HOMEOPATIA NA ONCOLOGIA EXPERIMENTAL: revisão sistemática

Resumo

De acordo com a Organização Mundial da Saúde (OMS), em 2020, o número estimado de novos casos de câncer aumentará para 16 milhões, sendo 10 milhões de mortes esperadas. Em alguns países, como Índia, a homeopatia tem sido utilizada como terapia complementar e alternativa para o tratamento de diferentes tipos de células tumorais. Muitos aspectos ligados à atividade biológica das altas diluições vêm sendo demonstrados por diferentes grupos de pesquisa, mas os efeitos desses medicamentos em células tumorais ainda permanecem pouco elucidados. Objetivos: A meta foi avaliar crítica e sistematicamente os aspectos metodológicos e os resultados obtidos, itens fundamentais na elucidação dos mecanismos de ação das formulações homeopáticas usadas em Oncologia. Método: Foram selecionados 91 artigos originais, publicados entre 2002 e 2018, escritos na língua inglesa e indexados no banco de dados do PubMed. Para a análise sistemática, os critérios de exclusão qualitativos foram aplicados em uma segunda seleção, de onde restaram 22 artigos para análise. Os critérios de seleção foram: 1) artigos de revisão; 2) artigos de avaliação clínica em humanos ou veterinária, 3) artigos que não estivessem na plataforma PubMed, mais antigos que 2002 ou que não estivessem na língua inglesa. A análise crítica dos artigos selecionados seguiu os critérios descritos pelo PRISMA. Resultados: Na análise sistemática dos trabalhos selecionados, observou-se que a Ásia é o continente com maior número de artigos e autores, com especial foco na Índia, onde a prática clínica é bastante consolidada e institucionalizada. Alguns dos artigos selecionados mostram aspectos inovadores do ponto de vista metodológico e podem ser ponto de partida para novas linhas de pesquisa. Nota-se, em função do tempo, a melhoria progressiva da qualidade e do rigor metodológico dos trabalhos, o que permite discernir quais são os fatores mais e menos importantes para o resultado final. Conclusão: A revisão sistemática dos artigos selecionados, submetidos a critérios de inclusão e exclusão e à análise minuciosa da metodologia, foi uma maneira eficaz de se observar a confiabilidade dos dados disponíveis na literatura, além de revelar aspectos importantes para o aprimoramento de pesquisas futuras na área, sobretudo quanto ao refinamento metodológico e ao impacto dos dados obtidos na orientação de políticas públicas envolvendo o uso da homeopatia como terapia complementar na oncologia.
According to the World Health Organization (WHO), in 2020 the estimated number of new cases of cancer will increase to 16 million, with 10 million expected deaths. In some countries, such as India, homeopathy has been used as complementary and alternative therapy for the treatment of different types of tumor cells. Many aspects related to the biological activity of high dilutions have been demonstrated by different research groups, but the effects of these drugs on tumor cells remain poorly elucidated. Objectives: The goal was to evaluate, critically and systematically, the methodological aspects and the results obtained, fundamental items in the elucidation of the mechanisms of action of homeopathic formulations used in oncology. Methods: 91 original articles were selected, being all published between 2002 and 2018, written in the English language and indexed in the PubMed database. For the systematic analysis, the qualitative exclusion criteria were applied in a second selection, from which 22 articles were left for analysis. The exclusion criteria were: 1) review articles; 2) human or veterinary clinical evaluation articles, 3) articles that were not on the PubMed platform, older than 2002 or were not written in the English. The critical analysis of the selected articles followed the criteria described by "PRISMA". Results: In the systematic analysis of the selected papers, it was observed that Asia is the continent with the greatest number of articles and authors, with a focus on India, where clinical practice is well established and institutionalized. Some of the selected articles show innovative aspects from a methodological point of view and may be the starting point for new lines of research. It is possible to observe the progressive improvement of the quality and the methodological accuracy of the work, which allows to discern which are the most (and less) important methodological factors to reach sound conclusions. Conclusion: The systematic review of selected articles, submitted to inclusion and exclusion criteria and to the detailed analysis of the methodology is an effective way of observing the reliability of the data available in the literature, besides revealing important aspects for the improvement of future studies in the area, regarding the methodological refinement and the impact of the obtained data in the orientation of public policies involving the use of homeopathy as complementary therapy in oncology.
