracic cavity, MMP can be localized or generalized to all cavity structures, and its clinical signs depend on this localization. Although some alternative therapies are being discussed, few studies are conclusive, with surgical intervention as the leading therapeutic option. Given this context, this report aimed to describe a case of MMP located in the mediastinum of a bitch treated with radical excision through mediated sternotomy. Case: A 7-year-old bitch of the Shar-pei breed was referred for care due to progressive weight loss and intense dyspnea. During the physical examination, dyspnea and muffled lung sounds were noted. The patient underwent hemodialysis, which showed neutrophilic leukocytosis. An abdominal ultrasound was also performed and revealed mild abdominal effusion, and chest radiography revealed an extensive tumor covering the entire chest cavity. Thoracocentesis was performed, and the material analyzed was a malignant exudate; the patient was referred to median sternotomy for exploratory purposes, and afterward, total macroscopic extirpation of the tumor was performed. A sample was sent for histopathology, and malignant mesothelioma was confirmed. The patient was discharged after 8 days of hospitalization with home treatment and did not return to the hospital. Upon contacting the guardian, we were informed that the animal had died 154 days after the procedure due to unknown causes. Discussion: Malignant pleural mesothelioma affects humans and animals; it is associated with the risk factor of contact with asbestos and the use of flea antiparasitic drugs. In small animals, its incidence is rare, albeit mesotheliomas have been reported in wild and large animals. The clinical signs are related to the location of the neoplasm. When it is located in the thoracic region, dyspnea, muffled lung sounds, cyanosis, and pleural effusion are observed in most cases. Diagnosis is usually late and incidental, although some tests, such as ultrasonography, magnetic resonance imaging, tomography, radiography, and needle biopsy, can help in the diagnosis. Histopathology is the exam of choice for definitive diagnosis, as it helps one observe the proliferation of neoplastic mesothelial cells, atypical mitosis figures, and marked cellular pleomorphism. Many therapeutic options have been discussed, including chemotherapy, immunotherapy, and anti-tumor immunization, although there is little scientific proof of their efficacy in animals. The current treatment of choice is tumor excision by surgical procedure with a palliative objective since the prognosis of the disease is unfavorable. Minimally invasive video surgery has been gaining more and more space in veterinary medicine and has proven successful in numerous cases of thoracic masses. In the present report, we chose to perform median sternotomy for total excision due to the extension of the mass that occupied the thoracic cavity practically in its entirety. Further research should be conducted to help in palliative treatments and increase the survival of patients with mesotheliomas, given that most studies are done in humans and not animals. We conclude that median sternotomy is still the therapeutic option of choice for the palliative treatment of patients with extensive thoracic pleural mesotheliomas.

Background: Cenurosis is a parasitic disease caused by Coenurus cerebralis, an intermediate form of Taenia multiceps multiceps, causing a fatal disease in production animals. Its adult form (Taenia) lodges in the small intestine of canids and can infect several intermediate hosts. Sheep are the main species affected by the disease, having nervous symptoms as one of its manifestations. This study aimed to describe the changes observed in computed tomography, as well as the clinical findings of a case of Cenurosis in a sheep on the western border of Rio Grande do Sul. Case: A ram was referred to the Centro Universitário da Região da Campanha (URCAMP) with neurological signs. Clinical, hematological, radiographic, tomographic and necroscopic evaluation of the animal was performed. There were no significant hematological and radiographic changes. During the neurological examination, corneal opacity was found in the right eyeball, associated with a visual deficit in the same eye. Also, when stimulated to move, it was possible to observe ataxia with ambulation to the left side, with right lateral displacement of the head. The tomography showed a hypodense area of approximately 3 cm at the base of the brain, in the region of the thalamus and third ventricle. Macroscopically, a translucent spherical cyst with approximately 4 cm in diameter was observed, containing the protoescolex/scolices of Taenia multiceps in the same region. Discussion: Computed tomography identified the presence of an apparently circular volume of approximately 4 cm in diameter, causing ventricular dilatation. This evidence of ventricular dilation corroborates aspects described in the literature, which found bilateral dilation of the ventricles by compression promoted by a cyst in the 4th ventricle. As the location of the cyst was located at the base of the brain, in the region of the third ventricle, it would be difficult to perform the surgery in the treatment of this case. Signs of ataxia, walking movements, decreased reflexes, nystagmus, unilateral blindness and lateral decubitus are commonly observed. Reports described that the main neurological alterations observed in 20 sheep with cenurosis were postural deficit, locomotion alteration, visual deficit and behavioral alterations, with more than 50% of the animals showing at least one of these signs. The main neurological signs observed in the case described were changes in gait, with walking, blindness, lateral head deviation and ataxia. These signs guide the clinical diagnosis of cenurosis. At necropsy, a spherical translucent cyst with approximately 4 cm in diameter was observed, containing the protoescolex/scolices of Taenia multiceps, which extended dorsally from the third ventricle to the base of the thalamus, laterally displaced to the right side. Rostrocaudal the lesion extended rostrally across the entire length of the thalamus to the beginning of the caudate nucleus and caudally to the base of the caudal colliculus, causing compression of the adjacent parenchyma, enabling the diagnosis of cenurosis. Similar findings are described as Coenurus cerebralis. This lesion pattern macroscopically characterizes the parasite lesions. Thus, it is concluded that the visualization of a hypodense area in CNS regions of sheep with neurological signs allows the diagnosis of cenurosis and precise location of the lesion. The description of this case adds information so that other professionals in the field can be successful in diagnosing the disease.

Resumo

This case report presents a celioscopy procedure followed by partial salpingectomy (PS) in a pregnant bitch referred to elective video assisted ovariohysterectomy. Through celioscopy it was visualized the embryonic vesicles and PS was performed after conversion to minioceliotomy. The puppies were born by eutocic delivery without clinical alterations. The case suggested that short and regular pneumoperitoneum is not deleterious to dog fetus and video surgery could be used in pregnant bitches. PS allows the fetus development and promotes bitch neutering.


Este trabalho relata um caso de celioscopia seguido de salpingectomia parcial (SP) em fêmea canina gestante encaminhada à ovariohisterectomia eletiva videoassistida. Por celioscopia foram identificadas vesículas gestacionais e a SP foi aplicada após conversão para miniceliotomia. Não foram observadas alterações clínicas nos filhotes, os quais nasceram por parto eutócico. Sugere-se que o pneumoperitônio regular por tempo reduzido não seja deletério para fetos caninos e a videocirurgia pode ser aplicada em caninos gestantes. A SP permite a continuidade do desenvolvimento fetal e a esterilização de fêmeas caninas.

For ethical and organic reasons, surgical and analgesic techniques that minimize and adequately control pain should be studied. Ovariohysterectomy is the most common surgical technique in veterinary medicine and is performed in female dogs for elective or therapeutic purposes. Both conventional and minimally invasive surgical techniques are used in veterinary medicine; however, the minimally invasive laparoscopic technique is considered superior to the conventional technique because it is associated with a less painful and faster post-operative recovery. However, for various reasons, the laparoscopic technique is not yet widely used in veterinary medicine, and the conventional ovariohysterectomy is still the most common. The aim of this study, therefore, was to validate the efficacy of the combination of dipyrone, scopolamine, and meloxicam after conventional or two-port laparoscopic-assisted ovariohysterectomies in female dogs. Additionally, this study sought to assess recovery after the two surgical modalities while receiving the same analgesic protocol. Fourteen healthy adult female dogs were spayed and then evaluated using the Visual Analogue Scale, the Melbourne Scale, and the short form of the Glasgow Composite Measure Pain Scale for 72 hours after surgery. The analgesic protocol was efficient for pain control after both techniques, with minimal differences between the groups.


Devido a razões éticas e orgânicas, técnicas cirúrgicas e analgésicas que minimizam e controlam adequadamente a dor devem estudadas. A ovário-histerectomia é a técnica cirúrgica mais realizada em medicina veterinária, seja aplicada com fins eletivos ou terapêuticos. As técnicas cirúrgicas convencionais e minimamente invasivas são utilizadas em medicina veterinária, e as laparoscópicas são consideradas superiores, pois promovem menor dor e mais rápida recuperação pós-operatória. Embora esta modalidade cirúrgica seja considerada superior, por diferentes razões ainda não é tão difundida na medicina veterinária, sendo a técnica convencional de ovário-histerectomia a mais utilizada em cadelas. Por estas razões, o objetivo deste estudo foi validar a eficácia do protocolo dipirona, escopolamina e meloxicam em cadelas submetidas a ovário-histerectomia convencional ou videoassistida por dois portais, assim como avaliar a recuperação dos animais submetidos a duas modalidades cirúrgicas junto ao referido protocolo analgésico. Assim, 14 cadelas adultas saudáveis foram submetidas a ovário-histerectomia e avaliadas por meio da Escala Visual Analógica, Escala de Melbourne e Forma reduzida da Escala Composta de Glasgow até 72 horas após a realização do procedimento cirúrgico. O protocolo analgésico foi eficiente para controle da dor nos animais submetidos a ambas as técnicas cirúrgicas, e mínimas diferenças foram observadas entre os grupos.

Background: Ovariohysterectomy (OVH) is one of the most applied techniques in Veterinary Medicine. Despite some analgesic protocols being used after this procedure in order to control post operatory pain, there is a lack of knowledge of the efficacy after conventional and two port laparoscopic-assisted techniques in dog female. Although the combined formulation of metamizol and scopolamine is used, there is no knowledge about the efficacy and collateral effects in a 6 h regimen after OVH. The aim of this research was to evaluate the efficacy of metamizol and scopolamine to analgesia after two techniques of OVH in dog female. Materials, Methods & Results: Fifteen adults and healthy dog female were submitted to OVH, being eight to the conventional procedure and seven to video assisted technic with two ports. The patients received a combination of metamizol and scopolamine, 6-6 h during 48 h and were evaluated before surgery, 60 min after extubation and on 1 h interval during the f irst 8 h, and at 12, 18, 24, 36, 48 and 72 h after surgery. For this, the University of Melbourne Pain Scale (UMPS) and the short-form of the Glasgow Composite Measure Pain Scale (CMPS-SF) were applied by three evaluators blinded to surgical technic. Glucoses measurement was realized at 1, 6, 12, 24, 48 and 72 h after surgery. Analgesic rescue was not necessary for any animal. Punctuation of UMPS in GC decreases at 8 h and at 5 h, and to GV at 12 and 24 h. Difference between groups happened at 3 h with larger average to GV; at 48 h the punctuation in GC was lager. Considering CMPS-SF to CG, higher average was observed at 1 h and decreased at 2, 3, 8 and 18. At GV highest average points was verified at 1, 2 and 3 h and decreased at 5, 8 and 18 h. Correlation was strong and positive between each two evaluators. Animals of both groups had increased average...

Background: The lymphatic system plays an important role in the local control of neoplasms as well as in the transportation of tumor cells, which represents the most common pathway of metastasis. From the tumorigenesis, there is a remarkable enlargement of the tumor drainage network towards the regional and target lymph nodes. Since the mammary lymphatic system of animals with neoplasms as well as the demarcation of the pelvic and abdominal caudal chain are poorly studied, the aim of the present study is to report a case of iliac lymphadenectomy after intrauterine lymphatic dyeing in a dog with breast neoplasm, which allowed the identification of metastasis in the medial iliac lymph node.Case: A 7-year-old, 13.4 kg, non neutered, mixed breed, female dog was diagnosed with ulcerative neoplasm in the left caudal abdominal breast, which presented evolution of approximately 90 days, presenting ulceration for 10 days, according to the tutor. After a clinical evaluation, thoracic radiography and abdominal ultrasonography were requested, which did not present metastasis suggestive images. Therefore, left unilateral total mastectomy was indicated. Retro-umbilical celiotomy was performed for the administration of 0.5 mL of 1% intrauterine methylene blue (body of the uterus), which allowed lymphatic mapping and medial iliac lymphadenectomy. Subsequently, ovariohysterectomy was performed and after celiorrhaphy, axillary lymphadenectomy was performed, followed by left unilateral mastectomy and concomitant removal of the superficial inguinal lymph node. The patient had a prompt recovery and was discharged from hospital in six hours postoperatively. The excised material was referred for histopathological examination, which revealed the presence of metastasis of simple tubulopapillary carcinoma of the mammary gland in the left medial and superficial inguinal lymph nodes, while the axillary lymph node had no metastases.[...]

Background: Dioctophyme renale, also known as “giant kidney worm”, is a cosmopolitan parasite, which usually affects domestic and wildlife canids in wetlands. Nephrotomy and nephrectomy are the only treatment options. Laparoscopic nephrectomy for dioctophymosis was first performed in veterinary patients in the late 90’s. Canine pyometra is one of the most frequent surgical disorder in the small animal setting. The purpose of this study was to report a case of successful one-stage laparoscopic approach for nephrectomy and ovariohysterectomy in a bitch.Case: A female stray dog, rescued from a riverside community, was presenting hematuria. Abdominal echography revealed presence of Dioctophyme renale worms within the right renal parenchyma. Moreover, the uterus was moderately filled by anechoic content. The patient was clinically stable and undergone laparoscopy. A 10-mm trocar was established at the middle third of the right flank, followed by 12-mmHg CO2 insufflation. The telescope was inserted and a giant worm was seen free within the abdominal cavity. A second 10-mm port was inserted cranial and dorsally to the first one. The parasite was retrieved using a laparoscopic Kelly forceps. A third 5-mm trocar was inserted in a classic laparoscopic triangulation port positioning. Renal artery and vein were individually triple ligated using titanium clips and then transected. The right ovarian pedicle was sequentially coagulated and transected using laparoscopic bipolar forceps. The right kidney was then dissected and detached from the abdominal wall, followed by cauterization and transection of the left ovarian pedicle. The uterus was exposed out of the abdominal cavity through the 5-mm port access. Uterine body and vessels were double ligated using polyglactin 910, with transfixating ligatures, and then transected. The opening of the retrieval bag was exposed through the cranial port site incision.[...]

The telescope was inserted and a giant worm was seen free within the abdominal cavity. A second 10-mm port was inserted cranial and dorsally to the first one. The parasite was retrieved using a laparoscopic Kelly forceps. A third 5-mm trocar was inserted in a classic laparoscopic triangulation port positioning. Renal artery and vein were individually triple ligated using titanium clips and then transected. The right ovarian pedicle was sequentially coagulated and transected using laparoscopic bipolar forceps. The right kidney was then dissected and detached from the abdominal wall, followed by cauterization and transection of the left ovarian pedicle. The uterus was exposed out of the abdominal cavity through the 5-mm port access. Uterine body and vessels were double ligated using polyglactin 910, with transfixating ligatures, and then transected. Background: Perineal hernia may be unilateral or bilateral and results from weakening and separation of the pelvic diaphragm muscles, favoring the abdominal viscera herniation into the perineal subcutaneous. The factors described as possible etiology of this affection are prostate hyperplasia, senile muscular atrophy, myopathy and hormonal imbalances. Several herniorrhaphy techniques are available in literature presenting different rates of success leaving the question whether there is any technique that is predominantly better than others are. This case report aims to describe five cases of perineal hernia treated with four different techniques performed by four experienced surgeons.Cases: Case 1. Semitendinous muscle transposition: A 8-year-old intact male Shitzu (6 kg) was referred for the evaluation of a swelling in the right perineal region. The mass was soft and regressed into de pelvic cavity during palpation. Abdominal ultrasound and radiography were taken, which reveled perineal hernia and a mild prostate enlargement. The patient was prepared for hernia repair with semitendinous muscle transposition and orchiectomy. Case 2. Internal obturator muscle transposition: A 9-year-old intact male mixed breed dog (18kg) presenting a two-week long unilateral (right-side) perineral hernia. Ultrasound exam revealed a cystic prostate and adipose tissue as the hernia content. After clinical and image examination the patient was submitted to surgical repair using the internal obturator muscle transposition technique and orchiectomy. Case 3. When manifested, these signals refer to the peripheral or central nervous system and the most common include: vestibular disease, seizures, laryngeal paralysis, polyneuropathy and paralysis of the facial nerve. Several reports of neurological disorders associated with hypothyroidism are found in literature, basically international. In the national literature, however, there are few reports on the subject. Thus, the aim of this study was to report a case of facial paralysis associated with hypothyroidism in a dog. Case: A male canine, the boxer race, with 7-year-old were referred to the Veterinary Medical Teaching Hospital of the UFSM with a history of difficult water and food intake and asymmetry of the face for seven days. On neurological examination, the animal found itself alert and locomotion, postural reactions and segmental reflexes without changes. In the evaluation of the cranial nerves, there was a menace response absent the right side, however with preserved vision, palpebral and lip ptosis of the right side and reflection palpebral absent on the same side. Background: Meningioma is the most common brain tumor in dogs. These extra-axial tumors originate in one of the meninges, and arachnoid is the most common. Several retrospective studies of brain tumors are found in the international literature. However there are few researches in the national literature. The purpose of this study is to report twelve dogs with brain meningioma. The breed, gender, age, neurological signs, the brain location, the clinical evolution, the tumor classification and diagnostics tests were investigated. Cases: Twelve dogs were attended at Veterinary Hospital of Santa Maria University: six mixed-breeds; four Boxer; a Dachshund; and a Poodle. The age ranged from eight to 14 years, with average of ten years and nine months old. The clinical signs observed were generalized seizures (9/12); behavioral changes (6/12); walk in circle (5/12); swallowing difficulty (2/12 [16%]); hypermetria (2/12); central vestibular syndrome (1/12); and amaurosis (1/12). The presumptive diagnosis was brain neoplasm in all dogs. Six dogs were underwent symptomatic treatment with corticosteroids and anticonvulsants; two dogs were underwent corticosteroids; and four were euthanized without performing treatment. Background: Renal urolithiasis is the third most common disease of the urinary tract of dogs. In humans, staghorn lithiasis affects 1 to 1.5% of the population, often women over 50 years old. In veterinary medicine, this type of lithiasis has been little reported, and there are few descriptions of treatment, both in small and large animals. The objective of this work is to report a rare case of renal staghorn stones treated by laparoscopic nephrotomy. Case: A six-year-old female schnauzer with 6 kg body weight was evaluated clinically in order to spay. According to the owner, the animal showed polydipsia. Additional tests such as blood count, serum biochemistry, radiography and abdominal ultrasound were done, showing hematological examinations within the standards for the species. After the ultrasound we noticed an asymmetric kidneys with the right kidney with preserved cortical layer and hyperechoic line corticomedullary interface, with posterior acoustic shadow and left kidney with relative/corticomedullary differentiation and pelvis preserved without alteration; this evocative image of kidney stones in kidney right. In abdominal radiographs was possible to visualize radiopaque mass in the right kidney compatible with renal staghorn stones. Excretory urography was done, which demonstrated evident excretion of contrast material in the left kidney and the right kidney doubtful.[...]

Background: Melanoma is a primary ocular neoplasm that may affect the iris, ciliary body, the choroid coat and others components of the eye. The ocular melanomas have a low metastatic level in dogs when compared to cats. Old animals are more affected than the young ones and there is no breed predilection. A complete eye examination and ocular ultrasound will lead to the diagnosis that will be further confirmed through histopathology. Current treatment of choice is based on the size and the structures involved by the tumor that can range from local resection to orbital exenteration. The objective of this study is to report a case of iris melanoma involving the ciliary body and the choroid coat of a Poodle, emphasizing its clinical manifestation, diagnosis and treatment. Case: This study reports a case of an intact male canine Poodle, 15 years old, weighing 5 kg that was referred for care to the Veterinary Teaching Hospital of the Federal University of Santa Maria, Brazil. Background: Intervertebral disc disease is a usual neurological disorder in dogs, which the spinal cord is compressed by intervertebral disc extrusion or protrusion. Fibrocartilaginous embolic myelopathy isan acute infarction disease of spinal cord caused by fibrocartilaginous emboli histologically identical to the nucleus pulposus of intervertebral discs. The aim of this report is present two cases of fibrocartilaginous embolic myelopathy in dogs with intervertebral disc disease. Cases: Two dogs - dachshund breed - were referred to the Veterinary Medical Teaching Hospital of the UFSM. In the first case, the dog was three years old and the owner described complete inability to move the pelvic limbs. The neurological examination revealed paraplegia, postural reactions deficits, normal spinal reflexes and muscle tonus in the hind limbs, pain on palpation in T12-T11 region and loss of deep pain in hind limbs. In the other case, the dog was seven years old and the owner described partial inability to move four limbs. The neurological examination revealed non-ambulatory tetraparesis, postural reactions deficit, normal spinal reflexes in four limbs and pain on palpation in cervical spine. Background: Vascular ring anomaly is considered a hereditary change. It occurs when the fourth right aortic arch persists instead of the fourth left aortic arch, forming a vascular ring that compress the esophagus causing esophageal dilatation cranial to the compression site. Diagnosis is based upon anamnesis, physical examination, clinical signs and complementary exams (radiograph, complete blood count and biochemical tests). The clinical treatment is inadequate, since the dilatation persists and tends to progress causing irreversible tissue injury. Surgical correction is indicated and consists of the arterial ligament resection and correction of the esophageal stenosis. Case: The objective of the current study is to report two case of persistence of right aortic arch in two cats referred to the Veterinary Hospital of the Federal University of Santa Maria. On physical examination the patients were alert, in good nutritional status, adequately hydrated and presented normal colored mucous membranes. Each animal underwent radiography and esophagogram with barium sulphate. The exam demonstrated partial esophageal dilation cranial to the cardiac silhouette, suggestive of vascular ring anomaly.
