Environmental factors are at least as important as genetic factors for the development of obsessive-compulsive symptoms (OCS), but the identification of such factors remain a research priority. Our study aimed to investigate the association between a broad scope of potential risk factors and OCS in a large community cohort of children and adolescents. We evaluated 1877 participants and their caregivers at baseline and after 3 years to assess various demographic, prenatal, perinatal, childhood adversity, and psychopathological factors. Mean age at baseline was 10.2 years (SD 1.9) and mean age at follow-up was 13.4 years (SD 1.9). Reports of OCS at baseline and follow-up were analyzed using latent variable models. At preliminary regression analysis, 15 parameters were significantly associated with higher OCS scores at follow-up. At subsequent regression analysis, we found that eight of these parameters remained significantly associated with higher follow-up OCS scores while being controlled by each other and by baseline OCS scores. The significant predictors of follow-up OCS were: lower socioeconomic status (p = 0.033); lower intelligence quotient (p = 0.013); lower age (p < 0.001); higher maternal stress level during pregnancy (p = 0.028); absence of breastfeeding (p = 0.017); parental baseline OCS (p = 0.038); youth baseline anxiety disorder (p = 0.023); and youth baseline OCS scores (p < 0.001). These findings may better inform clinicians and policymakers engaged in the mental health assessment and prevention in children and adolescents.

Background: Thalamic volume measures have been linked to obsessive-compulsive disorder (OCD) in children and adolescents. However, it is unclear if alterations in thalamic volumes occur before or after symptom onset and if there is a relation to the presence of sub-clinical obsessive-compulsive symptoms (OCS). Here, we explore the relationship between OCS and the rate of thalamic volume change in a cohort of children and youth at high risk to develop a mental disorder. A secondary aim was to determine if there is a relationship between OCS and the individual's OCD polygenic risk score (OCD-PRS) and between the rate of thalamic volume change and the OCD-PRS. Methods: The sample included 378 children enrolled in the longitudinal Brazilian High-Risk Cohort for Mental Conditions. Participants were assessed for OCS and the symmetrized percent change (SPC) of thalamic volume across two time-points separated by 3 years, along with the OCD-PRS. Zero-altered negative binomial models were used to analyze the relationship between OCS and thalamic SPC. Multiple linear regressions were used to examine the relationship between thalamic SPC and OCD-PRS. Results: A significant relationship between OCS and the right thalamus SPC (p = 0.042) was found. There was no significant relationship between changes in thalamic volume SPC and OCD-PRS. Conclusions: The findings suggest that changes in the right thalamic volume over the course of 3 years in children may be associated to OCS. Future studies are needed to confirm these results and further characterize the specific nature of OCS symptoms associated with thalamic volumes.

OBJECTIVE: The objective is to evaluate the prevalence and associated clinical characteristics of eating disorders (ED) in patients with obsessive-compulsive disorder (OCD). METHOD: This is a cross-sectional study comparing 815 patients with OCD. Participants were assessed with structured interviews and scales: SCID-I, Y-BOCS, Dimensional Y-BOCS, BABS, Beck Depression and Anxiety Inventories. RESULTS: Ninety-two patients (11.3%) presented the following EDs: binge-eating disorders [= 59 (7.2%)], bulimia nervosa [= 16 (2.0%)], or anorexia nervosa [= 17 (2.1%)]. Compared to OCD patients without ED (OCD-Non-ED), OCD-ED patients were more likely to be women with previous psychiatric treatment. Mean total scores in Y-BOCS, Dimensional Y-BOCS, and BABS were similar within groups. However, OCD-ED patients showed higher lifetime prevalence of comorbid conditions, higher anxiety and depression scores, and higher frequency of suicide attempts than did the OCD-Non-ED group. Primarily diagnosed OCD patients with comorbid ED may be associated with higher clinical severity. DISCUSSION: Future longitudinal studies should investigate dimensional correlations between OCD and ED.

BACKGROUND: A small proportion of individuals with Tourette syndrome (TS) have a lifelong course of illness that fails to respond to conventional treatments. Open label studies have suggested that low frequency (1-Hz) repetitive transcranial magnetic stimulation (rTMS) targeting the supplementary motor area (SMA) may be effective in reducing tic severity. OBJECTIVE/HYPOTHESIS: To examine the efficacy of rTMS over the SMA for TS in a randomized double-blind sham-controlled trial (RCT). METHODS: We conducted a two-site RCT-rTMS with 20 adults with severe TS for 3 weeks. Treatment consisted of 15 sessions (1-Hz; 30 min; 1800 pulses per day) of active or sham rTMS at 110% of the motor threshold over the SMA. A subsequent 3 week course of active rTMS treatment was offered. RESULTS: Of the 20 patients (16 males; mean age of 33.7 ± 12.2 years), 9 received active and 11 received sham rTMS. After 3 weeks, patients receiving active rTMS showed on average a 17.3% reduction in the YGTSS total tic score compared to a 13.2% reduction in those receiving sham rTMS, resulting in no statistically significant reduction in tic severity (P = 0.27). An additional 3 week open label active treatment for those patients (n = 7) initially randomized to active rTMS resulted in a significant overall 29.7% reduction in tic severity compared to baseline (P = 0.04). CONCLUSION: This RCT did not demonstrate efficacy of 3-week SMA-targeted low frequency rTMS in the treatment of severe adult TS. Further studies using longer or alternative stimulation protocols are warranted.

OBJECTIVE: Patients with obsessive-compulsive disorder (OCD) often present with deficits in episodic memory, and there is evidence that these difficulties may be secondary to executive dysfunction, that is, impaired selection and/or application of memory-encoding strategies (mediation hypothesis). Semantic clustering is an effective strategy to enhance encoding of verbal episodic memory (VEM) when word lists are semantically related. Self-initiated mobilization of this strategy has been associated with increased activity in the prefrontal cortex, particularly the orbitofrontal cortex, a key region in the pathophysiology of OCD. We therefore studied children and adolescents with OCD during uncued semantic clustering strategy application in a VEM functional magnetic resonance imaging (fMRI)-encoding paradigm. METHOD: A total of 25 pediatric patients with OCD (aged 8.1-17.5 years) and 25 healthy controls (HC, aged 8.1-16.9) matched for age, gender, handedness, and IQ were evaluated using a block design VEM paradigm that manipulated semantically related and unrelated words. RESULTS: The semantic clustering strategy score (SCS) predicted VEM performance in HC (p < .001, R(2) = 0.635), but not in patients (p = .099). Children with OCD also presented hypoactivation in the dorsomedial prefrontal cortex (cluster-corrected p < .001). Within-group analysis revealed a negative correlation between Yale-Brown Obsessive Compulsive Scale scores and activation of orbitofrontal cortex in the group with OCD. Finally, a positive correlation between age and SCS was found in HC (p = .001, r = 0.635), but not in patients with OCD (p = .936, r = 0.017). CONCLUSION: Children with OCD presented altered brain activation during the VEM paradigm and absence of expected correlation between SCS and age, and between SCS and total words recalled. These results suggest that different neural mechanisms underlie self-initiated semantic clustering in OCD.

BACKGROUND: Recent findings suggest that acute-phase rheumatic fever (RF) patients present with higher frequencies of obsessive-compulsive disorder (OCD) and tic disorders. Until now, there have been no such studies in RF in non-acute phases. OBJECTIVE: To verify whether patients with a history of RF with or without Sydenham's chorea (SC) present with higher rates of OCD, tic disorders, and other obsessive-compulsive (OC) spectrum disorders (such as body dysmorphic disorder [BDD]) than controls. METHOD: Between February 1999 and December 2002, 59 consecutive outpatients with non-acute RF (28 with and 31 without SC) from an RF clinic and 39 controls from an orthopedics clinic were blindly assessed for OC spectrum disorders using structured interviews to assign DSM-IV diagnosis. Data were analyzed with Fisher exact and chi(2) tests to compare frequencies of disorders, and Kaplan-Meier survival analyses were used to obtain age-corrected rates. RESULTS: The age-corrected rates of tic disorders were higher in patients with RF without SC (N = 3; 14.39%) (p =.003) when compared with controls. Age-corrected rates for OC spectrum disorders (OCD, tic disorders, and BDD) combined were higher both in RF without SC (N = 4; 20.65%) and RF with SC (N = 5; 19.55%) groups than in controls (N = 1; 2.56%) (p =.048). CONCLUSIONS: RF, even in the non-acute phase, may increase the risk for some OC spectrum disorders, such as OCD, tic disorders, and BDD. These data, although preliminary, reinforce the idea that OC spectrum disorders may share common underlying pathophysiologic mechanisms and vulnerability factors with RF or that RF could trigger central nervous system late manifestations such as OC spectrum disorders.

BACKGROUND: Although knowledge on developmental psychiatry is fundamental for the early recognition, treatment, and prevention of mental disorders, this subject has not been incorporated into the medical curriculum or psychiatric practice in Brazil. OBJECTIVE: To evaluate the effect of a short course on developmental psychiatry for undergraduate students and to expand education policies concerning developmental psychiatry in Brazil. METHODS: Before and after attending an extracurricular 12-hour, 4-day course on the fundamentals of developmental psychiatry, undergraduate health sciences students were tested regarding their knowledge of the subject. The pre-test/post-test included 12 randomly selected multiple-choice questions designed to evaluate knowledge related to developmental psychiatry and was administered together with a questionnaire designed to evaluate students' attitudes. To compare performances between groups, nonparametric analyses of ordinal categorical data were employed. RESULTS: The final sample comprised 43 students. The mean post-test score was significantly higher than the mean pre-test score (65.0% vs 39.9%; p < 0.0001). We found that strongly positive attitudes correlated with better performance. The 3rd and 4th year medical students performed better than the 1st and 2nd year medical students and the non-medical students. Sex differences favoring males were also observed. CONCLUSION: Our findings encourage additional educational policies related to developmental psychiatry which may result in direct clinical implications.

OBJECTIVE: This study aims to compare the prevalence of obsessive-compulsive spectrum disorders (OCSD) in psychiatric outpatients with and without a history of rheumatic fever (RF). METHODS: An analytical cross-sectional study assessing a large sample of consecutive psychiatric outpatients at a Brazilian private practice was conducted during a 10-year period. Psychiatric diagnoses were made by a senior psychiatrist based on Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition. Best-estimate diagnosis procedure was also performed. RESULTS: The total sample comprised 678 subjects, 13 of whom (1.92%) presented with a previous history of RF. This group showed a higher prevalence of subclinical obsessive-compulsive disorder (P=.025) and OCSD (P=.007) when compared to individuals with no such history. CONCLUSIONS: A previous history of RF was associated with OCSD. These results suggest that clinicians should be encouraged to actively investigate obsessive-compulsive symptoms and related disorders in patients with a positive history of RF.

CONTEXTO: Apesar de vital para o reconhecimento precoce, tratamento e prevenção de transtornos mentais, a psiquiatria do desenvolvimento ainda não foi incorporada à grade curricular ou prática psiquiátrica no Brasil. OBJETIVO: Avaliar o impacto de um curso extracurricular em psiquiatria do desenvolvimento na graduação e a viabilidade de expansão de políticas de ensino em psiquiatria do desenvolvimento no Brasil. MÉTODO: Antes e após assistirem um curso de 12 horas durante 4 dias sobre fundamentos da psiquiatria do desenvolvimento, estudantes de graduação em saúde responderam um questionário composto por 12 questões de múltipla escolha randomizadas para avaliação de retenção do conhecimento e questões destinadas à avaliação de atitudes. A análise estatística incluiu testes não-paramétricos de variáveis categóricas ordinais. RESULTADOS: Em uma amostra final composta por 43 estudantes, verificou-se um desempenho significativamente superior no pós-teste (65.0 por cento vs. 39.9 por cento; p < 0.0001). Atitudes positivas em relação ao curso relacionaram-se a um melhor desempenho. Estudantes dos 3º e 4º anos de medicina apresentaram resultados superiores quando comparados aos do 1º e 2º anos de medicina e aos estudantes de outras áreas da saúde. Constatou-se melhor desempenho no sexo masculino. CONCLUSÃO: Este estudo encoraja políticas de ensino em psiquiatria do desenvolvimento que poderão ter implicações clínicas diretas.

O objetivo central desta tese de doutorado foi investigar as características clínicas de sintomas obsessivo-compulsivos (SOC), como fenômeno intermediário entre o desenvolvimento normal e o transtorno obsessivo-compulsivo (TOC), em uma ampla amostra comunitária (não-clínica) composta por crianças em idade escolar (6 a 12 anos) e seus familiares biológicos. Para tal, determinou-se a prevalência e a distribuição sociodemográfica dos SOC descrevendo sua fenomenologia caracterizada a partir de dimensões de SOC, agregação familiar, associação com outras comorbidades psiquiátricas e outras variáveis de comprometimento clínico (ex: fatores de risco, problemas sociais, escolares e de comportamento). Dividimos o presente estudo em duas etapas. Na Etapa I, o objeto de estudo foram 9.937 crianças de 6 a 12 anos regularmente matriculadas em escolas públicas (crianças-index) e seus familiares biológicos (n total=29.459). Nesta etapa utilizou-se a Family History Screening (FHS), escala de rastreamento para sintomas psiquiátricos internacionalmente validada, e um módulo adicional com sete itens para identificar quatro dimensões de SOC ("Agressão/ sexual/ religiosa"; "arranjo/ simetria"; "contaminação/ lavagem" e colecionismo"). Nessa primeira etapa obtivemos dados sobre 9.937 crianças-index (podendo ser irmãos entre si), 3.305 irmãos biológicos (13 a 18 anos) e 16.218 pais. As mães biológicas foram informantes em 88% das entrevistas. Os SOC estiveram presentes em 19.4% da amostra total, sendo 14,7% das crianças-index; 15,6% dos irmãos; 34,6% das mães e 12,1% dos pais. A presença dos SOC foi associada ao sexo masculino e aumento da idade em crianças e adolescentes. Houve agregação familiar das dimensões de SOC nas famílias, sendo que a dimensão de "contaminação/ lavagem" foi a mais familiar (OR: 1,44; IC 95% 1,23-1,67; p < 0,001). Crianças-index com SOC apresentaram maior frequência de outros sintomas psiquiátricos, bem como maior comprometimento escolar...


The present thesis investigated the clinical characteristics of obsessive-compulsive symptoms (OCS), as an intermediate phenomenon between normal development and obsessive-compulsive disorder (OCD) by assessing an extensive community (non- clinical) sample of schoolchildren (6-12 years) and their biological relatives. We determined the prevalence and sociodemographic status of OCS, describing its phenomenology characterized from OCS dimensions, familial aggregation, association with other psychiatric comorbidities, and other variables of clinical impairment (e.g.: risk factors , social, school and behavior problems). The study was divided in two phases. In phase I, 9,937 children (aged 6 to 12 years) enrolled in regular public schools (index-children) and their biological relatives (overall n = 29,459) were assessed. In this phase, we used the Family History Screening (FHS), an internationally validated instrument developed for psychiatric symptoms assessment. An additional seven-item module to identify four OCS dimensions ("aggressive/ sexual/ religious"; "symmetry/ arranging", "contamination/ cleaning" and "hoarding ") was also used. In the first phase data on 9,937 index-children (may be siblings to each other), 3,305 biological siblings (13-18 years) and 16,218 parents were obtained. The biological mothers were informants in 88 % of the interviews. OCS were present in 19.4 % of the total sample, 14.7 % of index-children, 15.6 % of siblings, 34.6 % of mothers and 12.1 % of parents. The presence of OCS was associated with male gender and increasing age in children and adolescents. Familial aggregation of OCS dimensions was found; the "contamination/ cleaning" was the most familial dimension (OR: 1.44; 95% IC 1.23 to 1.67; p < 0.001). OCS were associated with higher frequency of other psychiatric symptoms as well as greater rates of social/ school problems and searching for previous treatments. CONTEXTO: A literatura mostra comorbidade entre jogo patológico, abuso de substâncias e transtornos afetivos (maior prevalência de depressão) em adultos. É alta a prevalência de depressão nos idosos, tornando-os vulneráveis à dependência de jogos de azar e ao abuso de substâncias. Com o aumento desta população em todo o mundo, faz-se necessário mais pesquisas sobre esse tema. OBJETIVOS: Avaliar o grau de envolvimento do idoso que aposta em jogos de azar e sua correlação com uso de substâncias. MÉTODOS: Revisão bibliográfica dos bancos de dados MEDLINE, LILACS e PsycINFO, sem limite de tempo. RESULTADOS: Idosos apostadores recreativos de jogos de azar apresentam consumo principalmente de álcool e boa avaliação de saúde geral. Idosos com problemas com jogo apresentam abuso de álcool, mais transtornos psiquiátricos e doenças cardiovasculares que os idosos jogadores recreativos. Quando comparados com adultos ou jovens jogadores, os idosos apresentam menos problemas relacionados ao consumo de álcool. CONCLUSÕES: Assim como nos adultos, também existe correlação entre apostas em jogos de azar e consumo de substâncias nos idosos. Quanto mais problemático o envolvimento com jogo, mais problemático é o consumo de substâncias (álcool) e maior prevalência de transtornos do humor (depressão). No entanto, não se pode afirmar uma relação causal entre esses fatores, sendo necessária realização de mais pesquisas sobre esse tema.


BACKGROUND: Literature presents comorbidity between pathological gambling, substance use/abuse and affective disorders (depression being the most prevalent) in adults. The prevalence of depression disorder in elderly people is high, making them vulnerable to pathological gambling and substance abuse. With the increase of this population worldwide, more research regarding these issues has become necessary. OBJECTIVES: Assess the involvement of the elderly in gambling activities and its correlation with substance abuse. METHODS: Literature review of MEDLINE, LILACS and PsycINFO databases without time limits. RESULTS: Elderly recreational gamblers present alcohol consumption and good general health. Elderly problem gamblers present alcohol abuse, with more psychiatric disorders and heart disease than recreational gamblers. When compared with adult or young gamblers, the elderly present fewer problems with alcohol consumption. Conclusions: As with adults, in the elderly a correlation also exists between gambling and substance abuse. The more problematic the gambling is, the more problematic the substance (alcohol) abuse, and the higher the prevalence of mood disorders (depression). A causal relationship between these factors can not yet be affirmed however, as more research is needed.

CONTEXTO: O abuso de álcool é considerado atualmente um dos maiores problemas de saúde devido ao impacto mundial que esta problemática gera em termos sociais e financeiros. OBJETIVOS: Discutir, a partir do referencial econômico, as conseqüências sociais do abuso do álcool e introduzir os conceitos relacionados à economia da saúde. MÉTODOS: Revisão da literatura com base nos sistemas Medline, LILACS e Scielo de artigos publicados nas línguas, portuguesa e inglesa no período de 1987 a 2007, e posterior busca manual com base na bibliografia de interesse dos artigos selecionados. RESULTADOS: Foram encontrados 50 artigos sobre estudos econômicos e conceitos econômicos em diversas áreas; desses, 15 diretamente relacionados ao custo social do abuso do álcool. O método de apuração dos custos e a composição dos itens de custos foram semelhantes em todos os estudos. CONCLUSÕES: Os estudos econômicos se apresentam incipientes no Brasil e avançados nos países desenvolvidos. A necessidade de pesquisas nacionais que abordem o custo social do abuso do álcool tem como uma das finalidades promover subsídios para se pensar e efetivar a construção de políticas públicas mais consistentes e que estejam em consonância com o perfil epidemiológico e socioeconômico do país.


BACKGROUND: Alcohol abuse is currently considered one of the largest worldwide health problems due to its social and financial impact. OBJECTIVES: To discuss the economic and social impact of alcohol abuse and to introduce concepts related to health economics. METHODS: Literature review of articles written in Portuguese and in English (published between 1987 and 2007) stored in the MEDLINE, LILACS and Scielo databases was carried out. RESULTS: Fifty articles on economic studies and economic concepts in various areas were found, among which 15 were directly related to the social costs of alcohol abuse. The costs assessment methods and the composition of cost items were similar in all studies. CONCLUSIONS: Economic studies are new in Brazil, but are advanced in developed countries. National research on the social costs of alcohol abuse is necessary in order to promote knowledge as a means of creating and implementing public policies that are more consistent with and in alignment with the epidemiological and socioeconomic profile of the country.

RESUMO

Este artigo descreve a história clínica e o manejo de um paciente masculino adulto com esquizofrenia catatônica refratária a dois neurolépticos típicos (haloperidol e clorpromazina) e a outro agente atípico (risperidona), e com antecedente de dois episódios de síndrome neuroléptica maligna em vigência de neurolépticos típicos. Os autores optaram pela associação de eletroconvulsoterapia (ECT) e olanzapina (7,5 mg). Foram obtidos consideráveis benefícios para o paciente.

RESUMO

A partir de um relato de caso, os autores discutem a evolucão histórica do conceito de estado-misto. Objetiva-se questionar as limitacões do manejo clínico destes pacientes, além de se ressaltar o papel da eletroconvulsoterapia nesses casos. Os dados relatados foram obtidos pela anamnese com o paciente e com seus familiares, além de notas em prontuários médicos.

RESUMO

Estudos recentes sugerem uma associação entre febre reumática (FR) e transtornos do espectro obsessivo-compulsivo, o que levou à hipótese de que alterações na resposta imune pudessem ter um papel na etiologia destes últimos. Um marcador biológico que talvez identifique maior susceptibilidade para o desenvolvimento de FR e desses transtornos neuropsiquiátricos tem causado grande interesse na literatura. Trata-se do D8/17, um anticorpo monoclonal contra um antígeno de membrana de linfócitos B. Neste artigo introduzimos conhecimentos sobre o D8/17 e discutimos suas implicações como um possível marcador biológico de transtornos neuropsiquiátricos associados ou não à FR.
