Avaliação da técnica de termografia por infravermelho para diagnóstico e acompanhamento clínico de alterações articulares crônicas em cães

Resumo

Esta pesquisa teve por objetivo a avaliação da termografia por infravermelho para o diagnóstico e acompanhamento clínico de alterações articulares crônicas em cães. Para o estudo foram selecionados 31 animais, de diversas raças, pesos e idades, divididos em 2 grupos: cães hígidos (N=17) ou com Doença Articular Degenerativa (DAD; N=14). Todos os animais passaram por avaliação clínica que incluiu anamnese, exame físico geral e específico do sistema osteoarticular. Adicionalmente, exames complementares de imagem foram empregados para confirmação do diagnóstico definitivo no grupo de animais com DAD. Todos os cães foram submetidos à perimetria dos membros torácicos e pélvicos, goniometria das articulações úmero-radio-ulnares, rádio-cárpicas, fêmur-tibio-patelares e tíbio-társicas, tanto em flexão quanto em extensão, além da análise termográfica de todas as articulações (FLIR® E40, EUA). Os dados gerados foram submetidos à análise estatística descritiva, além de análise de variância e teste de correlação para determinação das possíveis associações entre os achados do exame clínico ortopédico e os resultados do termograma articular. Não foram observadas variações significativas para a angulação dos membros torácicos comparados aos animais hígidos e portadores de DAD;, no entanto, a capacidade de flexão (P=0,0453) e extensão (P=0,0058) da articulação tíbio-társica direita e flexão da articulação tíbio-társica esquerda (P=0,0354) foram influenciadas pela existência de doença articular prévia. Não foram observadas diferenças significativas para a temperatura das diferentes articulações pesquisadas quando comparados os termogramas de animais hígidos e portadores de DAD, exceto para a mensuração termográfica da face cranial da articulação rádio-cárpica esquerda, que apresentou temperatura mais baixa (28,57±0,82) para animais com distúrbios articulares (p=0,0121) enquanto para animais saudáveis foi de 30,88±0,38. Frente aos achados, concluiu-se que a termografia por infravermelho não representou uma técnica eficiente para o diagnóstico e identificação de doenças articulares crônicas em cães.
This research aimed to evaluate infrared thermography for the diagnosis and clinical monitoring of chronic joint changes in dogs. For the study, 31 animals were selected, of different breeds, weights and ages, divided into 2 groups: healthy dogs (N = 17) or with Degenerative Joint Disease (DJD) (N = 14). All animals underwent clinical evaluation that included anamnesis, general and specific physical examination of the osteoarticular system. Additionally, complementary imaging tests were used to confirm the definitive diagnosis in the group of animals with DAD. All dogs underwent thoracic and pelvic limb perimetry, humeri-radio-ulnar, radio-carpal joints, tibio-patellar and tibio-tarsal joints, both in flexion and extension, in addition to thermographic analysis of all joints (FLIR® E40, USA). The data generated were subjected to descriptive statistical analysis, in addition to analysis of variance and correlation test to determine the possible associations between the findings of the orthopedic clinical examination and the results of the joint thermogram. No significant variations were observed for the angulation of the thoracic limbs when comparing healthy animals with DAD; however, the flexion capacity (P = 0.0453) and extension (P = 0.0058) of the right tibio-tarsal joint and flexion of the left tibio-tarsal joint (P = 0.0354) were influenced by the existence of disease previous articulation. No significant differences were observed for the temperature of the different joints studied when comparing the thermograms of healthy animals with DAD, except for the thermographic measurement of the cranial face of the left radio-carpal joint, which had a lower temperature (28.57 ± 0,82) for animals with joint disorders (p = 0.0121) while healthy dogs were 30,88±0,38. In conclusion, infrared thermography was not efficient in the assessing of chronic joint diseases in dogs.
