Purpose To compare Fructose-1,6-Bisphosphate (FBP) to Histidine-Tryptophan-Ketoglutarate (HTK) in liver preservation at cold ischemia. Methods Male rats (Sprague-Dawley: 280-340g) divided into three groups (n=7): Control; Fructose-1,6-bisphosphate (FBP); Histidine-Tryptophan-Ketoglutarate (HTK). Animals underwent laparotomy-thoracotomy for perfusion of livers with saline. Livers were removed and deposited into solutions. Mitochondria were isolated to determine State 3 (S3), State 4 (S4), Respiratory Control Ratio (RCR) and Swelling (S). Liver enzymes (AST, ALT, LDH) were determined in solution. At tissue, Malondialdehyde (MDA) and Nitrate (NOx) were determined. All parameters were analyzed at 0.6 and 24 hours of hypothermic preservation. Statistics analysis were made by Mann-Whitney test (p 0.05). Results Regarding ALT, there was a difference between FBP-6h/HTK-6h, lower in HTK. Regarding AST, there was a significant difference between FBP-24h/HTK-24h, lower in FBP. Regarding NOx, there was a difference between 0h and 6h, as well as 0h and 24h for both solutions. Regarding S3, there was a significant difference in 24h compared to Control-0h for both solutions, and a significant difference between FBP-6h/FBP-24h. Regarding S4, there was a difference between Control-0h/HTK-24h and FBP-24h/HTK-24h, higher in HTK. There was a difference between Control-0h/FBP-24h for Swelling, higher in FBP. Conclusion Fructose-1,6-Bisphosphate showed better performance at nitrate and aspartate aminotransferase compared to histidine-tryptophan-ketoglutarate.(AU)

Purpose To evaluate the effects of treatment with Indigo Carmine (IC) on rat livers subjected to ischemia-reperfusion injury. Methods The animals were subdivided into 4 groups: 1.SHAM group(SH) - saline; 2.SHAM group with IC-2mg/Kg(SHIC); 3.IR group - rats submitted to ischemia and reperfusion with saline(IR); 4.IR group with IC-2mg/Kg(IRIC). The IR protocol consists of liver exposure and administration of drug or saline intravenously, followed by 60 minutes of ischemia and 15 of reperfusion. Liver samples were collected for biochemical analysis. Results State 3 of mitochondrial respiration showed a significant worsening of the IRIC group in relation to all others. State 4 showed a difference between IRIC and SHIC. The Respiratory Control Ratio showed statistical decrease in IR and IRIC versus Sham. The osmotic swelling showed significant difference between SHxIR; SHICxIRIC and SHxIRIC. There was a significant increase in ALT in the IRIC group in relation to all the others. Concerning the nitrate dosage, there was a decrease in the group treated with IC(IRxIRIC). There was no difference regarding the dosage of Malondialdehyde. Conclusion IC was not able to protect mitochondria from IR injury and proved to be a potentiating agent, acting in synergy with the IR injury promoting damage to the hepatocyte membranes.(AU)

Early cancer diagnosis, new therapies that increased survival of patients, besides the increasingly elderly population are some factors would be associated with possible cancer dissemination in patients under cardiopulmonary bypass (CPB) cardiac surgery. Also, the benefits, and risks, regarding long-term survival, have not yet been established. Therefore, cardiac surgery morbimortality may be superior in patients with cancer disease. Also, immunologic and inflammatory changes secondary to CPB can also increase tumor recurrence. After a brief introduction and CPB immunologic the two main topic subjects included: 1) Combined heart surgery and lung resection and; 2) Possible influence of neoplasia type. After observing the relative literature scarcity, we keep the opinion that CPB has a modest association with cancer progression and that CPB and cancer dissemination should be a logical but unlikely association.(AU)

It is well known that during hepatic operative procedures, it is often critical that the irrigation is interrupted to avoid possible bleeding, blood transfusions, variable intensities, and their short and long-term consequences. It was believed in the past that the flow interruption should not exceed 20 minutes, which limited the use of this maneuver. However, it has been postulated that ischemia could be maintained for more than 60 minutes in healthy livers. The present paper review includes: 1) A brief introduction to justify the rationale of the review design; 2) Aspects of the pathophysiology of the three stages of the liver ischemia-reperfusion injury; 3) The innate and acquired immunity; 4) Oxidative stress; 5) Apoptosis and autophagy, Some essential biomarkers (Tumor Necrosis Factor-α, nitric oxide, metalloproteinases); and, finally; 6) Preventive ("cheating") strategies, non-pharmacological and pharmacological options to treat the liver IR injury.(AU)

Purpose:To analyze the effect of methylene blue (MB) therapy during the liver ischemia-reperfusion injury (I/R) process.Methods:Thirty-five male Wistar rats were used, (70%) submitted to partial ischemia (IR) or not (NIR) (30%) were obtained from the same animal. These animals were divided into six groups: 1) Sham (SH), 2) Sham with MB (SH-MB); 3) I/R, submitted to 60 minutes of partial ischemia and 15 minutes of reperfusion; 4) NI/R, without I/R obtained from the same animal of group I/R; 5) I/R-MB submitted to I/R and MB and 6) NI/R-MB, without I/R. Mitochondrial function was evaluated. Osmotic swelling of mitochondria as well as the determination of malondialdehyde (MDA) was evaluated. Serum (ALT/AST) dosages were also performed. MB was used at the concentration of 15mg/kg, 15 minutes before hepatic reperfusion. Statistical analysis was done by the Mann Whitney test at 5%.Results:State 3 shows inhibition in all ischemic groups. State 4 was increased in all groups, except the I/R-MB and NI/R-MB groups. RCR showed a decrease in all I/R and NI/R groups. Mitochondrial osmotic swelling showed an increase in all I/R NI/R groups in the presence or absence of MB. About MDA, there was a decrease in SH values in the presence of MB and this decrease was maintained in the I/R group. AST levels were increased in all ischemic with or without MB.Conclusions:The methylene blue was not able to restore the mitochondrial parameters studied. Also, it was able to decrease lipid peroxidation, preventing the formation of reactive oxygen species.(AU)

PURPOSE: Cardiopulmonary bypass (CPB) procedures are thought to activate systemic inflammatory reaction syndrome (SIRS). Strategies to curb systemic inflammation have been previously described. However, none of them is adequate, since "curbing" the extent of the inflammatory response requires a multimodal approach. The aim of the present mini-review is to discuss the main key points about the main principles in cardiopulmonary bypass curbing inflammation. METHODS: No systematic literature search (MEDLINE) and extracted data from the accumulated experience of the authors. The preconceived idea of an association between severe inflammation and coagulation disorders is reviewed. Also, some fundamental concepts, CPB inflammatory biomarkers, the vasoplegic syndrome and the need forindividual CPB protocols for children, diabetes and old patients, are discussed. CONCLUSION: The ways in which surgical technique (atraumatic vein harvest, biocompatibility and shear resistance of the circuit, monitoring, minimizing organ ischemia, minimal cross-clamping trauma, and blood management) are thought to curb SIRS induced by CPB and affect positively the patient outcome.Improved patient outcomes are strongly associated with these modalities of care, more than single or combinatorial drug strategies (aprotinin, tranexamic acid, pentoxifylline) or CPB modalities (minicircuits, heparin-coated circuits, retrograde autologous prime).(AU)

PURPOSE:To create in vitro a model to generate acidosis by CO2bubbling "organ chambers", which would be useful for researchers that aim to study the effects of acid-base disturbs on the endothelium-dependent vascular reactivity.METHODS:Eighteen male Wistar rats (230-280g) were housed, before the experiments, under standard laboratory conditions (12h light/dark cycle at 21°C), with free access to food and water. The protocol for promoting in vitro respiratory acidosis was carried out by bubbling increased concentrations of CO2. The target was to achieve an ideal way to decrease the pH gradually to a value of approximately 6.6.It was used, initially, a gas blender varying concentrations of the carbogenic mixture (95% O2 + 5% CO2) and pure CO2.RESULTS:1) 100% CO2, pH variation very fast, pH minimum 6.0; 2) 90%CO2 pH variation bit slower, pH minimum6.31; 3) 70%CO2, pH variation slower, pH minimum 6.32; 4) 50% CO2, pH variation slower, pH minimum 6:42; 5) 40 %CO2, Adequate record, pH minimum 6.61, and; 6) 30 %CO2 could not reach values below pH minimum 7.03. Based on these data the gas mixture (O2 60% + CO2 40%) was adopted,CONCLUSION:This gas mixture (O2 60% + CO2 40%) was effective in inducing respiratory acidosis at a speed that made, possible the recording of isometric force.(AU)

PURPOSE: The rationale of the present review is to analize the activity of Rosmarinus officinalis in the the cardiovascular system METHODS: A MEDLINE database search (from January 1970 to December 2011) using only rosmarinic acid as searched term. RESULTS: The references search revealed 509 references about rosmarinic acid in 40 years (the first reference is from 1970). There is a powerful prevalence of antioxidant and cancer studies. Other diseases are few cited, as inflammation, brain (Alzheimer and Parkinson disease) and, memory; allergy; diabetes; atherosclerosis, and; hypertension. It is necessary to consider the complete absence of studies on coronary artery disease, myocardial ischemia, heart failure or ischemia/reperfusion injury. CONCLUSION: Rosmarinic acid is underestimated as an experimental cardiovascular drug and deserves more attention.(AU)
OBJETIVO: A justificativa da revisão é analisar a atividade de Rosmarinus officinalis no sistema cardiovascular MÉTODOS: Uma busca de banco de dados MEDLINE (de janeiro de 1970 a dezembro de 2011), utilizando apenas o ácido rosmarínico como termo pesquisado. RESULTADOS: A busca referências revelou 509 referências sobre o ácido rosmarínico em 40 anos (a primeira referência é de 1970). Há uma prevalência poderoso antioxidante e estudos do câncer. Outras doenças são citados alguns, como o cérebro, inflamação (de Alzheimer e doença de Parkinson) e, a memória, hipertensão, alergia, diabetes, aterosclerose, e. É necessário ter em conta a ausência completa de estudos sobre a doença de artéria coronária, isquemia do miocárdio, insuficiência cardíaca ou isquemia / lesão de reperfusão. CONCLUSÃO: O ácido rosmarínico é subestimado como uma droga experimental cardiovascular e merece mais atenção.(AU)

PURPOSE: To verify if the methylene blue (MB) administration prevents and/or reverses the compound 48/80 (C48/80)-induced anaphylactic shock in pigs. METHODS: Female Dalland pigs were anesthetized and had the hemodynamic parameters recorded during the necessary time to administer some drugs and observe their effect. The animals were randomly assigned to one of the five groups: 1) control; 2) MB: the animals received a bolus injection of MB (2 mg/kg) followed by continuous infusion of MB (2.66 mg/Kg/h delivered by syringe infusion pump); 3) C48/80: the animals received a bolus injection of C48/80 (4 mg/kg); 4) C48/80+MB: the animals received a bolus injection of C48/80 (4 mg/kg) and 10 minutes after the C48/80 administration the animals received a bolus injection of MB (2 mg/kg) followed by continuous infusion of MB (2.66 mg/Kg/h delivered by syringe infusion pump); 5) MB+C48/80: the animals received a bolus injection of MB (2 mg/kg) and 3 minutes later they received a bolus injection of C48/80 (4 mg/kg). RESULTS: The intravenous infusion of MB alone caused no changes in the mean arterial pressure (MAP) showing that the administered MB dose was safe in this experimental model. The C48/80 was effective in producing experimental anaphylactic shock since it was observed a decrease in both MAP and cardiac output (CO) after its administration. The MB did not prevent or reverse the C48/80-induced anaphylactic shock in this model. In fact, the MAP of the animals with anaphylactic shock treated with MB decreased even more than the MAP of the animals from the C48/80 group. On the other hand, the C48/80-induced epidermal alterations disappeared after the MB infusion. CONCLUSION: Despite our data, the clinical manifestations improvement brings some optimism and does not allow excluding the MB as a possible therapeutic option in the anaphylactic shock.(AU)
OBJETIVO: Verificar se a administração de azul de metileno (AM) previne e/ou reverte o choque anafilático induzido por composto 48/80 (C48/80) em suínos. MÉTODOS: Porcos fêmeas Dalland foram anestesiados e tiveram os parâmetros hemodinâmicos registados durante o tempo necessário para administrar algumas drogas e observar seu efeito. Os animais foram aleatoriamente destribuídos em um dos cinco grupos: 1) controle, 2) AM: os animais receberam uma injeção em bolus de AM (2mg/kg), seguido de infusão contínua de AM (2,66mg/Kg /h por bomba de infusão de seringa); 3) C48/80: os animais receberam uma injeção em bolus de C48/80 (4mg/kg); 4) C48/80 + AM: os animais receberam uma injeção em bolus de C48/80 (4mg/kg) e 10 minutos após a administração de C48/80 os animais receberam uma injeção em bolus de AM (2mg/kg), seguido de infusão contínua de AM (2,66mg/kg/h por bomba de infusão de seringa); 5) AM+C48/80: os animais receberam uma injeção em bolus de AM (2mg/kg) e três minutos depois, receberam uma injeção em bolus de C48/80 (4mg/kg). RESULTADOS: A infusão intravenosa de AM não causou mudanças na pressão arterial média (PAM), mostrando que a dose de AM administrada foi segura neste modelo experimental. O C48/80 foi eficaz na indução do choque anafilático experimental, uma vez que foi observada redução na PAM e débito cardíaco (DC), após a sua administração. O AM não preveniu ou reverte o choque anafilático induzido por C48/80 neste modelo. Na verdade, a PAM dos animais com choque anafilático tratados com AM diminuiu mais do que o PAM dos animais do grupo C48/80. Por outro lado, as alterações epidérmicas induzidas pelo C48/80 desapareceu após a infusão do AM. CONCLUSÃO: Apesar dos resultados a melhora clínica das manifestações anafiláticas permite considerar a possibilidade do azul de metileno como opção terapêutica no tratamento do choque anafilático.(AU)

Purpose: Standardization of a simple and low cost technique of exhaled breath condensate (EBC) collection to measure nitrite. Methods: Two devices were mounted in polystyrene boxes filled either with crushed ice/salt crystals or dry ice/crushed ice. Blood samples were stored at -70º C for posterior nitrite dosages by chemiluminescence and the Griess reaction. Results: a) The use of crushed ice/dry ice or salt revealed sufficient EBC room air collection, but was not efficient for patients under ventilation support; b) the method using crushed ice/salt collected greater EBC volumes, but the nitrite concentrations were not proportional to the volume collected; c) The EBC nitrite values were higher in the surgical group using both methods; d) In the surgical group the nasal clip use diminished the EBC nitrite concentrations in both methods. Conclusions: The exhaled breath condensate (EBC) methodology collection was efficient on room air breathing. Either cooling methods provided successful EBC collections showing that it is possible to diminish costs, and, amongst the two used methods, the one using crushed ice/salt crystals revealed better efficiency compared to the dry ice method.(AU)
Objetivo: Padronizar técnica simples e barata de coleta do condensado do ar exalado pulmonar (CEP) para medir nitrito. Métodos: Dois dispositivos foram montados em caixas de isopor e preenchidos com gelo picado/sal grosso ou gelo picado/gelo seco. Amostras de sangue foram armazenadas a -70º C para dosagem de nitrito por quimiluminescência e pela reação de Griess. Resultados: a) a utilização de gelo picado/gelo seco ou sal foi eficiente para a coleta em respiração espontânea, mas ineficiente durante ventilação mecânica; b) o método gelo picado/sal coletou volumes maiores, sem aumento proporcional do nitrito; c) os valores do nitrito foram mais elevados no grupo cirúrgico utilizando os dois métodos; d) no grupo cirúrgico com clipe nasal ocorreu diminuição do nitrito em ambos os métodos. Conclusões: A metodologia do condensado do ar exalado pulmonar (CEP) foi eficiente na coleta respirando em ar ambiente. Os dois métodos de congelamento foram eficientes mostrando que é possível diminuir os custos, e, entre os dois métodos utilizados, o uso de gelo picado/sal mostrou melhor eficiência quanto ao volume da coleta do CEP em comparação com o uso de gelo seco.(AU)

PURPOSE: The purpose of this study is to compare the resting energy expenditure (REE) obtained by indirect calorimetry (IC) to the REE calculated by predictive equations (Brandi and Liggett) using the oxygen consumption (VO2) obtained by Fick's method in septic patients. METHODS: Prospective study in septic patients admitted in an intensive care unit of a university hospital. Fifteen adult patients (11 men and four women) were studied. VO2 measurements were made using a portable metabolic cart connected to a respirator and a pulmonary artery catheter. RESULTS: The APACHE II at admission was 22.6±7.2 with a mortality risk of 46.1±27.7 percent. The mean REE obtained by IC and by the Brandi and Liggett equations were 1699±271, 1815±355 and 1361±277 kcal.day-1 respectively. There were no statistically significant differences between the two methods, with the two means showing a difference of 8.7 percent. REE values were close for IC x BRANDI (r=0.80), but the IC x LIGGETT correlation was only 0.58. CONCLUSIONS: The results suggest that REE can be easily calculated by obtaining VO2 with the Swan-Ganz catheter and using the Brandi equation even for septic patients under mechanical ventilation.(AU)
OBJETIVO: O objetivo desse estudo foi o de comparar o consumo de oxigênio em repouso (COR) obtido por calorimetria indireta (CI) com valores do COR calculados por equações preditivas (Brandi e Ligget) que utilizam o consumo de oxigênio (VO2) obtido pelo método de Fick em pacientes sépticos. MÉTODOS. Estudo prospectivo em pacientes sépticos internados em unidade de terapia intensiva de um hospital universitário. Quinze pacientes adultos (onze homens e quatro mulheres). As medidas de VO2 foram realizadas usando um aparelho metabólico portátil conectado ao respirador e por cateter em artéria pulmonar. RESULTADOS: O índice APACHE II na admissão foi 22.6±7.2 com risco de mortalidade de 46.1±27.7 por cento. As médias do COR obtidas por CI e pelas equações de Brandi and Liggett foram, respectivamente, 1699±271, 1815±355 e 1361±277 kcal.day-1 Não se observaram diferenças estatisticamente significantes entre os dois métodos com as duas médias mostrando uma diferenças de 8.7 por cento. Os valores de COR Também foram próximo dos valores de CI x BRANDI (r=0.80), mas a correlação com CI x LIGGETT apenas 0.58. CONCLUSÕES: Os resultados sugerem que o COR pode ser facilmente calculado para obter-se o VO2 com o cateter de Swan-Ganz e pelo uso da equação de Brandi para pacientes sépticos em ventilação mecânica.(AU)

PURPOSE: Study hemodynamic pattern and lipoperoxidation during methylene blue (MB) treatment on taurocholate - enterokinase induced acute pancreatitis (AP). METHODS: Thirty pigs were equally divided in control group; MB group; AP group; MB previous AP group; and MB after 90 min of induced AP group. MB was given iv in a bolus dose (2mg.kg-1) followed by maintenance dose (2 mg.kg-1.h-1). Hemodynamic parameters were recorded continuously during 180 min by Swan-Ganz catheter. Blood samples were taken every 60 min to determine arterial and venous nitrate, malondialdehyde (MDA) and amylase. Pancreatic tissue was removed for histopathologic study. RESULTS: In AP group MBP and CO decreased over time 33 percent (p<0.05) and 52 percent (p<0.05), respectively. In MB previous induced-AP group, there was 70 minutes delay (p<0.05) to decrease MBP and CO. In MB group arterial and venous nitrite decreased (p<0.05) over time. MB infusion increased (p>0.05) serum MDA when associated to AP. After induced AP, MB did not reverse MBP and CO decrease. There was no difference in serum amylase and necro-hemorrhagic findings with MB treatment. CONCLUSIONS: In this taurocholate-induced AP model MB treatment delayed hemodynamic shock and decreases serum nitrate levels but increases serum MDA levels. No volemic replacement was done and it may have been a mitigated factor to a poor tissue perfusion and impairment microcirculation. Further investigations are needed to elucidate MB treatment role during AP treatment.(AU)
OBJETIVO: estudar o perfil hemodinâmico e a lipoperoxidação durante o tratamento com azul de metileno (AM) de pancreatite aguda (PA) induzida por taurocolato-enteroquinase. MÉTODOS: Trinta porcos foram igualmente divididos em: grupo controle, grupo AM; grupo PA; grupo AM prévio à PA; grupo AM após 90 minutos após a indução da PA. O AM foi administrado sob a forma de bolus EV (2mg.kg-1) seguido por dose de manutenção (2 mg.kg-1.h-1). Os parâmetros hemodinâmicos foram registrados continuamente durante 180 min com auxílio de cateter de Swan-Ganz. Amostras sanguíneas foram colhidas a cada 60 min para a determinação arterial e venosa de nitrato, malondialdeido (MDA) and amilase. Removeu-se tecido pancreático para estudo histopatológico. RESULTADOS: No grupo PA a pressão arterial media (PAM) e o débito cardíaco (DC) diminuíram respectivamente 33 por cento (p<0.05) e 52 por cento (p<0.05) no decorrer do tempo. No grupo AM prévio à indução da PA ocorreu 70 minutes de demora (p<0.05) para as diminuições da PAM e DC. No grupo AM houve diminuição temporal do nitrato arterial e venoso (p<0.05). A infusão de AM aumentou os valores de MDA sérico quando associado a PA (p>0.05). Após a indução da PA a infusão de AM não reverteu as quedas da PA e DC. Não houve diferenças nos níveis de amilase sérica e achados histológicos com o tratamento com o azul de metileno. CONCLUSÕES: No presente modelo de PA induzida por taurocolato o AM retardou o desenvolvimento do choque circulatório, diminuiu os níveis de nitrato mas aumentou os níveis de MDA. Não se realizou nenhum tipo de reposição volêmica que poderia melhorar a perfusão tecidual e melhora da microcirculação. Investigações adicionais são necessárias para elucidar o papel terapêutico do AM no tratamento da PA aguda.(AU)

PURPOSE: To evaluate liver alterations caused by biliary obstruction and drainage. METHODS: Thirty-nine male Wistar rats were randomly distributed in 4 groups: BO (n=18) bile duct ligation for 20 days, with a periodic evaluation of liver histological alterations, Doppler echography portal flow and measurements of NO and malondialdehyde (MDA); BO/DB (n=13) bile duct occlusion for 20 days followed by biliary drainage by choledochoduodenal anastomosis, 5 days follow-up, same BO group parameters evaluations; group CED (n=4) sham operation and portal flow evaluation trough 20 days; CHB (n=4) sham operation, with hepatic biopsy on 25th day and followed-up trough 25 days, by the same parameters of group BO, with exception of portal flow. Direct bilirubin (DB) and alkaline phosphatase (AP) were evaluated in the group BO, BO/DB and CHB. RESULTS: The bile duct ligation led to an increase of DB and AP, development of liver histological alterations, reduction of portal flow and increase of plasmatic NO and of MDA levels. The bile duct clearing resulted in a reduction of DB, AP, NO, MDA histological alterations and increase of portal flow. CONCLUSION: The biliary occlusion resulted in cholestasis and portal flow reduction, besides the increase of plasmatic NO and of hepatic MDA levels, and histological liver alterations, with a tendency of normalization after the bile duct clearing.(AU)
OBJETIVO: Avaliar alterações hepáticas resultantes do processo de obstrução/desobstrução biliar. MÉTODOS: Trinta e nove ratos Wistar foram distribuídos aleatoriamente em 4 grupos: OB (n=18), oclusão biliar, seguimento de 20 dias, avaliação das alterações histológicas hepáticas, do fluxo portal e dosagens de NO e de malondialdeído (MDA); grupo OB/DB (n=13) oclusão biliar por 20 dias seguida de desobstrução biliar por anastomose colédoco-duodenal, seguimento por 5 dias e avaliação dos mesmos parâmetros do grupo BO; grupo CED (n=4) operação simulada, avaliação do fluxo portal e seguimento por 25 dias; grupo CHB (n=4) operação simulada com biópsia hepática no 25º dia, e seguido por 25 dias com avaliação periódica dos mesmos parâmetros do grupo BO, com exceção da ecografia Doppler. Nos grupos BO, OB/DB e CHB foram avaliadas também a bilirrubina direta (BD) e a fosfatase alcalina (FA). RESULTADOS: A oclusão do colédoco causou aumento da BD e da FA, desenvolvimento de alterações hepáticas, redução do fluxo portal, com aumentos do NO plasmático e do MDA. A desobstrução biliar resultou na redução da BD, da FA, das alterações hepáticas, aumento do fluxo portal e redução do NO e do MDA. CONCLUSÃO: A obstrução biliar resultou em colestase, alterações histológicas hepáticas, diminuição do fluxo portal, aumentos do NO plasmático e MDA hepático, com tendência à normalização após a desobstrução biliar.(AU)

PURPOSE: This study sought to evaluate the efficiency of glycol methacrylate-embedding medium to detect morphological alterations of human saphenous vein submitted to brief and crescent pressurizations. METHODS: Saphenous veins of 20 CABG patients were randomly distributed into four experimental groups (control, 100, 200 and 300 mmHg pressures during 15 seconds). To quantify the percentage of endothelium spread over vein surface a microscope magnification of 100x was used for measurements. Morphometric analysis was performed using videomicroscopy with the Leica Qwin software in conjunction with a Leica microscope, videocamera, and an on-line computer. RESULTS: A slight tendency of quantitative increase was observed in all parameters including percentage of endothelium spread over vein surface and thickness of saphenous vein walls (intima and media layers). CONCLUSIONS: The glycol methacrylate-embedding allowed sections with adequate resolution of structural details and revealed to be an extremely useful method to study pressurized human saphenous veins.(AU)
OBJETIVO: Avaliar a inclusão em glicol metacrilato para estudar alterações morfológicas de veias safenas humanas submetidas a pressurizações breves e crescentes. MÉTODOS: Veias safena de 20 pacientes submetidos a cirurgia de revascularização do miocárdio foram distribuídas ao acaso em quatro grupos experimentais (controle, pressões de 100, 200 e 300 mmHg durante 15 segundos). Para quantificar a percentagem da superfície venosa recoberta por endotélio utilizou-se o aumento de 100x. A análise morfométrica foi realizada utilizando-se videomicroscopia com auxílio do software Leica Qwin em conjunto com um microscópio Leica e videocâmera, acoplados a um computador. RESULTADOS: Observou-se uma leve tendência de aumento quantitativo de todos os parâmetros avaliados, incluindo a percentagem de superfície recoberta por endotélio e a espessura das paredes das veias safenas. CONCLUSÕES: A inclusão em glicol metracrilato permitiu secções com adequada resolução dos detalhes estruturais, revelando-se um método extremamente útil para o estudo de veias safenas humanas pressurizadas.(AU)

PURPOSE: To study the interatrial conduction times and atrial node performance in patients submitted to mitral valve surgery with the aid of temporary atrial epicardic electrodes. METHODS: The atriograms were carried out in the first postoperative day and before the hospital discharge of ten consecutive patients. RESULTS: Sixty percent of the patients could complete the post-operative study protocol. The main results were: a) Post-operative arrhythmias were detected in 50 percent of the patients; b) There were no statistical differences between the pre and post-operative 12 lead EKGs. c) The interatrial conduction time (IACT) ranged from 90 to 140ms in the first post-operative day, and from 110 to 130ms at hospital discharge; d) The sinus node recovery time (SNRT) ranged from 250 to 560 ms in the first post-operative day and from 180 to 360ms at hospital discharge; e) The sinus atrial conduction time (SACT) remained between 70 and 140ms, both in the first post-operative day and at hospital discharge, and; f) The IACT was normal in patients whose left atrium (LA) was less than 50mm in diameter but supra normal in the remaining cases. CONCLUSIONS: Sinus node function and inter-atrial conduction are not altered by mitral valve operation. Post-operative programmed epicardic atrial stimulation is easy and safe.(AU)
OBJETIVO: Estudar os tempos de condução interatrial e a função do nó sinusal em pacientes submetidos a tratamento cirúrgico. MÉTODOS: Foram estudados 10 pacientes adultos consecutivos submetidos à operação de correção de valvopatia mitral. Registraram-se atriogramas usando eletrodos epimiocárdicos cirurgicamente implantados. Os atriogramas foram obtidos no primeiro dia do pós-operatório e antes da alta hospitalar. RESULTADOS: Os principais achados foram: a) A incidência de arritmias atriais até alta hospitalar foi de 50 por cento; b) O tempo de condução interatrial (TCIA) variou de 90 a 140 ms no 1ºPO e 110 a130 ms antes da alta hospitalar; c) O tempo de recuperação do nó sinusal (TRNS) variou de 250 a 560 ms no 1ºPO e180 a 360 ms antes da alta hospitalar; d) O tempo de condução sinoatrial (TCSA) variou de 70 a 140 ms tanto no 1ºPO, como antes da alta hospitalar e; d) O tempo de condução interatrial (TCIA) foi normal em pacientes com átrio esquerdo menores do que 50 mm e supranormal nos outros casos. CONCLUSÃO: As funções do nó sinusal e a condução interatrial não foram alteradas pelo tratamento cirúrgico da valvopatia mitral. A estimulação atrial epicárdica programada é segura de fácil realização.(AU)
