AVALIAÇÃO DO EFEITO DO DECANOATO DE NANDROLONA EM LESÃO MEDULAR EXPERIMENTAL EM RATOS

Resumo

O trauma medular agudo é causa comum de disfunção neurológica em animais que compromete as funções motora, sensitiva e autonômica, podendo resultar em incapacidade permanente. O decanoato de nandrolona (DN) é um esteroide amplamente estudado por seu efeito anabólico predominante e baixo potencial androgênico. É descrito como estimulador de crescimento e promotor de resistência celular em diversos tecidos, inclusive o neurológico. Objetivos: Avaliar o efeito do DN em ratos submetidos ao trauma medular agudo. Materiais e métodos: Foram utilizados 32 ratos (Rattus novergicus, linhagem Wistar), adultos jovens, com peso entre 240 e 260 gramas, divididos em três grupos: grupo submetido à lesão medular aguda e tratamento (GNAN) (n = 13); grupo controle submetido à lesão medular sem tratamento (GCON) (n = 13); o terceiro grupo submetido à laminectomia sem lesão medular (GLAM) (n = 6), para controle de alterações decorrentes do procedimento. Para a produção da lesão medular utilizou-se um aparato de metal com liberação de peso de 20g de 25 cm de altura. Após exposição do canal medular uma haste de metal com diâmetro de 2mm era posicionada em contato com a medula, e ao liberar o peso, a haste era atingida e promovia a lesão medular. Após produção da lesão, administrou-se DN na dose de 2 mg/Kg, via intramuscular após o procedimento cirúrgico. Os animais foram avaliados quanto à recuperação da função motora em cinco momentos: 24 h, 48 h, 72 h, 7 e 14 dias pós-lesão medular. No 14º dia realizou- se eutanásia e coleta dos fragmentos da medula espinhal e bexiga urinária para avaliação histológica. Resultados e Discussão: Os animais submetidos à lesão medular apresentaram paraplegia, não pontuando na escala BBB. A partir do sétimo dia pós-lesão observou-se melhora locomotora gradativa nos grupos com variações na escala. Após lesão medular, todos os animais manifestaram retenção urinária. O retorno da função ocorreu em média no quinto dia pós-lesão, não havendo diferença significativa entre os grupos. A bexiga urinária foi avaliada qualitativamente quanto a presença de infiltrado inflamatório e hemorragia. Os critérios histológicos avaliados nos segmentos medulares foram hemorragia, cromatólise, necrose neuronal, astrocitose e astrogliose, células de Gitter, leuco e poliomielomalácia. A avaliação locomotora dos animais submetidos à lesão medular aguda revelou variações quanto a intensidade da lesão nos animais do GNAN e GCON. O tempo foi fator determinante na evolução clínica dos animais, sem evidências da influência do DN. As avaliações histológicas medulares apresentaram variações quanto à intensidade da lesão, com tendência de menor intensidade nos segmentos cranial e epicentro da lesão dos animais submetidos ao tratamento com DN, porém não constatou-se evidências estatísticas significativas. As avaliações histopatológicas da bexiga urinária demonstraram a existência de resposta inflamatória caracterizada cistite linfohistiocitária e neutrofílica, nos animais do GNAN e GCON, sem interferência do DN quanto à presença da alteração. Conclusões: O método de lesão medular utilizado reproduziu de maneira heterogênea, incapacidade funcional, sensorial e motora grave em ratos. O DN na posologia avaliada não influenciou de forma significativa no retorno da função locomotora e na intensidade das alterações medulares histopatológicas.
Spinal cord injury is a common cause of neurological dysfunction in animals that compromises motor, sensory and autonomic functions and may result in permanent disability. Nandrolone decanoate (DN) is a steroid widely studied for its predominant anabolic effect and low androgenic potential. It is described as a growth promoter and promoter of cell resistance in several tissues, including the neurological. Objectives: To evaluate the effect of DN in rats submitted to spinal cord injury. Materials and methods: Thirty-two rats (Rattus novergicus, Wistar line), young adults, weighing between 240 and 260 grams, were divided into three groups: group submitted to acute spinal cord injury and treatment (GNAN) (n = 13); control group submitted to untreated spinal cord injury (GCON) (n = 13); the third group submitted to laminectomy without spinal cord injury (GLAM) (n = 6), to control procedural alterations. For the production of the spinal cord injury, a 20 g 25 cm high metal release device was used. After exposure of the medullary canal a metal rod with a diameter of 2 mm was placed in contact with the bone marrow, and when releasing the weight, the stem was reached and promoted the spinal cord injury. After injury, 2 mg / kg of DN was administered intramuscularly after the surgical procedure. The animals were evaluated for motor function recovery in five moments: 24 h, 48 h, 72 h, 7 and 14 days after spinal cord injury. On the 14th day, euthanasia and collection of the fragments of the spinal cord and urinary bladder were performed for histological evaluation. Results and Discussion: The animals submitted to the spinal cord injury presented paraplegia, not scoring on the BBB scale. From the seventh postoperative day, there was gradual locomotive improvement in the groups with variations in the scale. After spinal cord injury, all animals showed urinary retention. The return of the function occurred on average on the fifth post-injury day, with no significant difference between the groups. The urinary bladder was evaluated qualitatively for the presence of inflammatory infiltrate and hemorrhage. The histological criteria evaluated in the spinal segments were hemorrhage, chromatolysis, neuronal necrosis, astrocytosis and astrogliosis, Gitter cells, leuco and poliomyelomalacia. The locomotor evaluation of the animals submitted to the acute medullary lesion revealed variations in the lesion intensity in the GNAN and GCON animals. The time was a determining factor in the clinical evolution of the animals, without evidence of the influence of DN. The medullary histological evaluations presented variations regarding the intensity of the lesion, with a tendency of less intensity in the cranial segments and epicenter of the lesion of the animals submitted to the treatment with DN, but no significant statistical evidences were verified. Histopathological evaluations of the urinary bladder demonstrated the presence of an inflammatory response characterized by lymphohistiocytic and neutrophilic cystitis, in the animals of GNAN and GCON, without interference of DN in the presence of the alteration. Conclusions: The spinal cord injury method used heterogeneously reproduced functional, sensory and motor incapacity in rats. The DN in the dosage evaluated did not significantly influence the return of the locomotor function and the intensity of histopathological changes in the spinal cord.
