Resumo

O hipoadrenocorticismo canino é uma endocrinopatia caracterizada, na sua forma mais comum, pela deficiência de glicocorticoides e mineralocorticoides. Pacientes não tratados podem desenvolver a forma aguda da doença (crise addisoniana), apresentação grave com risco de óbito, caso não sejam realizadas as intervenções adequadas. Entre as alterações apresentadas estão: distúrbios eletrolíticos (como hiponatremia, hipercalemia, hiperfosfatemia e hipercalcemia), desequilíbrio hídrico (desidratação e choque hipovolêmico) e acidobásico. Fluidoterapia intravenosa adequada é a base do manejo inicial da crise addisoniana, que irá reestabelecer o volume intravascular e hidratação, reduzindo a concentração sérica de fósforo e potássio, a acidose lática, a azotemia pré-renal e a acidose metabólica. A reposição de glicocorticoides também contribui para a normalização dessas alterações. Entre as possíveis complicações do tratamento está a mielinólise pontina, decorrente de aumento rápido na concentração sérica de sódio. Diante disso, o conhecimento dos distúrbios associados à essa endocrinopatia auxilia na suspeição e manejo adequado da doença.(AU)


Canine hypoadrenocorticism is an endocrinopathy characterized, in its most common form, by glucocorticoid and mineralocorticoid deficiency, often resulting from immune-mediated destruction of the adrenal glands. Untreated patients may develop the acute form of the disease (Addisonian crisis), a severe presentation with a risk of death if appropriate interventions are not performed. Among the changes presented are: electrolyte disorders (such as hyponatremia, hyperkalemia, hyperphosphatemia and hypercalcemia), water imbalance (dehydration and hypovolemic shock) and acid-base disorder. Adequate intravenous fluid therapy is the basis for the initial management of Addisonian crisis, which will restore intravascular volume and hydration, reducing serum phosphorus and potassium concentrations, lactic acidosis, pre-renal azotemia and metabolic acidosis. Glucocorticoid replacement also contributes to the normalization of these changes. Among the possible complications of the treatment is pontine myelinolysis, resulting from a rapid increase in serum sodium concentration. In view of this, knowledge of the disorders associated with this endocrinopathy helps in the suspicion and adequate management of the disease.(AU)

Resumo

Background: Myelitis is the inflammation of the spinal cord parenchyma alone, whereas meningitis is the inflammation of the meninges. Steroid-responsive meningitis-arteritis (SRMA) is a meningomyelitis in which the major lesions involve the meninges, not the spinal cord parenchyma, and respond well to glucocorticoid treatment. However, myelitis in dogs has rarely been reported, and myelitis with a good response to glucocorticoid treatment without relapse has not been reported. This report describes 5 cases of steroid-responsive myelitis (SRM) in dogs. Cases: Case 1. A 8-year-old intact female Cocker Spaniel presented with progressive nonambulatory paraplegia. Whole spinal parenchymal lesions were identified using magnetic resonance imaging (MRI) scan. Mononuclear pleocytosis with increased total protein levels was the only abnormal finding on cerebrospinal fluid (CSF) analysis. Prednisolone (PDS) was administered followed by dose tapering according to therapeutic response. Cyclosporine was administered until the termination of PDS. Since then, no recurrence of neurological symptoms has been observed. Follow-up MRI and CSF analysis revealed resolution of previously observed abnormal findings. Case 2. A 2-year-old intact female Maltese presented with non-progressive paraparesis. A spinal parenchymal lesion in the lumbosacral region was observed on MRI. PDS was administered and slowly tapered at approximately 3-week intervals. No recurrence of neurological symptoms was observed after the treatment. Case 3. A 6-year-old intact female Miniature Pinscher presented with neck pain, along with leukocytosis and neutrophilia. Cervical spinal parenchyma lesions were revealed through MRI. Increased total protein concentration with mixed cell pleocytosis was observed on CSF analysis. Immunomodulatory therapy, similar to that in case 2, was initiated. A second MRI and CSF analysis revealed an improvement in the previously observed abnormalities. Case 4. A 2-year-old, intact female Toy Poodle presented with acute paraplegia and back pain. Lesions were observed in the spinal parenchyma at the T12-L3 levels on MRI. The treatment was conducted as in case 2. During treatment, neurological symptoms, including paraplegia and back pain, were not observed. Follow-up MRI revealed improvement in the spinal lesion. Case 5. A 6-month-old, castrated male Standard Poodle presented with progressive paraparesis. On MRI, lesions were observed in the T11-T13 regions. Immunomodulation therapy, similar to that in case 2, was initiated. No recurrence of neurological symptoms was observed after treatment initiation Discussion: SRM is similar to SRMA in terms of good steroid-responsiveness and noninfectious inflammation etiology; however, it does not exactly satisfy the diagnostic criteria for SRMA, nor does it progress similarly. The characteristics of SRM that do not satisfy the diagnostic criteria of SRMA include the absence of fever, C-reactive protein elevation, hyperglobulinemia, and relapse, and the presence of spinal parenchymal lesions without parenchymal or meningeal enhancement on MRI. It is also a seemingly different from spinal cord-only meningoencephalomyelitis of unknown origin due to its better treatment response and prognosis. However, the dogs in the present report with SRM satisfied the diagnostic criteria for transverse myelitis in human patients. Therefore, SRM, including good steroid responsiveness and good prognosis without relapse, may represent a novel type of meningomyelitis.

Resumo

O hipoadrenocorticismo primário ou doença de Addison é uma endocrinopatia que envolve a deficiência de glicocorticoides, comumente associada a deficiência mineralocorticoide. O tratamento mais utilizado para reposição hormonal envolve a associação de fludrocortisona e prednisona. O pivalato de desoxicorticosterona (DOCP) tem sido demonstrado na literatura como superior no controle eletrolítico no paciente com hipoadrenocorticismo, e sempre deve ser associado a prednisona/prednisolona. Neste trabalho, relatamos o benefício do DOCP e o protocolo de ajuste de dose desse fármaco em duas etapas em dois casos distintos. Houve uma eficácia superior no controle eletrolítico e redução dos efeitos colaterais com o protocolo de DOCP e prednisolona em relação ao uso de fludrocortisona em ambos os pacientes.(AU)


Primary hypoadrenocorticism or Addison's disease is an endocrinopathy involving glucocorticoid deficiency, commonly associated with mineralocorticoid deficiency. The most commonly used treatment for hormone replacement involves the combination of fludrocortisone and prednisone. Deoxycorticosterone pivalate (DOCP) has been shown in the literature to be superior in electrolyte control in patients with hypoadrenocorticism, and should always be associated with prednisone/prednisolona. In this study, we report the benefit of DOCP and the two-stage dose adjustment protocol for this drug in 2 different cases. There was superior efficacy in electrolyte control and reduction of side effects with the DOCP and prednisolone protocol compared to the use of fludrocortisone in both patients.(AU)


El hipoadrenocorticismo primario o enfermedad de Addison es una endocrinopatía que cursa con deficiencia de glucocorticoides, comúnmente asociada con deficiencia de mineralocorticoides. El tratamiento más utilizado para el reemplazo hormonal consiste en la combinación de fludrocortisona y prednisona. El pivalato de desoxicorticosterona (DOCP) ha demostrado en la literatura ser superior en el control de electrolitos en pacientes con hipoadrenocorticismo, y siempre debe asociarse con prednisona/prednisolona. En este trabajo, reportamos el beneficio de DOCP y el protocolo de ajuste de dosis en dos etapas para este fármaco en 2 casos diferentes. Hubo una eficacia superior en el control de electrolitos y la reducción de los efectos secundarios con el protocolo DOCP y prednisolona en comparación con el uso de fludrocortisona en ambos pacientes.(AU)

A celulite canina juvenil é uma afecção rara, que acomete, em sua maioria, filhotes entre três semanas e seis meses de idade, cuja patogênese não está completamente elucidada; entretanto, pressupõe-se estar relacionada à falha imunológica e a reações à vacina polivalente, podendo ocorrer predisposição racial. Como manifestações clínicas, o cão apresenta pápulas, pústulas, alopecia, edema, eritema, crostas e cicatrizes, especialmente em pálpebras, lábios e focinhos, sendo também acometido o pavilhão auricular. Além dessas alterações dermatológicas, nota-se linfoadenomegalia regional. O diagnóstico se baseia em exclusão de dermatopatias que mimetizam o quadro clínico, e o tratamento precisa ser realizado de forma precoce e adequada, normalmente se utilizando doses altas de glicocorticoides. Quando iniciada a terapia precoce, o prognóstico é bom, embora as cicatrizes possam ser permanentes. No presente estudo, foi relatado um caso de filhote fêma mestiça de poodle, que começou a apresentar sinais de otite purulenta, lesões edemaciadas, crostosas e supurativas em região periocular, perilabial, mentoniana, pápulas e pústula em pavilhão auricular, linfoadenomegalia de linfonodos submandibulares e poplíteos, sendo que os sintomas começaram após aplicação da vacina polivalente canina. Foi iniciado tratamento à base de glicocorticoide e, devido à resposta rápida à terapia, foi iniciado desmame da medicação, quando a paciente teve quadro de recidiva, que se resolveu após reajustes terapêuticos.


Canine juvenile cellulitis is a rare condition that affects, in its majority, puppies between three weeks and six months of age, whose pathogenesis has not been completely elucidated; however, it is assumed to be related to immunological failure and reactions to the polyvalent vaccine, which may lead to racial predisposition. As clinical manifestations, the dog presents papules, pustules, alopecia, edema, erythema, crusts and scars, especially on the eyelids, lips and snouts, and the ear pinna is also affected. In addition to these dermatological changes, regional lymphadenomegaly is noted. Diagnosis is based on the exclusion of dermatopathies that mimic the clinical picture, and treatment needs to be carried out early and properly, usually using high doses of glucocorticoids. When therapy is started early, the prognosis is good, although scarring may be permanent. In the present study, a case of a crossbred female poodle pup was reported, which began to show signs of purulent otitis, swollen, crusty and suppurative lesions in the periocular, perilabial, mental region, papules and pustules in the pinna, lymphadenomegaly of submandibular lymph nodes and poplites, and the symptoms started after application of the polyvalent canine vaccine. Glucocorticoid-based treatment was started and, due to the rapid response to therapy, weaning from the medication was started, when the patient had a relapse, which resolved after therapeutic adjustments

Resumo

Objetivou-se relatar um caso de dermatite piotraumática em um canino adulto, tratado sem uso de antimicrobianos associado à laserterapia. Foi encaminhado para atendimento clínico, um cão macho, 8 anos de idade, apresentando lesões em pele, alopecia e seborreia oleosa intensa. Na avaliação clínica, o animal apresentou os parâmetros normais para espécie, entretanto em decorrência das lesões apresentadas foram solicitados exames citológico e parasitológico de pele, que evidenciaram a presença de processo inflamatório agudo com evidenciação sugestiva de bactérias e Demodex canis no parasitológico, respectivamente. O tratamento proposto consistiu no uso de xampu a base de Clorexidina, suplementos a base de vitaminas e minerais, antiparasitário, glicocorticóide oral e posteriormente tópico, ômega3 e imunoestimulante, além de uma sessão de laserterapia. No último retorno, o animal apresentou o pelo refeito. A dermatite piotraumática, apesar de fácil diagnóstico, requer total atenção no que concerne a resposta individualizada de cada animal perante a afecção, neste caso o acompanhamento constante realizado pelo médico veterinário, proporcionou o estabelecimento da conduta terapêutica mais adequada, sem o uso de antimicrobianos em associação a laserterapia, para resolução do problema e o restabelecimento da saúde do animal.


The aim of this study was to report a case of pyotraumatic dermatitis in an adult canine that was treated without the use of antimicrobials associated to laser therapy. An 8-year-old male dog with skin lesions, alopecia, and severe oily seborrhoea was referred for clinical care. In the clinical evaluation, the animal presented normal parameters for the species, however due to the lesions presented, cytopathological and parasitological skin examinations were requested, which evidenced the presence of an infectious inflammatory process, suggestive of bacteria, at the cytopathological examination, and Demodex canis in the parasitological assay. The proposed treatment consisted of baths with shampoo based on chlorhexidine with miconazole, supplements based on vitamins and minerals, antiparasitic, oral glucocorticoid and later topical, omega 3 and immunostimulant, in addition to a phototherapy session. On the last return, the animal presented the remade hair. Pyotraumatic dermatitis, despite being easy to diagnose, requires full attention regarding the individualized response of each animal to the condition. In this case, the constant monitoring performed by the veterinarian, provided the establishment of the most appropriate therapeutic approach to solve the problem and restore the animal's health without the use of antimicrobials, and with the additional laser therapy.

Background: Canine eosinophilic folliculitis is a dermatological disease of acute onset with development of erosive to ulcerative papular lesions, especially on the nasal bridge, that may cause severe skin abnormalities leading to discomfort and pain to the patient. The aim of this report was to characterize a case of a canine eosinophilic folliculitis with papular, ulcerative and crusting dermatitis on the nasal bridge, papules on eyelid and pinna, with confirmed diagnosis based on aspiration cytology, history and response to immunosuppressive therapy with glucocorticoid. Case: An 1-year-old intact Daschund was attended showing an acute onset (over 4 h) of generalized urticarial reaction and nonpruriginous lesion at the muzzle with mild serosanguineous exudate, which persisted for 96 h when the dog was evaluated. It was observed a papular and ulcerative dermatitis with serosanguineous exudate and hematic crusts at nasal bridge, papules measuring 2 mm in diameter in the medial and lateral canthus of the left eyelid, ulcerative papule with hematic crust in the border of left ear pinna, multifocal papules on the skin, dyskeratosis and generalized hair loss. The patient was anesthetized for blood sampling (CBC and serum biochemistry), lesions fine-needle aspiration, scraping and imprint for cytological examination, bacterial culture and nasal turbinates radiography. Fragments for histopathological evaluation were also collected. Erythrogram and platelet evaluation were unremarkable. Leukogram revealed leukocytosis (neutrophilia, lymphocytosis, monocytosis and eosinophilia). Serum biochemistry revealed hyperalbuminemia and discrete hyperproteinemia; values of alanine aminotransferase, creatinine and globulins were within normal range. In cytological examination, intense cellularity was observed with predominance of eosinophils (60%), neutrophils (35%), macrophages performing cytophagocytosis (5%) and degenerated cells. There was no bacterial growth within 48 h after incubation of nasal bridge lesion swab. There were no abnormalities identified at radiographic evaluation of nasal turbinates. As the patient was already with antibiotic therapy and steroidal anti-inflammatory, it was opted to maintain it, since interruption between the day of examination and laboratory results could cause more prejudice than benefit, corticosteroid dose, however, was readjusted (prednisone 2 mg/kg/per os/every 24h). After 1 week of treatment the owner reported significant improvement of clinical signs without any further complaint. Discussion: Typically, type I hypersensitivity reactions such as insect bites do not exceed clinical signs of erythema, local edema and pruritus, with spontaneous remission of clinical signs within few hours after exposure to the antigen. Eosinophilic folliculitis, however, may cause more severe clinical alterations, such as pain, apathy and hyporexia. Nasal bridge is the predominant site described to be affected in cases of eosinophilic folliculitis, being auricular pinna, thorax and limbs considered atypical presentations which can delay proper diagnosis, since in endemic regions for diseases such as visceral leishmaniasis, infectious etiology may be listed first. Differential diagnosis also includes superficial pyoderma, juvenile cellulitis, pemphigus foliaceus and pharmacodermia. The case described in this report emphasize the importance of an accurate diagnosis as well as an early and adequate treatment in order to promote satisfactory response. Also, highlights inadequate use of antimicrobials as a direct consequence of lack of laboratorial investigation.

Resumo

Feline Plasmacytic Pododermatitis is an uncommon dermatological disorder that affects one or multiple paw pads and leads to swelling, ulceration, hemorrhage, scaling, erythema, and striation. Its etiopathogenesis is poorly understood, however, an immune-mediated basis is strongly suggested. A two-year-old neutered, mixed breed, male cat was referred to the Veterinary Clinic of the Federal University of Santa Catarina (UFSC), Curitibanos Campus, with a brief clinical history of progressive lesions on palmar and plantar pads for one month. On physical examination, the patient had enlarged popliteal and inguinal lymph nodes, and paw pads with thin, swollen, hyperemic, erythematous, and purple color skin with multiple white striations. Histopathological diagnosis confirmed plasmacytic pododermatitis, and glucocorticoid therapy with prednisolone was prescribed. In the present report, the patient's clinical follow-up was compromised, as the tutor did not perform the prescribed treatment and did not return with the animal for further evaluations.


A pododermatite plasmocitáriafelina é uma desordem dermatológica incomum que acomete os coxins plantares e/ou palmares, levando a edema, ulceração, hemorragia, descamação, eritema e estrias esbranquiçadas. Sua etiologia e patogenia são desconhecidas, mas acredita-se que tenha origem imunomediada. Foi atendido na Clínica Veterinária Escola da UFSC, CampusCuritibanos, um felino, macho, sem raça definida, 2 anos de idade, castrado, com histórico de lesões nos coxins palmares e plantares há um mês, com evolução progressiva. Ao exame físico apresentava-se com os linfonodos poplíteos e inguinais aumentados, além de coxins com pele fina, edemaciada, hiperêmica, com eritema, púrpura e estrias esbranquiçadas. O diagnóstico histopatológico confirmou pododermatite plasmocitária e foi prescrito tratamento à base de prednisolona. Não foi possível acompanhar a evolução clínica do paciente, pois o tutor não realizou o tratamento prescrito e não trouxe o animal para novas avaliações.

Resumo

Feline Plasmacytic Pododermatitis is an uncommon dermatological disorder that affects one or multiple paw pads and leads to swelling, ulceration, hemorrhage, scaling, erythema, and striation. Its etiopathogenesis is poorly understood, however, an immune-mediated basis is strongly suggested. A two-year-old neutered, mixed breed, male cat was referred to the Veterinary Clinic of the Federal University of Santa Catarina (UFSC), Curitibanos Campus, with a brief clinical history of progressive lesions on palmar and plantar pads for one month. On physical examination, the patient had enlarged popliteal and inguinal lymph nodes, and paw pads with thin, swollen, hyperemic, erythematous, and purple color skin with multiple white striations. Histopathological diagnosis confirmed plasmacytic pododermatitis, and glucocorticoid therapy with prednisolone was prescribed. In the present report, the patient's clinical follow-up was compromised, as the tutor did not perform the prescribed treatment and did not return with the animal for further evaluations.(AU)


A pododermatite plasmocitáriafelina é uma desordem dermatológica incomum que acomete os coxins plantares e/ou palmares, levando a edema, ulceração, hemorragia, descamação, eritema e estrias esbranquiçadas. Sua etiologia e patogenia são desconhecidas, mas acredita-se que tenha origem imunomediada. Foi atendido na Clínica Veterinária Escola da UFSC, CampusCuritibanos, um felino, macho, sem raça definida, 2 anos de idade, castrado, com histórico de lesões nos coxins palmares e plantares há um mês, com evolução progressiva. Ao exame físico apresentava-se com os linfonodos poplíteos e inguinais aumentados, além de coxins com pele fina, edemaciada, hiperêmica, com eritema, púrpura e estrias esbranquiçadas. O diagnóstico histopatológico confirmou pododermatite plasmocitária e foi prescrito tratamento à base de prednisolona. Background: Cryptococcosis is a serious fungal infection contracted by humans and animals, and the most common systemic mycosis found in cats. This disease is often contracted through inhalation of fungal propagules. The Central Nervous System (CNS) may be infected through local extension (nasal and frontal sinuses) or via hematogenous route. Similarly to CNS bacterial infection, the clinical signs of neurological dysfunction may be attributed to mass effect (gelatinous mass of fungal microorganisms and fungal granuloma formation) or to a more disseminated inflammatory response to invading microorganisms. The objective of this study is to report one case of a patient with cryptococcal granulomas in the central nervous system and one case of a patient with neurological signs associated to a cryptococcosis. Cases: Case 1. A 3-year-old male mixed breed feline was admitted to a veterinary clinic, located in Porto Alegre, RS, Southern Brazil. The patient presented unsourced behavioral changes, vestibular ataxia and dysphagia caused by inability of coordination. The following tests were performed: complete blood count test, biochemical analysis, computed tomography scan (CT scan), fluid analysis, radiography and toxoplasmosis test. The following medicine were administrated for treatment: fluconazole, dexamethasone, mannitol, phenobarbital and levetiracetam. Fluid therapy was also part of the treatment. Immediately after death, the cat was submitted for necropsy, and a fungal granulomatous meningoencephalomyelitis was diagnosed. Cryptococcus sp. was identified as the causal agent through pathological findings, fungal culture and PCR analysis. Case 2. One year later, another feline was admitted to the same clinic (a 2-year-old female mixed breed) presenting hypersalivation, tremors and excessive vocalization. Glicocorticoides são amplamente utilizados na clínica de pequenos animais, entretanto seu uso contínuo pode causar efeitos colaterais. Os gatos são considerados menos susceptíveis a esses efeitos do que outras espécies, mas existem poucos trabalhos abordando os efeitos adversos em felinos. O objetivo deste estudo foi avaliar possíveis alterações laboratoriais, histopatológicas e do grau de atenuação radiográfica do parênquima hepático de gatas submetidas à terapia com prednisolona. Um ensaio clínico foi realizado em quatro gatas hígidas, as quais receberam prednisolona, por via oral, na dose de 3mg/kg, durante 60 dias consecutivos. Nos achados histopatológicos após 60 dias de tratamento, observou-se desorganização dos cordões de hepatócitos e degeneração vacuolar, além de necrose de hepatócitos, porém não foram observados sinais de fibrose no parênquima hepático. Os dados da tomografia computadorizada demonstram aumento do grau de atenuação do parênquima hepático a partir do 30º dia da administração de prednisolona, que persistiu até o final do experimento. No presente estudo, foi possível caracterizar a existência de hepatopatia esteroidal em gatos em estágios precoces da terapia com prednisolona.(AU)

The objective of this study was to evaluate possible laboratory and histopathologic changes, as well as changes to the degree of radiographic attenuation of the hepatic parenchyma in cats treated with prednisolone. A clinical trial was done in four healthy cats, who received prednisolone orally at 3mg/kg during 60 consecutive days. In the histopathologic findings at 60 days of treatment, there were disorganized hepatocyte chords and vacuolar degeneration, as well as hepatocyte necrosis, however, there were no signs of fibrosis in the hepatic parenchyma. Data obtained via computed tomography showed increase of the degree of attenuation in the hepatic parenchyma from day 30 of prednisolone therapy, which persisted until the end of the experiment. In the present study, it was possible to characterize the existence of steroidal hepathopathy in cats in the early stages of prednisolone therapy.(AU)

Background: Bursa of Fabricius (BF) and the thymus are primary lymphoid organs of poultry and play a major role in avian immunity. Enteric system is also involved in immunity. Several pathologic conditions directly impact BF and thymus size, and also affect intestinal parameters. Besides, there are several immune system depressor agents which affect birds. The selection of glucocorticoid as inducer of immunosuppression is applied in many experiments; however there are few studies that are applied to the reality in the field. In this context, the aim of this study was to evaluate the effects of dexamethasone as an inducer of immunosuppression on lymphoid organs and microscopic structures of the jejunum.Materials, Methods & Results: One-day-old chicks were used as a control group (n = 8) and the treated group (n = 25) received intramuscular dexamethasone on 21, 23, 24 and 26 day-old. Control birds and treated birds were euthanized 8, 16, 24, 32 and 40 h after inoculation; four control birds and six treated birds were euthanized on the eighth day after the last inoculation. Thymus, BF and jejunum were collected during the necropsy. The selected organs were processed, stained with hematoxylin and eosin and photographed. The BF and thymus cuts were evaluated by three histopathologists to determine the depletion score. Ten villi of each jejunum were evaluated for width and length of villi, depth crypt, microvillus length, enterocyte length of each villus, and wall thickness. Treated birds presented a mean weight lower than control group during all the experiment. The mean weight and the relative weight of the BF and thymus of control birds were significantly higher than treated ones. The lymphocyte depletion in BF and thymus scores differed significantly between groups, being higher in the group challenged with dexamethasone. There were no significant differences between groups for depth of crypt, height of core and height of microvilli.[...]

The analysis of the health status of sentinel animals is a key element for the evaluation of ecosystem health conditions, since these animals respond to small and large scale changes in ecological factors and the quality of the environment. In the marine environment, the changes are systemic and accumulate impacts of coastal zones and oceanic activities, with aquatic mammals being species that reflect changes in biological and health parameters. For these animals several studies have reported an increase in the frequency and diversity of diseases, including infections by pathogens from the terrestrial environment. This increase may be related to impairment of the immune system activity of these organisms in response to potential synergistic factors. In cetaceans, immunosuppression may be caused by infection with viral agents, such as Morbillivirus, which induce severe lymphoid depletion and is responsible for several cases of mass mortality; chemical contamination, highlighting the organochlorines and trace elements (mainly mercury and cadmium); and even by chronic stress. Anthropic impacts are important stressors, and the consequences are more evident in animals of coastal habits, which leads to a constant release of glucocorticoid hormones and consequent lymphoid depletion, a mechanism similar to that occurring in terrestrial mammals. Immunosuppressed animals are susceptible to opportunistic diseases, some more severe and rare, with the risk of decline of populations. The objective of this review was to provide information on the aspects related to immunosuppression in cetaceans, associating etiological factors and pathological findings, and to highlight the relevance of the evaluation of the endocrine and immune system of marine animals as a reflection of the health status of marine ecosystems.(AU)


A análise do estado de saúde de animais sentinela é elemento-chave para avaliação das condições da saúde ecossistêmica, pois estes animais respondem a mudanças em pequena e larga escala em fatores ecológicos e da qualidade do ambiente. No ambiente marinho, as alterações são sistêmicas e acumulam impactos das zonas costeiras e de atividades oceânicas, sendo os mamíferos aquáticos espécies que refletem as mudanças em parâmetros biológicos e de saúde. Para estes animais diversos estudos vêm relatando um aumento da frequência e diversidade de doenças, incluindo infecções por patógenos provenientes do ambiente terrestre. Este aumento pode estar relacionado ao comprometimento da atividade do sistema imunológico destes organismos em resposta à potenciais fatores sinérgicos. Em cetáceos, a imunossupressão pode ter como causas a infecção por agentes virais, como o Morbillivirus, que induz severa depleção linfoide e é responsável por diversos casos de mortalidade em massa; a contaminação química, destacando os organoclorados e elementos traço (principalmente o mercúrio e o cádmio); e mesmo pelo estresse crônico. Impactos antrópicos são importantes agentes estressores, sendo mais evidentes as consequências em animais de hábitos costeiros, os quais acarretam uma constante liberação de hormônios glicocorticoides e consequente depleção linfoide, mecanismo semelhante ao que ocorre nos mamíferos terrestres. Os animais imunossuprimidos são vulneráveis às doenças oportunistas, algumas mais severas e raras, com risco de dizimar populações. O objetivo desta revisão foi compilar informações sobre os aspectos relacionados à imunossupressão em cetáceos, associando fatores etiológicos e achados patológicos, e destacar a relevância da avaliação do sistema endócrino e imunológico em animais marinhos como reflexo do estado de saúde dos ecossistemas marinhos.(AU)

The idiopathic hypereosinophilic syndrome (IHES) is a rare disease, characterized by variable eosinophilia and its massive infiltration into various organs. This study aimed to report clinical-laboratory findings and therapy in a canine with IHES. A one-year, 10-month-old male Rottweiler dog had a history of emesis, weight loss, hyporexia, and persistent eosinophilia on prior hematological exams. Vaccinations and worming were up to date. A complete blood count, serum biochemistry, bone marrow cytology, serology for leishmaniasis, chromatographic immunoassay for the detection of Dirofilaria immitis antigen, and abdominal ultrasound were requested. The tests for infectious diseases were negative. Blood biochemistry revealed no significant changes. An intense eosinophilia was observed in the hematology. A large number of cell precursors of the eosinophilic lineage were detected in the bone marrow cytology. Abdominal ultrasound showed thickening of intestinal loops. Considering the clinical and laboratory findings, the diagnosis of IHES was defined. Prednisolone treatment was instituted. The recurrence of peripheral eosinophilia occurred on the 35th day after therapy initiation. At that time, we opted to suspend the use of prednisolone and indicate the administration of deflazacort. With follow-up, therapeutic success with deflazacort was demonstrated, promoting the complete regression of clinical and ultrasound signs. The last glucocorticoid was maintained, but with a gradual dose reduction. The recognition of clinical and laboratory manifestations related to canine IHES is essential to establish an adequate diagnosis and therapy. Deflazacort emerges as a promising drug for controlling this disease.

Resumo

Antivenoms or antitoxins have been effectively used for more than a century. During this time, these products have always proven to be highly effective in the treatment of infections and envenomations. However, antivenoms did not exhibit good safety results in their initial applications. After many improvements, antivenoms have substantially better safety profiles but still have some side effects. Due to the occurrence of adverse reactions, the practice of using premedication with the intent to decrease side effects has become accepted or mandatory in many countries. The drugs used for premedication belong to the histamine H1 antagonist, glucocorticoid and catecholamine groups. Currently, this practice is being questioned due to low or controversial efficacies in clinical assays. In this article, we discuss the causes of adverse reactions, the mechanisms of drugs that block the undesired effects and the results obtained in clinical trials. Although these three families of drugs could have positive effects on reducing adverse reactions, only adrenaline has demonstrated positive results in clinical assays.(AU)
