Resumo

A síndrome da hiperestesia felina é uma doença ainda pouco compreendida, normalmente de caráter idiopático, que pode ocasionar vários sinais clínicos, relacionados a sensibilidade extrema em uma área da coluna, quase sempre no dorso do animal. O diagnóstico baseia-se na exclusão principalmente de alterações dermatológicas e neurológicas, e apresenta uma diversidade de medicações como terapêutica. Objetivou-se neste trabalho relatar um caso de hiperestesia felina, em um macho, castrado, de dois anos de idade, que não respondeu satisfatoriamente aos tratamentos convencionais, sendo necessária a implementação de opções terapêuticas integrativas, como aromaterapia e óleo de Cannabis.


Feline hyperesthesia syndrome is a disease that is still poorly understood, usually of an idiopathic nature, which can cause several clinical signs, related to extreme sensitivity in an area of the column, almost always on the animal's back. The diagnosis is based on the exclusion mainly of dermatological and neurological alterations, and presents a variety of medications as therapy. The present study described a case of feline hyperesthesia in a two-year-old male, castrated, which did not respond satisfactorily to conventional treatments, requiring the implementation of integrative therapeutic options, such as aromatherapy and Cannabis oil.


El síndrome de hiperestesia felina es una enfermedad aún poco conocida, generalmente de carácter idiopático, que puede provocar varios signos clínicos, relacionados con una sensibilidad extrema en una zona de la columna, casi siempre en la espalda del animal. El diagnóstico se basa en la exclusión principalmente de alteraciones dermatológicas y neurológicas, y presenta una variedad de medicamentos como terapia. El presente trabajo describe un caso de hiperestesia felina en un macho de dos años, castrado, que no respondió satisfactoriamente a los tratamientos convencionales, requiriendo la implementación de opciones terapéuticas integradoras, como la aromaterapia y el aceite de Cannabis.

Resumo

A Cannabis sativa é uma planta que apresenta vários benefícios terapêuticos para animais, como tratamento da dor neuropática, inflamatória e osteoartrose. A dor é bastante recorrente na rotina clínica, sendo importante seu manejo para que seja ofertada uma melhor qualidade e conforto de vida para o paciente. O estudo objetivou identificar, a partir de evidências científicas, as características da utilização medicinal do uso de Cannabis Sativa no tratamento da dor crônica no cão, utilizando um dos seus princípios ativos, canabidiol (CBD). Foi feito uma revisão bibliográfica onde foi realizada a busca de estudos experimentais e relatos de caso em bases de dados eletrônicos, sendo incluídas fontes contendo a utilização do CBD em animais, que abordaram controle da dor, assim como escore avaliativo da dor antes, durante e após o tratamento proposto. Após eleger e analisar 54 estudos percebe-se que na medicina veterinária o uso do canabidio é insuficiente, uma vez que o foco da maior parte dos estudos clínicos é voltado para medicina humana. Ainda assim, a utilização de CBD mostrou-se eficaz, confirmando uma nova alternativa para o controle da dor em animais.(AU)


Cannabis sativa is a plant that has several therapeutic benefits for animals, such as the treatment of neuropathic and inflammatory pain and osteoarthritis. Pain is quite recurrent in the clinical routine, and its management is important to offer a better quality and comfort of life for the patient. The study aimed to identify, based on scientific evidence, the characteristics of the medicinal use of Cannabis Sativa in the treatment of chronic pain in dogs, using one of its active principles, cannabidiol (CBD). A bibliographical review was carried out in which experimental studies and case reports were searched in electronic databases, including sources containing the use of CBD in animals, which addressed pain control, as well as pain assessment score before, during and after the proposed treatment. After choosing and analyzing 54 studies, it is clear that in veterinary medicine the use of CBD is few, and the focus of clinical studies is on human medicine. The use of CBD proved to be effective, thus confirming a new alternative for pain control in animals.(AU)


El cannabis sativa es una planta que tiene varios beneficios terapéuticos para los animales, como el tratamiento del dolor neuropático e inflamatorio y la osteoartritis. El dolor es bastante recurrente en la rutina clínica, y su manejo es importante para ofrecer una mejor calidad y comodidad de vida al paciente. El estudio tuvo como objetivo identificar, con base en la evidencia científica, las características del uso medicinal de Cannabis Sativa en el tratamiento del dolor crónico en perros, utilizando uno de sus principios activos, el cannabidiol (CBD). Se realizó una revisión bibliográfica en la que se buscaron estudios experimentales y reportes de casos en bases de datos electrónicas, incluyendo fuentes que contengan el uso de CBD en animales, que abordaran el control del dolor, así como la puntuación de evaluación del dolor antes, durante y después del tratamiento propuesto. Después de elegir y analizar 54 estudios, queda claro que en medicina veterinaria el uso de cannabidio es insuficiente, ya que el foco de la mayoría de los estudios clínicos está en la medicina humana. Aun así, el uso de CBD demostró ser efectivo, confirmando una nueva alternativa para el control del dolor en animales.(AU)

Background: Nonambulatory flaccid tetraparesis can be the result of diseases of the peripheral nervous system and it is characterized by generalized lower motor neuron (LMN) signs, as weakness, tetraparesis/tetraplegia, decreased muscle tone and reflexes. The term polyneuropathy is used for dysfunction of multiple peripheral nerves. In Brazil, there are several etiologies for polyneuropathy in dogs, such as acute idiopathic polyradiculoneuritis, botulism and myasthenia gravis. Toxoplasma gondii is an uncommon cause of LMN diseases in dogs. The aim of this report was to describe a case of flaccid tetraplegia toxoplasmosis in an adult dog with a Toxoplasma gondii serology with a markedly elevated IgG titer of 1:4096. Case: A 4-year-old intact mongrel male dog, weighing 19.6 kg, was referred to the Veterinary Medical Teaching Hospital of the Universidade Estadual de Londrina (UEL) with a 5-day history of weakness that progressed to tetraparesis. Physical examination revealed no significant changes other than the dull and unkempt coat. Neurologic examination revealed severe tetraparesis that was worse in the pelvic limbs, with decreased muscle tone in all four limbs. Postural reactions and the interdigital reflex were absent in all four limbs, as was the patellar reflex, but pain perception was present. There were no clinical signs of dysfunction on examination of the cranial nerves. Laboratory tests were performed, and creatine kinase was elevated (819 U/L). Blood was drawn to look for antibodies to Toxoplasma gondii and Neospora caninum class IgG using the indirect immunofluorescence technique. The antibody titer for Toxoplasma gondii (IgG) was 1:4096. A chest radiograph was performed to look for megaesophagus, and a pulmonary pattern suggestive of mild diffuse pneumonia was observed. Treatment was performed with sulfamethoxazole and trimethoprim, and the dog's condition improved slightly. Discussion: Based on lower motor neuron findings, the neurologic lesion was localized in the nerve roots, peripheral nerves, neuromuscular junctions, or muscles. The most important diseases in the list of differential diagnoses were immune-mediated or infectious polyradiculoneuritis (toxoplasmosis, neosporosis), myasthenia gravis, toxic polyneuropathy (botulism, chronic organophosphate poisoning), and paraneoplastic polyneuropathy. Among these differential diagnoses, polyradiculoneuritis is one of the most common. It is an idiopathic inflammatory disease. Exposure to raccoon saliva (in the U.S.), vaccination, or infection have been proposed as precipitating causes, but the triggers of this disease remain unknown. Serology for neosporosis was negative, while IgG titers for toxoplasmosis were 1:4096. In a previous study, dogs with acute polyradiculoneuritis were more likely to have T. gondii IgG serum antibody titers than dogs without neurologic signs. Infection with the protozoa T. gondii and N. caninum can cause intense polyradiculoneuritis in dogs accompanied by myositis, especially in puppies. One treatment trial was based on the administration of sulfonamide-trimethoprim with pyrimethamine, whose efficacy in the treatment of toxoplasmosis in dogs has also been reported in the literature. Neurologic deficits improved slightly, and there is a possibility that certain signs may not disappear completely because of the permanent damage caused by inflammation of the nervous system, as observed in the present case. The case had the limitation that it was not possible to perform other laboratory tests to demonstrate histopathologically the presence of Toxoplasma gondii organisms in muscles or nerves. Recovery of normal function is less likely in protozoan polyradiculoneuritis than in noninfectious polyradiculoneuritis. Thus, in the present case, the main suspicion was polyradiculoneuritis secondary to toxoplasmosis. Although it is a rare condition, it is important to consider toxoplasmosis in dogs with LMN-type tetraparesis or tetraplegia.

Resumo

Dipyrone is an effective analgesic for managing moderate or severe postoperative pain and can be used alone for mild pain or in combination with other analgesics for any type of pain. This study aimed to examine the administration of dipyrone by continuous infusion (CI) as an adjuvant analgesic in the intraoperative period for bitches undergoing elective ovariohysterectomy (OH) and its effect on these patients' cardiorespiratory parameters. Twenty bitches underwent an elective OH procedure. The pre-anesthetic agent was a combination of acepromazine and morphine. Propofol was used to induce anesthesia, and isoflurane was used for maintenance. Subsequently, the animals were randomly allocated into two groups: the dipyrone group (DG) received a bolus dose of dipyrone (25 mg kg-1) by CI at a rate of 10 mg kg-1 h, and the control group (CG) received a bolus dose and a CI of 0.9% NaCl solution, both groups at a rate of 5 mL kg-1 h. The parametric variables were analyzed by ANOVA, followed by Tukey's test (p <0.05). The paired t-test (p <0.05) was used for comparison between the groups. Statistical differences were observed for heart rate, systolic, diastolic, and mean arterial pressure, respiratory rate, and blood glucose between the periods in both groups. There were differences only in the basal values of MAP between the groups; however, most values remained within the physiological range for the species. Using the drug as an adjuvant to anesthesia did not alter cardiorespiratory parameters, and it can be used as an adjuvant in analgesia during the intraoperative period of OH.(AU)


A dipirona é um analgésico eficaz para o manejo da dor pós-operatória moderada ou grave, podendo ser utilizada isoladamente em dores leves ou associada a outros analgésicos em qualquer tipo de dor. O presente estudo teve como objetivo avaliar a utilização de infusão contínua (IC) de dipirona no período transoperatório de cadelas submetidas à Ovariohisterectomia (OH) eletiva como adjuvante analgésico, avaliando seu efeito sobre parâmetros cardiorrespiratórios desses pacientes. Para o estudo, 20 cadelas foram submetidas ao procedimento de OH eletiva. A medicação pré-anestésica foi composta pela associação de acepromazina e morfina, seguida da indução com propofol e manutenção com isofluorano. Posteriormente, os animais foram alocados, aleatoriamente, em dois grupos: dipirona (GD), que receberam bolus de dipirona (25 mg kg-1) seguido da IC do fármaco na taxa de 10 mg kg-1 h, e grupo controle (GC), cujos animais receberam o bolus e IC de solução de NaCl 0,9%, ambos os grupos na velocidade de 5 mL kg-1 h. Variáveis paramétricas foram analisadas pela ANOVA seguida pelo teste de Tukey (p <0,05) e comparadas entre os grupos pelo teste t pareado (p <0,05). Foram observadas diferenças estatísticas entre os momentos em ambos os grupos em relação à FC, FR, PAS, PAM, PAD e glicemia. Entre os grupos, houve apenas diferenças nos valores basais de PAM, no entanto, a maioria dos valores manteve-se dentro da faixa considerada fisiológica para a espécie. A utilização do fármaco como adjuvante a anestesia não alterou os parâmetros cardiorrespiratórios, podendo ser utilizado como adjuvante na analgesia durante o transoperatório de OH.(AU)

After analgesic administration, we evaluated general activity in the Open-Field and anxiety-like behavior in the Elevated Plus Maze of vasectomized mice. We divided C57BL/6J male mice into eight groups: saline, three non-operated control groups treated with 10 mg/kg meloxicam, 20 mg/kg tramadol, or both intraperitoneally, and four vasectomized mice groups treated with the same analgesic protocol as the control groups. One group of vasectomized mice received both treatments and an additional 10 mg/kg lidocaine at the incision site. We conducted the vasectomy via scrotal approach under isoflurane inhalation anesthesia and performed behavioral tests after full anesthesia recovery. Mice treated with meloxicam demonstrated low ambulation, spontaneous activity, and rearing frequency. Mice treated with tramadol showed spontaneous behavior compared with the saline control. Due to behavior changes demonstrated by meloxicam controls, we were unable to identify whether meloxicam provided adequate analgesia. Vasectomized mice treated with tramadol showed general activity behavior similar to their control but displayed significantly less rearing, suggesting that they were under potential signs of pain or discomfort. In conclusion, the Open Field test and the Elevated Plus Maze can usefully pre-evaluate analgesic protocols to identify possible interference caused by adverse drug effects. For future directions, an appropriate regimen of meloxicam and tramadol for enhancing mice welfare post vasectomy should be better investigated.

This study aimed to determine the psychometric properties and clinical use of the Brazilian version of the Canine Brief Pain Inventory (CBPI) in dogs with hip dysplasia (HD). Forty-three dogs with HD and 16 clinically normal dogs were enrolled. The HD dogs were treated daily with 4.4mg/kg carprofen (GT = 21) or placebo (GP = 19), for four weeks. Owners completed the CBPI at two weeks (W-2) and immediately before the start of the treatment (W0), two (W2) and four (W4) weeks during treatment, and two weeks (W6) after the end of treatment. The internal structure was accessed, and the Cronbach's alpha coefficient was 0.97, indicating the high internal consistency of the instrument. Principal Component Analysis (PCA) suggested the retention of one component, which accounted for 78% of the variability. The ROC curve analysis concluded that the score 3 has an excellent performance to discriminate between normal and possible HD dogs (AUC of 0.973). There was no difference between dogs treated with carprofen versus placebo. The instrument in Portuguese showed construct and criterion validity and reliability to be used in dogs with HD.


Objetivou-se determinar as propriedades psicométricas e a utilidade clínica do Breve Inventário de Dor Canina na língua portuguesa, em cães com displasia coxofemoral (DCF). O inventário foi preenchido por tutores de 43 animais com DCF e por 16 tutores de cães saudáveis. Os animais com DCF foram tratados com carprofeno 4,4mg/kg (GT = 21) ou placebo (GP =19), administrados uma vez ao dia durante quatro semanas. As avaliações foram realizadas duas semanas e imediatamente antes do tratamento, duas e quatro semanas durante o tratamento e após duas semanas do término do tratamento. A estrutura interna calculada pelo alfa de Cronbach = 0,97 indicou alta consistência dos dados. A análise dos componentes principais identificou a retenção de apenas um componente responsável por 78% da variabilidade dos dados. A análise da curva ROC indicou que o escore 3 discrimina cães saudáveis de cães com possível DCF (ASC de 0,973). Não houve diferença entre os cães tratados com carprofeno daqueles que receberam placebo. O questionário apresentou validade de constructo e critério e confiabilidade e pode-se empregá-lo para avaliar a dor crônica em cães com osteoartrite em países de língua portuguesa.

Resumo

Purpose: To analyze the effect and mechanism of dexmedetomidine (DEX) analgesia pretreatment on functional chronic visceral pain in rats. Methods: Rats were divided into six groups: W1, W2, W3, W4, W5, and W6. The behavioral changes and electrophysiological indexes of rats in each group before and after DEX treatment were detected. Results: The levels of abdominal withdrawal reflex (AWR) in W5 and W6 groups were significantly lower than those in group W3, while the levels of thermal withdrawal latency (TWL) and mechanical withdrawal threshold (MWT) were significantly higher than those in group W3 (p < 0.05). The electromyographic signals of W1, W5, and W6 groups showed little fluctuation, while those of groups W2, W3, and W4 showed obvious fluctuation. TLR4 mRNA expression, IRF3, P65, and phosphorylation levels in W4, W5, and W6 groups were significantly lower than those in group W2 (p < 0.05). Conclusions: Dexmedetomidine epidural anesthesia pretreatment could significantly inhibit visceral pain response in rats with functional chronic visceral pain, and its mechanism was related to the activation of TLR4 in spinal dorsal horn tissue of rats and the activation inhibition of IRF3 and P65 in the downstream key signals.

Resumo

A 14-month-old female Texel sheep that came from a herd made up of 19 animals showed haemoglobinuria, apathy, and anorexia, and died two days after the start of the clinical signals. The sheep remained in a natural grassland, where trailers were repaired, and multiple copper wires were deposited on the pasture. The animal had tachycardia, tachypnoea, pale mucous membranes, groaning pain on abdominal palpation, circling, head pressing, intensely hemolyzed plasma, and intense azotaemia. The necropsy showed focally extensive oedema in the inguinal and medial region of pelvic limbs, kidneys dark brown, and liver diffusely yellow with an evident moderate diffuse lobular pattern. The abomasum had a considerable amount of enameled material of thickness, firm to the cut, with 1-5 mm (copper wires). Histopathological examination showed marked diffuse tubular and glomerular coagulative necrosis in the kidneys, in addition to neutrophils, macrophages, lymphocytes, and plasma cells with moderate multifocal nephritis. The liver showed centrilobular necrosis, moderate hepatocellular edema, multifocal cholestasis, and in the lungs and brain mild to moderate diffuse edema. Copper content in the frozen liver (in natura) reached 1,598 mg/kg. Copper mesh ingestion led to sheep poisoning, which in this case was considered an atypical form of chronic primary copper poisoning.


Um ovino Texel de 14 meses de idade, que fazia parte de um rebanho de 19 animais, apresentou hemoglobinúria, apatia, anorexia e morreu dois dias após o início dos sinais clínicos. Os ovinos permaneciam em campo nativo, onde eram realizados consertos de trailers, e múltiplos fios de cobre ficavam depositados na pastagem. O animal apresentou taquicardia, taquipneia, mucosas pálidas, gemido de dor à palpação abdominal, além de andar em círculo, e pressão da cabeça contra obstáculos, plasma intensamente hemolisado e azotemia intensa. Na necropsia, havia edema na região inguinal e medial de membros pélvicos focalmente extenso, rins enegrecidos, e o fígado estava difusamente amarelado, com padrão lobular evidente difuso moderado. No abomaso, havia grande quantidade de material esmaltado, com 1-5mm de espessura, firme, que rangia ao ser cortado (fios de cobre). No exame histopatológico nos rins, havia necrose tubular e glomerular hemoglobinúrica difusa acentuada, além de nefrite de neutrófilios, macrófagos, linfócitos e plasmócitos multifocal moderada. No fígado, havia necrose centrolobular, tumefação hepatocelular e colestase multifocais moderadas; nos pulmões e no cérebro, edema difuso discreto a moderado. A dosagem de Cu no fígado revelou a presença de 1598mg/kg. A ingestão de malhas de cobre levou à intoxicação do ovino que, nesse caso, foi considerada uma forma atípica de intoxicação primária crônica por cobre.

Resumo

Background: Laminitis is characterized by an inflammation of the laminar structures of the hoof, which results in lamellar degradation of the suspensory apparatus of the distal phalanx. Despite being a common disease in the equine clinic, it is a medical emergency, which can lead the affected animals not return to sports activities. Due to the severity of laminitis and the poor prognosis, the objective was to report the case of a horse with chronic laminitis with sole perforation in all limbs treated with therapeutic shoeing using a type of horseshoe that has a horizontal crossbar and is filled with mass. Fast-drying epoxy, which has its use little described in the literature Case: A 4-year-old Crioula horse, weighing 325 kg, used in long noose competitions, was referred to the Veterinary Hospital of UFPR with chronic laminitis. The animal had reluctance to move, walk on a trestle position, grade V lameness and phalanx rotation of all limbs. The treatment of laminitis was carried out in the field, for two months, which consisted of trimming and shoeing with a horseshoe in the shape of a heart, and due to the failure of the treatment instituted, the patient presented clinical worsening. The treatment in the HV consisted of cleaning the perforated region of the sole with hydrogen peroxide and 10% iodine and dressing with cotton boots with EVA rubber on the sole to provide comfort. Corrective trimming was performed in order to remove excess forceps, lower the heel, and align the distal phalanx with the hoof wall, using a rasp. Orthopedic shoeing was performed with a normal horseshoe with a horizontal crossbar in the central region. The sole was filled with a quick-drying epoxy putty (Poxilina®), with the aim of preventing sole movement, blocking the rotation of the phalanges and improving the concavity of the hoof. Discussion: In chronic laminitis, the signs observed are claudication and deformation of the hoof, flat sole, enlargement of the white line, uneven growth of the hoof wall, hemorrhage in the abaxial white line, cracks in the hoof wall, which are observed parallel to the coronary band, in addition to phalanx rotation; in this clinical case, all these signs were observed. The trimming of horses with laminitis consists of removing the heels and decreasing the dorsal wall of the hoof, in this case, the trimming performed improved the clinical improvement of the animal five days after the procedure. The horseshoe used in the treatment of this animal has the objective of creating a weight-bearing surface on the middle of the sole, close to the tip of the frog, in this way the weight is not on the supporting edge of the clamp and, in this way, it reduces the pressure. over the most affected area. Despite the scarcity of reports on the use of this type of horseshoe, in this case it played a fundamental role in the treatment of laminitis. In this way, it is an alternative that must be taken into account when choosing the treatment, in view of its effectiveness and lower cost when compared to other methods. This case demonstrates the severity of laminitis in all limbs, especially as it presents a perforation of the sole. It is important to highlight that trimming and shoeing performed at the recommended interval (30-45 days) contributes a lot to the success of the treatment and the return of the animal to sports practice. The horseshoe with a horizontal crossbar was effective in the treatment of chronic laminitis of the forelimbs and pelvic limbs in the reported animal, with progressive improvement of the patient who returned to normal daily activities, without the presence of signs of pain or signs of laminitis after treatment.

Background: Diseases of the bovine digit and hoof remain major problems in dairy farming and the beef cow industry. Severe claudication accompanying swelling and pain at the lesion is often observed in deep digital sepsis, septic arthritis, severe sole ulcers, and chronic interdigital phlegmon. In addition, digital amputation is often performed in cases of white line disease or severe trauma, such as bone fractures. There are few reports of amputation of the medial digit of the bovine front hoof. In this case, amputation of the medial digit of the front leg of a pregnant cow was performed; its hospitalization and return to productivity is reported. Case: The bovine was a 9-year-old Holstein cow in the 6th month of pregnancy, weighing 671 kg. Upon admission, the cow could not bear weight on its right front leg, and swelling and a purulent discharge were observed in the coronary area of the medial digit. X-ray examination results indicated a periosteal reaction centered on the 3rd phalanx of the medial digit of its right front leg hoof and slight periosteal reactions at the adaxial sides of the middle and proximal phalanges, strongly suggestive of septic arthritis caused by infection. According to the X-ray examination results, no abnormalities of the lateral digit of the front right hoof were observed; thus, it was estimated that the post-amputation hoof would be able to bear weight. Therefore, a decision was made to perform immediate amputation of the medial digit of the right front leg hoof to reduce pain for improved delivery of the offspring and improved milk production during the lactation period, rather than allowing the lesion progress until the dry period or the expected date of delivery. According to the X-ray examination results, amputation of the 1st phalanx alone was expected to suffice for removal of the cause of the pain; thus, a decision was made to perform amputation at the edge adjacent to the 2nd phalanx. The right front medial digit was prepared for aseptic surgery, and infiltration anesthesia was performed. An incision was made with a surgical knife at the interdigit of the right front limb. A wire saw was inserted into the site to cut the 2nd phalanx in the anti-axial direction down to the skin to resect the lesion. White viscous pus was discharged at the resection site; therefore, the wound was lavaged with tap water and packed with povidone iodine-impregnated gauze. The wound was also covered with a diaper and dressed with non-elastic and elastic bandages. The dressing was changed daily to aid wound healing. No problems were observed in the standing-up motion or other relevant movements immediately after the surgery. Granulation tissue formed rapidly, approximately 2 weeks after the surgery, and the lesion dried gradually. Approximately one month after the surgery, the subject exhibited little difficulty in both standing up and walking. The subject successfully delivered its 7th offspring at the farm on postoperative day 93. Discussion: The animal of this study was a 9-year-old, pregnant cow, and although amputation of the front medial digit is a relatively rare procedure, the cow was able to deliver and return to production, owing to sufficient postoperative treatment and care. The case also demonstrated the advantage of X-ray examination in bovine hoof diseases for accurate diagnosis, precise operation, and prognostic assessment.

Resumo

Background: Superficial necrolytic dermatitis (SND), hepatocutaneous syndrome (HCS), metabolic epidermal necrosis (MEN), and necrolytic migratory erythema (NME) are useful terms to describe a disease that likely has a multifactorial etiopathogenesis. SND is a rare and fatal disease characterized by skin lesions and liver disease. Common skin lesions include hyperkeratosis, fissures, erosion, ulceration, crusting, exudation from the paws, face, perianal regions, and pressure points. This case report aimed to report the case of a bitch that developed the rare Superficial Necrolytic Dermatitis disease, emphasizing the clinical signs of the disease, and the importance of complementary exams such as abdominal ultrasound and skin biopsy for the definitive diagnosis. Case: A 9-year-old, mixed-breed, neutered female was referred for clinical examination with 5 months history of hyperkeratosis and ulceration of the paw pads, presenting pain, lameness and weight loss. Abdominal ultrasound revealed a liver with heterogeneous echotexture, mixed echogenicity, irregular and poorly delimited margins with hypoechoic nodules throughout like honeycombs. The gallbladder was visualized with a moderately thick layer. Histological analysis confirmed the diagnosis of SND. Skin biopsies showed an increase in thickness of the epidermis due to irregular hyperplasia and proliferation of keratinocytes in the basal layer of the epidermis, pallor of the spinous layer of the epidermis and important parakeratosis. Due to the progression of the disease, significant worsening of the patient's clinical condition and pain, associated with the impossibility of cure, the animal was submitted to euthanasia. A necropsy was performed to allow assessment of the liver and pancreas. The biopsies showed a severe proliferative chronic hepatitis, steatosis and cholestasis associated with pancreatitis and necrotic multifocal proliferative fibrinopurulent areas in the pancreas. Discussion: Clinical signs such as lethargy, inappetence, weight loss, as well as the dermatological signs presented by this bitch are nonspecific clinical signs and require a deeper clinical, pathological and histopathological diagnostic investigation to reach the diagnosis of this disease. The definitive diagnosis is made on the basis of a characteristic honeycomb pattern in the liver or associated with a neoplastic finding in the pancreas on ultrasound examination and confirmed by histopathological evaluation of skin biopsies. Palliative treatment with corticosteroid anti-inflammatories, improvement in feed quality, with higher nutritional and protein intake and intravenous amino acid supplementation are suggested by some authors as treatment alternatives. However, not all owners can afford a costly lifetime treatment. With the progressive worsening of the condition, many owners opt for euthanasia as a way to shorten the suffering of the animal. This decision is not an easy one to make. Despite the poor prognosis of the disease, treatment options should be tried by veterinarians and owners prior to the option of euthanasia. However, new affordable nutritional and pharmacological strategies to treat or control the disease are needed in order to improve quality of life of SND patients.

Background: Coxofemoral dysplasia (CFD) is the abnormal development of the hip joint, mostly affecting large breeds, and is characterized by subluxation or complete luxation of the femoral head. Among the conservative therapeutic options, cell therapies with stem cells for CFD provides potential by the countless possibilities of therapeutic application, especially those related to the chronic and/or degenerative diseases treatment, which could be a key point for the bone and joint repair. The objective was to report a bilateral CFD case in a dog, treated with intra-articular application of mesenchymal stem cells (MSC), with 0, 30, 60 and 90 days, and further evaluations. Case: A 2-year-old male Akita dog was referred to the Veterinary Hospital Harmonia (HVH) located in the city of Recife, Pernambuco, presenting clinical signs of hip pain, lameness and pelvic limbs hopping. By orthopedic examination, pain was observed at the cranial and caudal extension of the pelvic limb, and at flexion, abduction and adduction of the limb, as well as moderate muscle atrophy and presence of joint crackling. By coxofemoral radiography performed in ventrodorsal projection, it were detected bilateral femoral head subluxation, thickening of the femoral neck and the presence of intraarticular osteophytes. These findings are compatible with the degeneration caused by the presence of CFD. Laboratory tests performed such as hemogram and biochemical had no changes. Thus, the patient had intra-articular application of allogeneic MSC, derived from adipose tissue, obtained by private company. For stem cell applications, arthrocentesis of the hip joints was performed one at a time, using a 5 mL syringe and 16G needle for aspiration of 2 mL synovial fluid, for discard and application of stem cells. Background: Hypothyroidism is characterized by hypofunction of the thyroid gland. It results in deficient production of thyroid hormones. Neurological disorders resulting from hypothyroidism are rare, which highlights the importance of this study. This study reports a case of hypothyroidism in a dog with neurological clinical signs, that was treated at the Universidade Estadual de Santa Cruz's Veterinary Hospital (HV-UESC). Case: A 4-year-old male intact Dogo Argentino breed dog, weighing 64 kg, presenting obesity, anorexia, prostration, walking in circles, and chronic dermatopathy was presented at HV-UESC. Upon physical examination, the animal presented a deficit of proprioception in the 4 limbs, with preserved superficial and deep pain. No alteration was observed in the ears, that could explained the clinical signs. In terms of dermatopathy, the animal presented symmetrical alopecia in the lateral region of the thighs and tail. Blood samples were collected for a complete blood count and biochemical tests of urea, creatinine, ALT, AST and cholesterol. Imaging radiography and ultrasonography were performed, which ruled out thoracic and abdominal changes that could be related to the case. Prior to receiving the blood test results, idiopathic encephalitis was suspected and enrofloxacin and prednisone were prescribed for 7 days. During the medication period, previous exams were provided, which indicated only increased cholesterol (500 mg/dL). The animal showed no improvement with the prescribed medication. In view of the clinical signs presented by the patient and the results of the additional tests, hormonal disease was suspected, compatible with hypothyroidism. Thus, hormonal tests of total T4, free T4, and TSH were requested, leading to verification of reduced total T4 (0.3 ng/dL) and free T4 (0.15 ng/dL) levels, and confirming the dysfunction of the thyroid gland. The previous treatment was suspended and thyroid hormone replacement was initiated. After 3 days of treatment, the neurological signs regressed and the animal became more active; after 30 days, the areas of alopecia decreased. Although the patient did not receive the recommended clinical follow-up for such cases, it was possible to establish the ideal levothyroxine dosage for the dog after appropriate adjustments, which permitted thyroid hormone levels to return to normal. Discussion: This report refers to a case of hypothyroidism in a giant dog breed. The dog in the report showed clinical signs of a dermatological, metabolic, and neurological nature, which is consistent with a lack of thyroid hormone. The main signs presented by the animal were neurological, such as walking in circles and a deficit of proprioception in the four limbs. These clinical signs are rarely mentioned in the literature associated with hypothyroidism. Laboratory abnormalities are correlated with the severity and chronicity of the disease. The animal showed a decrease in total T4 and free T4, which is to be expected in a hypothyroid animal. As a result, levothyroxine replacement treatment was initiated. The dose used for the dog, which is the recommended dose in the literature, greatly increased its total T4 levels. As a result, the dose was readjusted after a new clinical evaluation. The rate of metabolism and absorption of levothyroxine varies widely and is independent of weight. The patient showed quickly improvement in neurological signs, activity level, and serum cholesterol rate. Regarding dermatological signs and body condition, there was a more gradual improvement. This corroborates what is mentioned in the literature, which indicates that improvements can take many months

Background: Mastitis is an inflammation of the mammary gland or udder, regardless of its origin, severity, or evolution. Bilateral total mastectomy is indicated in cases of chronic suppurative mastitis, gangrenous mastitis, udder neoplasm or hyperplasia. For mastectomy, the supine position is recommended and, as it is a long-term procedure, general anesthesia was chosen together with the tumescence technique with the objective of transanesthetic and postoperative analgesia. The present report aims to report the general anesthesia protocol used for 2 goats submitted to bilateral total mastectomy associated with a locoregional block by tumescence, a technique not described in the goat species. Cases: Two female goats, mixed breed, approximately 3 years old, were admitted to the Hospital Veterinário de Grandes Animais (HVGA) of the Universidade Federal Rural do Rio de Janeiro (UFRRJ), with a history of recurrent mastitis. There was an increase in udder volume and the presence of purulent secretion during milking. One of the animals had given birth about 20 days ago. Surgical treatment through bilateral total mastectomy was recommended for both animals, as they did not present a satisfactory response to antimicrobial therapy, excessive enlargement and functional loss of the mammary system. For the procedure, food fasting for 48 h and water fasting for 24 h was established. The anesthetic protocols used consisted of previous sedation with xylazine1 [Xilazin® - 0.05 mg/kg, IM] anesthetic induction with ketamine2 [Ketalex® - 10 mg/kg, IV] and midazolam3 [Dormire® - 0.1 mg/kg, IV]. Orotracheal intubation and connection to the anesthetic circuit were performed for maintenance through inhalation anesthesia with isoflurane4 (Forane®) and oxygen therapy. Then, in bottle of lactated ringer's solution5 [Linhamax® - 500 mL], lidocaine 2%6 without vasoconstrictor [Lidovet® - 40 mL] and adrenaline7 [Adren® - 25 mg/mL, 0.5 mL] were added, 10 mL/kg of the solution were injected into the subcutaneous tissue of the region of the mammary chains utilizing a Klein cannula after local antisepsis. After the administration of the tumescent solution, it was observed that the area involved is presented with the formation of a gel and there is minimal bleeding, compared to the conventional technique. The animals were monitored for the degree of analgesia, recording vital signs every 15 min at the end of the surgery, dipyrone9 [D-500® - 25 mg/kg, IV], morphine7 [Sulfato de Morfina® - 0.1 mg/kg, SC], and meloxicam10 [Maxican® 0.5 mg/kg, IM, 3 consecutive days] for postoperative analgesia. Additionally, tetanus serum1 [Vencosat® - 50,000 IU, single dose] and of oxytetracycline¹ [Oxitetraciclina LA® - 10 mg/kg, IM, 5 applications on alternate days)]. In the following 24 h, no signs of pain were observed on palpation of the surgical wound in the animals, but goat 1 was apathetic and inappetent, with improvement in the clinical picture only 48 h after surgery. Discussion: The choice of the surgical procedure resulted in an improvement in the clinical conditions of the animals and the anesthetic protocol using inhalation anesthesia and locoregional block (tumescence) proved to be efficient to perform in goats, contributing to transanesthetic and postoperative analgesia, being easily applied to ruminants that need surgical interventions in the region of mammary chains.

Many variations of ovariohysterectomy techniques have been described, including the traditional one and minimally invasive procedures. Non-laparoscopic Snook hook technique is an alternative for performing minimally invasive ovariohysterectomy. Few studies have been carried out in order to assess pain in animals submitted to minimally invasive surgeries, especially involving one of the most performed surgical procedures in veterinary practice. The aim of this study was to evaluate surgical duration, intraoperative nociception and acute postoperative pain after traditional ovariohysterectomy or minimally invasive non-laparoscopic technique in dogs using Snook hook. The hypothesis is that non-laparoscopic minimally invasive ovariohysterectomy would be faster and less painful than the conventional technique. Thirty dogs were divided into Traditional Group (TG = 15) and Minimally Invasive Group (MIG = 15). Heart rate, respiratory rate, systolic blood pressure, body temperature, oxyhemoglobin saturation, end-tidal carbon dioxide concentration (ETCO2) and end-tidal isoflurane concentration were evaluated before the surgery begins (M0), during incision (M1), clamping of the first ovarian pedicle (M2), second ovarian pedicle (M3), uterine cervix (M4), abdominal suture (M5) and at the end of surgery (M6). The modified Glasgow Pain Scale was used for acute postoperative pain assessment and Visual Analogue Scale (VAS) was used to assess the sensitivity of surgical wound. The level of significance established for all statistical analysis was 5%. Statistical differences were not observed between groups considering total surgical time and postoperative acute pain intensity (P > 0.05), in spite of MIG having shorter duration of surgery. There was no statistical difference between groups considering all intraoperative parameters except respiratory rate (TG < MIG; P < 0.05) and ETCO2 (MIG < TG; P < 0.05) at the moment of traction of the first ovarian pedicle (M2). Pain assessment by VAS showed statistical difference 24h after the end of surgery (TG < MIG) (P < 0.05). Both procedures were similar regarding intraoperative nociception and acute postoperative pain. It is possible that the sensation of pain in both procedures was blocked by the effectiveness of analgesics, once they might cause an inhibition of painful behaviors limiting a possible difference in pain identification. Higher respiratory stress observed in MIG at M2 and higher pain score by VAS noted in MIG 24 h after the end of surgery can be justified by greater traction of ovarian pedicle, due to limited surgical access of minimally invasive technique. Minimally invasive ovariohysterectomy non-laparoscopic seems to be potentially faster, probably due to the smaller size of the abdominal incision, which takes less time to be closed. In the present study, both techniques were performed by an experienced surgeon, providing safe procedures, nevertheless it is important to emphasize that iatrogenic injury can be caused by surgeons non-proficient in the Snook hook technique, considering the limited visualization of anatomical abdominal structures. Data obtained indicate that traditional ovariohysterectomy and non-laparoscopic Snook hook technique promote similar intraoperative nociception and acute postoperative pain, however minimally invasive procedure is potentially faster with less surgical trauma.(AU)

This study aimed to evaluate the postoperative analgesic efficacy of dexmedetomidine alone and when associated with morphine in patients under general inhalational anesthesia with isoflurane and undergoing elective ovariohysterectomy (OH). Twenty healthy bitches were selected via physical and laboratory examinations. Prior to the study, they underwent a 24-h period of adaptation to the environment and observers. They were then randomly divided into two groups: the dexmedetomidine group receiving a dose of 10 µg/kg, and dexmedetomidine group (10 µg/kg) associated with morphine receiving a dose of 0.3 mg/kg, administered via the intramuscular route. Thereafter, patients were induced with propofol and maintained with isoflurane diluted in 100% oxygen administered through a calibrated vaporizer. To verify postoperative analgesia, pain assessments were performed using the modified Glasgow (EGM) and Melbourne (EM) scales. For sedation assessment, the Dobbins scale was used at different times: before the administration of pre-anesthetic medication (T0) and at another six times in the postoperative period, 1 (T1), 2 (T2), 4 (T3), 8 (T4), 12 (T5), and 24 (T6) h after orotracheal extubation. No statistical differences were observed between groups in the scales according to Dunn's Kruskal-Wallis post hoc test, and between the times a statistical difference was noticed by the Friedman test (p<0.05) for the EGM and EM scale scores, but there was no need for analgesic rescue. Therefore, we found that isolated dexmedetomidine and morphine produced adequate analgesic effects in the postoperative period of bitches submitted for OH.


Objetivou-se avaliar a eficácia analgésica pós-operatória da dexmedetomidina isolada e associada à morfina em pacientes submetidas à anestesia geral inalatória com isoflurano e submetidas à ovariohisterectomia eletiva (OH). Vinte cadelas saudáveis foram selecionadas a partir de exames físico e laboratoriais. Previamente ao estudo, foram submetidas a um período de 24 horas de adaptação ao ambiente e aos observadores. Ato contínuo, foram divididas aleatoriamente em dois grupos: grupo dexmedetomidina, na dose de 10 µg/Kg (GD) e grupo dexmedetomidina (10 µg/Kg) associado com morfina, na dose de 0,3 mg/Kg (GDM), administrados pela via intramuscular (IM). Ato contínuo, as pacientes foram induzidas com propofol e mantidas com isoflurano diluído em oxigênio 100% e administrado através de vaporizador calibrado. Para verificação da analgesia pós-operatória foram realizadas avaliações de dor por meio de escalas de Glasgow Modificada (EGM) e de Melbourne (EM). Para avaliação de sedação, por meio da escala de Dobbins (ED), em diferentes tempos: antes da administração da MPA (T0), e em mais 6 tempos no período pós-operatório, uma (T1), duas (T2), quatro (T3), oito (T4), 12 (T5) e 24 (T6) horas após a extubação orotraqueal. Não foram observadas diferenças estatísticas entre os grupos nas escalas de acordo com o teste de Kruskal-Wallis post-hoc de Dunn; e entre os tempos notou-se diferença estatística pelo teste de Friedman (p<0,05) para EGM e EM, porém não houve necessidade de resgate analgésico. Portanto, conclui-se que a dexmedetomidina isolada e associada a morfina produziu efeito analgésico adequado no período pós-operatório de cadelas submetidas a OH.

Abstract Ph1 is a neurotoxin purified from spider venom that acts as a high-voltage-activated (HVA) calcium channel blocker. This spider peptide has shown a high selectivity for N-type HVA calcium channels (NVACC) and an analgesic effect in several animal models of pain. Its activity was associated with a reduction in calcium transients, glutamate release, and reactive oxygen species production from the spinal cord tissue and dorsal ganglia root (DRG) in rats and mice. It has been reported that intrathecal (i.t.) administration of Ph1 to treat chronic pain reverted opioid tolerance with a safer profile than -conotoxin MVIIA, a highly selective NVACC blocker. Following a recent development of recombinant Ph1 (CTK 01512-2), a new molecular target, TRPA1, the structural arrangement of disulphide bridges, and an effect on glial plasticity have been identified. CTK 01512-2 reproduced the antinociceptive effects of the native toxin not only after the intrathecal but also after the intravenous administration. Herein, we review the Ph1 antinociceptive activity in the most relevant pain models and its mechanisms of action, highlighting the impact of CTK 01512-2 synthesis and its potential for multimodal analgesia.

Background: Complete avulsion of the hoof in horses, also known as exungulation, is not a commonly reported injury and usually leads to euthanasia due to the great amount of tissue loss, intense pain, secondary complications, expensive and lengthy treatment. It can involve deep structures and cause different complications leading to chronic lameness. In stallions affected by such injury, the reproductive tract and performance may also be affected. The aim of this study was to report a case of complete avulsion of the right front hoof in a Criollo stallion and subsequent bilateral testicular degeneration. Case: A 10-year-old Criollo stallion was referred to the Veterinary Clinical Hospital of the Federal University of Pelotas (HCV- UFPel) with a complete avulsion of the left front hoof. At admission, the stallion had clinical parameters compatible with intense pain and blood loss. Evaluation of the wound demonstrated that the distal end of the third phalanx (P3) was exposed but no fracture was detected on radiological evaluation. No other structure was apparently affected. Initially, anti-inflammatory (phenylbutazone) and opioid (morphine) was given for pain control and supportive fluid therapy was started to restore hydration. Antibiotic (Sulfamethoxazole with trimethoprim) was administered for 10 days. Continued therapy with phenylbutazone, pentoxifylline, omeprazole and supplementation with methionine, biotin and zinc was also given. Local treatment was carried out by cleaning the wound, applying an antimicrobial ointment and dressing it with a bandage. Wound management was adapted according to the evolution and healing process. The stallion was kept in stall rest during its hospitalization time. In the second month after the injury, accumulation of liquid in the scrotum was observed. Clinical and ultrasound evaluation lead to a presumptive diagnosis of testicular degeneration. The stallion was discharged after three months when the wound was almost healed and the hoof had started to grow. Six month later, a follow up by the referring vet showed that the hoof was almost completely grown and the x-ray assessment demonstrated a dorsal rotation and resorption of the distal end of the third phalanx. Discussion: The stallion of this report had a complete avulsion of the hoof capsule caused by trauma. Conservative treatment was established including wound cleaning and dressing to avoid contamination, control of pain and inflammation, antimicrobial care and supplementation to support hoof growth. Time period for wound healing and hoof growth was in agreement with other cases described previously. Bone sequestrum of the distal end of the third phalanx, and detachment of a fragment were observed in this case, followed by bone resorption. The stallion was closely monitored to prevent laminitis in the contralateral limb and no alterations were detected during the treatment period. Testicular degeneration was observed, probably caused as a consequence of hoof avulsion and due to a long period of stall rest. Degenerative alterations in testicles interfere with thermoregulation and spermatogenesis, affecting semen quality and reproductive performance. Rotation of the third phalanx was also observed six months later caused by the hoof loss. In conclusion, the patient of this report had a complete regrowth of the hoof capsule although a long intensive treatment was necessary to achieve this result. As a consequence, testicles degeneration may happen impairing its function as a stallion.(AU)

Phα1ß is a neurotoxin purified from spider venom that acts as a high-voltage-activated (HVA) calcium channel blocker. This spider peptide has shown a high selectivity for N-type HVA calcium channels (NVACC) and an analgesic effect in several animal models of pain. Its activity was associated with a reduction in calcium transients, glutamate release, and reactive oxygen species production from the spinal cord tissue and dorsal ganglia root (DRG) in rats and mice. It has been reported that intrathecal (i.t.) administration of Phα1ß to treat chronic pain reverted opioid tolerance with a safer profile than ω-conotoxin MVIIA, a highly selective NVACC blocker. Following a recent development of recombinant Phα1ß (CTK 01512-2), a new molecular target, TRPA1, the structural arrangement of disulphide bridges, and an effect on glial plasticity have been identified. CTK 01512-2 reproduced the antinociceptive effects of the native toxin not only after the intrathecal but also after the intravenous administration. Herein, we review the Phα1ß antinociceptive activity in the most relevant pain models and its mechanisms of action, highlighting the impact of CTK 01512-2 synthesis and its potential for multimodal analgesia.
