Análise dos Fluxos de Profilaxia Antirrábica e da Participação do Médico Veterinário no Acompanhamento Animal em Mordeduras Caninas na Fronteira Sul do Brasil

Resumo

O protocolo de prevenção da raiva humana pós mordedura orienta a observação dos cães e gatos por 10 dias após a agressão. Neste contexto, a atuação do médico veterinário junto à vigilância em saúde é determinante no protocolo da Agência Nacional de Vigilância Sanitária para o acompanhamento do animal e tratamento antirrábico humano. O estudo teve como objetivo verificar se existe padronização de conduta de profilaxia antirrábica pós exposição entre os estados da região Sul e se a presença efetiva do médico veterinário na saúde pública interfere no tipo de conduta adotada. O estudo avaliou municípios com população entre 10 a 50 mil habitantes de três regionais de saúde: Frederico Westphalem/RS com cinco municípios; Extremo Oeste 2/SC com sete municípios e Francisco Beltrão/PR com nove municípios. Para cada município foram enviados questionários com perguntas que avaliaram a atuação do médico veterinário junto à vigilância epidemiológica. Acessando as fichas do Sistema de Informação de Agravos de Notificação do ano de 2019 foram obtidos dados referentes ao tipo de tratamento pós exposição: Observação, Vacina, Observação + Vacina, Soro + Vacina ou Soro. Para análise estatística dos dados foi utilizada a análise descritiva e aplicado o método Qui-quadrado. Dentre os municípios pesquisados 93% enviaram respostas. Em 77% do total de casos notificados, a conduta foi observação do animal agressor, respectivamente RS com 74%, SC 83% e PR 76%. Sobre o acompanhamento do animal, os municípios responderam que em 64% das vezes não é realizado por médico veterinário, podendo ser outro profissional da saúde, ou mesmo o dono do animal. Apenas cinco municípios informaram dispor de médico veterinário para acompanhamento do animal, quatro no PR e um em SC. Paraná e SC foram os únicos a dispor de ficha de acompanhamento do animal. Comparando os estados, houve diferença significativa (p<0,05) para a maioria das condutas. Dispensa de tratamento em SC foi inferior ao PR e RS. As condutas de observação do animal e de vacina foram maiores no PR, e observação + vacina foi superior no PR e RS. Não houve diferença entre estados para soro+vacina. Apesar de não ser possível comprovar estatisticamente, sugere-se que o predomínio da conduta de observação do animal esteja relacionado à presença de um maior número de médicos veterinários no PR e da existência da ficha de acompanhamento em SC. Foi possível concluir que não existem padronizações de condutas com rotinas e protocolos, bem como fichas de acompanhamento para os estados da região Sul do Brasil, nem mesmo entre municípios do mesmo estado. Os municípios de pequeno porte da região Sul possuem deficiência de profissionais Médicos Veterinários para o acompanhamento do animal agressor.
The post-bite human rabies prevention protocol guides the observation of dogs and cats for 10 days after the aggression. In this context, the role of the veterinarian with health surveillance is crucial in the protocol of the National Health Surveillance Agency for the monitoring of the animal and human anti-rabies treatment. The study aimed to verify if there is a standardization of post-exposure rabies prophylaxis conduct among the states of the South region and if the effective presence of the veterinarian in public health interferes with the type of conduct adopted. The study evaluated municipalities with a population between 10 and 50 thousand inhabitants from three health regions: Frederico Westphalem/RS with five municipalities; Far West 2/SC with seven municipalities and Francisco Beltrão/PR with nine municipalities. Questionnaires were sent to each municipality with questions that evaluated the performance of the veterinarian with the epidemiological surveillance. By accessing the 2019 Notifiable Diseases Information System forms, data were obtained regarding the type of post-exposure treatment: Observation, Vaccine, Observation + Vaccine, Serum + Vaccine or Serum. For statistical analysis of the data, descriptive analysis was used and the Chi-square method was applied. Among the cities surveyed, 93% sent answers. In 77% of all reported cases, the conduct was observation of the aggressor animal, respectively RS with 74%, SC 83% and PR 76%. Regarding the monitoring of the animal, the municipalities responded that in 64% of the time it is not carried out by a veterinarian, and it may be another health professional, or even the owner of the animal. Only five municipalities reported having a veterinarian to monitor the animal, four in PR and one in SC. Paraná and SC were the only ones to have an animal follow-up form. Comparing the states, there was a difference significant (p<0.05) for most conducts. Treatment dispensation in SC was inferior to PR and RS. The behaviors of animal observation and vaccine were higher in PR, and observation + vaccine was higher in PR and RS. There was no difference between states for serum+vaccine. Although it is not possible to prove it statistically, it is suggested that the predominance of the animal observation behavior is related to the presence of a greater number of veterinarians in PR and the existence of a follow-up form in SC. It was possible to conclude that there is no standardization of conduct with routines and protocols, as well as follow-up sheets for the states of the southern region of Brazil, not even between municipalities in the same state. Small municipalities in the South region have a shortage of Veterinary Professionals to monitor the aggressor animal.
