Resumo

This manuscript described the anatomopathological and immunohistochemical findings in a rare case of mammary fibroadenomatoid hyperplasia in a 12-month-old Holstein heifer. A yellow, multilobulated, firm 20cm x 9cm x 6.5cm mass affecting the right quarters of the udder was observed. Total mastectomy was performed. Microscopic evaluation revealed severe hyperplasia of the mammary epithelium and numerous well-differentiated and mildly pleomorphic acini. Additionally, moderate proliferation of the fibrous connective tissue and the myoepithelial cells near the proliferating acini was evident. About 50% of the proliferating epithelial cells showed positive nuclear labeling for estrogen and progesterone receptors, and approximately one-third were positive for Ki-67. In addition, the myoepithelial cells exhibited diffuse nuclear immunoreactivity for p63. Based on the clinical, morphologic, and immunohistochemical findings, a diagnosis of mammary fibroadenomatoid hyperplasia with probable influence of ovarian steroids was made.


Este artigo descreve os achados anatomopatológicos e imuno-histoquímicos de um caso raro de hiperplasia fibroadenomatoide mamária em uma novilha Holandesa de 12 meses. Observou-se uma massa amarelada, firme, multilobulada, medindo 20 x 9 x 6,5 cm e afetando os quartos direitos do úbere. Mastectomia total foi realizada. Microscopicamente, notou-se hiperplasia acentuada do epitélio mamário e numerosos ácinos levemente pleomórficos e bem diferenciados. Proliferação moderada do tecido conjuntivo fibroso e das células mioepiteliais próximos aos ácinos proliferados era também evidente. Aproximadamente 50% das células epiteliais proliferadas mostraram marcação nuclear positiva para receptores de estrógeno e progesterona e cerca de um terço dessas células era positivo para Ki-67. Além disso, as células mioepiteliais exibiram reação nuclear e difusa para p63. Com base nos achados clínicos, morfológicos e imuno-histoquímicos, firmou-se o diagnóstico de hiperplasia fibroadenomatoide mamária com provável influência de esteroides ovarianos.

Backgroun d: Feline mammary hyperplasia is a nonneoplastic condition characterized by a rapid hypertrophy and hyperplasia of stroma and ductal epithelium of mammary glands due to hormonal stimuli. This condition is frequently seen in young female cats and is associated with an increase in serum endogenous progesterone or application of exogenous progestogen used as contraceptive. The present study aimed to describe a case of feline mammary hyperplasia treated with the association of aglepristone and acupuncture. Case : A female cat was attended at Hospital de Clínicas Veterinárias of Universidade Federal do Rio Grande do Sul presenting a history of increasing mammary volume after a single progestogen injection. The contraceptive administration was made by the patient owner due to signs of her first estrus and starting mammary development after this application four days ago. The patient had a generalized increase in breast, with solid consistence and high temperature. The ovariectomy was performed through the flank and, 24 days after surgery, had not been seen any mammary reduction, it was decided to aglepristone administration at 10 mg/kg subcutaneously, once a day, for three consecutive days. Acupuncture was performed once or twice a week, stimulating the acupuncture points Yintang, SP6, ST36, ST40, GV14, GV20, GV24, LIV2, LIV3, LIV13, LIV14, CV4, CV8, CV10, CV17, BL23 and LI4, that have, among other indications, analgesic, anti- inflammatory, soothing and immune stimulant actions. Tissue involution had began after antiprogestin applications and some acupuncture sessions. However, there was formation of an abscess in breast that prevents further tissue remission and was only detected after fistulation, delaying patient recovery. After abscess content drainage there was a significant mammary volume reduction and restoration of breast normal size occurred in seven days. Discussion : The patient had received one exogenous progestogen injection and was presenting generalized mammary in - crease, symmetrically and bilaterally, with solid consistence and high temperature, allowing the achievement of a diagnosis based on history and clinical signs. The patient was treated surgically by ovariectomy performed through the flank due to the oversized mammary glands, removing endogenous source of progesterone and preventing the recurrence of disease, as recommended by several authors. Aglepristone, that acts by blocking progesterone receptors, was administered at a dose of 10 mg/kg as recommended by manufacturer and aimed to accelerate mammary volume reduction. This antiprogestin has been indicated by literature when surgical treatment is not effective. During aglepristone action the patient was sub - mitted to acupuncture sessions with intention of accelerate mammary tissue regression and healing process and improve her immunity. Among several indications of acupuncture, it is reported the promotion of analgesia, immune modulation and activation of regenerative processes, which occur due to release of chemicals and neuropeptides as a result of nervous system stimulation. The association of aglepristone with acupuncture was effective in mammary hyperplasia treatment because, despite an abscess formation at the site, there was a rapid mammary involution after drainage of purulent content.(AU)

Resumo

The present study was designed to investigate the significance of mitotic index and AgNOR count in canine mammary tumours. Samples from 74 grossly suspected cases of bitches for mammary tumour were collected from different veterinary hospitals in 10% buffered formalin, of which 65 were confirmed as tumours on histopathological examination. Among them, 11 (16.92%) were benign and 54 (83.08%) were malignant tumours. Benign tumours included benign mixed mammary tumour (36.36%), fibroadenoma (27.27%), duct papilloma (18.18%) and simple adenoma (18.18%). The malignant mammary tumours comprised of papillary adenocarcinoma (27.78%), malignant mixed mammary tumour (25.92%), solid carcinoma (18.52%), squamous cell carcinoma (5.56%), fibrosarcoma (5.56%), infiltrative adenocarcinoma (3.7%), mucinous carcinoma (3.7%) and each (1.54%) of osteochondrosarcoma, carcinosarcoma, myxosarcoma, intraductal carcinoma and spindle cell carcinoma (malignant myoepithelioma). Mitotic index and AgNOR counts were significantly (P<0.05) differ for benign and malignant mammary tumours. Among malignant mammary tumours mitotic index ranged from 1.08 to 4.19 with solid carcinoma showing the highest index (4.197±1.570) and osteochondrosarcoma the lowest (1.08±0.0) while in benign mammary tumours, it ranged from 0.63 t0 0.9. AgNOR counts were significantly lower (2.57± 0.68) in benign mammary tumours than malignant (3.38±1.01). From this study it was concluded that mitotic index and AgNOR count was a good indicator of transformation of tumours towards the malignancy

Resumo

Descrevem-se seis casos de alteração fibroadenomatosa mamária felina (AFAMF) associada à administração de uma única injeção de acetato de medroxiprogesterona (AMP) na dose recomendada pelos fabricantes. A doença foi observada em gatas jovens não-castradas atendidas em dois hospitais veterinários de 1999 a 2001. O diagnóstico de AFAMF foi feito com base no histórico, sinais clínicos e achados macroscópicos e confirmado histologicamente em quatro dos seis casos. Sugere-se que a ocorrência da AFAMF esteja associada ao efeito prolongado de uma única injeção de AMP de depósito administrada em situações nas quais o emprego dessa terapia não é recomendado. (AU)
