Tricomoníase feminina no sul do Brasil: relação com HIV, fatores de risco e métodos diagnósticos

Resumo
A tricomoníase é uma infecção sexualmente transmissível curável, causada pelo protozoário Trichomonas vaginalis. No Brasil, a prevalência varia de 1,9% a 37,5%, dependo da população estudada e das técnicas de diagnóstico empregadas. A infecção pelo protozoário está associada a diversas complicações de saúde reprodutiva, incluindo o aumento à exposição ao Vírus da Imunodeficiência Humana (HIV). Os objetivos deste estudo foram determinar a frequência e os preditores de risco da infecção por T. vaginalis, entre as mulheres HIV positivas e negativas, atendidas no Ambulatório de Ginecologia e Obstetrícia da Faculdade de Medicina da Universidade Federal de Pelotas (UFPel), correlacionando com os níveis de carga viral e contagem de linfócitos CD4, além de comparar a sensibilidade e especificidade do exame direto e do cultivo in vitro, utilizados no diagnóstico. Foi realizado um estudo transversal com 267 mulheres, durante os meses de janeiro a dezembro de 2015. As pacientes foram entrevistadas e as amostras foram colhidas para detecção de T. vaginalis através da cultura em meio Diamond e exame a fresco. Esfregaços com conteúdo vaginal foram coletados e corados pela técnica de Gram. A prevalência da tricomoníase entre as mulheres estudadas foi de 6,4%. Entre as mulheres HIV positivas, a prevalência foi de 3,9% e nas HIV negativas, foi de 7,9%, dados estatisticamente similares (p0,05). Foram fatores de risco significativos para a infecção, o tabagismo (OR= 3,52), o prurido vaginal (OR=4,43) e a presença de vaginose bacteriana (OR= 5,29). Adicionalmente, foi constatado elevado índice de desconhecimento (90,3%) sobre a tricomoníase na população estudada. Do total de infectadas, verificou-se que 17,6% eram assintomáticas. Os sinais e sintomas mais frequentes foram corrimento vaginal, prurido e odor fétido. O status HIV, bem como o nível de CD4 e carga viral não foram associados à infecção pelo protozoário entre mulheres HIV soropositivas. Em relação à comparação entre técnicas, verificou-se que o exame a fresco, usado como rotina, detectou 47,05% das infectadas. A prevenção da tricomoníase, assim como outras DSTs, na população, é uma grande iniciativa, que pode ser feita, entre outras medidas, através da orientação em saúde pública.Trichomoniasis is an curable sexually transmitted infection caused by the protozoan Trichomonas vaginalis. The protozoan infection is associated with several reproductive health complications, including increased exposure to Human Immunodeficiency Virus (HIV). The objectives of this study were to determine the frequency and risk predictors of infection with T. vaginalis, between the positive and HIV negative women seen at the Clinic of Obstetrics and Gynecology, School of Medicine, Federal University of Pelotas (UFPel), correlating with levels viral load and CD4 lymphocyte count and to compare the sensitivity and specificity of the wet mount and in vitro culture, used in the diagnosis. A cross-sectional study of 267 women during the months from January to December 2015. The patients were interviewed and samples were collected for detection of T. vaginalis by culture through Diamond and wet mount was carried out. Smears with vaginal content were collected and stained by the Gram technique. The prevalence of trichomoniasis among women studied was 6.4%. Among HIV positive women, the prevalence was 3.9% and the negative HIV was 7.9%, statistically similar data (p = 0.05). There were significant risk factors for infection, smoking (OR = 3.52), vaginal itching (OR = 4.43) and the presence of bacterial vaginosis (OR = 5.29). Additionally, it was found high unfamiliarity index (90.3%) of trichomoniasis in the population studied. Of total infected, it was found that 17.6% were asymptomatic. The most common signs and symptoms were vaginal discharge, itching and fetid odor. HIV status and CD4 levels and viral load were not associated with protozoan infection among women HIV seropositive. Regarding comparison techniques, it was found that the wet mount, used routinely detected only 47.05% of infected. Prevention of trichomoniasis, and other STDs in the population, is a major initiative that can be made, among other measures through the public health orientation.
