OBJECTIVE: To determine the prevalence of allergic conjunctivitis in children in selected schools in Karachi, Pakistan. METHODS: This cross-sectional study was conducted in September and October 2008 as part of the School Eye Health Initiative launched by the Aga Khan University, Karachi and the Adamjee Eye Hospital, Karachi. Children in the selected schools underwent vision assessment and a slit lamp examination by an ophthalmologist. The main objective was to detect allergic conjunctivitis which was ascertained by the presence of papillae in the upper tarsal conjunctiva, redness of the eyes, and presence or history of itching and burning. RESULT: A total of 818 children aged 5-19 years were examined. Of these, 19.2% (95% CI 16.5% to 21.9%) had allergic conjunctivitis. There was a significant association between increasing age and allergic conjunctivitis (Odds ratio: 1.19, 95% CI: 1.12, 1.26; P < 0.001). Boys had a higher burden of allergic conjunctivitis than girls; however this difference was not statistically significant (Odds ratio: 1.31, 95% CI: 0.90, 1.91, p = 0.153). CONCLUSION: There is a very high prevalence of allergic conjunctivitis in children in the selected schools in Karachi. Further research is needed to identify factors that contribute to such a high burden of this condition and to assess whether a similar pattern is also observed elsewhere.

AIM OF THE STUDY: The present investigation was carried out to provide the pharmacological basis for the medicinal use of Terminalia bellerica in hyperactive gastrointestinal and respiratory disorders. MATERIALS AND METHODS: Crude extract of Terminalia bellerica fruit (Tb.Cr) was studied in in vitro and in vivo. RESULTS: Tb.Cr caused relaxation of spontaneous contractions in isolated rabbit jejunum at 0.1-3.0mg/mL. Tb.Cr inhibited the carbachol (CCh, 1microM) and K(+) (80mM)-induced contractions in a pattern similar to that of dicyclomine, but different from nifedipine and atropine. Tb.Cr shifted the Ca(++) concentration-response curves to right, like nifedipine and dicyclomine. In guinea-pig ileum, Tb.Cr produced rightward parallel shift of acetylcholine-curves, followed by non-parallel shift at higher concentration with the suppression of maximum response, similar to dicyclomine, but different from nifedipine and atropine. Tb.Cr exhibited protective effect against castor oil-induced diarrhea and carbachol-mediated bronchoconstriction in rodents. In guinea-pig trachea, Tb.Cr relaxed the CCh-induced contractions, shifted CCh-curves to right and inhibited the contractions of K(+). Anticholinergic effect was distributed both in organic and aqueous fractions, while CCB was present in the aqueous fraction. CONCLUSIONS: These results indicate that Terminalia bellerica fruit possess a combination of anticholinergic and Ca(++) antagonist effects, which explain its folkloric use in the colic, diarrhea and asthma.

RESUMO

Pantothenate kinase-associated neurodegeneration is an autosomal-recessive disorder associated with the accumulation of iron in the basal ganglia. The disease presents with dystonia, rigidity, and gait impairment, leading to restriction of activities and loss of ambulation. The disorder is caused by defective iron metabolism associated with mutations in the PANK2 gene, which codes for the pantothenate kinase enzyme. We report on a mutation screen conducted in two siblings to establish a molecular diagnosis of the disease and a genetic test for the family.

OBJECTIVES: Perception and attitude regarding prenatal screening and induced abortion vary across different populations. This study assesses the attitudes and perceptions regarding prenatal screening and induced abortion among Pakistani adults. METHODS: We conducted a cross-sectional study among adults (18+) coming to the Aga Khan University Hospital, a private tertiary care hospital in Karachi, Pakistan. RESULTS: Majority (65%) of the study population had knowledge about prenatal screening and it was acceptable to most (85.5%) of them. Significant proportion had high acceptance for induced abortion (23%) of a fetus that has serious congenital anomalies. On the other hand, 15% were unwilling to consider termination of pregnancy (TOP) in any circumstances. Women had more favorable attitude toward induced abortion. Most of the respondents (63%) were in favor of abortion if fetal death was imminent as a result of a congenital abnormality. Majority wanted mutual consultation of husband and wife for making decision regarding induced abortion (84%). CONCLUSION: There was a considerable discord in opinion about abortion in the study population. Health care providers should involve both parents in making decisions about abortions and counsel them adequately about congenital disorders.
