Resumo

Prothrombin time (PT) and the activated partial thromboplastin time (aPTT) are useful tools for the diagnosis and monitoring of coagulation disorders in Veterinary Medicine. Our objectives were: to establish reference intervals (RI) for PT and a PTT for the dog using the Start®4 (Stago), to compare the obtained RI with literature; to evaluate the effects of gender and age on the coagulation profile. Plasma samples of 122 healthy dogs (57 males; 65 females) aged between 4 months and 18 years, divided into three age groups (0-2 years old; 3-10 years old; > 10 years old) and grouped in to males and females were analysed. The RI were estimated following the ASVCP guidelines with the Reference Value Advisor software. The RI were: PT 6.7'' to 10.8''; aPTT 9.0'' to 14.8''. PT was significantly higher in females than in males. Dogs aged 10 years or older have significantly higher mean aPTT times than younger dogs. RI comparison showed a considerable percentage of cases outside the reference RI of the literature (PT - 79.3%; aPTT - 77.1%), demonstrating the need of each laboratory to calculate its own RI. The RI established in this study are applicable for the coagulation profile assessment in dogs.


O tempo de protrombina (TP) e o tempo de tromboplastina parcial ativada (TTPa) são ferramentas úteis para o diagnóstico e monitorização das alterações da coagulação em Medicina Veterinária. Os objetivos deste estudo foram: estabelecer intervalos de referência (IR) para TP e TTPa para o cão utilizando o Start®4 (Stago), de modo a comparar os IR obtidos com a literatura; avaliar os efeitos do sexo e da idade no perfil da coagulação. Foram usadas amostras de plasma de 122 cães saudáveis (57 machos; 65 fêmeas) com idades entre quatro meses e 18 anos, divididos em três grupos (0-2 anos; 3-10 anos; > 10 anos) e agrupados em machos e fêmeas. Os IR foram calculados seguindo as diretrizes da ASVCP com o software Reference Value Advisor. Os IR obtidos foram: PT 6,7 '' a 10,8 ''; TTPa 9,0 '' a 14,8 ''. O TP foi significativamente maior nas fêmeas do que nos machos. Os cães com 10 anos ou mais apresentaram tempos médios de TTPa significativamente maiores do que cães mais jovens. A comparação de IR mostrou uma percentagem considerável de casos fora do IR de referência da literatura (TP - 79,3%; TTPa - 77,1%), confirmando a necessidade de cada laboratório calcular seu próprio IR. Os IR estabelecidos neste estudo são aplicáveis na avaliação do perfil hemostático em cães.

Purpose: In this study, hemostatic efficacy of Ankaferd Blood Stopper (ABS), a new generation hemostatic agent, was compared in the presence of heparin effect. Methods: Forty-eight Wistar albino rats were divided into two main groups as heparinized and nonheparinized, and these two main groupswere divided into six subgroups as control, Surgicel and ABS (n = 8). Grade 2 liver injury was performed on rats as standard. All groups were compared in terms of weight, laceration surface area, prothrombin time (PT), activated partial thromboplastin time (aPTT), international normalized ratio (INR), bleeding time, bleeding amount, hemoglobin (Hb) levels, macroscopic and microscopic reactions to the agent used. Results: Whereas there was no statistically significant difference between weight, laceration surface area, PT, INR and preoperative Hb values in the heparinized and nonheparinized groups, postoperative Hb, bleeding time, bleeding amount and aPTT values were statistically different (p 0.05). In the heparin-hemostat interaction, the ABS group had the lowest bleeding in the heparinized group in terms of the amount of bleeding compared to the control and Surgicel groups (F = 0.764; p = 0.047). In macroscopic and microscopic comparison, there was no difference between the groups in terms of cell necrosis andfresh bleeding (p > 0.05), it was found that the Surgicel group had statistical significantly higher reaction scores (p < 0.05) than the other groups in terms of other parameters. Conclusions: Ankaferd Blood Stopper can be safely and effectively used in surgical practice and in patients with additional diseases requiring heparinization, since it causes minimal reaction in the liver and decreases the amount of bleeding especially in the heparinized group.(AU)

ABSTRACT Purpose: To compare biological compatibility, hemostasis, and adhesion formation between oxidized regenerated cellulose and lyophilized hydrolyzed porcine collagen in liver trauma. Methods: Forty male Wistar rats constituted two groups: group A (oxidized cellulose) and group B (lyophilized hydrolyzed collagen). Standardized liver trauma was made, and the hemostatic agent was applied. Animals in subgroups A7 and B7 were submitted to euthanasia and relaparotomy after seven days, and in subgroups A14 and B14 after 14 days. Macroscopic and microscopic results were evaluated. Results: There was no fluid in the cavity in any of the animals, and adhesions were present in all of them. In the analysis after seven days, the adhesions were grades 3 or 4 and consisted of omentum, small intestine, and abdominal wall (p<0.05). In both groups, the mesh was surrounded by a capsule, which was not observed after 14 days. In the evaluation after 14 days, adhesions were grades 2 or 3 (p>0.05). The microscopic examination showed subacute and chronic reactions, in both groups and in both timepoints, with similar frequency. The intensity of fibrosis always presented positive scores. Microabscesses and xanthomatous macrophages were observed in both groups. Conclusions: There was no superiority of one agent over the other.

Fibrin biopolymers, previously referred as "fibrin glue" or "fibrin sealants", are natural biomaterials with diverse applications on health. They have hemostatic, adhesive, sealant, scaffold and drug delivery properties and have become widely used in medical and dental procedures. Historically, these biomaterials are produced from human fibrinogen and human or animal thrombin, and the possibility of transmission of infectious diseases by human blood is not ruled out. In the 1990s, to overcome this problem, a new heterologous biomaterial composed of a thrombin-like enzyme purified from Crotalus durissus terrificus venom and a cryoprecipitate rich in fibrinogen extracted from buffaloes Bubalus bubalis blood has been proposed. Therefore, a systematic review of studies on exclusively heterologous fibrin sealants published between 1989 and 2018 was carried out using the following databases: PubMed, SciELO and Google Scholar. The keyword used was "heterologous fibrin sealant". The search resulted in 35 scientific papers in PubMed, four in SciELO and 674 in Google Scholar. After applying the inclusion/exclusion criteria and complete reading of the articles, 30 studies were selected, which formed the basis of this systematic review. It has been observed that the only completely heterologous sealant is the one produced by CEVAP/UNESP. This heterologous biopolymer is proven effective by several studies published in refereed scientific journals. In addition, clinical trials phase I/II for the treatment of chronic venous ulcers authorized by the Brazilian Health Regulatory Agency (ANVISA) were completed. Preliminary results have indicated a safe and promising effective product. Phase III clinical trials will be proposed and required to validate these preliminary findings.(AU)

Resumo

Background In most cases of envenoming by the green habu Viridovipera stejnegeri in Taiwan coagulopathy is not observed. Case presentation Herein, we describe the case of a patient with liver cirrhosis who developed venom-induced consumptive coagulopathy after V. stejnegeri bite. Laboratory investigation revealed the following: prothrombin time > 100 s (international normalized ratio > 10), activated partial thromboplastin time > 100 s, fibrinogen < 50 mg/dL, and fibrin degradation product > 80 μg/mL. The patient recovered after administration of bivalent hemorrhagic antivenom, vitamin K, fresh frozen plasma and cryoprecipitate. Conclusion The liver, directly involved in the acute phase reaction, is the main responsible for neutralization of animal toxins. Any patient with history of liver cirrhosis bitten by a venomous snake, even those whose venoms present low risk of coagulopathy, should be very carefully monitored for venom-induced consumptive coagulopathy (VICC), since the hemostatic balance may be disrupted.(AU)

PURPOSE: To investigate whether topically administered hemostatic agents ankaferd blood stopper and microporous polysaccharide hemospheres can decrease epidural fibrosis after laminectomy in rats. METHODS: Eighteen adult male Sprague-Dawley rats were equally and randomly divided into three groups. In the treatment groups, ankaferd blood stopper and microporous polysaccharide hemospheres topically administrated upon duramater surface after laminectomy. Fibroblast count, epidural fibrosis and arachnoidal involvement were evaluated and graded histopathologically. RESULTS: Our data revealed that the count of fibroblasts, the grading of epidural fibrosis and arachnoideal involvement in the rats treated with microporous polysaccharide hemospheres were significantly less than the control group. Although the arachnoideal involvement in ankaferd blood stopper group were significantly less than the control group, there were no statistical differences when comparing the grading of epidural fibrosis and the fibroblasts count between the treatment groups and the control group. CONCLUSION: The ankaferd blood stopper and microporous polysaccharide hemospheres reduced epidural fibrosis and arachnoideal involvement after laminectomy in rats.(AU)

Resumo

PURPOSE: To quantify the amount of bone formation in the calvarial region of Wistar rats after craniotomy using bone wax as a haemostatic agent. METHODS: Surgery to produce bilateral, symmetric, full-thickness cranial defects (area: 18 mm2) was performed in eight animals. The right side of the cranium remained open and the edges of the left side osseous defect was covered with bone wax. Calvaria were imaged immediately after surgery and 12 weeks postoperatively by computerized tomography. The areas of the bone defects were measured in three-dimensional images using Magics 13.0 (Materialise-Belgic, software CAD). RESULTS: The average amount of bone formation on the left and right side respectively was 4.85 mm2 and 8.16 mm2. Statistically significant differences between the amount of bone formation on the left and right sides were seen. CONCLUSIONS: Bone wax significantly diminishes the rate of bone formation in calvarial defects in a rat model.(AU)


OBJETIVO: Quantificar a formação óssea da região da calvaria de ratos Wistar submetidos à craniotomia com a utilização de cera de osso como agente hemostático. MÉTODOS: Cirurgia para realizar um defeito ósseo craniano bilateral, simétrico (área: 18 mm2) e com espessura total foi realizado em oito animais. O lado direito do crânio permaneceu aberto e as extremidades do defeito ósseo do lado esquerdo foram recobertas com cera de osso. O crânio foi submetido à avaliação radiológica imediatamente após a cirurgia e 12 semanas após a cirurgia com a utilização de tomografia computadorizada. As áreas dos defeitos ósseos foram medidas através de imagens tridimensionais e utilizando o programa de computador Magics 13.0 (Materialise-Belgic, software CAD). RESULTADOS: A quantidade média de formação óssea no lado esquerdo e direito foi respectivamente de 4.85 mm2 e 8.16 mm2. Diferença estatisticamente significante foi observada entre o lado direito e esquerdo. CONCLUSÕES: A cera de osso diminuiu significativamente a formação óssea nos defeitos ósseos em modelo animal.(AU)

BACKGROUND: The ideal therapy for hemorrhoids is always debated. For early grades of the disease, many different modalities of treatment have been proposed. Some are effective but are more painful, others are less painful but their efficacy is not assured on long term. Infrared photocoagulation has emerged as a new addition to the list. In this procedure, the tissue is coagulated by infrared radiation. During treatment, mechanical pressure and radiation energy are applied simultaneously to ablate the blood supply to the hemorrhoidal mass. METHODS: In the present retrospective study, the effect of infrared coagulation on patients with early grades of hemorrhoids is described. In a separate study, a comparison is made between Infrared coagulation and rubber band ligation in terms of their effectiveness and discomfort. RESULTS: 212 patients were treated by infrared coagulation and were followed up for a period of 18 months. Only 28 patients had persistence or recurrence of bleeding. Overall ratio of comfort and patient satisfaction from pain and bleeding was quite satisfactory. The comparative study showed that though rubber band ligation is more effective, it is a more painful procedure. CONCLUSION: These studies shows that Infrared coagulation for hemorrhoids in early stages could prove to be a easy and effective alternative to conventional methods as it is quick, less painful and safe. The procedure can be repeated in case of recurrence and should be considered as the first choice in early hemorrhoids.


OBJETIVO: Mostrar os benefícios do uso dos raios infravermelhos como alternativa no tratamento de hemorróidas sangrantes, comparado a outras modalidades habitualmente utilizadas. MÉTODOS: 212 pacientes com hemorróidas sangrantes foram tratados com raios infravermelhos e foram acompanhados por período de 18 meses a fim de se observar os efeitos do procedimento. Comparou-se com outras modalidades usuais de tratamento. RESULTADOS: Apenas 28 pacientes tiveram persistência ou recidiva de sangramento. O procedimento mostrou vantagens comparado a outros. CONCLUSÃO: A coagulação de hemorróidas sangrantes nos estágios iniciais por raios infravermelhos mostrou ser uma alternativa simples, eficaz e segura, em comparação a outros procedimentos convencionais. O procedimento é rápido e menos doloroso. Pode ser repetido em caso de recidiva e deve ser considerado como primeira escolha em hemorróidas precoces.

Resumo

A hemostasia é um evento biológico passível de ser avaliado e estudado, assim como seus distúrbios. Há situações durante a anestesia que podem cursar com alterações hemostáticas culminando com aumento do sangramento ou até mesmo hemorragias graves. Os tempos de coagulação têm recebido especial atenção tendo-se em vista os diferentes contratempos hemostáticos que pode ocorrer durante o procedimento anestésico-cirúrgico. Sendo assim, nesse estudo buscou-se estabelecer os valores padrões para tempo de protrombina (TP) e de tromboplastina parcial ativada (TTPA) para cães submetidos a diferentes procedimentos cirúrgicos. Foram estudados 50 cães hígidos para padronização dos valores de TTPA e TP utilizando aparelho automático. Os valores de TTPA estavam dentro dos valores de normalidade da literatura (6,9 a 17,6 segundos) e valores de TP discretamente maiores (de 6,65 a 12,8 segundos). Foram estudadas 20 cadelas classificadas como ASA I e 18 cães ASA II e III. Nestes animais observaram-se aumentos significativos de valores de TTPA (de 12,04 para 14,29 segundos em ASA I, com P<0,0378; e de 13,4 para 15,11 segundos nos cães ASA II e III, P<0,0067) e de TP (de 8,36 para 9,7 segundos em ASA I, P<0,0323; e de 8,32 para 9,34 segundos nos caes ASA II e III, P< 0,0084) entre os momentos pré e pós-anestésicos Estes aumentos acompanham quedas da pressão coloidosmótica, indicando que o processo de anestesia, cirurgia e fluidoterapia causam hemodiluição, e conseqüente aumento nos tempo de coagulação


Hemostasis is a biological event that could be evaluated and studied, just like yours disturbs. There are situations in to anesthetic procedures that could curse with hemostatics disturbs causing bleeding increase or even critical blood loss. The blood clotting times have been received special attention because different hemostatic setbacks that could occur during the anesthetic and surgical procedures. This way, this study look for establish the values for reference to protrombine time (PT) and to activated partial tromboplastin time (APTT) for dogs. Were studied 50 healthy dogs to standardize the values of PT and APTT utilizing an automatic instrument. The APTT values was in agreement with the literature values (6,9 to 17, 6 seconds), and the PT values was discret increased when compared with literature (from 6,65 to 12,8 seconds). Was studied 20 bitches classifieds as ASA I and 18 dogs classifieds as ASA II and III. In this animals observed significant increase values of APTT (from 12,04 to 14,29 seconds in the ASA I, P<0,0378; and from 13,4 to 15,11 seconds in the dogs ASA II and III, P<0,0067) and of PT (from 8,36 to 9,7 seconds in ASA I, P<0,0323; and from 8,32 to 9,34 seconds in the ASA II and III dogs) between the pre and post anesthetic moments. These increases attendance decease in the colloid osmotic pressure, indicating that the anesthetic, surgical and fluid therapy procedures can cause hemodilution, and consequent increase in the blood clotting times
