Avaliação dos efeitos in vitro do Dimetilsulfóxido em células sinoviais de equinos e de sua utilização intra-articular como tratamento para sinovite aguda

Resumo

As lesões articulares na espécie equina causam inúmeras perdas econômicas e funcionais, com objetivo de minimizá-las vários estudos sobre técnicas e terapias são realizadas e se fazem necessárias. Acredita-se que o dimetilsulfóxido (DMSO) tenha ação moduladora nos processos inflamatórios articulares agudos e ação sobre radicais livres, porém sua eficácia ainda é dúbia. Este estudo teve como objetivo avaliar os efeitos do DMSO sobre células sinoviais e a lavagem articular com diferentes concentrações de DMSO (5% e 10%), em um modelo experimental de indução da sinovite aguda em equinos, com LPS. Para tanto, este estudo foi dividido em duas etapas. A primeira etapa visou verificar o efeito do DMSO sobre células sinoviais, através da análise de viabilidade celular após a estimulação das células sinoviais pelo LPS e tratamento com o DMSO. Para a segunda etapa do estuo foram utilizadas 26 articulações tibiotársicas, distribuídas aletoriamente em quatro grupos. Os grupos foram tratados e organizados da seguinte forma: grupo solução de ringer lactato; grupo DMSO 5% em solução de ringer lactato; grupo DMSO 10% em solução de ringer lactato; e Grupo controle (apenas indução da sinovite). Foram realizadas coletas do líquido sinovial (LS) antes da infiltração das articulações com LPS, na dose de 0,5 ng (M0), após 1h (M1), antes das lavagens (M8), e após 24h e 48h (M24 e M48) da indução da sinovite, as amostras de LS foram utilizadas para contagem total de células nucleadas e diferenciação celular, quantificação de PGE2, interleucina-1, interleucina-6, TNF-, AH, CS, ureia, proteína total e análise do burst oxidativo. As células sinoviais estimuladas por LPS e tratadas com DMSO 5% apresentaram menor porcentagem de células viáveis após vinte e quatro horas, entretanto após 48h somente as células tratadas com ringer lactato apresentaram menor porcentagem de células viáveis. O equipamento Lameness Locator® foi utilizado para verificar a presença de dor articular, previamente a todas coletas. Todos os grupos apresentaram aumento na contagem total de células nucleadas, concentração de PGE2, IL-1, IL-6 e proteína total no M8. Sendo que os grupos DMSO 5% e DMSO 10% apresentaram diminuição da contagem total de células nucleadas após a lavagem articular. A concentração de PGE2 e IL-1 no grupo DMSO 10% foi menor no M48 em relação ao M8, e o grupo ringer lactato apresentou menor concentração de IL-1 que os grupos DMSO 5% e controle no M24. O grupo DMSO 10% apresentou menor concentração de IL-6 em relação aos grupos DMSO 5% e ringer lactato no momento 24. O TNF- apresentou aumento em todos os grupos no M1. Os grupos DMSO 10% e controle apresentaram menor concentração de TNF- nos M24 e M48 em relação ao M1. Os grupos DMSO 5% e controle não apresentaram diferença na concentração de IL-10, porém os grupos ringer lactato e DMSO 10% apresentaram maior concentração no M48. Os grupos DSMO 10% e ringer lactato apresentaram menor concentração de AH no M24. Todos os grupos apresentaram aumento da concentração de CS após as lavagens articulares. Não houve diferença na avaliação de dor articular, bem como na análise do burst oxidativo e concentração de ureia. Os diferentes grupos apresentaram similaridade de comportamento das células e citocinas estudadas no líquido sinovial ao longo do experimento, contudo o grupo DMSO 10% foi o que apresentou mais rapidamente o retorno próximo aos valores basais. Os resultados apresentados neste estudo permitem concluir que frente à necessidade de se realizar lavagens articulares a utilização de solução de ringer lactato com DMSO 10% é mais indicada que o uso de ringer lactato ou solução com DMSO a 5%. Entretanto, nenhum tratamento foi capaz de demonstrar efeito condroprotetor.
Joint injuries in the equine species cause numerous economic and functional losses, in order to minimize them, several studies on techniques and therapies are performed and are necessary. Dimethylsulfoxide (DMSO) is thought to have modulating action on acute joint inflammatory processes and action on free radicals, but its efficacy is still dubious. This study aimed to evaluate the effects of DMSO on synovial cells and joint lavage with different concentrations of DMSO (5% and 10%) in an experimental model of induction of acute synovitis in horses with LPS. Therefore, this study was divided into two steps. The first step aimed to verify the effect of DMSO on synovial cells by analyzing cell viability after stimulation of synovial cells by LPS and treatment with DMSO. For the second stage of the study, 26 tibiotarsal joints were used, divided into four groups. The groups were treated and organized as follows: ringer lactate solution group; 5% DMSO group in ringer lactate solution; 10% DMSO group in ringer lactate solution; and Control group (synovitis induction only). Synovial fluid (LS) was collected prior to infiltration of the joints with LPS at a dose of 0.5 ng (M0), after 1h (M1), before washes (M8), and after 24h and 48h (M24 and M48), from synovitis induction. LS samples were used for total nucleated cell count and cell differentiation, PGE2 quantification, interleukin-1, interleukin-6, TNF-, HA, CS, urea, total protein and burst oxidative analysis. The synovial cells stimulated by LPS and treated with DMSO 5% presented lower percentage of viable cells-after 24 hours, however after 48h only the cells treated with Ringer lactate showed lower percentage of viable cells. Lameness Locator® equipment was used to verify the presence of joint pain, prior to all collections. All groups showed an increase in total nucleated cell count, PGE2, IL-1, IL-6 and total protein in M8. The DMSO 5% and DMSO 10% groups showed a decrease in the total nucleated cell count after joint washing. The concentration of PGE2 and IL-1 in the 10% DMSO group was lower in M48 compared to M8, and the ringer lactate group presented lower IL-1 concentration than the 5% DMSO and control groups in M24. The 10% DMSO group showed lower IL-6 concentration compared to the 5% DMSO and ringer lactate groups at time 24. TNF- increased in all groups at M1. The 10% DMSO and control groups presented lower TNF- concentration in M24 and M48 in relation to M1. The 5% DMSO and control groups showed no difference in IL-10 concentration, but the ringer lactate and 10% DMSO groups showed higher concentration in M48. The DSMO 10% and ringer lactate groups presented lower HA concentration in M24. All groups showed increased concentration of CS after joint washes. There was no difference in joint pain evaluation, oxidative burst analysis and urea concentration. The different groups showed similarity of behavior of cells and cytokines studied in the synovial fluid throughout the experiment, however the 10% DMSO group showed the fastest return near basal values. The results presented in this study allow us to conclude that in view of the need to perform joint washes, the use of 10% DMSO lactated ringer's solution is better than the use of 5% DMSO lactated ringer's solution. However, no treatment was able to demonstrate chondroprotective effect
