TEMPLATING DE HASTES PROTÉTICAS PARA ARTROPLASTIA TOTAL DE QUADRIL CANINA EM RADIOGRAFIAS OBTIDAS POR DIFERENTES DECÚBITOS ESTUDO EX-VIVO

Resumo

A artroplastia total do quadril representa a melhor opção para a restauração da movimentação articular e alívio de dor nos quadros de doença articular degenerativa em cães. O planejamento cirúrgico apresenta papel crucial na prevenção de falhas e consequente aumento dos índices de sucesso. Objetivou-se, no presente estudo, comparar o planejamento pré-operatório (templating) para aplicação da haste femoral protética utilizando a projeção radiográfica craniocaudal obtidas do decúbito ventrodorsal ou lateral em cães. Foram utilizados 45 fêmures provenientes de 23 cães submetidos à eutanásia por motivos diversos. O templating foi realizado por 3 avaliadores com prévia experiência em ATQ e os dados foram submetidos à análise não paramétrica, e comparados pelo teste de Kruskal-Wallis e pelo teste de Friedman, retirando-se o efeito da variação entre os observadores como bloco. O dimensionamento das próteses variou de acordo com a experiência do avaliador, sem correlações estatísticas entre as projeções testadas. Quando retirado o efeito interavaliadores os valores de preenchimento do canal medular femoral nas projeções radiográficas testadas não apresentaram diferenças estatísticas. O preenchimento femoral está diretamente relacionado ao tamanho da haste femoral protética, portanto, é possível inferir que o posicionamento lateral apresenta uma alternativa de posicionamento radiográfico para o planejamento do artroplastia total de quadril. Não foi possível definir um modelo de haste ou projeção superior por divergências encontradas em dados literários.
Total hip arthroplasty has been an effective option to restore joint junction. Preoperative planning plays an important role at preventing failures and, consequently, increasing success rates. The aim of this study was to compare the preoperative templating for femoral stem application using craniocaudal radiographic projection obtained from ventrodorsal or lateral positioning in dogs. Forty-five femora from 23 euthanized dogs were used. The templating was performed by 3 experienced veterinary surgeons. The data was submitted to a non-parametric analysis and were compared by the Kruskal-Wallis test and the Friedman test, removing the effect of the variation among the observers as a block. The size of femoral stems varied according to the experience of the evaluator, however, when comparing the femoral filling between the radiographic projections, there was no statistical difference. The femoral filling is directly related to the femoral stem size, so it is possible to infer that the lateral positioning presents an alternative of radiographic positioning for the planning of the total hip arthroplasty in dogs. It was not possible to define a superior model or projection due to discrepancies between literary data.
